Saurav Kashyap ad537689c3 [SCSI] qla2xxx: Remove qla2x00_wait_for_loop_ready function.
This function can wait for 5min under certain scenarios. One of them is when
the port is down from switch and bus reset is issued. The bus reset used to
wait for 5 minutes for the loop and upper layer callers used to hang and give
stack trace because of getting stuck for 120 sec. It is legacy code that was
used when the driver used to do queuing of the commands.

Finn Thain 30c0527d15 m68k/mac: cleanup mac_irq_pending
mac_irq_pending() has only one caller (mac_esp.c). Nothing tests for Baboon, PSC or OSS pending interrupts. Until that need arises, let's keep it simple and remove all the unused abstraction. Replace it with a routine to check for SCSI DRQ.

Matthew Garrett e5a44df85e [SCSI] hpsa: Disable ASPM
The Windows driver .inf disables ASPM on hpsa devices. Do the same because the
selection of a non default ASPM policy can cause the device to hang.

Vasily Averin cf16123c9c [SCSI] aacraid: controller hangs if kernel uses non-default ASPM policy
Aacraid controller can hang on some nodes if kernel uses non-default
(powersave) ASPM policy.  Controller hangs shortly after successful load and
hardware detection. Scsi error handler detects this hang and tries to restart
hardware but it does not help.

Initially it was noticed on RHEL6-based openVZ kernel after backporting
aacraid driver from mainline (RHEL6 kernel with original driver works well)

This issue happens because default ASPM policy was changed in Red Hat
kernels. Therefore guys from Red Hat have noticed this problem long time ago:
on Fedora 12

In RHEL6 kernel this issue was fixed, ASPM was disabled in aacraid driver. In
kernel changelog I've found that seems it was done by Matthew Garrett: -
[scsi] aacraid: Disable ASPM by default (Matthew Garrett) [599735]

However seems this patch was not submitted to mainline. I've reproduced this
issue on vanilla 3.1.0 kernel booted with "pcie_aspm.policy=powersave" option,
So I believe it makes sense to do it now.

Hannes Reinecke 745718132c [SCSI] Silencing 'killing requests for dead queue'
When we tear down a device we try to flush all outstanding
commands in scsi_free_queue(). However the check in
scsi_request_fn() is imperfect as it only signals that
we _might start_ aborting commands, not that we've actually
aborted some.
So move the printk inside the scsi_kill_request function,
this will also give us a hint about which commands are aborted.

Anton Blanchard f7c9c6bb14 [SCSI] Fix block queue and elevator memory leak in scsi_alloc_sdev
When looking at memory consumption issues I noticed quite a
lot of memory in the kmalloc-2048 bucket:

Over 15MB. slub debug shows that cfq is responsible for almost
all of it:

In scsi_alloc_sdev we do scsi_alloc_queue but if slave_alloc
fails we don't free it with scsi_free_queue.

The patch below fixes the issue:

Moger, Babu ab72002bc5 [SCSI] scsi_dh_alua: Fix the time inteval for alua rtpg commands
This patch corrects the retry interval for alua rtpg command. Purpose was to retry the commands in seconds.
But that was not happening. Reason is msleep takes argument in milliseconds.

Also added minor text after successful attach.

Robin H. Johnson 99a700bcc7 [SCSI] mv_sas: OCZ RevoDrive3 & zDrive R4 support
In the OCZ RevoDrive3/zDrive R4 series, the "OCZ SuperScale Storage
Controller" with "Virtualized Controller Architecture 2.0" really seems
to be a Marvell 88SE9485 part, with OCZ firmware/BIOS.

Developed and tested on OCZ RevoDrive3 120GB [PCI 1b85:1021]

Should work on:
- OCZ RevoDrive3 (2x SandForce 2281)
- OCZ RevoDrive3 X2 (4x SandForce 2281)
- OCZ zDrive R4 CM84 (4x SandForce 2281)
- OCZ zDrive R4 CM88 (8x SandForce 2281)
- OCZ zDrive R4 RM84 (4x SandForce 2582)
- OCZ zDrive R4 RM88 (8x SandForce 2582)

All of this because a friend recently bought a OCZ RevoDrive3 and was
bitten by the lack of Linux support.

