The data read from the SKIPMAP registers is not immediately available
after writing and the driver panics when a packet is enqueued from the
interrupt handler. This patch adds an ndelay(195) before these registers
are read (delay value mentioned in section 15.1.1.3 of the ISP1760 data
sheet).

Roel Kluin pointed out that transfer_buffer_lengths in struct urb was
declared as an 'int'. This patch changes this field to be 'u32' to
prevent any potential negative conversion and comparison errors.
This triggered a few compiler warning messages when these fields were
being used with the min macro, so they have also been fixed up in this
patch.

The code in uhci-q.c doesn't have to use pseudo-negative values. I did
it that way because it was easy and because it would give the expected
output during debugging. But it doesn't have to work that way. Here's

Currently, the driver only supports PCI and PPC_OF but there are
boards like ARM RealView where this is a platform device. The patch adds
the necessary functions and registration to the isp1760-if.c file and
modifies the corresponding Makefile and Kconfig to be able to use this
driver even if PCI and PPC_OF are not enabled.

This patch (as1204) adds a software retry mechanism to ehci-hcd. It
gets invoked when the driver encounters transaction errors on an
asynchronous endpoint. On many systems, hardware deficiencies cause
such errors to occur if one device is unplugged while the host is
communicating with another device. With the patch, the failed
transactions are retried and generally succeed the second or third
time through.
This is based on code originally written by Koichiro Saito.

This patch (as1225) fixes a bug in ehci-hcd. The condition for
whether unlinked QHs can become IDLE should not be that the controller
is halted, but rather that the controller isn't running. In other
words when the root hub is suspended, the hardware doesn't own any
QHs.
This fixes a problem that can show up during hibernation: If a QH is
only partially unlinked when the root hub is frozen, then when the
root hub is thawed the QH won't be in the IDLE state. As a result it
can't be used properly for new URB submissions.

ehci-hcd uses usb_get_urb() and usb_put_urb() in an unbalanced way causing
isochronous URB's kref.counts incrementing once per usb_submit_urb() call.
The culprit is *usb being set to NULL when usb_put_urb() is called after URB
is given back.
Due to other fixes there is no need for ehci-hcd to deal with usb_get_urb()
nor usb_put_urb() anymore, so patch removes their usages in ehci-hcd.
Patch also makes ehci_to_hcd(ehci)->self.bandwidth_allocated adjust, if a
stream finishes.

Currently ITDs are immediately recycled whenever their URB completes.
However, EHCI hardware can sometimes remember some ITD state. This
means that when the ITD is reused before end-of-frame it may sometimes
cause the hardware to reference bogus state.
This patch defers reusing such ITDs by moving them into a new ehci member
cached_itd_list. ITDs resting in cached_itd_list are moved back into their
stream's free_list once scan_periodic() detects that the active frame has
elapsed.
This makes the snd_usb_us122l driver (in kernel since .28) work right
when it's hooked up through EHCI.

If a USB PCI controller is behind a cardbus bridge, we are trying to
restore its configuration registers too early, before the cardbus
bridge is operational. To fix this, call pci_restore_state() from
usb_hcd_pci_resume() and remove usb_hcd_pci_resume_early() which is
no longer necessary (the configuration spaces of USB controllers that
are not behind cardbus bridges will be restored by the PCI PM core
with interrupts disabled anyway).
This patch fixes the regression from 2.6.28 tracked as
http://bugzilla.kernel.org/show_bug.cgi?id=12659
[ Side note: the proper long-term fix is probably to just force the
unplug event at suspend time instead of doing a plug/unplug at resume
time, but this patch is fine regardless - Linus ]

This patch adds support for the FHCI USB controller, as found
in the Freescale MPC836x and MPC832x processors. It can support
Full or Low speed modes.
Quite a lot the hardware is doing by itself (SOF generation, CRC
generation and checking), though scheduling and retransmission is on
software's shoulders.
This controller does not integrate the root hub, so this driver also
fakes one-port hub. External hub is required to support more than
one device.

Commit a0d4922da2
(USB: fix up suspend and resume for PCI host controllers) attempted
to fix the suspend-resume of PCI USB controllers, but unfortunately
it did that incorrectly and interrupts are left enabled by the USB
controllers' ->suspend_late() callback as a result. This leads to
serious problems during suspend which are very difficult to debug.
Fix the issue by removing the ->suspend_late() callback of PCI
USB controllers and moving the code from there to the ->suspend()
callback executed with interrupts enabled. Additionally, make
the ->resume() callback of PCI USB controllers execute
pci_enable_wake(dev, PCI_D0, false) to disable wake-up from the
full power state (PCI_D0).

This patch (as1199) changes the initial wakeup settings for PCI USB
host controllers. The controllers are marked as capable of waking the
system, but wakeup is not enabled by default.
It turns out that enabling wakeup for USB host controllers has a lot
of bad consequences. As the simplest example, if a USB mouse or
keyboard is unplugged immediately after the computer is put to sleep,
the unplug will cause the system to wake back up again! We are better
off marking them as wakeup-capable and leaving wakeup disabled.

This patch (as1193b) enables wakeup during initialization for all PCI
host controllers, and it removes some code (and comments!) that are no
longer needed now that the PCI core automatically initializes wakeup
settings for all new devices.
The idea is that the bus should initialize wakeup, and the bus glue
or controller driver should enable it.

Use pci_wake_from_d3() for enabling and disabling wakeup,
instead of pci_enable_wake().
Carry out the actual state change while interrupts are
disabled.

messing around with the wakeup settings while the device
is in D3.

There is no need to disable port 1 on ISP1761. That port could
be used as an OTG port which would require a different init
sequence. However we don't have OTG support (yet) so we can use
it as a normal USB port.
This patch allows port 1 to be used a normal Port on the ISP1761.

Contains fixes so probe on x86 PCI runs, apparently I'm first to try
this. Several fixes to memory access to probe host scratch register.
Previously would bug check on chip_addr var used uninitialized.
Scratch reg write failed in one instance due to 16-bit initial access
mode, so added "& 0x0000ffff" to the readl as fix.
Includes some general cleanup - remove global vars, organize memory map
resource use.

A published errata for ppc440epx states, that when running Linux with
both EHCI and OHCI modules loaded, the EHCI module experiences a fatal
error when a high-speed device is connected to the USB2.0, and
functions normally if OHCI module is not loaded.
There used to be recommendation to use only hi-speed or full-speed
devices with specific conditions, when respective module was unloaded.
Later, it was observed that ohci suspend is enough to keep things
going, and it was turned into workaround, as explained below.

The 440EPx USB 2.0 Host controller is an EHCI compliant controller. In
USB 2.0 Host controllers, each EHCI controller has one or more companion
controllers, which may be OHCI or UHCI. An USB 2.0 Host controller will
contain one or more ports. For each port, only one of the controllers
is connected at any one time. In the 440EPx, there is only one OHCI
companion controller, and only one USB 2.0 Host port.
All ports on an USB 2.0 controller default to the companion
controller. If you load only an ohci driver, it will have control of
the ports and any deviceplugged in will operate, although high speed
devices will be forced to operate at full speed. When an ehci driver
is loaded, it explicitly takes control of the ports. If there is a
device connected, and / or every time there is a new device connected,
the ehci driver determines if the device is high speed or not. If it
is high speed, the driver retains control of the port. If it is not,
the driver explicitly gives the companion controller control of the
port.

Instead of waiting a painful 5000ms, quirk_usb_disable_ehci() now does a
1000ms loop to wait for the BIOS to acknowledge the handoff.
The five second delay is really quite irritating to have to deal with
every boot up, and I very seriously doubt any non-broken bios takes more
than a second to do the actual handoff.

The Orion ehci driver serves the Orion, kirkwood and DD Soc families.
Since each of those integrate a different USB phy we should have the
ability to use few initialization sequences or to leave the boot loader
phy settings as is.

This patch is required for all AMD SB600 revisions to avoid USB subsystem hang
symptom. The USB subsystem hang symptom is observed when the system has
multiple USB devices connected to it. In some cases a USB hub may be required
to observe this symptom.

