Here is a patch for ehca to use proper flag, ie. GFP_ATOMIC
resp. GFP_KERNEL, when calling get_zeroed_page() to prevent "Bug:
scheduling while atomic...". This error does not cause a kernel panic
but makes ipoib un-usable afterwards. It is reproducible on
2.6.20-rc4 if one does ifconfig down during a flood ping test. I have
not observed this error in earlier releases incl. 2.6.20-rc1.
This error occurs when a qp event/irq is received and ehca event
handler allocates a control block/page to obtain HCA error data block.
Use of GFP_ATOMIC when in interrupt context prevents this issue.

According to the Tavor and Arbel programmer's reference manuals, the
number of bytes transferred is not provided in the byte_cnt field of
the CQ entry for atomic operation completions. For atomic operations,
the number of bytes transferred is always 8 (when the status is
"success"), and this constant value should always be used by the
driver in the ib_wc entry returned, rather than using the CQE.

There's a problem with how rdma cm events are reported to userspace
that can lead to application crashes.
When a new connection request arrives, a context for the connection is
allocated in the kernel. The connection event is then reported to
userspace. The userspace library retrieves the event and allocates
its own context for the connection. The userspace context is
associated with the kernel's context when accepting. This allows the
kernel to give userspace context with other events.
A problem occurs if a second event for the same connection occurs
before the user has had a chance to call accept. The userspace
context has not yet been set, which causes the librdmacm to crash.
(This has been seen when the app takes too long to call accept,
resulting in the remote side timing out and rejecting the connection)
Fix this by ignoring events for new connections until userspace has
set their context. This can only happen if an error occurs on a new
connection before the user accepts it. This is okay, since the accept
will just fail later.

iSER limits the number of outstanding PDUs to send. When this threshold
is reached, it should return an error code (-ENOBUFS) instead of setting
the suspend_tx bit (which should be used only by libiscsi).

mthca_table_find() will return the wrong address when the table entry
being searched for is exactly at the beginning of a sglist entry
(other than the first), because it uses >= when it should use >.
Example: assume we have 2 entries in scatterlist, 4K each, offset is
4K. The current code will return first entry + 4K when we really want
the second entry.
In particular this means mapping an FMR on a memfree HCA may end up
writing the page table into the wrong place, leading to memory
corruption and also causing the HCA to use an incorrect address
translation table.

struct srp_device.fmr_page_mask was unsigned long, which means that
the top part of addresses above 4G was being chopped off on 32-bit
architectures. Of course nothing good happens when data from SRP
targets is DMAed to the wrong place.
Fix this by changing fmr_page_mask to u64, to match the addresses
actually used by IB devices.

Move the initialization of ipoib_neigh's skb_queue into
ipoib_neigh_alloc(), since commit 2745b5b7 ("IPoIB: Fix skb leak when
freeing neighbour") will make iterate over the skb_queue to free any
packets left over when freeing the ipoib_neigh structure.
This fixes a crash when freeing ipoib_neigh structures allocated in
ipoib_mcast_send(), which otherwise don't have their skb_queue
initialized.

During connection establishment, the passive side of a connection can
receive messages from the active side before the connection event has
been delivered to the user. Allow the passive side to send messages
in response to received data before the event is delivered. To handle
the case where the connection messages are lost, a new rdma_notify()
function is added that users may invoke to force a connection into the
established state.

Connection information was never given to the recipient of a
connection request or reply message. Only the event was delivered.
Report the connection data with the event to allows user to
reject the connection based on the requested parameters, or adjust
their resources to match the request.

Commit 51f65ebc ("IB/ipath - program intconfig register using new HT
irq hook"), which fixed interrupts for HyperTransport HCAs, broke PCI
Express HCAs, because for those HCAs, the driver uses the value of
pdev->irq before pci_enable_msi() and ends up getting a totally bogus
IRQ number. Fix this by using the value of pdev->irq after
pci_enable_msi().

ib_flush_fmr_pool() stashes away the request generation number
properly, but then goes ahead and rereads it every time it tests
whether the flush generation number has caught up. This means that
there is a theoretical possibility of livelock, if the request
generation number keeps getting bumped and the flush generation number
never catches up. The fix is simple: use the request generation
number read at the beginning of the function.
Also, atomic_inc() followed by atomic_read() can be replaced with
atomic_int_return(). There's no real requirement for atomicity here
but we might as well shrink the code.
This bug was discovered using David Binderman's list of "set but never
used" warnings from icc.

This facility provides three entry points:
ilog2() Log base 2 of unsigned long
ilog2_u32() Log base 2 of u32
ilog2_u64() Log base 2 of u64
These facilities can either be used inside functions on dynamic data:
int do_something(long q)

}
Or can be used to statically initialise global variables with constant values:
unsigned n = ilog2(27);
When performing static initialisation, the compiler will report "error:
initializer element is not constant" if asked to take a log of zero or of
something not reducible to a constant. They treat negative numbers as
unsigned.
When not dealing with a constant, they fall back to using fls() which permits
them to use arch-specific log calculation instructions - such as BSR on
x86/x86_64 or SCAN on FRV - if available.

ib_ucm_cleanup_events() holds file_mutex while calling ib_destroy_cm_id().
This can deadlock since ib_destroy_cm_id() flushes event handlers, and
ib_ucm_event_handler() needs file_mutex, too. Therefore, drop the
file_mutex during the call to ib_destroy_cm_id().

The ib_cm_establish() function is replaced with a more generic
ib_cm_notify(). This routine is used to notify the CM that failover
has occurred, so that future CM messages (LAP, DREQ) reach the remote
CM. (Currently, we continue to use the original path) This bumps the
userspace CM ABI.
New alternate path information is captured when a LAP message is sent
or received. This allows QP attributes to be initialized for the user
when a new path is loaded after failover occurs.

Fix following problems in process_req() relating to cancellation:
- Function is wrongly doing another addr_remote() when cancelled,
which is not required.
- Make failure reporting immediate by using time_after_eq().
- On cancellation, -ETIMEDOUT was returned to the callback routine
instead of the more appropriate -ECANCELLED (users getting notified
may want to print/return this status, eg ucma_event_handler).

It is possible to swap the CQs used for send_cq and recv_cq when
creating two different QPs. If these two QPs are then destroyed at
the same time, an AB-BA deadlock can occur because the CQ locks are
taken our of order. Fix this by always taking CQ locks in a fixed
order.

When initializing an mthca SRQ, the log_srq_size field should be the
log of the number of SRQ WQEs, not the log of the number of bytes in
the SRQ.
This affects only mthca drivers for memfree HCAs which set the initial
srq wqe counter (in the SW2HW transition) to a non-zero value.

This is a patch for ehca to fix a bug in prepare_sqe_to_rts(), which
used WQE address to iterate pending work requests. This might cause
an access violation since the queue pages can not be assumed to follow
each other consecutively. Thus, this patch introduces a few queue
functions to determine WQE offset based on its address and uses WQE
offset to iterate the pending work requests.

