main memory to the controller using a set of register commands; these
commands include setting up the context ("posting"), removing the
context ("invalidating"), and commands to control the scheduling of
commands and connections to that remote device ("suspensions" and
"resumptions"). There is a similar path to control RNC scheduling from
the protocol engine, which interprets the results of command and data
transmission and reception.
In general, the controller chooses among non-suspended RNCs to find one
that has work requiring scheduling the transmission of command and data
frames to a target. Likewise, when a target tries to return data back
to the initiator, the state of the RNC is used by the controller to
determine how to treat the incoming request. As an example, if the RNC
is in the state "TX/RX Suspended", incoming SSP connection requests from
the target will be rejected by the controller hardware. When an RNC is
"TX Suspended", it will not be selected by the controller hardware to
start outgoing command or data operations (with certain priority-based
exceptions).
As mentioned above, there are two sources for management of the RNC
states: commands from driver software, and the result of transmission
and reception conditions of commands and data signaled by the controller
hardware. As an example of the latter, if an outgoing SSP command ends
with a OPEN_REJECT(BAD_DESTINATION) status, the RNC state will
transition to the "TX Suspended" state, and this is signaled by the
controller hardware in the status to the completion of the pending
command as well as signaled in a controller hardware event. Examples of
the former are included in the patch changelogs.
Driver software is required to suspend the RNC in a "TX/RX Suspended"
condition before any outstanding commands can be terminated. Failure to
guarantee this can lead to a complete hardware hang condition. Earlier
versions of the driver software did not guarantee that an RNC was
correctly managed before I/O termination, and so operated in an unsafe
way.
Further, the driver performed unnecessary contortions to preserve the
remote device command state and so was more complicated than it needed
to be. A simplifying driver assumption is that once an I/O has entered
the error handler path without having completed in the target, the
requirement on the driver is that all use of the sas_task must end.
Beyond that, recovery of operation is dependent on libsas and other
components to reset, rediscover and reconfigure the device before normal
operation can restart. In the driver, this simplifying assumption meant
that the RNC management could be reduced to entry into the suspended
state, terminating the targeted I/O request, and resuming the RNC as
needed for device-specific management such as an SSP Abort Task or LUN
Reset Management request.

This patch fixes a bug in iscsit_allocate_iovecs() where iov_count was
incorrectly calculated using min(1UL, data_length / PAGE_SIZE) instead of
max(1UL, data_length / PAGE_SIZE), that ends up triggering an OOPs for
large block I/O when the SGL <-> iovec mapping exceeds the bogus iov_count
allocation size.
This is a regression introduced during the iscsi-target conversion back
to using core memory allocation here:

Add the ZTE (Vodafone) K3765-Z to the whitelist. This requires the
previous patch to make the whitelist with forced interface 4 generic
or the device fails to initialise. After applying this patch and
loading the Option driver without usb-modeswitch's bind all
interfaces trick, a wwan0 net interface and /dev/cdc-wdm0 device
file were created. Using Bjorn Mork's perl connection script a
connection was made to a mobile network using QMI and the network
interface's IPv4 address was configured OK.

Change the forced interface 4 whitelist to use the generic shared
binder instead of the Gobi specific one. Certain ZTE devices
(K3520-Z & K3765-Z) don't work with the Gobi version, but function
quite happily with the generic. This has been tested with the following
devices:
K3520-Z
K3565-Z
K3765-Z
K4505-Z
It hasn't been tested with the ZTE MF820D, which is the only other
device that uses this whitelist at present. Although Bjorn doesn't
expect any problems, any testing with that device would be appreciated.

Palmas has both Switched Mode (SMPS) and Linear (LDO) regulators in it.
This regulator driver allows software control of these regulators.
The regulators available on Palmas series chips vary depending on the muxing.
This is handled automatically in the driver by reading the mux info from OTP.

We need to use a different loop index for mlx4_counter_alloc() and for
device_create_file() iterations: the mlx4_counter_alloc() loop index
is used in the error flow to free counters.
If the same loop index is used for device_create_file() and, say, the
device_create_file() loop fails on the first iteration, the allocated
counters will not be freed.

The current error flow code was releasing the IB connection object and
calling iscsi_destroy_endpoint() directly without going through the
reference counting mechanism introduced in commit 39ff05d ("IB/iser:
Enhance disconnection logic for multi-pathing"). This resulted in a
double free of the iscsi endpoint object, which causes a kernel NULL
pointer dereference. Fix that by plugging into the IB conn reference
counting correctly.

Enable IB ULPs to use a larger portion of the device EQs (which map to
IRQs). The mlx4_ib driver follows the mlx4_core framework of the EQs
to be divided among the device ports. In this scheme, for each IB
port, the number of allocated EQs follows the number of cores, subject
to other system constraints, such as number available MSI-X vectors.

When the thin pool target clears the discard_passdown parameter
internally, it incorrectly changes the table line reported to userspace.
This breaks dumb string comparisons on these table lines in generic
userspace device-mapper library code and leads to tables being reloaded
repeatedly when nothing is actually meant to be changing.
This patch corrects this by no longer changing the table line when
discard passdown was disabled.
We can still tell when discard passdown is overridden by looking for the
message "Discard unsupported by data device (sdX): Disabling discard passdown."
This automatic detection is also moved from the 'load' to the 'resume'
so that it is re-evaluated should the properties of underlying devices
change.

