Quinn Tran 41dc529a46 qla2xxx: Improve RSCN handling in driver
Current code blindly does State Change Registration when
the link is up. Move SCR behind fabric scan, so that arbitrated
loop scan would not get erroneous error message.

Some of the other improvements are as follows

- Add session deletion for TPRLO and send acknowledgment for TPRLO.
- Enable FW option to move ABTS, RIDA & PUREX from RSPQ to ATIOQ.
- Save NPort ID early in link init.
- Move ABTS & RIDA to ATIOQ helps in keeping command ordering and
  link up sequence ordering.
- Save Nport ID and update VP map so that SCSI CMD/ATIO won't be dropped.
- fcport alloc does the initializes memory to zero. Remove memset to
  zero since It might corrupt link list.
- Turn off Registration for State Change MB in loop mode.

Quinn Tran 726b854870 qla2xxx: Add framework for async fabric discovery
Currently code performs a full scan of the fabric for
every RSCN. Its an expensive process in a noisy large SAN.

This patch optimizes expensive fabric discovery process by
scanning switch for the affected port when RSCN is received.

Currently Initiator Mode code makes login/logout decision without
knowledge of target mode. This causes driver and firmware to go
out-of-sync. This framework synchronizes both initiator mode
personality and target mode personality in making login/logout
decision.

This patch adds following capabilities in the driver

- Send Notification Acknowledgement asynchronously.
- Update session/fcport state asynchronously.
- Create a session or fcport struct asynchronously.
- Send GNL asynchronously. The command will ask FW to
  provide a list of FC Port entries FW knows about.
- Send GPDB asynchronously. The command will ask FW to
  provide detail data of an FC Port FW knows about or
  perform ADISC to verify the state of the session.
- Send GPNID asynchronously. The command will ask switch
  to provide WWPN for provided NPort ID.
- Send GPSC asynchronously. The command will ask switch
  to provide registered port speed for provided WWPN.
- Send GIDPN asynchronously. The command will ask the
  switch to provide Nport ID for provided WWPN.
- In driver unload path, schedule all session for deletion
  and wait for deletion to complete before allowing driver
  unload to proceed.

Himanshu Madhani 2c39b5ca2a qla2xxx: Remove SRR code
During initial implementation, tape support was included but not
enabled by default on target. So far, we don't see any target
customer requesting this support. Since this code is not being
used actively, we want to remove it and we will add back if there
are any request in future for SRR support.

Christoph Hellwig b6a05c823f scsi: remove eh_timed_out methods in the transport template
Instead define the timeout behavior purely based on the host_template
eh_timed_out method and wire up the existing transport implementations
in the host templates.  This also clears up the confusion that the
transport template method overrides the host template one, so some
drivers have to re-override the transport template one.

Christoph Hellwig 82ed4db499 block: split scsi_request out of struct request
And require all drivers that want to support BLOCK_PC to allocate it
as the first thing of their private data.  To support this the legacy
IDE and BSG code is switched to set cmd_size on their queues to let
the block layer allocate the additional space.

Quinn Tran 300af14bdb qla2xxx: Disable out-of-order processing by default in firmware
Out of order(OOO) processing requires initiator, switch
and target to support OOO. In today's environment, none
of the switches support OOO. OOO requires extra buffer
space which affect performance. By turning ON this feature
in QLogic's FW, it delays error recovery because dropped
frame is treated as out of order frame. We're turning OFF
this option of speed up error recovery.

Quinn Tran 5f35509db1 qla2xxx: Terminate exchange if corrupted
Corrupted ATIO is defined as length of fcp_header & fcp_cmd
payload is less than 0x38. It's the minimum size for a frame to
carry 8..16 bytes SCSI CDB. The exchange will be dropped or
terminated if corrupted.

Milan P. Gandhi c7702b8c22 scsi: qla2xxx: Get mutex lock before checking optrom_state
There is a race condition with qla2xxx optrom functions where one thread
might modify optrom buffer, optrom_state while other thread is still
reading from it.

In couple of crashes, it was found that we had successfully passed the
following 'if' check where we confirm optrom_state to be
QLA_SREADING. But by the time we acquired mutex lock to proceed with
memory_read_from_buffer function, some other thread/process had already
modified that option rom buffer and optrom_state from QLA_SREADING to
QLA_SWAITING. Then we got ha->optrom_buffer 0x0 and crashed the system:

This is mostly stuff which missed the initial pull.  There's a new
 driver: qedi, some ufs, ibmvscsis and ncr5380 updates plus some
 assorted driver fixes and also a fix for the bug where if a device
 goes into a blocked state between configuration and sysfs device add
 (which can be a long time under async probing) it would become
 permanently blocked.