In iwcm_deref_id(), the comment says : "If the last reference is being
removed and iw_destroy_cm_id is waiting, wake up the waiting
thread". The second part of the comment, "and iw_destroy_cm_id is
waiting," is wrong, since this function either wakes the waiter
already waiting in iwcm_deref_id, or enables it (so that when
wait_for_completion() is performed later, it will immediately return).

If we get IW_CM_EVENT_CONNECT_REQUEST message and encounter an error
(not in the LISTEN state, cannot create an id, cannot alloc
work_entry, etc), then the memory allocated by cm_event_handler() in
the event->private_data gets leaked. Since cm_work_handler has already
put the event on the work_free_list, this allocated memory is
leaked. High backlog value can allow DoS attacks.

Commit b3b30f5e ("IB/mthca: Recover from catastrophic errors")
introduced some section mismatch breakage, because the error recovery
code tears down and reinitializes the device, which calls into lots of
code originally marked __devinit and __devexit from regular .text.
Fix this by getting rid of these now-incorrect section markers.

Set the Scsi_Host's max_cmd_len from 12 (default) to 16 for
SRP. Otherwise scsi_dispatch_cmd() won't pass down certain commands

In queue_req(), use time_after_eq() instead of time_after()
for following reasons :
- Improves insert time if multiple entries with same time are
present.
- set_timeout need not be called if entry with same time
is added to the list (and that happens to be the entry
with the smallest time), saving atomic/locking operations.
- Earlier entries with same time are deleted first (fifo).

IPoIB assumes that high (reserved) octet in the hardware address is 0,
and copies it into the QPN. This violates RFC 4391 (which requires
that the high 8 bits are ignored on receive), and will result in an
invalid QPN being used when interoperating with IPoIB connected mode.

When ib_cancel_mad() is called, it puts the canceled send on a list
and schedules a "flushed" callback from process context. However,
this leaves a window where a receive completion could be processed
before the send is fully flushed.
This is fine, except that ib_find_send_mad() will find the MAD and
return it to the receive processing, which results in the sender
getting both a successful receive and a "flushed" send completion for
the same request. Understandably, this confuses the sender, which is
expecting only one of these two callbacks, and leads to grief such as
a use-after-free in IPoIB.
Fix this by changing ib_find_send_mad() to return a send struct only
if the status is still successful (and not "flushed"). The search of
the send_list already had this check, so this patch just adds the same
check to the search of the wait_list.

Change ehca's Kconfig to activates scaling code as default. After
several measurements we saw that this feature prevents dropped packets
(UD) in stress situation. Thus, enabling it helps to improve ehca's
bandwidth through IPoIB.

Assure 4K alignment for firmware control blocks in 64K page mode,
because kzalloc()'s result address might not be 4K aligned if 64K
pages are enabled. Thus, we introduce wrappers called
ehca_{alloc,free}_fw_ctrlblock(), which use a slab cache for objects
with 4K length and 4K alignment in order to alloc/free firmware
control blocks in 64K page mode. In 4K page mode those wrappers just
are defines of get_zeroed_page() and free_page().

Several fields in an incoming MAD extended info header were passed
into the MAD_IFC firmware command at incorrect offsets (mostly off by
4 bytes). As the result, the HCA will fail to generate traps in which
this info is needed (e.g. traps which include the GRH of the incoming
packet), in violation of the IB spec.

When a connection is started (a new connection or a recovered one),
iSER should prepare its resources for full-featured mode and only then
notify the iSCSI layer that it is ready to start queueing commands.

We discovered a problem when running IPoIB applications on multiple
CPUs on an Altix system. Many messages such as:
ib_mthca 0002:01:00.0: SQ 000014 full (19941644 head, 19941707 tail, 64 max, 0 nreq)
appear in syslog, and the driver wedges up.
Apparently this is because writes to the doorbells from different CPUs
reach the device out of order. The following patch adds mmiowb() calls
after doorbell rings to ensure the doorbell writes are ordered.

Don't attempt to set up the diagpkt device in the module init code.
Instead, wait until a piece of hardware is initialized. Fixes a
problem when loading the ib_ipath module when no InfiniPath hardware
is present: modprobe would go into the D state and stay there.

Since pr_debug() has changed from a macro to an inline function when
DEBUG is not defined, its arguments now need to be defined even when
debugging is off. Therefore to_event_str() and to_qp_state_str() need
to be moved out of #ifdef DEBUG. The compiler will throw the
definitions away if DEBUG is not defined, but it needs to be able to
see that the functions exist.

Currently a DREP is only sent in response to a DREQ if a connection
has been found matching the DREQ, and it is in the proper state. Once
a DREP is sent, the local connection moves into timewait. Duplicate
DREQs received while in this state result in re-sending the DREP.
However, it's likely that the local connection will enter and exit
timewait before the remote side times out a lost DREP and resends a DREQ.
To handle this, we send a DREP in response to a DREQ, even if a local
connection is not found. This avoids maintaining disconnected
id's in timewait states for excessively long times, just to handle a
lost DREP.

If the ib_cm module is unloaded while id's are still in timewait, the
CM will destroy the work queue used to process timewait. Once the
id's exit timewait, their timers will fire, leading to a crash trying
to access the destroyed work queue.
We need to track id's that are in timewait, and cancel their deferred
work on module unload.

Enable multiple concurrent connections to the same SRP target:
1) Use port GUID instead of node GUID in the initiator port
identifier. This allows connections to be made from multiple HCA
ports at the same time.
2) Let the user specify the identifier extention when adding the
device. This allows userspace to make multiple connections even
from the same port, if it wants too.
Without this, only one connection can be made from any given HCA, even
if it has multiple ports, because we don't use multi-channel mode, so
targets will only allow one connection from a given initiator port ID.

scsi_host_alloc() already allocates with kzalloc(), so the struct Scsi_Host
is zeroed out, including the private data portion. Remove the redundant
memset that zeros this out again in the SRP initiator.

The AMSO driver was not thread-safe in the post WR code and had
code that would sleep if the WR post FIFO was full. Since these
functions can be called on interrupt level I changed the sleep to a
udelay.

Maintain a per-CPU global "struct pt_regs *" variable which can be used instead
of passing regs around manually through all ~1800 interrupt handlers in the
Linux kernel.
The regs pointer is used in few places, but it potentially costs both stack
space and code to pass it around. On the FRV arch, removing the regs parameter
from all the genirq function results in a 20% speed up of the IRQ exit path
(ie: from leaving timer_interrupt() to leaving do_IRQ()).
Where appropriate, an arch may override the generic storage facility and do
something different with the variable. On FRV, for instance, the address is
maintained in GR28 at all times inside the kernel as part of general exception
handling.
Having looked over the code, it appears that the parameter may be handed down
through up to twenty or so layers of functions. Consider a USB character
device attached to a USB hub, attached to a USB controller that posts its
interrupts through a cascaded auxiliary interrupt controller. A character
device driver may want to pass regs to the sysrq handler through the input
layer which adds another few layers of parameter passing.
I've build this code with allyesconfig for x86_64 and i386. I've runtested the
main part of the code on FRV and i386, though I can't test most of the drivers.
I've also done partial conversion for powerpc and MIPS - these at least compile
with minimal configurations.
This will affect all archs. Mostly the changes should be relatively easy.
Take do_IRQ(), store the regs pointer at the beginning, saving the old one:
struct pt_regs *old_regs = set_irq_regs(regs);
And put the old one back at the end:
set_irq_regs(old_regs);
Don't pass regs through to generic_handle_irq() or __do_IRQ().
In timer_interrupt(), this sort of change will be necessary:
- update_process_times(user_mode(regs));
- profile_tick(CPU_PROFILING, regs);
+ update_process_times(user_mode(get_irq_regs()));
+ profile_tick(CPU_PROFILING);
I'd like to move update_process_times()'s use of get_irq_regs() into itself,
except that i386, alone of the archs, uses something other than user_mode().
Some notes on the interrupt handling in the drivers:
(*) input_dev() is now gone entirely. The regs pointer is no longer stored in
the input_dev struct.
(*) finish_unlinks() in drivers/usb/host/ohci-q.c needs checking. It does
something different depending on whether it's been supplied with a regs
pointer or not.
(*) Various IRQ handler function pointers have been moved to type
irq_handler_t.