Using kfifos for ID management was limiting the number of QPs and
preventing NP384 MPI jobs. So replace it with a simple bitmap
allocator.
Remove IDs from the IDR tables before deallocating them. This bug was
causing the BUG_ON() in insert_handle() to fire because the ID was
getting reused before being removed from the IDR table.

Add module option db_fc_threshold which is the count of active QPs
that trigger automatic db flow control mode. Automatically transition
to/from flow control mode when the active qp count crosses
db_fc_theshold.

Get FULL/EMPTY/DROP events from LLD. On FULL event, disable normal
user mode DB rings.
Add modify_qp semantics to allow user processes to call into the
kernel to ring doobells without overflowing.
Add DB Full/Empty/Drop stats.
Mark queues when created indicating the doorbell state.
If we're in the middle of db overflow avoidance, then newly created
queues should start out in this mode.
Bump the C4IW_UVERBS_ABI_VERSION to 2 so the user mode library can
know if the driver supports the kernel mode db ringing.

db_lock, a spin lock disabling BH too, used by the recovery thread and
the ring_tx_db() paths to allow db drop recovery.
Clean up initial DB avoidance code.
Add read_eq_indices() - this allows the LLD to use the PCIe mw to
efficiently read hw eq contexts.
Add cxgb4_sync_txq_pidx() - called by iw_cxgb4 to sync up the sw/hw
pidx value.
Add flush_eq_cache() and cxgb4_flush_eq_cache(). This allows iw_cxgb4
to flush the sge eq context cache before beginning db drop recovery.
Add module parameter, dbfoifo_int_thresh, to allow tuning the db
interrupt threshold value.
Add dbfifo_int_thresh to cxgb4_lld_info so iw_cxgb4 knows the threshold.
Add module parameter, dbfoifo_drain_delay, to allow tuning the amount
of time delay between DB FULL and EMPTY upcalls to iw_cxgb4.

Add platform-specific callback functions for interrupts. This is
needed to do a single read-clear of the CAUSE register and then call
out to platform specific functions for DB threshold interrupts and DB
drop interrupts.
Add t4_mem_win_read_len() - mem-window reads for arbitrary lengths.
This is used to read the CIDX/PIDX values from EC contexts during DB
drop recovery.
Add t4_fwaddrspace_write() - sends addrspace write cmds to the fw.
Needed to flush the sge eq context cache.

As iwlwifi use fat skbs, it should not pull too much data in skb->head,
and particularly no tcp data payload, or splice() is slower, and TCP
coalescing is disabled. Copying payload to userland also involves at
least two copies (part from header, part from fragment)
Each layer will pull its header from the fragment as needed.
(on 64bit arches, skb_tailroom(skb) at this point is 192 bytes)
With this patch applied, I have a major reduction of collapsed/pruned
TCP packets, a nice increase of TCPRcvCoalesce counter, and overall
better Internet User experience.
Small packets are still using a fragless skb, so that page can be reused
by the driver.

Palmas is a PMIC from Texas Instruments and this is the MFD part of the
driver for this chip. The PMIC has SMPS and LDO regulators, a general
purpose ADC, GPIO, USB OTG mode detection, watchdog and RTC features.

regulator_map_voltage_iterate() is for drivers implementing set_voltage_sel()
and list_voltage() to use it as their map_voltage() operation.
In this case, regulator_map_voltage_iterate() happen to be doing the same thing
as ab3100_get_best_voltage_index() function. So we can use it to replace
ab3100_get_best_voltage_index() function.

Here we setup the ab8500 regulator driver for DT. We first do
this in the normal way, by providing a match structure during
initialisation, but then we provide information so that
whilst probing we can use existing data structures to do DT
look-ups.

ab8500's probe() function is becoming quite large, so in the lead
up to Device Tree enablement which will fork the thread of execution
this patch splits it into 3 main areas; basic error checking will
remain in probe(), but regulator register initialisation and regulator
registration have been moved to their own functions which will
be called in sequence by probe() and the DT equivalent.

The reset date of the ST Micro version of PL031 is 2000-01-01. The
correct weekday for 2000-01-01 is saturday, but pl031 is initialized to
sunday. This may lead to alarm malfunction, so configure the correct
wday if RTC_DR indicates reset.