Notes from testing:
-------------------
- SMART works.
- VPD Device Identification is "OCZ-REVODRIVE3"
- Thin provisioning/TRIM seems to be implemented as WRITE SAME UNMAP,
  with deterministic (non-zero) read after TRIM, but I'm not sure if it
  works 100% in my testing.
- Some of the tuning in the firmware seems to ensure much better
  performance when in a RAID0 setup than using the two devices
  seperately.

I have not tested booting from the SSD, because all of this was
developed and tested remotely from the actual hardware.

Adds more cases to do flogi retry, now also retry
on getting bad response due to either no ELS response
or flogi response payload length not large enough.
In those cases flogi was not retried and that
was leaving lport offline.

Dan Williams 52d7463433 [SCSI] isci: revert bcn filtering
The initial bcn filtering implementation was validated on a kernel
baseline that predated the switch to new libata error handling.  Also,
prior to that conversion we borrowed the mvsas MVS_DEV_EH approach to
prevent the unwanted extra ap->ops->phy_reset(ap) that occurred in the
ata_bus_probe() path.

After the conversion to new libata eh resets at discovery are more
frequent and get filtered prematurely by IDEV_EH.  The result is that
our bcn filtering has been blocked from running and at discovery and it
appears to stall discovery completion to the point of triggering hung
task timeouts.  So, revert the implementation for now.  When it returns
it will go into libsas proper.

The domain rediscovery that takes place due to ->lldd_I_T_nexus_reset()
events should now be properly waited for by the ata_port_wait_eh() call
in ata_port_probe().  So the hard coded delay in the isci
->lldd_I_T_nexus_reset() and other libsas drivers should help debounce
the libsas thread from seeing temporary device removals.

Jeff Skirvin 8e35a1398c [SCSI] isci: Fix hard reset timeout conditions.
A hard reset can timeout before or after the last phy in the
port goes away.  If after, then notify the OS that the last
phy has failed.

The recovery for the failed hard reset has been removed.
This recovery code was unecessary in that the link would
recover from the failure normally by a new link reset sequence
or hotplug of the remote device.

Jeff Skirvin 98145cb722 [SCSI] isci: Fix task management for SMP, SATA and on dev remove.
libsas uses the LLDD abort task interface to handle I/O timeouts
in the SATA/STP and SMP discovery paths, so this change will terminate
STP/SMP requests. Also, if the device is gone, the lldd will prevent
libsas from further escalations in the error handler.

Scott Teel b7ec021fe6 [SCSI] hpsa: fix potential array overflow in hpsa_update_scsi_devices
The currentsd[] array in hpsa_update_scsi_devices had room for
256 devices.  The code was iterating over however many physical
and logical devices plus an additional number of possible external
MSA2XXX controllers, which together could potentially exceed 256.

We increased the size of the currentsd array to 1024 + 1024 + 32 + 1
elements to reflect a reasonable maximum possible number of devices
which might be encountered.  We also don't just walk off the end
of the array if the array controller reports more devices than we
are prepared to handle, we just ignore the excessive devices.

Andrew Vasquez d424754cbe [SCSI] qla2xxx: Correct inadvertent clearing of RISC_INTR status.
During heavy I/O (CPU-affinity mode enabled) and CLI/Agent
interactions, the driver would report periodic mailbox command
timeout statuses.  Within the CPU-affinity ISR handler, the
driver should check the 'disable-msix-handshake' flag in deciding
whether or not to clear HCCRX_CLR_RISC_INT.  The mode is not
specific to a dedicated queue, instead, applies to the current
'ha' context.

Bhanu Prakash Gollapudi 32c3045450 [SCSI] bnx2fc: Handle SRR LS_ACC drop scenario
When SRR LS_ACC is dropped, the driver was not issuing ABTS for SRR when it
times out. Since the target received SRR, it was able to send the XFER_RDY and
the the original IO request completed successfully. In this condition ABTS was
not sent during bnx2fc_srr_compl(). Fix this by first checking for ELS timeout
and issue ABTS before checking if original IO request is complete.