Move the call to ib_register_device() later, since a device should not
be registered until it is completely read to be used. This fixes
crashes that occur if an upper-layer driver such as IPoIB is loaded
before the ehca module.

The race is as follows:
A process : cma_process_remove() calls cma_remove_id_dev(),
which sets id state to CMA_DEVICE_REMOVAL and
calls wait_event(dev_remove).
B process : cma_req_handler() had incremented dev_remove,
and calls cma_acquire_ib_dev() and on failure
calls cma_release_remove(), which does a
wake_up of cma_process_remove(). Then
cma_req_handler() calls rdma_destroy_id();
A Process : cma_remove_id_dev() gets woken and checks the
state of id, and since it is still (wrongly)
CMA_DEVICE_REMOVAL, it calls notify_user(id)
and if that fails, the caller - cma_process_remove()
calls rdma_destroy_id(id). Two processes can
call rdma_destroy_id(), resulting in one
de-referencing kfreed id_priv.
Fix is for process B to set CMA_DESTROYING in cma_req_handler()
so that process A will return instead of doing a rdma_destroy_id().

In some places, particularly drivers and __init code, the init utsns is the
appropriate one to use. This patch replaces those with a the init_utsname
helper.
Changes: Removed several uses of init_utsname(). Hope I picked all the
right ones in net/ipv4/ipconfig.c. These are now changed to
utsname() (the per-process namespace utsname) in the previous
patch (2/7)

The resize CQ function changes the memory used to store the queue.
Other routines need to honor the lock before accessing the pointer
to the queue and verify that the head and tail are in range.

This change moves around port assignment so that it happens before any
memory is allocated. This allows memory to be allocated on an appropriate
CPU, which improves performance for users of /dev/ipath.

The EEPROM is read via programmable I/O pins. When the driver
is compiled -Os, the CPU can speculatively read the I/O
value before it is valid. This patch fixes the problem.

We can sometimes trigger parity errors due to processor speculative
reads to our write-combined memory (mostly seen on Woodcrest). Add a
stats counter for these.
Factored out the sendbuffererror buffer cancellation code so it can be
used in the new handling; suppress likely subsequent error messages if
within two jiffies of the cancellation.
Also restore 2 dropped TXE lines on hwe_bitsextant noticed while
debugging.

Fixed mismatch in linkstate/trainingstate shifts and masks in the

iSER uses the DMA mapping api to map the page holding the
SCSI command data to the HCA DMA address space. When the
command data is not aligned for RDMA, the data is copied
to/from an allocated buffer which in turn is used for
executing this command. The pages associated with the
command must be unmapped before being touched.

iSER uses a data transaction object (struct iser_dto) as part
of its IB data descriptors (struct iser_desc) management.
It also uses a hierarchy of connection structures pointing to
each other. A DTO may exist even after the iscsi_iser connection
pointed by it is destroyed (eg one that is bound to a post
receive buffer which was flushed by the IB HW). Hence DTOs need
point to the lowest connection, which is struct iser_conn.

This eliminates the i_blksize field from struct inode. Filesystems that want
to provide a per-inode st_blksize can do so by providing their own getattr
routine instead of using the generic_fillattr() function.
Note that some filesystems were providing pretty much random (and incorrect)
values for i_blksize.

The following patches reduce the size of the VFS inode structure by 28 bytes
on a UP x86. (It would be more on an x86_64 system). This is a 10% reduction
in the inode size on a UP kernel that is configured in a production mode
(i.e., with no spinlock or other debugging functions enabled; if you want to
save memory taken up by in-core inodes, the first thing you should do is
disable the debugging options; they are responsible for a huge amount of bloat
in the VFS inode structure).
This patch:
The filesystem or device-specific pointer in the inode is inside a union,
which is pretty pointless given that all 30+ users of this field have been
using the void pointer. Get rid of the union and rename it to i_private, with
a comment to explain who is allowed to use the void pointer. This is just a
cleanup, but it allows us to reuse the union 'u' for something something where
the union will actually be used.

0x08 is the HT capability, while PCI_CAP_ID_HT_IRQCONF would be
the subtype 0x80 that mpic_scan_ht_pic() uses.
Rename PCI_CAP_ID_HT_IRQCONF into PCI_CAP_ID_HT.
And by the way, use it in the ipath driver instead of defining its
own HT_CAPABILITY_ID.

When ipoib_ib_dev_flush() is called because of a port event, the
driver needs to rejoin all multicast groups, since the flush will call
ipoib_mcast_dev_flush() (via ipoib_ib_dev_down()). Otherwise no
(non-broadcast) multicast groups will be rejoined until the networking
core calls ->set_multicast_list again, and so multicast reception will
be broken for potentially a long time.

RFC 4391 ("Transmission of IP over InfiniBand (IPoIB)") says:
If the IB multicast group does not already exist, one must be
created first with the IPoIB link MTU. The MGID MUST use the same
P_Key, Q_Key, SL, MTU, and HopLimit as those used in the
broadcast-GID. The rest of attributes SHOULD follow the values used
in the broadcast-GID as well.
However, the current IPoIB driver is only setting the attributes
required by the InfiniBand spec to create a multicast group, so in
particular the MTU and HopLimit are not being set. Add these
attributes when creating MCGs, and also set the Rate attribute, since
IPoIB pays attention to that attribute as well.

If a QP has separate send and receive CQs, then the send CQ will never
have receive completions from that QP in it. So when cleaning the
send CQ, there's no need to pass in an SRQ pointer, even if the QP is
attached to an SRQ.

Trigger device remove and then add when a catastrophic error is
detected in hardware. This, in turn, will cause a device reset, which
we hope will recover from the catastrophic condition.
Since this might interefere with debugging the root cause, add a
module option to suppress this behaviour.

Fast Memory Registration (fmr) is used to register for rdma an sg whose
elements are not linearly sequential after dma mapping.
The IB verbs layer provides an "all dma memory MR (memory region)" which
can be used for RDMA-ing a dma linearly sequential buffer.
Change the code to use the dma mr instead of doing fmr when dma mapping
produces a single dma entry sg.