Pull two networking fixes from David S. Miller:
1) Thanks to Willy Tarreau and Eric Dumazet, we've unlocked a bug that's
been present in do_tcp_sendpages() since that function was written in
2002.
When we block to wait for memory we have to unconditionally try and
push out pending TCP data, otherwise we can block for an unreasonably
long amount of time.
2) Fix deadlock in e1000, fixes kernel bugzilla 43132

Commit 1cc0c998fd ("ACPI: Fix D3hot v D3cold confusion") introduced a
bug in __acpi_bus_set_power() and changed the behavior of
acpi_pci_set_power_state() in such a way that it generally doesn't work
as expected if PCI_D3hot is passed to it as the second argument.
First off, if ACPI_STATE_D3 (equal to ACPI_STATE_D3_COLD) is passed to
__acpi_bus_set_power() and the explicit_set flag is set for the D3cold
state, the function will try to execute AML method called "_PS4", which
doesn't exist.
Fix this by adding a check to ensure that the name of the AML method
to execute for transitions to ACPI_STATE_D3_COLD is correct in
__acpi_bus_set_power(). Also make sure that the explicit_set flag
for ACPI_STATE_D3_COLD will be set if _PS3 is present and modify
acpi_power_transition() to avoid accessing power resources for
ACPI_STATE_D3_COLD, because they don't exist.
Second, if PCI_D3hot is passed to acpi_pci_set_power_state() as the
target state, the function will request a transition to
ACPI_STATE_D3_HOT instead of ACPI_STATE_D3. However,
ACPI_STATE_D3_HOT is now only marked as supported if the _PR3 AML
method is defined for the given device, which is rare. This causes
problems to happen on systems where devices were successfully put
into ACPI D3 by pci_set_power_state(PCI_D3hot) which doesn't work
now. In particular, some unused graphics adapters are not turned
off as a result.
To fix this issue restore the old behavior of
acpi_pci_set_power_state(), which is to request a transition to
ACPI_STATE_D3 (equal to ACPI_STATE_D3_COLD) if either PCI_D3hot or
PCI_D3cold is passed to it as the argument.
This approach is not ideal, because generally power should not
be removed from devices if PCI_D3hot is the target power state,
but since this behavior is relied on, we have no choice but to
restore it at the moment and spend more time on designing a
better solution in the future.

The use of the inw/outw functions by the cs89x0 platform driver
results in NULL pointer references on ARM platforms and
platforms that do not provide ISA-style programmed I/O accessors.
Using inw/outw also accesses the wrong address space on platforms
that have a PCI I/O space that is not identity-mapped into the
physical address space.

While the RNC is suspended for I/O cleanup, the remote device can be
stopped and the RNC setup for destruction. These changes accomodate that

The RNC state machine would incorrectly transition from
SCI_RNC_AWAIT_SUSPENSION directly to SCI_RNC_INVALIDATING when a destruct
request was made. This would skip the increment of the suspension count
and the abort of pending TCs (although the invalidating state would at
least cleanup outstanding TCs).
Instead, the RNC will transition to SCI_RNC_SUSPENDED and then start the
destruction process.

In the link fail path, set IDEV_GONE for every device on the domain
when the last link in the port fails.
In the abort path functions like isci_reset_device, make sure that
there has not already been a detected domain failure with the device
by checking IDEV_GONE, before performing any kind of hard reset, SMP
phy control, or TMF operation.
The check for IDEV_GONE makes sure that the device in the abort path
really has control of the port with which it is associated. This
prevents starting hard resets at incorrect times and scheduling
unnecessary LUN resets for SATA devices.

There is an apparent HW lockup caused when the PE is disabled while there
is an outstanding TC in progress. This change puts the link into OOB to
force the TC to end before the PE is disabled.

This change adds timeouts to the RNC suspension wait. It makes the
suspend and resume timeouts the same.
The previous resume timeout of 5 ms was too short, and timeouts were
seen in resumptions of devices in the abort task/LUN reset path - which
would receive an RNC resumed message within a tenth of a second later.

Instead of immediately transitioning to the SCI_RNC_AWAIT_SUSPENSION
state, handle the SCI_RNC_RESUMING suspend transition from the
SCI_RNC_READY state like the SCI_RNC_INVALIDATING --> SCI_RNC_POSTING
transitions do now, by setting the destination state for the entry
into the READY state.

Completion of I/Os during the one of the abort path interface calls
from libsas can drive remote device state changes and the resumption
of the device RNC. This is a problem when the abort path is
attempting to cleanup outstanding I/O at the same time - the resumption
can prevent the termination from occuring correctly.

In the libsas error path, SATA disks require extra handling in
libata to recover operation. However, libsas expects to be able
to immediately recover all outstanding I/O once the error handler
escalation stops. This patch fixes the condition where the libata
error handler is scheduled for operation but libsas has already
deleted the outstanding sas_tasks.

The RNC can be any of the states in the loop from suspended to
ready when the API "suspend" or "resume" are called. This change
adds destination states parameters that control the suspension /
resumption action of the RNC statemachine for those transition states.

This commit changes the means by which outstanding I/Os are handled
for cleanup.
The likelihood is that this commit will be broken into smaller pieces,
however that will be a later revision. Among the changes:
- All completion structures have been removed from the tmf and
abort paths.
- Now using one completed I/O list, with the I/O completed in host bit being
used to select error or normal callback paths.

Add comprehensive decode for all TC completions that generate RNC
suspensions.
Note that this commit also removes unconditional resumptions of ATAPI
devices when in the SCI_STP_DEV_ATAPI_ERROR state, and STP devices
when in the SCI_STP_DEV_IDLE state. This is because the SCI_STP_DEV_IDLE
and SCI_STP_DEV_ATAPI state entry functions manage the RNC resumption.