If the IO and the corresponding ABTS are not responded by a target, cleanup the
IO and issue explicit logout when ulp timer expires while waiting for ABTS to
complete. Wait for the session to be ready before returning to the SCSI layer.
If the session is not ready let the SCSI-ml escalate the error recovery.

Petr Uzel c68bf8eeaa [SCSI] st: fix race in st_scsi_execute_end
The call to complete() in st_scsi_execute_end() wakes up sleeping thread
in write_behind_check(), which frees the st_request, thus invalidating
the pointer to the associated bio structure, which is then passed to the
blk_rq_unmap_user(). Fix by storing pointer to bio structure into
temporary local variable.

This bug is present since at least linux-2.6.32.

which only returns active volumes. In order to get info on inactive volumes,

MPI2_RAID_PGAD_FORM_GET_NEXT_CONFIGNUM, and traverse each config till the
iocstatus is MPI2_IOCSTATUS_CONFIG_INVALID_PAGE returned.
Added a change in the driver to remove the instance of
sas_device object when the driver returns "1" from the slave_configure callback.
Also fixed code to report the hot spares to the operating system with a /dev/sg
assigned.

This is due to the slave_configuration routine is getting called when
host reset is active, and config page reads are failing, and driver
attempts to added device with stale config data.

To fix the issue, added error checking in slave_configure to check
for configuration pages failing, and return "1" so the device  is
not configured.  The config pages are failing if raid volume is
configured while issuing a host reset, thus driver is reading stale
data and proceeding to attempt to add.  The fix is to return error
so the volume is not configured.

a SYNC_CACHE request to driver while the IO queues are locked due to host reset.

immediately when the driver receives the reply message, instead of waiting
on the hot plug worker threads.

active, thus preventing drives from getting properly deleted.

the devices that failed to be detected are not added to the list.

New feature Fast Load Support.

(1)Asynchronous SCSI scanning: This will allow the drivers to scan
for devices in parallel while other device drivers are loading at
the same time. This will improve the amount of time it takes for the
OS to load.

(2) Reporting Devices while port enable is active: This feature will
allow devices to be reported to OS immediately while port enable is
active. The previous implementation waits for port enable to complete,
and then report devices. This feature is only enabled on IT firmware
configurations when there are no boot device configured in BIOS Configuration
Utility, else the driver will wait till port enable completes reporting
devices. For IR firmware, this feature is turned off. This feature is to
address large SAS topologies (>100 drives) when the boot OS is using onboard
SATA device, in other words, the boot devices is not
connected to our controller.

(3) Scanning for devices after diagnostic reset completes: A new routine
_scsih_scan_start is added. This will scan the expander pages, IR pages,
and sas device pages, then reporting new devices to SCSI Mid layer. It
seems the driver is not supporting adding devices while diagnostic reset
is active. Apparently this is due to the sanity checks on
ioc->shost_recovery flag throughout the context of kernel work thread FIFO,
and the mpt2sas_fw_work.

Mike Christie 590134fa78 [SCSI] qla4xxx: export address/port of connection (fix udev disk names)
Udev uses the connection's persistent address/port in the
/dev/disk/by-path name. qla4xxx is not exporting this value
so its iscsi disk names have been lacking the iscsi info.
In the old driver we did not have this information but
we do now and can fix this by just setting the flags to tell
libiscsi/scsi_transport_iscsi to export this info in sysfs.

Brian King 4c647e909f [SCSI] ipr: Fix BUG on adapter dump timeout
If an adapter dump times out, the ipr driver will abort the
dump and proceed to reset and recover the adapter. When an
adapter dump completes, the work thread which is reading the
adapter dump will initiate an adapter reset to recover the
adapter. However, when the adapter dump gets aborted, the
work thread should not initiate an adapter reset, since an
adapter reset is already in progress. This fixes a case of
calling pci_block_user_cfg_access overlapped, which results
in a BUG.

adam radford f575c5d3eb [SCSI] megaraid_sas: Fix instance access in megasas_reset_timer
The following patch for megaraid_sas will fix a potential bad pointer access
in megasas_reset_timer(), when a MegaRAID 9265/9285 or 9360/9380 gets a
timeout.  megasas_build_io_fusion() sets SCp.ptr to be a struct
megasas_cmd_fusion *, but then megasas_reset_timer() was casting SCp.ptr to be
a struct megasas_cmd *, then trying to access cmd->instance, which is invalid.