ASL/PZL updates while the WUSB channel is inactive (i.e., the PZL and
ASL are stopped) may not complete. This causes hangs when removing the
whci-hcd module if a device is still connected (removing the device
does an endpoint_disable which results in an ASL update to remove the
qset).
If the WUSB channel is inactive the update can simply be skipped as the
WHC doesn't care about the state of the ASL/PZL.

This patch is required for AMD SB700 south bridge revision A12 and A13 to avoid
USB subsystem hang symptom. The USB subsystem hang symptom is observed when the
system has multiple USB devices connected to it. In some cases a USB hub may be
required to observe this symptom.
This patch works around the problem by correcting the internal register setting
that will help by changing the behavior of the internal logic to avoid the
USB subsystem hang issue. The change in the behavior of the logic does not
impact the normal operation of the USB subsystem.

The UWB radio manager coordinates the use of the radio between the
PALs that may be using it. PALs request use of the radio with
uwb_radio_start() and the radio manager will start beaconing if its
not already doing so. When the last PAL has called uwb_radio_stop()
beaconing will be stopped.
In the future, the radio manager will have a more sophisticated channel
selection algorithm, probably following the Channel Selection Policy
from the WiMedia Alliance when it is finalized. For now, channel 9
(BG1, TFC1) is selected.
The user may override the channel selected by the radio manager and may
force the radio to stop beaconing.
The WUSB Host Controller PAL makes use of this and there are two new
debug PAL commands that can be used for testing.

This patch (as1164) fixes a bug in the EHCI scheduler. The interval
value it uses is already in linear format, not logarithmically coded.
The existing code can sometimes crash the system by trying to divide
by zero.

This patch (as1165) makes a few small changes in the logic used by
ehci-hcd when it encounters a controller error:
Instead of printing out the masked status, it prints the
original status as read directly from the hardware.
It doesn't check for the STS_HALT status bit before taking
action. The mere fact that the STS_FATAL bit is set means
that something bad has happened and the controller needs to
be reset. With the old code this test could never succeed
because the STS_HALT bit was masked out from the status.
I anticipate that this will prevent the occasional "irq X: nobody cared"
problem people encounter when their EHCI controllers die.

Add ehci_shutdown() or ohci_shutdown() calls to the USB
PS3 bus glue. ehci_shutdown() and ohci_shutdown() do some
controller specific cleanups not done by usb_remove_hcd().
Fixes errors on shutdown or reboot similar to these:

When stopping the WUSB channel the host should send Channel Stop IEs giving
the WUSB Channel Time of the last MMC. Both WHCI and HWA hosts provide a

Some Toshiba Mobile I/O chips have OHCI controller built in.
E.g. the tc6393xb chip found in several Toshiba e-Series PDAs
and in Sharp Zaurus SL-6000 PDA. This adds platform glue

This patch (as1149) fixes an obscure problem in OHCI polling. In the
current code, if the RHSC interrupt status flag turns on at a time
when RHSC interrupts are disabled, it will remain on forever:
The interrupt handler is the only place where RHSC status
gets turned back off;
The interrupt handler won't turn RHSC status off because it
doesn't turn off status flags if the corresponding interrupt
isn't enabled;
RHSC interrupts will never get enabled because
ohci_root_hub_state_changes() doesn't reenable RHSC if RHSC
status is on!
As a result we will continue polling indefinitely instead of reverting
to interrupt-driven operation, and the root hub will not autosuspend.
This particular sequence of events is not at all unusual; in fact
plugging a USB device into an OHCI controller will usually cause it to
occur.
Of course, this is a bug. The proper thing to do is to turn off RHSC
status just before reading the actual port status values. That way
either a port status change will be detected (if it occurs before the
status read) or it will turn RHSC back on. Possibly both, but that
won't hurt anything.
We can still check for systems in which RHSC is totally broken, by
re-reading RHSC after clearing it and before reading the port
statuses. (This re-read has to be done anyway, to post the earlier
write.) If RHSC is on but no port-change statuses are set, then we
know that RHSC is broken and we can avoid re-enabling it.

This patch (as1147) fixes the remote-wakeup support for EHCI
controllers using the ARC/TDI "embedded-TT" core. These controllers
turn off the RESUME bit by themselves when a port resume is complete;
hence we need to keep separate track of which ports are suspended or
in the process of resuming.
The patch also makes a couple of small improvements in ehci_irq(),
replacing reads of the command register with the value already stored
in a local variable.

In case of error, the function isp1760_register returns an ERR
pointer, but never returns a NULL pointer. So after a call to this
function, a NULL test should be replaced by an IS_ERR test. Moreover,
we have noticed that:
(1) the result of isp1760_register is assigned through the function
pci_set_drvdata without an error test,
(2) if the call to isp1760_register fails, the current function
(isp1761_pci_probe) returns 0, and if it succeeds, it returns -ENOMEM,
which seems odd.
Thus, we suggest to move the test before the call to pci_set_drvdata
to correct (1), and to turn it into a non IS_ERR test to correct (2).

ehci_watchdog will wake up CPU very frequently so that CPU
stays at C3 very short, average residence time is about 50
ms on Aspire One, but we expect it should be about 1 second
or more, so this kind of periodic timer is very bad for power
saving.
We can't remove this timer because of some bad USB controller
chipset, but at least we should reduce its side effect to as
possible as low.
This patch can make CPU stay at C3 longer, average residence time
is about twice as long as original.

This patch (as1140) adds a little intelligence to the interrupt-URB
scheduler in uhci-hcd. Right now the scheduler is stupid; every URB
having the same period is assigned to the same slot. Thus a large
group of period-N URBs can fill their slot and cause -ENOSPC errors
even when all the lower-period slots are empty.
With the patch, if an URB doesn't fit in its assigned slot then the
scheduler will try using lower-period slots. This will provide
greater flexibility. As an example, the driver will be able to handle
more than just three or four mice, which the current driver cannot.

This patch is based on the following ideas:
1. Some usb devices (such as usb video class) have endpoints of high
interval attribute, so reading "periodic" file need more debug buffer
to accommodate the qh or itd schedule information. For example, 4KB
buffer is not enough for a single interrupt qh of 2ms period.
2. print a %p need 16 byte buffer on 64-bits arch, but 8 byte on 32-bits
arch. Add a extra bonus for 64-bits arch.

This patch (as1134) attempts to improve the way we handle OHCI
controllers with broken Root Hub Status Change interrupt support. In
these controllers the RHSC interrupt bit essentially never turns off,
making RHSC interrupts useless -- they have to remain permanently
disabled.
Such controllers should still be allowed to turn off their root hubs
when no devices are attached. Polling for new connections can
continue while the root hub is suspended. The patch implements this
feature. (It won't have much effect unless CONFIG_PM is enabled and
CONFIG_USB_SUSPEND is disabled, but since the overhead is very small
we may as well do it.)

This patch let a few discrete USB host controllers drivers (isp116x-hcd,
r8a66597-hcd and sl811-hcd) obtain IRQ flags from their IORESOURCE_IRQ
resource if configured as such, much like it's been done for the smc91x
driver.
It spares people writing support for specific boards the burden to
configure the interrupt controller independantly, and keeps all IRQ
related information in a single resource.
HCD that are integrally part of a SoC have been left aside, as there
is probably no "wiring" options...
Tested on an Xscale PXA-255 based platform with isp116x-hcd.

Direct access to USB host controller registers is considered to be not
portable, and is usually a bad sign for poorly abstracted interface.
Introduce .flags and .power_on_delay to "struct pxaohci_platform_data"
so that most platforms don't bother to write their own .init/.exit()
sequences.

As noted by Stefan Neis <Stefan.Neis@kobil.com>, we had a recent
regression with EHCI periodic transfers, in some (seemingly not
all that common) cases.
The root cause was that the schedule activation was only loosely
coupled to the addition or removal of transfers, so two different
execution contexts could both think they had to deactivate (or
conversely activate) the schedule. So this fix tightens that
coupling, managing it more like a refcount.

I noticed that the "Refactor "if (handshake()) state = HC_STATE_HALT"
patch from earlier this year perpetuated a potential problem: it can
mark the controller as halted when it's still running (but not acting
as, perhaps wrongly, expected).
That caused some hangs and crashes, rather than more polite failure
modes of a truly halted controller. This patch forces a true halt,
and emits a (previously missing) diagnostic.