As iser is able to use at most one rdma operation for the
execution of a scsi command, and registration of the sg
associated with scsi command has its restrictions, the code
checks if an sg is "aligned for rdma".
Alignment for rdma is measured in "fmr page" units whose
possible resolutions are different between HCAs and can be
smaller, equal or bigger to the system page size.
When the system page size is bigger than 4KB (eg the default
with ia64 kernels) there a bigger chance that an sg would be
aligned for rdma if the fmr page size is 4KB.
Change the code to create FMR whose pages are of size 4KB
and to take that into account when processing the sg.

Currently, the data length of a command coming down from scsi-ml
is limited only by the size of its sg list (sg_tablesize). The
max data length may be different for different page size values.
By setting max_sectors, we limit the data length to
max_sectors*512 bytes.

dma mapping may include a "compaction" of the sg associated with scsi command.
Hence, the size of the maximal prefix of the SG which is aligned for rdma must be
compared against the length of the dma mapped sg (mem->dma_nents) and not against
the size of it before it was mapped (mem->size).

Closes a window where address resolution can attach an rdma_cm_id to a
device during destruction of the rdma_cm_id. This can result in the
rdma_cm_id remaining in the device list after its memory has been

Add an iWARP Connection Manager (CM), which abstracts connection
management for iWARP devices (RNICs). It is a logical instance of the
xx_cm where xx is the transport type (ib or iw). The symbols exported
are used by the transport independent rdma_cm module, and are
available also for transport dependent ULPs.

The implementation assumes that any RMPP request that requires a
response uses DS RMPP. Based on the RMPP start-up scenarios defined
by the spec, this should be a valid assumption. That is, there is no
start-up scenario defined where an RMPP request is followed by a
non-RMPP response. By having this assumption we avoid any API
changes.
In order for a node that supports DS RMPP to communicate with one that
does not, RMPP responses assume a new window size of 1 if a DS ACK has
not been received. (By DS ACK, I'm referring to the turn-around ACK
after the final ACK of the request.) This is a slight spec deviation,
but is necessary to allow communication with nodes that do not
generate the DS ACK. It also handles the case when a response is sent
after the request state has been discarded.

Set the reject code properly when rejecting a request that contains an
invalid GID. A suitable GID is returned by the IB CM in the

Ordering of writethrough store buffers needs to be forced, and we need
to use ifdef to get writethrough behavior to InfiniPath buffers, because
there is no generic way to specify that at this time (similar to code
in char/drm/drm_vm.c and block/z2ram.c).

Improve performance of userspace post receive, post SRQ receive, and
poll CQ operations for ipath by allowing userspace to directly mmap()
receive queues and completion queues. This eliminates the copying
between userspace and the kernel in the data path.

Add a ib_uverbs_resize_cq_resp.driver_data field so that low-level

Lockdep warns when userspace creates a QP that uses different CQs for
send completions and receive completions, because both CQs are locked
and their mutexes belong to the same lock class. However, we know
that the mutexes are distinct and the nesting is safe (there is no
possibility of AB-BA deadlock because the mutexes are locked with
down_read()), so annotate the situation with SINGLE_DEPTH_NESTING to
get rid of the lockdep warning.

The SM LID used to send traps to is incorrectly set to port LID. This
is a regression from 2.6.17 -- after a PortInfo MAD is received, no
traps are sent to the SM LID. The traps go to the loopback interface
instead, and are dropped there. The SM LID should be taken from the
sm_lid of the PortInfo response.

3 seems like a low number of IB Communication Manager retries to set;
we see connections failing under stress, and in any case 3 just looks
like an arbitrary number. 15 is the max value allowed by the
InfiniBand spec.

When a send-only multicast group join fails, mcast->query must be set
to NULL. Otherwise, IPoIB will never retry the join and the multicast
group will never be reachable.

If there is a problem in the connection, the SCSI mid-layer will
eventually call srp_reset_host(), which will call srp_reconnect(), so
we do not need to schedule a call to srp_reconnect_work() from
srp_completion().
Removing this prevents srp_reset_host() from failing if a reconnect
scheduled from srp_completion() is already in progress, which in turn
was causing crashes as both SCSI midlayer and srp_reconnect() were
cancelling commands.

iSCSI RFC states that the first burst length must be smaller than the
max burst length. We currently assume targets will be good, but that may
not be the case, so this patch adds a check.
This patch also moves the unsol data out offset to the lib so the LLDs
do not have to track it.

It is supposed to be OK to call mthca_create_ah() and mthca_destroy_ah()
from any context. However, for mem-full HCAs, these functions use the
mthca_alloc() and mthca_free() bitmap helpers, and those helpers use
non-IRQ-safe spin_lock() internally. Lockdep correctly warns that
this could lead to a deadlock. Fix this by changing mthca_alloc() and
mthca_free() to use spin_lock_irqsave().

Leave all SRQ methods out of the device's uverbs_cmd_mask if the
device doesn't have SRQ support (because of ancient firmware) so that
we don't allow userspace to call the driver's create_srq method. This
fixes a userspace-triggerable oops caused by ib_uverbs_create_srq()
following the device's ->create_srq function pointer, which will be
NULL if the device doesn't support SRQs.

After commit 12bbb2b7be, when SM LID
change or LID change MAD also has a client reregistration bit set,
only CLIENT_REREGISTER event is generated.
As a result, the sa_query module and the cache module don't update the
port information, and ULPs (e.g. IPoIB) stop working. This is the
regression we observe as compared to 2.6.17.
Rather than generate multiple events (which would have negative
performance impact), let us simply let cache and SA query respond to
reregister event in the same way as to LID and SM change events.

When destroying a QP, mthca locks both the QP's send CQ and receive
CQ. However, the following scenario is perfectly valid:
QP_a: send_cq == CQ_x, recv_cq == CQ_y
QP_b: send_cq == CQ_y, recv_cq == CQ_x
The old mthca code simply locked send_cq and then recv_cq, which in
this case could lead to an AB-BA deadlock if QP_a and QP_b were
destroyed simultaneously.
We can fix this by changing the locking code to lock the CQ with the
lower CQ number first, which will create a consistent lock ordering.
Also, the second CQ is locked with spin_lock_nested() to tell lockdep
that we know what we're doing with the lock nesting.
This bug was found by lockdep.

Wait until all users have closed their device context before allowing
device unregistration to complete. This prevents a crash caused by
referring to stale data structures.
A better solution would be to have a way to revoke contexts rather
than waiting for userspace to close the context, but that's a much
bigger change that will have to wait. For now let's at least avoid
the crash.

Data corruption has been seen with Mellanox SRP targets when FMRs
create a memory region with I/O virtual address != 0. Add a
workaround that disables FMR merging for Mellanox targets (OUI 0002c9).

When we enter recovery and flush the running commands
we cannot freee the connection before flushing the commands.
Some commands may have a reference to the connection
that needs to be released before. iscsi_stop was forcing
the term and suspend too early and was causing a oops
in iser, so this patch removes those callbacks all together
and allows the LLD to handle that detail.

