Bjorn Helgaas fb127cb9de PCI: collapse pcibios_resource_to_bus
Everybody uses the generic pcibios_resource_to_bus() supplied by the core
now, so remove the ARCH_HAS_GENERIC_PCI_OFFSETS used during conversion.

Alex Chiang a7db504052 PCI: remove pcibios_scan_all_fns()
This was #define'd as 0 on all platforms, so let's get rid of it.

This change makes pci_scan_slot() slightly easier to read.

Fernando Luis Vázquez Cao 977badb2f3 PCI: remove pci_dac_dma_... APIs on mn10300
It seems that mn10300 made it upstream after Jan Beulich's pci_dac_dma_*
cleanup work and still defines pci_dac_dma_supported(). This API is not
required by the PCI subsystem anymore, so remove it.