Just loading instance from scmd->device->host->hostdata in
megasas_reset_timer() fixes the issue.

Mike Christie 2d63673b4d [SCSI] iscsi class: fix vlan configuration
Userspace was sending the priority/id part of the vlan tag
and sysfs was displaying the id in the vlan file. This
renames the vlan sysfs file to vlan_id to reflect that it
was showing the id and to match the vlan_priority file.
This also adds a ISCSI_NET_PARAM_VLAN_TAG iscsi nl command
to relfect that we are sending down the vlan/priority
part of the tag.

Dan Carpenter dc55b8274d [SCSI] aacraid: use lower snprintf() limit
This is just a cleanup, to silence static checker warnings.  It
doesn't change how the code works.

buf[] can either be BUF_SIZE if this is called from sysfs, or it can
be 16 if it's called from aac_get_adapter_info() via
aac_get_serial_number().  We use the smaller limit here.

sizeof(dev->supplement_adapter_info.MfgPcbaSerialNo) is 12 so there
is actually no chance of hitting either limit.

  statement to the end of the routine after setting return value
- moved the msleep(10) to the beginning of the wait loop for checking the
  SLIPort_Status register
- Added the code to follow the existing wait for SLIPort_Status register RDY,
  ERR, and RN bits to be set by the port before proceeding to perform PCI
  function reset.
- Do not override ulpCt_h and ulpCt_l for SLI 4 ports.
- For vport delete, call lpfc_nlp_put when the vport's vpi state is not
  marked with VPI_REGISTERED.
- Added missed fields into the driver's Controller Attributes Structure
- Changed ringing EQ/CQ/RQ doorbell register to be dependent on the size
  of the queue.
- Return -EACCES in issue_reset if cfg_enable_hba_reset is zero.
- Added new logging flag LOG_FCP_UNDER 0x00040000 to qualify underrun logging.
- Add a check in the fabric name display routine to display 0 if the port

During link down, iscsid tries to do re-login to failed session. In case of
link down-up-down, LLD was sending connection login failed event to iscsid,

Lalit Chandivade 166dd20d79 [SCSI] qla4xxx: Free Device Database (DDB) reserved by FW
Firmware reserves DDBs if there are entries in the FLASH.
So there are no free DDBs left when a iSCSI login is initiated
by user space tool like iscsiadm.
Since now login is not controlled by firmware, LLD need to free
up the DDBs after firmware init. This will ensure free DDBs are
available for iSCSI logins using iscsiadm.

The driver failed to export primary boot target if secondary target did not
exist in the FLASH. If boot targets are not valid then driver assumed 0 and
1 as default boot targets. Since these target did not exist in flash, the

Lalit Chandivade 4549415af6 [SCSI] qla4xxx: Do not add duplicate CHAP entry in FLASH
QLogic applications store the CHAP information in FLASH. During login,
authentication information is provided using an index into the CHAP region.

In order to support QLogic applications along with iscsiadm, updated the
LLD to not add duplicate CHAP entries in the CHAP region and preserve the
existing CHAP info in the CHAP region in FLASH.
This allows QLogic applications to pre-write the CHAP entries in the
CHAP region.

With iscsiadm, when the CHAP authentication information is sent to the LLD, the
LLD searches for the entry in CHAP region in FLASH, if exists then do not add.
If CHAP entry does not exist then add the CHAP entry in the CHAP region.

Jayamohan Kallickal e528586079 [SCSI] be2iscsi: Fix for kdump failure
This patch fixes the Kdump failure reported by Redhat wich was caused by
be2iscsi resetting the Chip when be2iscsi detects it is in crashdump mode. If
be2net was loaded before be2iscsi then this leaves be2net driver hanging on to
resources that the chip stopped recognising after the reset.