The neighbour ha field may get updated without destroying the
neighbour. In this case, the ha field gets out of sync with the
address handle stored in ipoib_neigh->ah, with the result that
the ah field would point to an incorrect path, resulting in all
packets being lost.

Validate MADs sent by userspace clients for spec compliance with
C13-18.1.1 (prevent duplicate requests and responses sent on the
same port). Without this, RMPP transactions get aborted because
of duplicate packets.
This patch is similar to that provided by Jack Morgenstein.

ipath_skip_sge() doesn't exactly duplicate the side effects of
ipath_copy_sge() if num_sge > 1 since it doesn't decrement ss->num_sge.
This could result in the sg_list being accessed out of bounds.
Since ipath_skip_sge() is almost always called with num_sge == 1,
the original "optimization" is almost never used.

I am still working on a proposal to remove the phys_to_virt() calls
in the ib_ipath driver. In the mean time, this patch allows SRP
to work by fixing the R_Key check and conversion from IB address
to kernel virtual address. It also returns the correct page size
for FMRs.

Lockdep warns because uverbs is trying to take uobj->mutex when it
already holds that lock. This is because there are really multiple
types of uobjs even though all of their locks are initialized in
common code.

ib_fmr_pool_map_phys gets the virtual address by pointer but never writes
there, and users (e.g. srp) seem to assume this and ignore the value
returned. This patch cleans up the API to get the VA by value, and updates
all users.

The device address contains unsigned character arrays, which contain raw GID
addresses. The GIDs may not be naturally aligned, so do not cast them to

If a user of the IB CM returns -ENOMEM from their connection callback, simply
drop the incoming REQ - do not attempt to send a reject. This should allow
the sender to retry the request.

mthca: initialize send and receive queue locks separately
lockdep identifies a lock by the call site of its initialization. By
initializing the send and receive queue locks in mthca_wq_init() we confuse
lockdep. It warns that that the ordered acquiry of both locks in
mthca_modify_qp() is recursive acquiry of one lock:

We can't tell for sure if any packets are in the infinipath receive buffer
when we shut down a chip port. Normally this is taken care of by orderly
shutdown, but when processes are terminated, or sending process has a bug, we
can continue to receive packets. So rather than writing zero to the address
registers for the closing port, we point it at a dummy memory.

We do a few more explicit checks for specific models, and now also support the
old PathScale serial number style, or new QLogic style.
This is backwards compatible with previous versions of software and hardware.
That is, older software will see a plausible serial number and correct GUID
when used with a new board, while newer software will correctly handle an
older board.

This patch separates QP state used for sending and receiving RC packets so the
processing in the receive interrupt handler can be done mostly without locks
being held. ACK packets are now sent without requiring synchronization with
the send tasklet.

Do an extra check to see if in-memory tail changed while processing packets,
and if so, going back through the loop again (but only once per call to
ipath_kreceive()). In practice, this seems to be enough to guarantee that if
we crossed the clearing of an interrupt at start of ipath_intr with a
scheduled tail register update, that we'll process the "extra" packet that
lost the interrupt because we cleared it just as it was about to arrive.

The problem was that I was updating the head register multiple times in the
rcvhdrq processing loop, and setting the counter on each update. Since that
meant that the tail register was ahead of head for all but the last update, we
would get extra interrupts. The fix was to not write the counter value except
on the last update.
I also changed to update rcvhdrhead and rcvegrindexhead at most every 16
packets, if there were lots of packets in the queue (and of course, on the
last packet, regardless).
I also made some small cleanups while debugging this.
With these changes, xeon/monty typically sees two openib packets per interrupt
on sdp and ipoib, opteron/monty is about 1.25 pkts/intr.
I'm seeing about 3800 Mbit/s monty/xeon, and 5000-5100 opteron/monty with
netperf sdp. Netpipe doesn't show as good as that, peaking at about 4400 on
opteron/monty sdp. Plain ipoib xeon is about 2100+ netperf, opteron 2900+, at
128KB

Also count the number of interrupts where that works (fastrcvint). On any
interrupt where the port0 head and tail registers are not equal, just call the
ipath_kreceive code without reading the interrupt status, thus saving the
approximately 0.25usec processor stall waiting for the read to return. If any
other interrupt bits are set, or head==tail, take the normal path, but that
has been reordered to handle read ahead of pioavail. Also no longer call
ipath_kreceive() from ipath_qcheck(), because that just seems to make things
worse, and isn't really buying us anything, these days.
Also no longer loop in ipath_kreceive(); better to not hold things off too
long (I saw many cases where we would loop 4-8 times, and handle thousands (up
to 3500) in a single call).

Made in-memory rcvhdrq tail update be in dma_alloc'ed memory, not random user
or special kernel (needed for ppc, also "just the right thing to do").
Some cleanups to make unexpected link transitions less likely to produce
complaints about packet errors, and also to not leave SMA packets stuck and
unable to go out.
A few other random debug and comment cleanups.
Always init rcvhdrq head/tail registers to 0, to avoid race conditions (should
have been that way some time ago).

Extend the get_sb() filesystem operation to take an extra argument that
permits the VFS to pass in the target vfsmount that defines the mountpoint.
The filesystem is then required to manually set the superblock and root dentry
pointers. For most filesystems, this should be done with simple_set_mnt()
which will set the superblock pointer and then set the root dentry to the
superblock's s_root (as per the old default behaviour).
The get_sb() op now returns an integer as there's now no need to return the
superblock pointer.
This patch permits a superblock to be implicitly shared amongst several mount
points, such as can be done with NFS to avoid potential inode aliasing. In
such a case, simple_set_mnt() would not be called, and instead the mnt_root
and mnt_sb would be set directly.
The patch also makes the following changes:
(*) the get_sb_*() convenience functions in the core kernel now take a vfsmount
pointer argument and return an integer, so most filesystems have to change
very little.
(*) If one of the convenience function is not used, then get_sb() should
normally call simple_set_mnt() to instantiate the vfsmount. This will
always return 0, and so can be tail-called from get_sb().
(*) generic_shutdown_super() now calls shrink_dcache_sb() to clean up the
dcache upon superblock destruction rather than shrink_dcache_anon().
This is required because the superblock may now have multiple trees that
aren't actually bound to s_root, but that still need to be cleaned up. The
currently called functions assume that the whole tree is rooted at s_root,
and that anonymous dentries are not the roots of trees which results in
dentries being left unculled.
However, with the way NFS superblock sharing are currently set to be
implemented, these assumptions are violated: the root of the filesystem is
simply a dummy dentry and inode (the real inode for '/' may well be
inaccessible), and all the vfsmounts are rooted on anonymous[*] dentries
with child trees.
[*] Anonymous until discovered from another tree.
(*) The documentation has been adjusted, including the additional bit of
changing ext2_* into foo_* in the documentation.

This file contains the processing carried over an SG list associated with
a SCSI command such that it can be registered with the IB verbs. The
registration produces a network virtual address (VA) and a remote access
key (RKEY or STAG in iSER spec notation) which are used by the target for
its RDMA operation.

iSER (iSCSI Extensions for RDMA) transport provider driver for the iSCSI
initiator, whose other parts (under drivers/scsi) are scsi_transport_iscsi
- the transport management module, iscsi_tcp - the TCP transport provider
module and libiscsi - a kernel library (module) implementing functionality
needed by both TCP and iSER transports. iSER is both a provider of the iSCSI
transport api and a SCSI low level driver.
This file contains internal data structures and non static service functions.