This patch (as1069c) changes the way OHCI root-hub status-change
interrupts are enabled. Currently a special HCD method,
hub_irq_enable(), is called when the hub driver is finished using a
root hub. This approach turns out to be subject to races, resulting
in unnecessary polling.
The patch does away with the method entirely. Instead, the driver
automatically enables the RHSC interrupt when no more status changes
are present. This scheme is safe with controllers using
level-triggered semantics for their interrupt flags.

On some AMD 700 series southbridges, ISO OUT transfers (such as audio
playback through speakers) on the USB OHCI controller may be corrupted
when an A-Link express power saving feature is active.
PLL power down mode in conjunction with link power management feature
L1 being enabled is the bad combination ... this patch prevents them
from being enabled when ISO transfers are pending.

According to Alan Stern, short packets are quite normal under
certain circumstances. This printk was triggered by usb to
serial converters on every packet and some usb sticks triggered
a few of those while plugging the stick.
This printks are now hidden unless USB debug mode is activated.

ISP1760 requires a delay of 90ns between programming the address and
reading the data. Current driver solves this by a mdelay(1) which is
very heavy weighted and slow. This patch applies the workaround from
the ISP1760 FAQ by using two different banks for PTD and payload data
and using a common wait for them. This wait is done by an additional
ISP1760 access (whose timing constraints guarantee the 90ns delay).
This improves speed when reading from an USB stick from:

This patch (as1114) fixes a problem that was revealed by an earlier
patch (as1069b). Some broken controllers seem never to turn off their
RHCS interrupt status bit, even when told to do so. As a result they
generate an interrupt storm and hang the system.
The patch avoids enabling RHSC interrupt requests when the RHCS status
bit is already set. This should have no adverse affects on normal
controllers, since they won't set the status bit until a root-hub
status change actually occurs, in which case we wouldn't enable RHSC
interrupt requests anyway -- we would wait until the status change had
been processed and cleared.

If you are using linked lists for queues list_splice() will not do what

A couple of USB register initializations had to be changed on MPC85xx
platforms. This is due to the internal SoC buses being different on
MPC83xx SoCs vs MPC85xx SoCs.
We currently handle this via an ifdef since 83xx and 85xx are mutually
exclusive kernel builds.

Various cleanups and fixes to the i2c code in ohci-pnx4008:
* Delete empty isp1301_command. The i2c driver command implementation
is optional, so there's no point in providing an empty
implementation.
* Give a name to isp1301_driver. I'm surprised that i2c-core accepted
to register this driver at all. I've chosen "isp1301_pnx" as the
name, because it's not a generic ISP1301 driver (much like the
isp1301_omap driver.) We might want to make the name even more
specific (but "isp1301_ohci_pnx4008" doesn't fit.)
* The ISP1301 is definitely not a hardware monitoring device.
* Fix a memory leak on failure in isp1301_attach. If
i2c_attach_client fails, the client is not registered so
isp1301_detach is never called and the i2c_client memory is lost.
* Use strlcpy instead of strcpy.

This adds support for hardware configurations that don't match the
chip default register settings (e.g., 16-bit data bus, DACK and
DREQ pulled up instead of down, analog overcurrent mode).
These settings are passed in via the OF device tree. The PCI
interface still assumes the same default values.

This patch fixes some performance bugs observed with some workloads
when unlinking EHCI queue header (QH) descriptors from the async ring
(control/bulk schedule).
The mechanism intended to defer unlinking an empty QH (so there is no
penalty in common cases where it's quickly reused) was not working as
intended. Sometimes the unlink was scheduled:
- too quickly ... which can be a *strong* negative effect, since
that QH becomes unavailable for immediate re-use;
- too slowly ... wasting DMA cycles, usually a minor issue except
for increased bus contention and power usage;
Plus there was an extreme case of "too slowly": a logical error in the
IAA watchdog-timer conversion meant that sometimes the unlink never
got scheduled.
The fix replaces a simple counter with a timestamp derived from the
controller's 8 KHz microframe counter, and adjusts the timer usage
for some issues associated with HZ being less than 8K.
(Based on a patch originally by Alan Stern, and good troubleshooting
from Leonid.)

If we do rmmod ohci_hcd while an application is doing something, the
following may happen:
- a control URB completes (in finish_urb) and the ohci's endpoint is
set into ED_UNLINK in ed_deschedule
- same URB is (re)submitted because of the open/close loop or other
such application behaviour
- rmmod sets the state to HC_STATE_QUESCING
- finish_unlinks happens at next SOF; normally it would set ed into
ED_IDLE and immediately call ed_schedule (since URB had extra TDs
queued), which sets it into ED_OPER. But the check in ed_schedule
makes it fail with -EAGAIN (which is ignored)
- from now on we have a dead URB stuck; it cannot even be unlinked
because the ed status is not ED_OPER, and thus start_ed_unlink is
not invoked.
This patch removes the check. In 2.6.25, all callers check for
__ACTIVE bit before invoking ed_schedule, which is more appropriate.

As RMK pointed out, considering the fact that the _only_ platform with
a PXA and SA1111 is the Lubbock, and that SA1111 DMA doesn't work there,
(i.e. the SA1111 OHCI doesn't work there) the SA1111 OHCI driver should
really be made SA11x0 specific.

Andrey Borzenkov reports that it resulted in a totally hung machine for
him when loading the OHCI driver. Extensive netconsole capture with
SysRq output shows that modprobe gets stuck in ohci_hub_status_data()
when probing and enabling the OHCI controller, see for example
http://lkml.org/lkml/2008/7/5/236
for an analysis.
The problem appears to be an interrupt flood triggered by the commit
that gets reverted, and Andrey confirmed that the revert makes things
work for him again.

This patch fixes a problem with OHCI where canceling bulk or
interrupt URBs may lose track of the right data toggle. This
seems to be a longstanding bug, possibly dating back to the
Linux 2.4 kernel, which stayed hidden because
(a) about half the time the data toggle bit was correct;
(b) canceling such URBs is unusual; and
(c) the few drivers which cancel these URBs either
[1] do it only as part of shutting down, or
[2] have fault recovery logic, which recovers.
For those transfer types, the toggle is normally written back
into the ED when each TD is retired. But canceling bypasses
the mechanism used to retire TDs ... so on average, half the
time the toggle bit will be invalid after cancelation.
The fix is simple: the toggle state of any canceled TDs are
propagated back to the ED in the finish_unlinks function.

This patch fixes a regression in the EHCI driver's TIMER_IO_WATCHDOG
behavior. The patch "USB: EHCI: add separate IAA watchdog timer" changed
how that timer is handled, so that short timeouts on the remaining
timer (unfortunately, overloaded) would never be used.
This takes a more direct approach, reorganizing the code slightly to
be explicit about only the I/O watchdog role now being overridable.
It also replaces a now-obsolete comment describing older timer behavior.

If the SM501 and another platform driver, such as the SM501
then we end up defining PLATFORM_DRIVER twice. This patch
seperated the SM501 onto a seperate define of SM501_OHCI_DRIVER
so that it can be selected without overwriting the original
definition.

This patch (as1099) fixes a performance regression in ehci-hcd. The
fundamental problem is that queue headers get removed from the
schedule too quickly, since the code checks for a counter advancing
rather than making an actual time-based check. The latency involved
in removing the queue header and then relinking it can severely
degrade certain kinds of workloads.
The patch replaces a simple counter with a timestamp derived from the
controller's uframe value. In addition, the delay for unlinking an
idle queue header is increased from 5 ms to 10 ms; since some
controllers (nVidia) have a latency of up to 1 ms for unlinking, this
reduces the relative impact from 20% to 10%.
Finally, a logical error left over from the IAA watchdog-timer
conversion is corrected. Now the driver will always either unlink an
idle queue header or set up a timer to unlink it later. The old code
would sometimes fail to do either.

This patch (as1098) changes the way ehci-hcd schedules its periodic
Iso transfers. That the current scheduling code is wrong is clear on
the face of it: Sometimes it returns -EL2NSYNC (meaning that an URB
couldn't be scheduled because it was submitted too late), but it does
this even when the URB_ISO_ASAP flag is set (meaning the URB should be
scheduled as soon as possible).
The new code properly implements as-soon-as-possible scheduling,
assigning the next unexpired slot as the URB's starting point. It
also is more careful about checking for Iso URB completion: It doesn't
bother to check for activity during frames that are already over,
and it allows for the possibility that some of the URB's packets may
have raced the hardware when they were submitted and so never got used
(the packet status is set to -EXDEV).
This fixes problems several people have experienced with USB video
applications.