This patch replaces  chip reset with Function reset so that only the
particular function and not the whole chip is affected

Meelis Roos 528c04ee6d [SCSI] qlogicpti: fix timeout
qlogicpti times out for some tape library operations (like mtx
inventory). It seems SCSI command timeout is hardcoded into the driver.
Fix it by propagating the timeout from scsi request to the controller as
suggested by James Bottomley.

Tested on Sun Ultra 1 with Sun StorEdge L8 Autoloader.

Wayne Boyer 14ed9cc7e7 [SCSI] ipr: Add support to flash FPGA and flash back DRAM images
The write buffer command is used to download and burn new IOA FW images.
The same interface can now be used to flash FPGA and flash back DRAM images.
To download and flash the new images takes more than 15 minutes, so increase
the write buffer command timeout to 30 minutes.

The FPGA and flash back DRAM images don't have the same card_type as the IOA FW
image. So, remove the sanity checking from the driver.  The adapter has sanity
checking and will only accept a valid image.

Dan Williams 1a34c06401 [SCSI] libsas: fix port->dev_list locking
port->dev_list maintains a list of devices attached to a given sas root port.
It needs to be mutated under a lock as contexts outside of the
single-threaded-libsas-workqueue access the list via sas_find_dev_by_rphy().
Fixup locations where the list was being mutated without a lock.

This is a follow-up to commit 5911e963 "[SCSI] libsas: remove expander
from dev list on error", where Luben noted [1]:

    > 2/ We have unlocked list manipulations in sas_ex_discover_end_dev(),
    > sas_unregister_common_dev(), and sas_ex_discover_end_dev()

    Yes, I can see that and that is very unfortunate.

Dan Williams ac013ed1cb [SCSI] isci: export phy events via ->lldd_control_phy()
Allow the sas-transport-class to update events for local phys via a new
PHY_FUNC_GET_EVENTS command to ->lldd_control_phy().  Fixup drivers that
are not prepared for new enum phy_func values, and unify
->lldd_control_phy() error codes.

These are the SAS defined phy events that are reported in a
smp-report-phy-error-log command:

 * /sys/class/sas_phy/<phyX>/running_disparity_error_count

Dan Williams b50102d3e9 [SCSI] isci: atapi support
Based on original implementation from Jiangbi Liu and Maciej Trela.

ATAPI transfers happen in two-to-three stages.  The two stage atapi
commands are those that include a dma data transfer.  The data transfer
portion of these operations is handled by the hardware packet-dma
acceleration.  The three-stage commands do not have a data transfer and
are handled without hardware assistance in raw frame mode.

stage1: transmit host-to-device fis to notify the device of an incoming
atapi cdb.  Upon reception of the pio-setup-fis repost the task_context
to perform the dma transfer of the cdb+data (go to stage3), or repost
the task_context to transmit the cdb as a raw frame (go to stage 2).

stage2: wait for hardware notification of the cdb transmission and then
go to stage 3.

stage3: wait for the arrival of the terminating device-to-host fis and
terminate the command.

To keep the implementation simple we only support ATAPI packet-dma
protocol (for commands with data) to avoid needing to handle the data
transfer manually (like we do for SATA-PIO).  This may affect
compatibility for a small number of devices (see
ATA_HORKAGE_ATAPI_MOD16_DMA).

If the data-transfer underruns, or encounters an error the
device-to-host fis is expected to arrive in the unsolicited frame queue
to pass to libata for disposition.  However, in the DONE_UNEXP_FIS (data
underrun) case it appears we need to craft a response.  In the
DONE_REG_ERR case we do receive the UF and propagate it to libsas.

Mark Salyzyn 6fbc769219 [SCSI] pm8001: missing break statements
Code Inspection: found two missing break directives. First one will
result in not retrying an a task that report
IO_OPEN_CNX_ERROR_HW_RESOURCE_BUSY, the second will result in cosmetic
debug printk conflicting statement stutter. Because checkpatch.pl came
up with a warning regarding unnecessary space before a newline on one of
the fragments associated with the diff context, I took the liberty of
fixing all the cases of this issue in the pair of files touched by this
defect. These cosmetic changes hide the break changes :-(

To help focus, break changes are in pm8001_hwi.c fragment line 1649 for
the IO_OPEN_CNX_ERROR_HW_RESOURCE_BUSY case statement and pm8001_sas.c
line 1000 deals with the conflicting debug print stutter.