In ib_uverbs_cleanup_ucontext(), when iterating through the lists of
objects, there's no reason to do list_del() to remove the objects,
since both the objects and the lists that contain them are about to be
freed anyway. Since list_del() is a moderately big inline function,
getting rid of this extra work saves quite a bit of .text:

In ib_uverbs_post_send(), move the "out:" label after the loop that
frees the list of work requests, since the only place that jumps there
is before any work requests could possibly be added to the list.
This removes a compile warning: "is_ud might be used uninitialized in
this function".

Various drivers use xmit_lock internally to synchronise with their
transmission routines. They do so without setting xmit_lock_owner.
This is fine as long as netpoll is not in use.
With netpoll it is possible for deadlocks to occur if xmit_lock_owner
isn't set. This is because if a printk occurs while xmit_lock is held
and xmit_lock_owner is not set can cause netpoll to attempt to take
xmit_lock recursively.
While it is possible to resolve this by getting netpoll to use
trylock, it is suboptimal because netpoll's sole objective is to
maximise the chance of getting the printk out on the wire. So
delaying or dropping the message is to be avoided as much as possible.
So the only alternative is to always set xmit_lock_owner. The
following patch does this by introducing the netif_tx_lock family of
functions that take care of setting/unsetting xmit_lock_owner.
I renamed xmit_lock to _xmit_lock to indicate that it should not be
used directly. I didn't provide irq versions of the netif_tx_lock
functions since xmit_lock is meant to be a BH-disabling lock.
This is pretty much a straight text substitution except for a small
bug fix in winbond. It currently uses
netif_stop_queue/spin_unlock_wait to stop transmission. This is
unsafe as an IRQ can potentially wake up the queue. So it is safer to
use netif_tx_disable.
The hamradio bits used spin_lock_irq but it is unnecessary as
xmit_lock must never be taken in an IRQ handler.

Currently, all userspace verbs operations that call into the kernel
are serialized by ib_uverbs_idr_mutex. This can be a scalability
issue for some workloads, especially for devices driven by the ipath
driver, which needs to call into the kernel even for datapath
operations.
Fix this by adding reference counts to the userspace objects, and then
converting ib_uverbs_idr_mutex into a spinlock that only protects the
idrs long enough to take a reference on the object being looked up.
Because remove operations may fail, we have to do a slightly funky
two-step deletion, which is described in the comments at the top of
uverbs_cmd.c.
This also still leaves ib_uverbs_idr_lock as a single lock that is
possibly subject to contention. However, the lock hold time will only
be a single idr operation, so multiple threads should still be able to
make progress, even if ib_uverbs_idr_lock is being ping-ponged.
Surprisingly, these changes even shrink the object code:

Documentation/infiniband/core_locking.txt says:
All of the methods in struct ib_device exported by a low-level
driver must be fully reentrant. The low-level driver is required to
perform all synchronization necessary to maintain consistency, even
if multiple function calls using the same object are run
simultaneously.
However, mthca's modify_qp, modify_srq and resize_cq methods are
currently not reentrant. Add a mutex to the QP, SRQ and CQ structures
so that these calls can be properly serialized.

Some error paths after the mthca_alloc_mailbox() call in mthca_modify_qp()
just do a "return -EINVAL" without freeing the mailbox. Convert these

The P_Key is provided into a SIDR REQ in two places, once as a
parameter, and again in the path record. Remove the P_Key as a
parameter and always use the one given in the path record.
This change has no practical effect on ABI functionality.

Misc cleanups in ib_srp:
1) I think that it is more efficient to move the req entries from req_list
to free_list in srp_reconnect_target (rather than rebuild the free_list).
(In any case this code is shorter).
2) This allows us to reuse code in srp_reset_device and srp_reconnect_target
and call a new function srp_reset_req.

There has been a change in the format of port identifiers between
revision 10 of the SRP specification and the current revision 16A.
Revision 10 specifies port identifier format as
lower 8 bytes : GUID upper 8 bytes : Extension
Whereas revision 16A specifies it as
lower 8 bytes : Extension upper 8 bytes : GUID
There are older targets (e.g. SilverStorm Virtual Fibre Channel
Bridge) which conform to revision 10 of the SRP specification.
The I/O class of revision 10 is 0xFF00 and the I/O class of revision
16A is 0x0100.
For supporting older targets, this patch:
1) Adds a new optional target creation parameter "io_class". Default
value of io_class is 0x0100 (i.e. revision 16A)
2) Uses the correct port identifier format for targets with IO class
of 0xFF00 (i.e. conforming to revision 10)

When creating a FMR pool, query the IB device and use the returned
max_map_map_per_fmr attribute as for the max number of FMR remaps. If
the device does not suport querying this attribute, use the original
IB_FMR_MAX_REMAPS (32) default.

Fix misaligned access faults on ia64: never cast a misaligned
neighbour->ha + 4 pointer to union ib_gid type; pass a void * pointer
instead. The memcpy was being optimized to use full word accesses
because the compiler thought that union ib_gid is always aligned.
The cast in IPOIB_GID_ARG is safe, since it is fixed to access each
byte separately.

The comparisons of priv->tx_tail to ah->last_send in ipoib_free_ah()
and ipoib_post_receive() are slightly unsafe, because priv->tx_lock is
not held and hence a stale value of ah->last_send might be used, which
would lead to freeing an AH before the driver was really done with it.
The simple way to fix this is to the optimization of early free from
ipoib_free_ah() and unconditionally queue AHs for reaping, and then
take priv->tx_lock in __ipoib_reap_ah().

Check GID/LID for requester side when searching for request which
matches received response. This is in order to guarantee uniqueness
if the same TID is used when requesting via multiple source LIDs (when
LMC is not zero). Use ports' cached LMC to perform the check.
Further, do not perform LID check for direct-routed packets, since
the permissive LID makes a proper check impossible.

It's perfectly valid for a connection to an SRP target to have a
request limit of 0, so get rid of the message about it, which can spam
kernel logs even with printk_ratelimit(). Keep a count of such events
in a "zero_req_lim" SCSI host attribute instead, so someone who cares
can look at the statistics.

Handle IB_CM_DREQ_ERROR and IB_CM_DREQ_RECEIVED events from the CM,
instead of just printing "Unhandled CM event". In the case of
DREQ_ERROR, just ignore the event -- a TIMEWAIT_EXIT will be generated
also. For DREQ_RECEIVED, send a DREP in response to shut the
connection down cleanly.

Interrupts will always be enabled in srp_remove_one(), so
spin_lock_irq() can be used instead of spin_lock_irqsave().
Also, the loop takes target->scsi_host->host_lock, so target->state
can just be set to SRP_TARGET_REMOVED witout testing the old value.

The SRP driver never sleeps while holding target_mutex, and it's just
used to protect some simple list operations, so hold times will be

The kernel has had wait_for_completion_timeout() for a long time now.
mthca should use it to handle FW commands timing out, instead of
implementing the same thing in a much more complicated way by using
wait_for_completion() along with a timer that does complete().