This patch (as1097) fixes a bug in the remote-wakeup handling in
ehci-hcd. The driver currently does not keep track of whether the
change-suspend feature is enabled for each port; the feature is
automatically reset the first time it is read. But recent changes to
the hub driver require that the feature be read at least twice in
order to work properly.
A bit-vector is added for storing the change-suspend feature values.

This patch (as1096) fixes an annoying problem: When a full-speed or
low-speed device is plugged into an EHCI controller, it fails to
enumerate at high speed and then is handed over to the companion
controller. But usbcore logs a misleading and unwanted error message
when the high-speed enumeration fails.
The patch adds a new HCD method, port_handed_over, which asks whether
a port has been handed over to a companion controller. If it has, the
error message is suppressed.

This patch (as1095) cleans up the HCD glue and several of the EHCI
bus-glue files. The ehci->is_tdi_rh_tt flag is redundant, since it
means the same thing as the hcd->has_tt flag, so it is removed and the
other flag used in its place.
Some of the bus-glue files didn't get the relinquish_port method added
to their hc_driver structures. Although that routine currently
doesn't do anything for controllers with an integrated TT, in the
future it might. So the patch adds it where it is missing.
Lastly, some of the bus-glue files have erroneous entries for their
hc_driver's suspend and resume methods. These method pointers are
specific to PCI and shouldn't be used otherwise.
(The patch also includes an invisible whitespace fix.)

Commit 7329e211b9 ("USB: root hubs don't
lie about their number of TTs") requires the various platform EHCI
glue modules to set ->has_tt if the root hub has a Transaction
Translator.
The Orion EHCI root hub does have a Transaction Translator, so set
->has_tt in ehci_orion_setup(). This fixes oopsing on plugging in a
low speed device.

This patch (as1084b) fixes the way uhci-hcd handles polling and
remote wakeups for its root hubs. When remote wakeup is disabled,
neither interrupts nor polling should be enabled during a root-hub
suspend. Likewise, if interrupts are enabled during suspend then
polling isn't needed.
Furthermore the EGSM (Enter Global Suspend Mode) bit shouldn't be set
in the Command register unless remote wakeup is enabled. Apparently
some controllers will issue a remote-wakeup interrupt whenever EGSM
is on, even if Resume-Detect interrupts are supposedly disabled.

might (should) work as well but the OTG isn't supported. Also ISO packets are

ehci_port_power() is called during initialization by all the

Simplify processing of completed qtds, and correct handling of short
reads, by removing two state variables:
- "qtd_status" wasn't needed. The current URB's status is either
OK (-EINPROGRESS) or some fault status. Once a fault appears,
the queue halts and any later QTDs are immediately removed, so
no temporary status is needed. (Or for typical short reads,
it's not treated as a fault, so no queue halt is needed.)
- "do_status" was erroneous. Because of how the queue is set up,
short control reads can (and should!) be treated like full size
reads, and cleaned up the usual way. The status stage will be
executed transparently, and usbcore handles the choice of whether
to report this status as unexected.
The "do_status" problem caused a rather perplexing timing-dependent
problem with usbtest case 10. Sometimes it would make the controller
skip a dozen transactions while (wrongly) trying to clean up after a
short transfer. Fortunately, removing a dcache contention issue made
this become trivial to reproduce (on one test rig), so enough clues
finally presented themselves ... I think this has been around for a
very long time, but was worsened by recent urb->status changes.

This patch (as1069b) changes the way OHCI root-hub status-change
interrupts are enabled. Currently a special HCD method,
hub_irq_enable(), is called when the hub driver is finished using a
root hub. This approach turns out to be subject to races, resulting
in unnecessary polling.
The patch does away with the method entirely. Instead, the driver
automatically enables the RHSC interrupt when no more status changes
are present. This scheme is safe with controllers using
level-triggered semantics for their interrupt flags.

This patch (as1068b) disables the RD interrupt flag when an OHCI root
hub is suspended with remote wakeup disabled. Although the spec
clearly states that this flag permits the controller to issue an
interrupt when a resume request from downstream is detected and not
when a local status change occurs, some controllers mistakenly use it
for both types of event.

When a USB device is suspended, whether or not it is enabled for
remote wakeup depends on the device_may_wakeup() setting. The setting
is then saved in the do_remote_wakeup flag.
Later on, however, the device_may_wakeup() value can change because of
user activity. So when testing whether a suspended device is or
should be enabled for remote wakeup, we should always test
do_remote_wakeup instead of device_may_wakeup(). This patch (as1076)
makes that change for root hubs in several places.
The patch also adjusts uhci-hcd so that when an autostopped controller
is suspended, the remote wakeup setting agrees with the value recorded
in the root hub's do_remote_wakeup flag.
And the patch adjusts ehci-hcd so that wakeup events on selectively
suspended ports (i.e., the bus itself isn't suspended) don't turn on
the PME# wakeup signal.

Provide better comments about qh_completions() and QTD handling.
That code can be *VERY* confusing, since it's evolved over a few
years to cope with both hardware races and silicon quirks.
Remove two unlikely() annotations that match the GCC defaults
(and are thus pointless); add an "else" to highlight code flow.
This patch doesn't change driver behavior.

This patch refactors some shutdown code so it can be shared between
ehci_stop() and ehci_shutdown().
This also fixes a couple potential bugs:
- ehci_shutdown() was not locking ehci->lock before halting the HC.
- ehci_shutdown() didn't disable the watchdog and IAA timers.
- ehci_stop() was resetting the host controller when it may have been
running, which the EHCI spec says "may result in undefined behavior".
ehci_stop() was calling port_power() to turn off the ports, which waited
20ms after applying the port change. The msleep was for the case where
the HC might take 20ms to turn the ports on; since we're shutting them
off, we can avoid the msleep and just use ehci_turn_off_ports().
ehci_stop() doesn't need to clear the intr_enable register or revert
ownership of the companion controllers to the BIOS, because the host
controller reset should have done that. There might be a buggy host
controller that doesn't follow the reset rules, but for now we assume
it's redundant code and remove it.
[ A subsequent patch will cancel the timers later ... this version
carries forward existing bugs where timers could get re-armed
after they're canceled. ]

Drivers in the ohci-hcd family should perform certain tasks whenever
their controller device is resumed. These include checking for loss
of power during suspend, turning on port power, and enabling interrupt
requests.
Until now these jobs have been carried out when the root hub is
resumed, not when the controller is. Many drivers work around the
resulting awkwardness by automatically resuming their root hub
whenever the controller is resumed. But this is wasteful and
unnecessary.
To simplify the situation, this patch (as1066) adds a new core
routine, ohci_finish_controller_resume(), which can be used by all the
OHCI-variant drivers. They can call the new routine instead of
resuming their root hubs. And ohci-pci.c can call it instead of using
its own special-purpose handler.

The .suspend and .resume method pointers in struct usb_hcd have not
been fully understood by host-controller driver writers. They are
meant for use with PCI controllers; other platform-specific drivers
generally should not refer to them.
To try and clarify matters, this patch (as1065) renames those methods
to .pci_suspend and .pci_resume. It eliminates corresponding dead code
and bogus references in the ohci-ssb and u132-hcd drivers.

Currently EHCI root hubs enumerate with a bDeviceProtocol code
indicating that they possess a Transaction Translator. However the
vast majority of controllers do not; they rely on a companion
controller to handle full- and low-speed communications. This patch
(as1064) changes the root-hub device descriptor to match the actual
situation.

This patch (as1063) fixes a bug in the way ohci-hcd resumes its
controllers. It leaves the Master Interrupt Enable bit turned off.
If the root hub is resumed immediately this won't matter. But if the
root hub is suspended (say because no devices are plugged in), it won't
ever wake up by itself.

I was converting a semaphore in this file to a mutex when I noticed that
this file has some fairly rampant style problems. Practically every line
has spaces instead of tabs .. Once I cleared that up, checkpatch.pl showed
a number of other problem.. I think this file might be a good one to review
for new style checks that could be added..