Michael Hernandez 5601236b6f scsi: qla2xxx: Add Block Multi Queue functionality.
Tell the SCSI layer how many hardware queues we have based on the number
of max queue pairs created. The number of max queue pairs created will
depend on number of MSI-X vector count.

This feature can be turned on via CONFIG_SCSI_MQ_DEFAULT or passing
scsi_mod.use_blk_mq=Y as a parameter to the kernel

In a target environment, there could be a user space daemon

mailbox commands immediately rather than waiting for timeout,
if driver detects that chip reset has been issued or chip reset
is in progress.

The previous commit 1535aa75a3 ("qla2xxx: fix invalid DMA access after
command aborts in PCI device remove") introduced a regression during an
EEH recovery, since the change to the qla2x00_abort_all_cmds() function
calls qla2xxx_eh_abort(), which verifies the EEH recovery condition but
handles it heavy-handed. (commit a465537ad1 "qla2xxx: Disable the
adapter and skip error recovery in case of register disconnect.")

This problem warrants a more general/optimistic solution right into
qla2xxx_eh_abort() (eg in case a real command abort arrives during EEH
recovery, or if it takes long enough to trigger command aborts); but
it's still worth to add a check to ensure the code added by the previous
commit is correct and contained within its owner function.

This commit just adds a 'if (!ha->flags.eeh_busy)' check around it.
(ahem; a trivial fix for this -rc series; sorry for this oversight.)

With it applied, both PCI device remove and EEH recovery works fine.

If a command is aborted in the kernel but not in the adapter, it might be
considered complete and its DMA memory released, but it is still alive in
the adapter, which will trigger an invalid DMA access upon its completion
(in the DMA operations to deliver the command response to the driver).

On powerpc platforms with IOMMU/EEH capabilities, the problem is observed
during PCI device removal with ongoing IO requests -- which might trigger
an EEH event very often, pointing to a 'TCE Request Page Access Error'.

In that path, which is qla2x00_remove_one(), the commands are aborted in
qla2x00_abort_all_cmds(), which does not perform an abort in the adapter
as is done in qla2xxx_eh_abort() for example.

So, this patch changes qla2x00_abort_all_cmds() to abort commands in the
adapter too, with a call to qla2xxx_eh_abort(), which already implements
all the logic to submit abort requests and handle responses.

When the driver is unloading, in qla2x00_remove_one(), there is a single
call/point in time to abort ongoing commands, qla2x00_abort_all_cmds(),
which is still several steps away from the call to scsi_remove_host().

If more commands continue to arrive and be processed during that
interval, when the driver is tearing down and releasing its structures,
it might potentially hit an oops due to invalid memory access:

Bill Kuzeja a5dd506e15 scsi: qla2xxx: Fix scsi scan hang triggered if adapter fails during init
A system can get hung task timeouts if a qlogic board fails during
initialization (if the board breaks again or fails the init). The hang
involves the scsi scan.

In a nutshell, since commit beb9e315e6 ("qla2xxx: Prevent removal and
board_disable race"):

...it is possible to have freed ha (base_vha->hw) early by a call to
qla2x00_remove_one when pdev->enable_cnt equals zero:

Almost always, the scsi_host_put above frees the vha structure
(attached to the end of the Scsi_Host we're putting) since it's the last
put, and life is good.  However, if we are entering this routine because
the adapter has broken sometime during initialization AND a scsi scan is
already in progress (and has done its own scsi_host_get), vha will not
be freed. What's worse, the scsi scan will access the freed ha structure
through qla2xxx_scan_finished:

The scsi scan keeps checking to see if a scan is complete by calling
qla2xxx_scan_finished. There is a timeout value that limits the length
of time a scan can take (hw->loop_reset_delay, usually set to 5
seconds), but this definition is in the data structure (hw) that can get
freed early.

This can yield unpredictable results, the worst of which is that the
scsi scan can hang indefinitely. This happens when the freed structure
gets reused and loop_reset_delay gets overwritten with garbage, which
the scan obliviously uses as its timeout value.

The fix for this is simple: at the top of qla2xxx_scan_finished, check
for the UNLOADING bit in the vha structure (_vha is not freed at this
point).  If UNLOADING is set, we exit the scan for this adapter
immediately. After this last reference to the ha structure, we'll exit
the scan for this adapter, and continue on.