Vasu Dev ed26cfece6 [SCSI] libfc: cache align struct fc_fcp_pkt fields
Re-arrange its fields to avoid padding and have better
cacheline alignments.

Removed not used start_time, end_time and last_pkt_time
fields.

This all reduced this struct size to 448 from 480 and
that also reduced one cacheline on x86_64 beside
eliminating 8 pads. However kept logical fields together.

The queue-depth for libsas-attached devices initializes to 32 and can
only be increased manually via sysfs to a max of 64, while mpt2sas
attached devices initialize to 254 and dynamically float via the
midlayer ->change_queue_depth interface.

No performance regression was observed with this change on the isci
driver.

Currently mvsas and pm8001 have custom ->slave_alloc implementations to
achieve this.  Uplevel it for all libsas drivers as isci encounters problems
with atapi devices when scanning past lun0.

Just do what Darrick suggested [1], and limit the scan for ata devices.

Roland Dreier 9bfacd01dc [SCSI] qla2xxx: Fix crash in qla2x00_abort_all_cmds() on unload
I hit a crash in qla2x00_abort_all_cmds() if the qla2xxx module is
unloaded right after it is loaded.  I debugged this down to the abort
handling improperly treating a command of type SRB_ADISC_CMD as if it
had a bsg_job to complete when that command actually uses the iocb_cmd
part of the union.  (I guess to hit this one has to unload the module
while the async FC initialization is still in progress)

It seems we should only look for a bsg_job if type is SRB_ELS_CMD_RPT,
SRB_ELS_CMD_HST or SRB_CT_CMD, so switch the test to make that explicit.

Following reports on the list, it looks like the 3e-9xxx driver will leak dma
mappings every time we get a transient queueing error back from the card.
This is because it maps the sg list in the routine that sends the command, but
doesn't unmap again in the transient failure path (even though the command is
sent back to the block layer).  Fix by unmapping before returning the status.

The root cause was an EEH error, which sent us down the offload_close path in
the cxgb3 driver, which in turn sets cdev->l2opt to NULL, without regard for
upper layer driver (like the cxgbi drivers) which might have execution contexts
in the middle of its use. The result is the oops above, when t3_l2t_get attempts
to dereference L2DATA(cdev)->nentries in arp_hash right after the EEH error handler sets it to NULL.

The fix is to prevent the setting of the NULL pointer until after there are no
further users of it.  The t3cdev->l2opt pointer is now converted to be an rcu
pointer and the L2DATA macro is now called under the protection of the
rcu_read_lock().  When the EEH error path:
t3_adapter_error->offload_close->cxgb3_offload_deactivate
Is exectured, setting of that l2opt pointer to NULL, is now gated on an rcu
quiescence point, preventing, allowing L2DATA callers to safely check for a NULL
pointer without concern that the underlying data will be freeded before the
pointer is dereferenced.

This has been tested by the reporter and shown to fix the reproted oops

Brian King 41e9a69641 [SCSI] ipr: Stop reading adapter dump prematurely
When the ipr driver decides to dump the adapter, it changes the
sdt_state to GET_DUMP, then prepares the adapter so that the dump
can be read. However, if the ipr worker thread wakes up for some
reason before the driver has put the adapter in a state where it
can succesfully dump the adapter, the driver will start dumping
the adapter too early, which can potentially trigger a BUG check
in the pci config blocking API. Fix this by adding a new
sdt_state to differentiate between the ipr driver wanting to dump
the adapter in the near future and wanting to dump the adapter now.

Support added for controllers capable of multi reply queues.

The following are the modifications to the driver to support NUMA.

1) Create the new structure adapter_reply_queue to contain the reply queue
   info for every msix vector.  This object will contain a
   reply_post_host_index, reply_post_free for each instance, msix_index, among
   other parameters.  We will track all the reply queues on a link list called
   ioc->reply_queue_list. Each reply queue is aligned with each IRQ, and is
   passed to the interrupt via the bus_id parameter.