Some EHCI fault paths with large control transfers aren't coded. Avoid
problems by rejecting transfers that may need two qTDs (16+ KB). This is
mostly paranoia; even 4 KB transfers are rare, and most HCDs use lower
limits (so it's unlikely anyone would ever try such a thing).

power.power_state is scheduled for removal. This patch (as1053)
removes all uses of that field from drivers/usb. Almost all of them
were write-only, the most significant exceptions being sl811-hcd.c and

- Un-break MODULE_ALIAS() directives after platform_bus hotplugging
acquired a backwards-incompatible change. (Which didn't fix ANY
of the in-tree drivers it prevented from hotplugging -- sigh.)
- Remove some bogus assignments of platform_bus_type; that's done by
the platform_bus code.
- Add some FIXMEs for drivers with that pointless two-level idiom for
probe() and remove() routines. ("Obfuscation" is a non-goal.)
That should help avoid future bus glue which copies that idiom.

This teaches EHCI how to to work around bugs in certain high speed
devices, by accomodating "bulk" packets that exceed the 512 byte
constant value required by the USB 2.0 specification. (Have a
look at section 5.8.3, paragraphs 1 and 3.)
It also makes the descriptor parsing code warn when it encounters
such bugs. (We've had reports of maybe two or three such devices,
all pretty recent.)
Such devices are nonconformant. The proper fix is have the vendors
of those devices do the simple, obvious, and correct thing ... which
will let them be used with USB hosts that don't have workarounds for
this particular vendor bug. But unless/until they do, we can at least
have one of the high speed HCDs work with such buggy devices.

This limits how long the OHCI port reset loop waits for the hardware
to do its job, if the controller either (a) dies, or (b) can't finish
the reset. Such limits are always a good idea.

This patch (as1047) removes the USB_PERSIST Kconfig option, enabling
it permanently. It also prevents the power/persist attribute from
being created for hub devices; there's no point in having it since
USB-PERSIST is always turned on for hubs.

This patch (as1044) causes EHCI port handover for non-high-speed
devices to occur during every root-hub resume, not just in cases where
the controller lost power or was reset. This is necessary because:
When some machines go into suspend, they remove power from
on-board USB devices while retaining suspend current for USB
controllers.
The user might well unplug a USB device while the system is
suspended and then plug it back in before resuming.
A corresponding change is made to the core resume routine; now
high-speed root hubs will always be resumed when the system wakes up,
even if they were suspended before the system went to sleep. If this
weren't done then EHCI port handover wouldn't work, since it is called
when the EHCI root hub is resumed.
Finally, a comment is added to the hub driver explaining the khubd has
to be freezable; if it weren't frozen then it could interfere with
port handover.

Code inspection discovered in 2 places timers were being incorrectly setup
using round_jiffies_relative(HZ). The timer would then fire at time (0 <= T <
HZ).

This patch, suggested by Alan Stern, fixes the hung USB issues
on my notebook from suspend/resume cycles.
It does so by eliminating some confusion about the internal state
machine associated with unlinking from the EHCI async schedule ring,
which caused a recent regression:

Make it possible to pass mbus_dram_target_info to the ehci-orion
driver via the platform data, make the ehci-orion driver program
the window registers based on this data if it is passed in, and
make the Orion platform setup code use this method instead of
programming the EHCI mbus window registers by hand.

The VIA VT6212 defaults to only waiting 1us between passes over EHCI's
async ring, which hammers PCI badly ... and by preventing other devices
from accessing the bus, causes problems like drops in IDE throughput,
a problem that's been bugging users of those chips for several years.
A (partial) datasheet for this chip eventually turned up, letting us
see how to make it use a VIA-specific register to switch over to the
the normal 10us value instead, as suggested by the EHCI specification
Solution noted by Lev A. Melnikovsky.
It's not clear whether this register exists on other VIA chips; we
know that it's ineffective on the vt8235. So this patch only applies
to chips that seem to be incarnations of the (discrete) vt6212.

The recent EHCI driver update to split the IAA watchdog timer out from
the other timers made several things work better, but not everything;
and it created a couple new issues in bugzilla. Ergo this patch:
- Handle a should-be-rare SMP race between the watchdog firing
and (very late) IAA interrupts;
- Remove a shouldn't-have-been-added WARN_ON() test;
- Guard against one observed OOPS;
- If this watchdog fires during clean HC shutdown, it should act
as a NOP instead of interfering with the shutdown sequence;
- Guard against silicon errata hypothesized by some vendors:
* IAA status latch broken, but IAAD cleared OK;
* IAAD wasn't cleared when IAA status got reported;
The WARN_ON is in bugzilla as 10168; the OOPS as 10078; these are
both regressions.

This patch removes the buffering of the status register.
USB core behavior has changed a bit and this buffering was not refreshed
at the right time. The core got buffered old value of HCRHPORT and it
did not detect any devices on boot.

USB: ehci: Fixes completion for multi-qtd URB the short read case
When use of urb->status in the EHCI driver was reworked last August
(commit 14c04c0f88), a bug was inserted
in the handling of early completion for bulk transactions that need
more than one qTD (e.g. more than 20KB in one URB).
This patch resolves that problem by ensuring that the early completion
status is preserved until the URB is handed back to its submitter,
instead of resetting it after each qTD.

During the last step of hibernation in the "platform" mode (with the
help of ACPI) we use the suspend code, including the devices'
->suspend() methods, to prepare the system for entering the ACPI S4
system sleep state.
But at least for some devices the operations performed by the
->suspend() callback in that case must be different from its operations
during regular suspend.
For this reason, introduce the new PM event type PM_EVENT_HIBERNATE and
pass it to the device drivers' ->suspend() methods during the last phase
of hibernation, so that they can distinguish this case and handle it as
appropriate. Modify the drivers that handle PM_EVENT_SUSPEND in a
special way and need to handle PM_EVENT_HIBERNATE in the same way.
These changes are necessary to fix a hibernation regression related
to the i915 driver (ref. http://lkml.org/lkml/2008/2/22/488).

The previous fix for a "sparse" warning in ehci_urb_dequeue() was
incorrect. After rescheduling interrupt transfers it returned the
URB's completion status, not status for the dequeue operation itself.
This patch resolves that issue, cleans up the code in the reschedule
path, and shrinks the object code by a dozen bytes.

While most isochronous endpoints have short polling intervals, the
EHCI driver won't necessarily handle larger ones correctly.
This patch switches to use a "u16" to represent those periods, not
a u8, since it can always work: the largest expressible period
is 2^15 units ... not the previous too-short limit of 128 frames
(full or low speeds) or microframes (high speed, 32 frames).
This bug is essentially theoretical, since the few ISO endpoints
I've seen which don't use one transfer per frame are high speed
ones using more than that (including high bandwidth, 24 KB/msec).

Some of the "EHCI ports reset forever" problems may be explained by
code paths which wrongly flagged resets as complete. This removes
two such paths; the ehci_hub_status_data() path should be the only one
to have an effect, since it was already properly flagged on the other

usb: ohci-sm501 driver V2
This patch adds sm501 ohci support. It's all very straightforward with the
exception of dma_declare_coherent_memory() and HCD_LOCAL_MEM. Together they
are used to ensure that usb data is allocated using dma_alloc_coherent(),
and that only valid dma memory is used to allocate from. This driver is
a platform device, and the mfd driver sm501.c is already creating one
usb host controller instance per sm501.

This has some bugfixes for the EHCI driver's ISO transfer scanning
logic. It was leaving ITDs and SITDs on the schedule too long, for
a few different reasons, which caused trouble.
(a) Look at all microframes for high speed transfers, not just
the ones we expect to have finished. This way transfers
ending mid-frame will complete without needing another IRQ.
This also minimizes bogus scheduling underruns (e.g. EL2NSYNC).
(b) When we encounter an ISO transfer (either speed, but this
hits mostly at full speed) that's not yet been completed,
immediately stop scanning; we've caught up to the hardware,
no matter what other indications might say.
(c) Always clean up ITDs (for high speed transfers) when the HC
is no longer running.
I'm not sure whether the last one has been observed before, but both
the others have been reported with "real world" audio and video code.