Create an SRP FMR pool on HCAs that support FMRs, and use FMRs to map
gather/scatter lists that have more than one entry into a single
memory region that appears virtually contiguous to the SRP target
(which is the RDMA initiator).
This patch bails out on FMR mapping for SCSI commands where the
gather/scatter list cannot be mapped into a single FMR because there
are sub-page-sized entries in middle of the list. An unaligned
start or end of the list is OK.

Kernel connection management agent over InfiniBand that connects based
on IP addresses. The agent defines a generic RDMA connection
abstraction to support clients wanting to connect over different RDMA
devices.
The agent also handles RDMA device hotplug events on behalf of clients.

Memfree firmware is in rare cases reporting WQE index == base - 1 in
receive completion with error, instead of (rq size - 1); base is 0 in
mthca. Here is a patch to avoid kernel crash and report a correct WR
id in this case.

mthca does not restore the following PCI-X/PCI Express registers after reset:
PCI-X device: PCI-X command register
PCI-X bridge: upstream and downstream split transaction registers
PCI Express : PCI Express device control and link control registers
This causes instability and/or bad performance on systems where one of
these registers is set to a non-default value by BIOS.

When ipoib_stop() is called it first calls netif_stop_queue() to stop
the kernel from passing more packets to the network driver. However,
the completion handler may call netif_wake_queue() re-enabling packet
transfer.
This might result in leaks (we see AH leaks which we think can be
attributed to this bug) as new packets get posted while the interface
is going down.

If we post a list of length exactly a multiple of 256, nreq in
doorbell gets set to 256 which is wrong: it should be encoded by 0.
This is because we only zero it out on the next WR, which may not be
there. The solution is to ring the doorbell after posting a WQE, not
before posting the next one.
This is the same bug that we just fixed for QPs with non-shared RQ.

The problem was that node A's sending thread, which handles sending RDMA
read response data, would write the trigger word, the last packet would
be sent, node B would send a new RDMA read request, node A's interrupt
handler would initialize s_rdma_sge, then node A's sending thread would
update s_rdma_sge. This didn't happen very often naturally but was more
frequent with 1 byte RDMA reads. Rather than adding more locking or
increasing the QP structure size and copying sge data, I modified the
copy routine to update the pointers before writing the trigger word to
avoid the update race.

The local loopback path for RC can lock the rkey table lock without
blocking interrupts. The receive interrupt path can then call
ipath_rkey_ok() and deadlock. Remove the redundant lock.

If a reconnection attempt fails, then SRP does two scsi_host_put()s.
This is a historical relic from an earlier version of the driver that
took a reference on the scsi_host before trying to reconnect, so get
rid of the extra scsi_host_put().

Sending a DREQ may fail, for example because the remote target has
already broken the connection. If so, then SRP should not wait for
the disconnection to complete, because it never will.

Setting fw_cmd_doorbell allows FW command to be queued using posted
writes instead of requiring polling on a "go" bit, so it should be a
performance boost. However, the option causes problems with at least
some device/firmware combinations, so set the default to 0 until we
understand what's going on better.

Fix race condition during destruction calls to avoid possibility of
accessing object after it has been freed. Instead of waking up a wait
queue directly, which is susceptible to a race where the object is
freed between the reference count going to 0 and the wake_up(), use a
completion to wait in the function doing the freeing.

The ipath driver's table of PCI IDs needs a { 0, } entry at the end.
This makes all of the device aliases visible to userspace so hotplug
loads the module for all supported devices. Without the patch,
modinfo ipath_core only shows:

When deleting a child interface with a non-default P_Key via
/sys/class/net/ibX/delete_child, the interface must be freed with

Fix races in in destroying various objects. If a destroy routine
waits for an object to become free by doing
wait_event(&obj->wait, !atomic_read(&obj->refcount));
/* now clean up and destroy the object */
and another place drops a reference to the object by doing
if (atomic_dec_and_test(&obj->refcount))
wake_up(&obj->wait);
then this is susceptible to a race where the wait_event() and final
freeing of the object occur between the atomic_dec_and_test() and the
wake_up(). And this is a use-after-free, since wake_up() will be
called on part of the already-freed object.
Fix this in mthca by replacing the atomic_t refcounts with plain old
integers protected by a spinlock. This makes it possible to do the
decrement of the reference count and the wake_up() so that it appears
as a single atomic operation to the code waiting on the wait queue.
While touching this code, also simplify mthca_cq_clean(): the CQ being
cleaned cannot go away, because it still has a QP attached to it. So
there's no reason to be paranoid and look up the CQ by number; it's
perfectly safe to use the pointer that the callers already have.

If a SCSI abort completes, or the command completes successfully, then
the driver must remove the command from its queue of pending
commands. Similarly, if a device reset succeeds, then all commands
queued for the given device must be removed from the queue.

GuidInfo records have 8 byte GUIDs in them, so an index should be
multiplied by 8 to get an offset. mthca_query_gid() was incorrectly
multiplying by 16.

Only check that RMPP version is not specified when MAD class does not
support RMPP. Just because a class is allowed to use RMPP doesn't
mean that rmpp_version needs to be set for the MAD agent to
register. Checking this was a recent change which was too pedantic.

The driver allocates SRQ WQEs size with a power of 2 size both for
Tavor and for memfree. For Tavor, however, the hardware only requires
the WQE size to be a multiple of 16, not a power of 2, and the max
number of scatter-gather allowed is reported accordingly by the
firmware (and this is the value currently returned by
ib_query_device() and ibv_query_device()).
If the max number of scatter/gather entries reported by the FW is used
when creating an SRQ, the creation will fail for Tavor, since the
required WQE size will be increased to the next power of 2, which
turns out to be larger than the device permitted max WQE size (which
is not a power of 2).
This patch reduces the reported SRQ max wqe size so that it can be used
successfully in creating an SRQ on Tavor HCAs.

When allocating gid_cache, use kmalloc(sizeof *gid_cache, ...) rather
than kmalloc(sizeof *pkey_cache, ...). It doesn't really matter which
one is used, since the size ends up the same either way, but it's much

The PCI spec recommends against drivers playing with a device's PCI
read burst size, and says that systems software should configure it.
And we actually have users that report that changing it from the
default set by BIOS hurts performance and/or stability for them. On
the other hand, the Mellanox Programmer's Reference Manual recommends
turning it up all the way to the maximum value. Some tests conducted
here in the lab do not show performance improvement from this tuning,
but this might be just me.
As a work-around, make this tuning an option, off by default (safe
value), with an eye towards removing it completely one day if no one
complains.

Make IPoIB's send and receive queue sizes tunable via module
parameters ("send_queue_size" and "recv_queue_size"). This allows the
queue sizes to be enlarged to fix disastrously bad performance on some
platforms and workloads, without bloating memory usage when large
queues aren't needed.

ipoib_mcast_restart_task() might free an mcast object while a join
request is still outstanding, leading to an oops when the query
completes. Fix this by waiting for query to complete, similar to what
ipoib_stop_thread() is doing. The wait for mcast completion code is
consolidated in wait_for_mcast_join().

