This finally makes TX on OFDM rates possible on my dev with PHY rev 4.
We still have lower performance than wl, but at least speeds around 15M
become possible.

This patch fixes tramissing on OFDM rates for PHYs 1 and 2. There is
still something wrong with PHYs 3+. Tests has shown decreasing of
performance on CCK rates by 1-2%, we have to live with that.
Additionaly this noticeably reduces amount of PHY errors. They were
mostly produced by auto-switching to higher rate for better
performanced, which resulted in no transmit at all and PHY errors.

The return value of the tx operation is commonly
misused by drivers, leading to errors. All drivers
will drop frames if they fail to TX the frame, and
they must also properly manage the queues (if they
didn't, mac80211 would already warn).
Removing the ability for drivers to return a BUSY
value also allows significant cleanups of the TX
TX handling code in mac80211.
Note that this also fixes a bug in ath9k_htc, the
old "return -1" there was wrong.

The flag isn't very descriptive -- the intention
is that the driver provides a TSF timestamp at
the beginning of the MPDU -- make that clearer
by renaming the flag to RX_FLAG_MACTIME_MPDU.

Needed to update to apply fixes for which the old branch was too
outdated.
As discussed we do not know band width at core reset time and it is not a good
idea to reset whole just to change band. So just set unconditionally 20 MHz
band width as default during core reset.
As for defines PHY clock changed to band width in specs and it makes much more
sens to call defines by band width which is self-explainable. Updated specs do
not mention 0 value, but comparing to old ones you can notice lineral relation
between PHY clock speed and band width. So it makes sense for 0x0 value to be
10 MHz band width.

"These tables came from reverse engineering the 5.10.56.46 version of the
Broadcom driver. Trace comparisons between b43 and the current Broadcom driver
(5.10.120.0) show byte reversals for the PHY register writes."

Instead of win_req_t, drivers are now requested to fill out
struct pcmcia_device *p_dev->resource[2,3,4,5] for up to four iomem
ranges. After a call to pcmcia_request_window(), the windows found there
are reserved and may be used until pcmcia_release_window() is called.

The default llseek operation is changing from
default_llseek to no_llseek, so all code relying on
the current behaviour needs to make that explicit.
The wireless driver infrastructure and some of the drivers
make use of generated debugfs files, so they cannot
be converted by our script that automatically determines
the right operation.
All these files use debugfs and they typically rely
on simple_read_from_buffer, so the best llseek operation
here is generic_file_llseek.

These changes may be slightly safer in some instances.
There are other kzalloc calls with a multiply, but those
calls are typically "small fixed #" * sizeof(some pointer)"
and those are not converted.

Currently, mac80211 translates the cfg80211
cipher suite selectors into ALG_* values.
That isn't all too useful, and some drivers
benefit from the distinction between WEP40
and WEP104 as well. Therefore, convert it
all to use the cipher suite selectors.

...and unregistration to core shutdown. Previously, the driver
remained registered even when the hardware was shutdown. That
causes the driver to return -ENODEV if the b43 device is IFF_DOWN.
This change causes the driver to disappear in that case, allowing
/dev/hwrng to still function if another hwrng device is available.