Small updates to the EHCI driver's ISO support:
- Get rid of the Kconfig option for full speed ISO. It may
not be perfect yet, but it hasn't appeared to be dangerous
and pretty much every configuration wants it.
- Instead of two places to disable an empty periodic schedule
after an ISO transfer completes, just have one.
- After the periodic schedule is disabled, we can short-circuit
the schedule scan ... it can't possibly have more work to do.
Assuming a typical config with split iso enabled, the only change
in behavior should be almost unobservable: quicker termination
of periodic scans when the schedule gets emptied.

In the EHCI driver, itd->usecs[8] is used in periodic_usecs(), indexed by
uframe. For an ITD's unused uframes it is 0, else it contains the same
value as itd->stream->usecs. To check if an ITD's uframe is used, we can
instead test itd->hw_transaction[uframe]: if used, it will be nonzero no
matter what endianess is used.
This patch replaces those two uses, eliminates itd->usecs[], and saves
eight bytes from each ITD.

Update the ohci-at91 bus glue to start understanding about the per-port
power switch GPIOs it's given (on the sam9263-ek and potentially other
boards). For the moment this just claims them and forces them active
(assuming active-low power enables) whenever the HCD is loaded.
The assumption is still that board setup configures the GPIOs. Using
gpio_request() tracks actual usage and guards against conflict.

Kernel bugzilla entry #9569 reports a potential OOPS in some code
supporting the integrated root hub TT support used on ARC/TDI
derived cores. (This seems to have been a longstanding issue.)

This adds a workaround for an issue reported with ISO transfers
on some EHCI controllers, most recently with VIA KT800 and PS3
EHCI silicon.
The issue is that the silicon doesn't necessarily seem to be done
using ISO DMA descriptors (itd, sitd) when it marks them inactive.
(One theory is that the ill-defined mechanism where hardware caches
periodic transfer descriptors isn't invalidating their state...)
With such silicon, quick re-use of those descriptors makes trouble.
Waiting until the next frame seems to be a sufficient workaround.
This patch ensures that the relevant descriptors aren't available
for immediate re-use. It does so by not recycling them until after
issuing the completion callback which would reuse them by enqueueing
an URB and thus (re)allocating ISO DMA descriptors.

This patch (as1028) was mostly written by David Brownell; I made only
a few changes (extra log info and a small bug fix -- which might
account for why David's version had to be reverted). It adds a new
watchdog timer to the ehci-hcd driver to be used exclusively for
detecting lost or missing IAA notifications.
Previously a shared timer had been used, which may have led to some
problems as reported by Christian Hoffmann.

The OHCI driver's IRQ handler, while processing a WDH interrupt, masks
and unmasks it. I believe this is both broken (the write may still be
posted during the donelist processing it's trying to safeguard) and
useless as this IRQ may not be reissued until it's acked (unless this
legacy code is an uncommented workaround for some chip erratum).

This patch should *not* break compatibility with older device trees
which do not use the 'fsl,' prefix. The drivers will still bind against
the older names also.

The OHCI IRQ handler has an optimisation that avoids reading some
chip registers when the controller reports that the interrupt was
triggered *only* because completed requests were written into the
controller's "done list" and handed to the host.
This mechanism can't be used on some controllers. Among others, it
fails for the SA1111 and the AMCC 440EP PowerPC processor.
This patch removes the optimisation and makes the code clearer.

Host controller IRQs are supposed to be serviced with interrupts
disabled. This patch (as1026) adds an IRQF_DISABLED flag to all the
controller drivers that lack it. It also replaces the
spin_lock_irqsave() and spin_unlock_irqrestore() calls in uhci_irq()
with simple spin_lock() and spin_unlock().
This fixes Bugzilla #9335.

A recent patch added software synchronization during EHCI startup,
so ports aren't switched away from the companion controllers after
resets have started. This patch adds a short delay letting hardware
finish that port switching before any new resets begin ... so both
ends of that hardware race window are closed.

This patch (as1003) changes uhci-hcd to treat the URB_ISO_ASAP flag
the same as other host controller drivers, namely, to schedule an Iso
URB for the first available time slot that hasn't already expired.
URBs in which the flag isn't set will be scheduled for the first slot
following the last URB, even if it has expired.
This fixes a problem reported by Martin Bachem.

This patch (as999) fixes a problem that sometimes shows up when host
controller driver modules are loaded in the wrong order. If ehci-hcd
happens to initialize an EHCI controller while the companion OHCI or
UHCI controller is in the middle of a port reset, the reset can fail
and the companion may get very confused. The patch adds an
rw-semaphore and uses it to keep EHCI initialization and port resets
mutually exclusive.

Add OHCI big endian frame_no quirk. The frame_no value stored in the
HCCA is a 16 bit field at a specific offset, but since not all CPUs can
do 16-bit memory accesses it's used as a 32 bit field. And that's why
big-endian OHCI must shift 16 bits ... unless the spec is not followed.
Currently there's one MPC52xx platform that doesn't need the shift. This
patch adds a new "big endian frame_no" quirk to control that at runtime.

This patch (as972) changes ohci-hcd so that after an error occurs, the
remaining TDs for the URB will be skipped over entirely instead of
going through the donelist. This enables the driver to give back the
URB as soon as the error is detected, avoiding the need to store the
error status in urb->status.

This patch (as970) adds a new urb->unlinked field, which is used to
store the status of unlinked URBs since we can't use urb->status for
that purpose any more. To help simplify the HCDs, usbcore will check
urb->unlinked before calling the completion handler; if the value is
set it will automatically override the status reported by the HCD.

This patch (as969) continues the ongoing changes to the way HCDs
report URB statuses. The programming interface has been simplified by
making usbcore responsible for clearing urb->hcpriv and for setting
-EREMOTEIO status when an URB with the URB_SHORT_NOT_OK flag ends up
as a short transfer.
By moving the work out of the HCDs, this removes a fair amount of
repeated code.

This patch (968) changes the way uhci-hcd reports status for
Isochronous URBs. Until now urb->status has been set to the last
detected error code. But other HCDs don't do this; they leave the
status set to 0 and report errors only in the individual iso packet
descriptors. So this patch removes the extra computation and makes
uhci-hcd behave like the others.

It is not necessary to powerdown the ports on ehci_pci_reinit() when the
chip reset already did that. Removing this saves 20ms during restart
after poweroff paths (which OLPC uses a lot).
To ensure driver startup then behaves consistently, force a reset during
driver startup. (Not doing this was an accident of some previous changes
to the init sequence.)
Make the corresponding change in the PS3 support. It's not clear what
ehci-fsl should do here; it has similar code to the PS3.

This patch (as954) implements a suggestion of David Brownell's. Now
the host controller drivers are responsible for linking and unlinking
URBs to/from their endpoint queues. This eliminates the possiblity of
strange situations where usbcore thinks an URB is linked but the HCD
thinks it isn't. It also means HCDs no longer have to check for URBs
being dequeued before they were fully enqueued.
In addition to the core changes, this requires changing every host
controller driver and the root-hub URB handler. For the most part the
required changes are fairly small; drivers have to call
usb_hcd_link_urb_to_ep() in their urb_enqueue method,
usb_hcd_check_unlink_urb() in their urb_dequeue method, and
usb_hcd_unlink_urb_from_ep() before giving URBs back. A few HCDs make
matters more complicated by the way they split up the flow of control.
In addition some method interfaces get changed. The endpoint argument
for urb_enqueue is now redundant so it is removed. The unlink status
is required by usb_hcd_check_unlink_urb(), so it has been added to
urb_dequeue.

The ZF Micro OHCI controller exhibits unexpected behavior that seems to be
related to high load. Under certain conditions, the controller will
complete a TD, remove it from the endpoint's queue, and fail to add it to
the donelist. This causes the endpoint to appear to stop responding. Worse,
if the device is removed while in that state, OHCI will hang while waiting
for the orphaned TD to complete. The situation is not recoverable without
rebooting.
This fix enhances the scope of the existing OHCI_QUIRK_ZFMICRO flag:
1. A watchdog routine periodically scans the OHCI structures to check
for orphaned TDs. In these cases the TD is taken back from the
controller and completed normally.
2. If a device is removed while the endpoint is hung but before the
watchdog catches the situation, any outstanding TDs are taken back
from the controller in the 'sanitize' phase.
The ohci-hcd driver used to print "INTR_SF lossage" in this situation;
this changes it to the universally accurate "ED unlink timeout". Other
instances of this message presumably have different root causes.
Both this Compaq quirk and a NEC quirk are now properly compiled out for
non-PCI builds of this driver.