This problem is hard to hit, but I have run into it doing negative
testing many times now (with a test specifically designed to bring it
out), so I can verify that this fix works. My testing has been against a
RHEL7 driver variant, but the bug and patch are equally relevant to to
the upstream driver.

Sawan Chandak 783e0dc4f6 qla2xxx: Check for device state before unloading the driver.
During hot swap of PCI device, there can be PCI error on device,
during normal driver unload. The race between normal driver unload and
driver unload due to PCI error, can lead to system crash.Fix is to check
if there is unload going on and allow that function to unload the driver.

Johannes Thumshirn ef8d1d51aa qla2xxx: setup data needed in ISR before setting up the ISR
qla2xxx first calls request_irq() and then does the setup of the queue
entry data needed in the interrupt handlers in when using MSI-X. This
could lead to a NULL pointer dereference when an IRQ fires between the
request_irq() call and the assignment of the qentry data structure to
the rsp->msix field. A possible case for such a race would be in the
kdump case when the HBA's IRQs are still enabled but the driver is
undergoing a new initialisation and thus is not aware of already
activated IRQs in the HBA.

That macros uses logical "&&" instead of bit-wise "&" which is
apparently wrong. Since the macro is completely unused, simply

Tina Ruchandani 5ea33eb573 qla2xxx: Remove use of 'struct timeval'
struct register_host_info stores a 64-bit UTC system time timestamp.
This patch removes the use of 'struct timeval' to obtain that timestamp
as its tv_sec value will overflow on 32-bit systems in year 2038 beyond.
The patch uses ktime_get_real_seconds() which returns a 64-bit seconds value.

Chad Dupuis 080c9517ea qla2xxx: Set relogin flag when we fail to queue login requests.
If we fail to queue an srb for an async login we should set the
relogin flag so it will be retried as the reason for the queuing
failure was most likely transient.  Failure to do this can lead to
failed paths as login is never retried if the relogin flag is not
set.

[ Upstream Commit 84e32a06f4 ]

Commit 84e32a0 ("qla2xxx: Use pci_enable_msix_range() instead of
pci_enable_msix()") introduced a regression when target mode is enabled.
In qla24xx_enable_msix(), ha->max_rsp_queues was incorrectly set
to a value higher than the number of response queues allocated causing
an invalid dereference. Specifically here in qla2x00_init_rings():

Quinn Tran a07100e00a qla2xxx: Fix TMR ABORT interaction issue between qla2xxx and TCM
During lun reset, TMR thread from TCM would issue abort
to qla driver.  At abort time, each command is in different
state.  Depending on the state, qla will use the TMR thread
to trigger a command free(cmd_kref--) if command is not
down at firmware.

This looks wrong because we are comparing a boolean with an

Driver has following initialization sequence for Target mode

1. Driver initialization starts

2. ISP Abort is scheduled when the target is enabled.

qla2xxx [0000:04:00.0]-4807:25: ISP abort scheduled
qla2xxx [0000:04:00.0]-00af:25: Performing ISP error recovery - ha=ffff880caa9e0000.

3. DPC thread starts the ISP Abort

4. While DPC is resetting the chip and initializing the firmware, we get
   async events from the firmware about P2P mode, LOOP UP and PORT UPDATE.

5. PRLI from a initiator is delivered to us followed by a PLOGI and then a
   SCSI command which creates a session.

6. If the SCSI command is a WRITE in this case, we issue XFR RDY and it
   gets dropped as can be seen with messages RESET-XFR because ISP Abort
   is still active

qla2xxx [0000:04:00.0]-e902:25: RESET-XFR active/old-count/new-count = 1/1/1.

7. If the SCSI command is a READ, we issue RESPONSE and they get dropped
   as well because Abort is still active.

qla2xxx [0000:04:00.0]-e901:25: RESET-RSP active/old-count/new-count = 1/1/1

8. Now eventually, ISP Abort finishes clearing the DPC flags.

qla2xxx [0000:04:00.0]-8822:25: qla2x00_abort_isp succeeded.
qla2xxx [0000:04:00.0]-4808:25: ISP abort end.

9. Since we dropped SCSI commands silently (without any responses sent
   to the initiator) initiator waits for a SCSI timeout (which is 60
   seconds in our case), Sends an ABTS which fails since there
   no se_cmd found for the tag that ABTS is referencing as the
   commands were cleaned up in Step 6 and 7.