(2) The driver will figure out the msix_vector_count from the PCIe MSIX
    capabilities register instead of the IOC Facts->MaxMSIxVectors. This is
    because the firmware is not filling in this field until the driver has
    already registered MSIX support.

(3) If the ioc_facts reports that the controller is MSIX compatible in the
    capabilities, then the driver will request for multiple irqs.  This count
    is calculated based on the minimum between the online cpus available and
    the ioc->msix_vector_count.  This count is reported to firmware in the
    ioc_init request.

(4) New routines were added _base_free_irq and _base_request_irq, so
    registering and freeing msix vectors were done thru simple function API.

(5) The new routine _base_assign_reply_queues was added to align the msix
    indexes across cpus. This will initialize the array called
    ioc->cpu_msix_table.  This array is looked up on every MPI request so the
    MSIxIndex is set appropriately.

(6) A new shost sysfs attribute was added to report the reply_queue_count.

(7) User needs to set the affinity cpu mask, so the interrupts occur on the
    same cpu that sent the original request.

Dan Williams ad4f4c1de8 [SCSI] isci: initial sgpio write support
Basic support to initialize the gpio unit, accept an incomming
SAS_GPIO_REG_TX_GP bitstream, and translate it to the ODx.n fields in
the hardware registers.  If register indexes outside the supported range
are specified in the SMP frame we simply accept the write and return how
many registers (SFF-8485) were written (libsas reports this as residue
in the request).

Dan Williams 8ec6552f4a [SCSI] libsas: sgpio write support
Add SFF-8485 v0.7 / SAS-1 smp-write-gpio register support to libsas.
Defer SAS-2 support unless/until it defines an sgpio interface.

Minimum implementation needed to get the lights blinking.
try_test_sas_gpio_gp_bit() provides a common method to parse the
incoming write data (raw bitstream), and the to_sas_gpio_gp_bit() helper
routine can be used as a basis for the set/clear operations for the
'read' implementation.  Host implementations parse as many bits
(ODx.[012]) as are locally supported and report the number of registers
successfully written.  If the submitted data overruns the internal
number of registers available report the write as a success with the
number of bytes remaining reported in ->resid_len.

Example (assuming an active backplane) set the "identify" pattern for
the first 21 devices:

smp_write_gpio --count=2 --data=92,49,24,92,24,92,49,24 -t 4 --index=1 /dev/bsg/sas_hostX

Mark Salyzyn 24926dadc4 [SCSI] libsas: fix failure to revalidate domain for anything but the first expander child.
In an enclosure model where there are chaining expanders to a large body
of storage, it was discovered that libsas, responding to a broadcast
event change, would only revalidate the domain of first child expander
in the list.

The issue is that the pointer value to the discovered source device was
used to break out of the loop, rather than the content of the pointer.

This still remains non-compliant as the revalidate domain code is
supposed to loop through all child expanders, and not stop at the first
one it finds that reports a change count. However, the design of this
routine does not allow multiple device discoveries and that would be a
more complicated set of patches reserved for another day. We are fixing
the glaring bug rather than refactoring the code.

started. Because of this driver can send discovery solicitation on a stale

Jiri Pirko 4bc71cb983 net: consolidate and fix ethtool_ops->get_settings calling
This patch does several things:
- introduces __ethtool_get_settings which is called from ethtool code and
  from drivers as well. Put ASSERT_RTNL there.
- dev_ethtool_get_settings() is replaced by __ethtool_get_settings()
- changes calling in drivers so rtnl locking is respected. In
  iboe_get_rate was previously ->get_settings() called unlocked. This
  fixes it. Also prb_calc_retire_blk_tmo() in af_packet.c had the same
  problem. Also fixed by calling __dev_get_by_index() instead of
  dev_get_by_index() and holding rtnl_lock for both calls.
- introduces rtnl_lock in bnx2fc_vport_create() and fcoe_vport_create()
  so bnx2fc_if_create() and fcoe_if_create() are called locked as they
  are from other places.
- use __ethtool_get_settings() in bonding code