Three main sets of changes:
1) dmi_get_system_info() return value should have been marked const,
since callers should not be changing that data.
2) const-ify DMI internals, since DMI firmware tables should,
whenever possible, be marked const to ensure we never ever write to
that data area.
3) const-ify DMI API, to enable marking tables const where possible
in low-level drivers.
And if we're really lucky, this might enable some additional
optimizations on the part of the compiler.
The bulk of the changes are #2 and #3, which are interrelated. #1 could
have been a separate patch, but it was so small compared to the others,
it was easier to roll it into this changeset.

Correctly initialize the on-chip EHCI controller on the AMCC PPC440EPx.
Fix "USB 0.0" initialization message, and properly put the controller
into a known state before starting it.
Add "FIXME" comment to the au1xxx bus glue which is doing the same wrong
thing here. (Who maintains that, now that AMD sold off Alchemy?) Remove
some false copyright attributions which were somehow placed in the au1xxx
bus glue then copied into ppc-soc.

This reverts commit 196705c9bb. It was
reported to cause a regression by Daniel Exner, and Arjan van de Ven
points out that we actually already have infrastructure in place for
setting limits on acceptable DMA latency that would be the much more
correct fix for the problem with some Broadcom EHCI controllers.
Fixed up trivial conflicts due to the changes to support big-endian host
controller descriptors in drivers/usb/host/{ehci-sched.c,ehci.h}.

Slab destructors were no longer supported after Christoph's
c59def9f22 change. They've been
BUGs for both slab and slub, and slob never supported them
either.
This rips out support for the dtor pointer from kmem_cache_create()
completely and fixes up every single callsite in the kernel (there were
about 224, not including the slab allocator definitions themselves,
or the documentation references).

This patch (as931b), adapted from a patch by Olav Kongas, makes a small
set of conservative changes to the isp116x-hcd driver in preparation
for the removal of urb->status.
finish_request() is moved up in the source and is called
as soon as the URB is known to have completed, rather than
after all the active endpoints have been scanned.
The status of a completed URB is kept in a local variable
and copied to urb->status only when the URB is about to be
given back.
-EREMOTEIO error status for control transfers is set after
the status stage rather than when the short packet arrives.
Some unnecessary uses of urb->lock are removed.

Root hubs can't be resumed if their parent controller device is still
suspended. This patch (as925) adds a check for that condition in
hcd_bus_resume() and prevents it from being treated as a fatal
controller failure.
ehci-hcd is updated to add the corresponding test. Unnecessary
debugging messages are removed from uhci-hcd and dummy-hcd. The
error return code from dummy-hcd is changed to -ESHUTDOWN, the same as
the others. ohci-hcd doesn't need any changes.
Suspend handling in the non-PCI host drivers is somewhat hit-and-miss.
This patch shouldn't have any effect on them.

According to the USB Specification Revision 2.0 chapter 11.12.5
a hub experiencing an over-current condition must place all
affected ports in the powered-off state. It seems that some root
hubs need port power to be cycled by software in order to get back
to normal functionality after an over-current condition ... like
the EHCI implementation on an MPC8343E.

This patch fixes a silicon bug in some NEC OHCI chips. The bug appears
at random times and is very, very difficult to reproduce. Without the
following patch, Linux would shut the chip and its associated devices
down. In Apple PowerBooks this leads to an unusable keyboard and mouse
(SSH still working). The idea of restarting the chip is taken from
public Darwin code.

For MPC831x support, change the ehci-fsl driver to preserve
bits set in platform code. Add a common CONFIG_USB_EHCI_FSL
to indicate presence of Freescale EHCI SOC. Add FSL_USB2_DR_OTG
operating mode support, thus both host and device can work for the
mini-ab receptacle. Note: this doesn't enable OTG protocol
support.

This patch (as895) fixes up a loose end in the port-handover code for
the USB-Persist facility. A special case occurs when a high-speed
device is attached to a port which the user has designated to run at
full-speed only; the port must be disabled before the handover can
take place.

This patch (as887) changes the way ehci-hcd and ohci-hcd handle a loss
of VBUS power during suspend. In order for the USB-persist facility
to work correctly, it is necessary for low- and full-speed devices
attached to a high-speed port to be handed back to the companion
controller during resume processing.
This entails three changes: adding code to ehci-hcd to perform the
handover, removing code from ohci-hcd to turn off ports during
root-hub reinit, and adding code to ohci-hcd to turn on ports during
PCI controller resume. (Other bus glue resume methods for platforms
supporting high-speed controllers would need a similar change, if any
existed.)

I would like to submit Renesas R8A66597 USB HCD driver.
R8A66597 is Renesas USB 2.0 host and peripheral combined
controller device originally designed for embedded products.
As a limitation of this device, it does not support externel
hub more than 2 tier, and cannot communicate with a USB
device more than 10. Then this device is not compatible with
EHCI and/or OHCI, I wrote driver support patch based on
sl811 code.
This driver has the following unique specifications:
- Implement transfer timeout to share one pipe with plural endpoint.
- Detach detection of a USB device connected to externel hub.
The driver has been tested external hub, usb-hdd, usb-cdrom,
usb-speaker, mice, keyboard, and usbtest driver.

This patch implements supports for EHCI controllers whose in-memory
data structures are represented in big-endian format. This is needed
(unfortunately) for the AMCC PPC440EPx SoC EHCI controller; the EHCI
spec doesn't specify little-endian format, although that's what most
other implementations use.
The guts of the patch are to introduce the hc32 type and change all
references from le32 to hc32. All access routines are converted from
cpu_to_le32(...) to cpu_to_hc32(ehci, ...) and similar for the other
"direction". (This is the same approach used with OHCI.)
David fixed:
Whitespace fixes; refresh against ehci cpufreq patch; move glue
for that PPC driver to the patch adding it; fix free symbol
capture bugs in modified "constant" macros; and make "hc32" etc
be "le32" unless we really need the BE options, so "sparse" can
do some real good.

EHCI controllers that don't cache enough microframes can get MMF errors
when CPU frequency changes occur between the start and completion of
split interrupt transactions, due to delays in reading main memory
(caused by CPU cache snoop delays).
This patch adds a cpufreq notifier to the EHCI driver that will
inactivate split interrupt transactions during frequency transitions.
It was tested on Intel ICH7 and Serverworks/Broadcom HT1000 EHCI
controllers.

Instead of all drivers reading pci config space to get the revision
ID, they can now use the pci_device->revision member.
This exposes some issues where drivers where reading a word or a dword
for the revision number, and adding useless error-handling around the
read. Some drivers even just read it for no purpose of all.
In devices where the revision ID is being copied over and used in what
appears to be the equivalent of hotpath, I have left the copy code
and the cached copy as not to influence the driver's performance.
Compile tested with make all{yes,mod}config on x86_64 and i386.

This patch (as901) fixes an oversight in ohci-hcd. The
hub_status_data routine must not try to access the controller's
memory-mapped registers if the controller is in a low-power state;
such attempts will cause a crash on some architectures (such as PPC).

This patch (as882) fixes a problem with the EHCI BIOS handoff. On my
machine, the BIOS configures the controller and the handoff fails,
leaving the controller configured. During resume-from-disk, this
confuses ehci-hcd into thinking that the controller has not been
tampered with.
The problem is fixed by turning off the Configured Flag whenever a
BIOS handoff is attempted, whether it succeeds or not.

Remove the old crisv10 HCD ... it can't have built for some time,
doesn't even have a Kconfig entry, was the last driver not to have
been converted to the "hcd" framework, and considering the usbcore
changes since its last patch was merged, has just got to buggy as
all get-out.
I'm told Axis has a new driver, and will be submitting it soon.