10. Initiator send an IO after the ABTS which succeed fine.

To fix the above case, the following changes have been made:
  - To prevent target from dropping commands silently, use the online flag
    instead to check for an active chip reset. Once the port is online during
    a chip reset phase, we are good to process the commands.
  - Clean up qla2x00_restart_isp to not set the online flag and process ATIO
    as it is unnecessary. During a chip reset, interrupts are enabled only
    after setting the online flag to 1, so ATIO's won't be missed and hence
    no need to process ATIO's after setting the online flag.

Quinn Tran 2f424b9b36 qla2xxx: Move atioq to a different lock to reduce lock contention
99% of the time the ATIOQ has SCSI command.  The other 1% of time
is something else.  Most of the time this interrupt does not need
to hold the hardware_lock.  We're moving the ATIO interrupt thread
to a different lock to reduce lock contention.

Quinn Tran 7560151b6b qla2xxx: Remove dependency on hardware_lock to reduce lock contention.
Sessions management (add, deleted, modify) currently are serialized
through the hardware_lock.  Hardware_lock is a high traffic lock.
This lock is accessed by both the transmit & receive sides.

Sessions management is now moved off to another lock call sess_lock.
This is done to reduce lock contention and increase traffic throughput.

Alexei Potashnik b7bd104e6f qla2xxx: Wait for all conflicts before ack'ing PLOGI
Until now ack'ing of a new PLOGI has only been delayed if there
was an existing session for the same WWN. Ack was released when
the session deletion completed.

If there was another WWN session with the same fc_id/loop_id pair
(aka "conflicting session"), PLOGI was still ack'ed immediately.
This potentially caused a problem when old session deletion logged
fc_id/loop_id out of FW after new session has been established.

Two work-arounds were attempted before:
1. Dropping PLOGIs until conflicting session goes away.
2. Detecting initiator being logged out of FW and issuing LOGO
to force re-login.

This patch introduces proper solution to the problem where PLOGI
is held until either existing session with same WWN or any
conflicting session goes away. Mechanism supports one session holding
two PLOGI acks as well as one PLOGI ack being held by many sessions.

Alexei Potashnik 71cdc07964 qla2xxx: Delete session if initiator is gone from FW
1. Initiator A is logged in with fc_id(1)/loop_id(1)
2. Initiator A re-logs in with fc_id(2)/loop_id(2)
3. Part of old session deletion async logoout for 1/1 is queued
4. Initiator B logs in with fc_id(1)/loop_id(1), starts
   passing data and creates session.
5. Async logo from 3 is processed by DPC and sent to FW

Now initiator B has the session but is logged out from FW.

This condition is detected first with CTIO error 29 at which
point we should delete current session. During session
deletion we will send LOGO to initiator to force re-login.

Under rare circumstances initiator might be logged out of FW,
not have driver session, but still think it's logged in.
E.g. the above sequence plus session deletion due to re-config.
Incoming commands will fail to create local session because
initiator is not found in FW. In this case we also issue LOGO
to initiator to force him re-login.

Finally this patch fixes exchange leak when commands where
received in logged out state. In this case loop_id must be
set to FFFF when corresponding exchange is terminated. The
patch modifies exchange termination to always use FFFF,
since in certain scenarios it's impossible to tell whether
command was received in logged in or logged out state.

Christoph Hellwig 64d513ac31 scsi: use host wide tags by default
This patch changes the !blk-mq path to the same defaults as the blk-mq
I/O path by always enabling block tagging, and always using host wide
tags.  We've had blk-mq available for a few releases so bugs with
this mode should have been ironed out, and this ensures we get better
coverage of over tagging setup over different configs.

Christoph Hellwig 2eafd72939 target: use per-attribute show and store methods
This also allows to remove the target-specific old configfs macros, and

Nicholas Bellinger 8f9b565482 target/qla2xxx: Honor max_data_sg_nents I/O transfer limit
This patch adds an optional fabric driver provided SGL limit
that target-core will honor as it's own internal I/O maximum
transfer length limit, as exposed by EVPD=0xb0 block limits
parameters.

This is required for handling cases when host I/O transfer
length exceeds the requested EVPD block limits maximum
transfer length. The initial user of this logic is qla2xxx,
so that we can avoid having to reject I/Os from some legacy
FC hosts where EVPD=0xb0 parameters are not honored.

When se_cmd payload length exceeds the provided limit in
target_check_max_data_sg_nents() code, se_cmd->data_length +
se_cmd->prot_length are reset with se_cmd->residual_count
plus underflow bit for outgoing TFO response callbacks.
It also checks for existing CDB level underflow + overflow
and recalculates final residual_count as necessary.

Note this patch currently assumes 1:1 mapping of PAGE_SIZE
per struct scatterlist entry.