Push translation of static rate to HCA format into low-level drivers,
where it belongs. For static rate encoding, use encoding of rate
field from IB standard PathRecord, with addition of value 0, for
backwards compatibility with current usage. The changes are:
- Add enum ib_rate to midlayer includes.
- Get rid of static rate translation in IPoIB; just use static rate
directly from Path and MulticastGroup records.
- Update mthca driver to translate absolute static rate into the
format used by hardware. This also fixes mthca's static rate
handling for HCAs that are capable of 4X DDR.

sysfs. Also, don't allow CONFIG_INFINIBAND_MTHCA_DEBUG to be disabled
unless CONFIG_EMBEDDED is selected. We want users (and especially
distros) to have this turned on unless they really need to save space,
because by the time we want debugging output, it's usually too late to
rebuild a kernel.

We traced this back to local_completions unlocking mad_agent_priv->lock
while still keeping a pointer into local_list. A later call to
list_del(&local->completion_list) would then corrupt the list.
To fix this, remove the entry from local_list after looking it up but
before releasing mad_agent_priv->lock, to prevent cancel_mads from
finding and freeing it.

The ipathfs filesystem contains files that are not appropriate for
sysfs, because they contain binary data. The hierarchy is simple; the
top-level directory contains driver-wide attribute files, while numbered
subdirectories contain per-device attribute files.
Our userspace code currently expects this filesystem to be mounted on
/ipathfs, but a final location has not yet been chosen.

EEPROM support, interrupt handling, statistics gathering, and write
combining management for x86_64.
A note regarding i2c: The Atmel EEPROM hardware we use looks like an
i2c device electrically, but is not i2c compliant at all from a
functional perspective. We tried using the kernel's i2c support to
talk to it, but failed.
Normal i2c devices have a single 7-bit or 10-bit i2c address that they
respond to. Valid 7-bit addresses range from 0x03 to 0x77. Addresses
0x00 to 0x02 and 0x78 to 0x7F are special reserved addresses
(e.g. 0x00 is the "general call" address.) The Atmel device, on the
other hand, responds to ALL addresses. It's designed to be the only
device on a given i2c bus. A given i2c device address corresponds to
the memory address within the i2c device itself.
At least one reason why the linux core i2c stuff won't work for this
is that it prohibits access to reserved addresses like 0x00, which are
really valid addresses on the Atmel devices.

The ipath driver is a low-level driver for PathScale InfiniPath host
channel adapters (HCAs) based on the HT-400 and PE-800 chips, including
the InfiniPath HT-460, the small form factor InfiniPath HT-460, the
InfiniPath HT-470 and the Linux Networx LS/X.
The ipath_driver.c file contains much of the low-level device handling
code.

Received responses are currently matched against sent requests based
on TID only. According to the spec, responses should match based on
the combination of TID, management class, and requester LID/GID.
Without the additional qualification, an agent that is responding to
two requests, both of which have the same TID, can match RMPP ACKs
with the incorrect transaction. This problem can occur on the SM node
when responding to SA queries.

ipoib_hard_header() needs to handle the case that daddr is NULL. This
can happen when packets are injected via a raw socket, and IPoIB
shouldn't oops in this case.

The previous patch for Tavor broke MemFree logic.
The driver should perform limit check only for Tavor. For MemFree,
the check is incorrect, since ds (WQE stride) is always a power-of-2
(although the max_desc_size may not be).
In Tavor, however, WQE stride and desc_size are the same, and are not
necessarily power-of-2. The check was really for the WQE stride (and
it Tavor, we use max_desc_size for the stride).

The recently merged patch to create a fake scatterlist for non-SG SCSI
commands had a bug: the driver ended up doing dma_unmap_sg() on a
scatterlist scmnd->request_buffer rather than the fake scatter list it
created. Fix this so that the driver unmaps the same thing it maps.

This patch causes the network interface to respond to P_Key change
events correctly. As a result, you'll see a child interface in the
"RUNNING" state (netif_carrier_on()) only when the corresponding P_Key
is configured by the SM. When SM removes a P_Key, the "RUNNING" state
will be disabled for the corresponding network interface. To
implement this, I added IB_EVENT_PKEY_CHANGE event handling. To
prevent flushing the device before the device is open by the "delay
open" mechanism, I added an additional device flag called
IPOIB_FLAG_INITIALIZED.
This also prevents the child network interface from trying to join to
multicast groups until the PKEY is configured. We used to get error
messages like:
ib0.f2f2: couldn't attach QP to multicast group ff12:401b:f2f2:0:0:0:ffff:ffff
in this case. To fix this, I just check IPOIB_FLAG_OPER_UP flag in
ipoib_set_mcast_list().

If the call to mthca_MODIFY_QP() failed, then mthca_modify_qp() would
still do some things it shouldn't, such as store away attributes for
special QPs. Fix this, and simplify the code, by simply jumping to
the exit path if mthca_MODIFY_QP() fails.

With the current IPoIB driver, the status of network interfaces stays
"RUNNING" even if the link goes down (for example because a cable is
unplugged). Fix this by flushing the IPoIB interface when the link
goes down.

Guarantee the calculated work queue entry size does not exceed the max
allowable WQE size when creating an SRQ. This is a problem with Arbel
in Tavor-compatibility mode because the current WQE size computation
method rounds up to next power of 2.

Since the SCSI midlayer is moving towards entirely getting rid of
commands with use_sg == 0, we should treat this case as an exception.
Therefore, change the IB SRP initiator to create a fake scatterlist
for these commands with sg_init_one(). This simplifies the flow of
DMA mapping and unmapping, since SRP can just use dma_map_sg() and
dma_unmap_sg() unconditionally, rather than having to choose between
the dma_{map,unmap}_sg() and dma_{map,unmap}_single() variants.

struct neigh_ops currently has a destructor field, which no in-kernel
drivers outside of infiniband use. The infiniband/ulp/ipoib in-tree
driver stashes some info in the neighbour structure (the results of
the second-stage lookup from ARP results to real link-level path), and
it uses neigh->ops->destructor to get a callback so it can clean up
this extra info when a neighbour is freed. We've run into problems
with this: since the destructor is in an ops field that is shared
between neighbours that may belong to different net devices, there's
no way to set/clear it safely.
The following patch moves this field to neigh_parms where it can be
safely set, together with its twin neigh_setup. Two additional
patches in the patch series update ipoib to use this new interface.

In neigh_add_path(), the queue of delayed packets can never be full,
because the queue is always freshly created and cannot be found by any
other code path. In fact, the test of the queue length is worse than
useless: if somehow the test ever triggered and path_rec_start() also
failed, then dev_kfree_skb_any() will be called twice on the same skb.
Fix this by deleting the useless test. Pointed out by Michael
S. Tsirkin <mst@mellanox.co.il>.

After removing the port from port_list, ib_mad_port_close flushes
port_priv->wq before destroying the special QPs. This means that a
completion event could arrive, and queue a new work in this work queue
after flush.
This patch also removes an unnecessary flush_workqueue():
destroy_workqueue() already includes a flush.