Use the new ohci-pci quirk infrastructure to address the problem it was
created to address: a quirk specific to the Portege 4000, in buzilla as

Also fix a misuse of "__devinit" for the quirk functions. It must not

are never supposed to fail. Unsettlingly, one of them has shown up in
a user's log! Maybe making the warning more visible and having the
call-stack information available will help pinpoint the source of the

This patch removes the unnecessary bit number from CKENnn_XXXX
definitions for PXA, so that
CKEN0_PWM0 --> CKEN_PWM0
CKEN1_PWM1 --> CKEN_PWM1
...
CKEN24_CAMERA --> CKEN_CAMERA
The reasons for the change of these defitions are:
1. they do not scale - they are currently valid for pxa2xx, but
definitely not valid for pxa3xx, e.g., pxa3xx has bit 3 for camera
instead of bit 24
2. they are unnecessary - the peripheral name within the definition
has already announced its usage, we don't need those bit numbers
to know which peripheral we are going to enable/disable clock for
3. they are inconvenient - think about this: a driver programmer
for pxa has to remember which bit in the CKEN register to turn
on/off
Another change in the patch is to make the definitions equal to its
clock bit index, so that
#define CKEN_CAMERA (24)
instead of
#define CKEN_CAMERA (1 << 24)
this change, however, will add a run-time bit shift operation in
pxa_set_cken(), but the benefit of this change is that it scales
when bit index exceeds 32, e.g., pxa3xx has two registers CKENA
and CKENB, totally 64 bit for this, suppose CAMERA clock enabling
bit is CKENB:10, one can simply define CKEN_CAMERA to be (32 + 10)
and so that pxa_set_cken() need minimum change to adapt to that.

There is one significant difference between the behavior of root hubs
(as embodied in host controller hardware) and external hubs: When a
remote-wakeup signal is received, an external hub sends an interrupt
message at the _end_ of the resume sequence but a root hub generates
and interrupt at the _beginning_ of the resume sequence. The host
system must poll for the end of the sequence.
When ehci-hcd was converted to interrupt-driven operation instead of
using polling, the remaining need for this particular poll was
overlooked. This patch (as894) fixes the problem.

This patch (as871) fixes a problem introduced by an earlier change.
It turns out that some systems really do need to have a terminating
skeleton QH present whenever FSBR is on. I don't know any way to tell
which systems do need it and which don't; the easiest answer is to
have it there always.
This fixes the NumLock-hang bug reported by Jiri Slaby.

This patch (as863) fixes a problem encountered sometimes when resuming
a port on a UHCI controller. The hardware may turn off the
Resume-Detect bit before turning off the Suspend bit, leading usbcore
to think that the port is still suspended and the resume has failed.
The patch makes uhci_finish_suspend() wait until both bits are safely
off.

This patch (as856) attempts to improve the performance of uhci-hcd by
removing the asynchronous skeleton Queue Headers. They don't contain
any useful information but the controller has to read through them at
least once every millisecond, incurring a non-zero DMA overhead.
Now all the asynchronous queues are combined, along with the period-1
interrupt queue, into a single list with a single skeleton QH. The
start of the low-speed control, full-speed control, and bulk sublists
is determined by linear search. Since there should rarely be more
than a couple of QHs in the list, the searches should incur a much
smaller total load than keeping the skeleton QHs.

The AT91SAM9261 needs to activate an AHB clock (HCK0) to use the USB Host
controller. Previously clock.c would just enable it at startup, but now

This patch (as850b) disables remote wakeup (and everything else!) on
all EHCI ports when the shutdown() method is called. If remote wakeup
is left active then some systems will reboot instead of powering off.
This fixes Bugzilla #7828.

After Al Viro (finally) succeeded in removing the sched.h #include in module.h
recently, it makes sense again to remove other superfluous sched.h includes.
There are quite a lot of files which include it but don't actually need
anything defined in there. Presumably these includes were once needed for
macros that used to live in sched.h, but moved to other header files in the
course of cleaning it up.
To ease the pain, this time I did not fiddle with any header files and only
removed #includes from .c-files, which tend to cause less trouble.
Compile tested against 2.6.20-rc2 and 2.6.20-rc2-mm2 (with offsets) on alpha,
arm, i386, ia64, mips, powerpc, and x86_64 with allnoconfig, defconfig,
allmodconfig, and allyesconfig as well as a few randconfigs on x86_64 and all
configs in arch/arm/configs on arm. I also checked that no new warnings were
introduced by the patch (actually, some warnings are removed that were emitted
by unnecessarily included header files).

Now that port status change notifications are interrupt-driven,
ehci-hcd needs to tell usbcore when a remote-wakeup resume operation
is finished -- we can no longer rely on the core to poll and find
out. This patch (as843) uses the root-hub status timer to force a
poll after the resume is complete.
The patch also changes the test for detecting when the TDRSMDN resume
period has expired. It's necessary to use time_after_eq() instead of
time_after(), since the polling is triggered precisely by a timer.
The same change is made for TDRSTR reset expiration, for consistency.

Switch ehci-hcd to use the new polling scheme, which reports root
hub status changes via the interrupt handler, in an asynchronous
fashion. Doing so disables polling for status changes (whose handler is
rh_timer_func).
Tested on a Geode GX machine, which is now capable of running at =~ 5
timer interrupts per second (in the -rt tree), resulting in significant
power savings.

This patch (as710) adds a sysfs class-device attribute file named
"companion" for EHCI controllers. The file contains a list of port
numbers that are dedicated to the companion controller; by writing a
port number to the file the user can force a high-speed device
attached directly to the computer to run at full speed. (As far as I
know it is not possible to do this for a device attached to an
external hub.) A port is removed from the file by writing the
negative of its port number.
Several users have asked for this facility and it seems like a useful
thing to have. Every now and then one runs across a device which
behaves much better at full speed than at high speed.

This patch (as709) changes the way ehci-hcd presents port status
values for ports owned by the companion controller. It no longer
hides the information; in particular, it allows the core to see the
disconnect event that occurs when a full- or low-speed device is
switched over to the companion. This is required for the next patch
in this series.

The attached patch fixes the unbalanced calls to enable_irq_wake() and
disable_irq_wake() in the AT91 USB Host driver.
It should resolve these kernel messages:

(The original code was debugged before a bug in the genirq wakeup irq

Isochronous queues don't need a dummy TD because the Queue Header
isn't managed by the hardware. This patch (as836) removes the
unnecessary dummy TDs.
The patch also fixes a long-standing typo in a comment (a "don't" was
missing -- potentially very confusing!).

This patch (as768) improves the debugging checks for the uhci-hcd
frame list. The number of entries displayed is limited to 10, and the
driver now checks for the correct Skeleton QH link value at the end of
each chain of Isochronous TDs. The code to compute these link values
is now used in two spots, so it is moved into its own separate
subroutine.

The previous model had the module_init & module_exit function in the
bus glue .c files themselves. That's a problem if several glues need
to be selected at once and the driver is built has module. This case
is quite common in embedded system where you want to handle both the
integrated ohci controller and some extra controller on PCI.
The ohci-hcd.c file now provide the module_init & module_exit and
appropriate driver registering/unregistering is done conditionally,
using #ifdefs.

This patch implements supports for EHCI controllers whose MMIO
registers are big endian and enables that functionality for
the Toshiba SCC chip. It does _not_ add support for big endian
in-memory data structures as this is not needed for that chip
and I hope it will never be.
The guts of the patch are to convert readl(...) to
ehci_readl(ehci, ...) and similarly for register writes.

and big endian descriptors in order to support the Toshiba SCC
implementation which has big endian registers but little endian

This patch applies David Brownell's suggestion for reworking the
OHCI quirk mechanism via a table of PCI IDs. It adapts the existing
quirks to use that mechanism.
This also moves the quirks to reset() as suggested by the comment
in there. This is necessary as we need to have the endian properly
set before we try to init the controller.

Instead of matching all motherboards whose name contains "A7V8X" for a
remote-wakeup hardware bug, this patch (as829) matches only those
boards whose name is exactly equal to "A7V8X". Later motherboards
don't seem to have the bug.
(In fact, it's possible that only one motherboard in the world has the
bug. With only one user reporting problems, it's hard to tell.)

ELAN's U132 is a USB to CardBus OHCI controller adapter,
designed specifically for CardBus 3G data cards to
function in machines without a CardBus slot.
The "ftdi-elan" module is a USB client driver, that detects
a supported CardBus OHCI controller plugged into the
U132 adapter and thereafter provides the conduit for
for access by the "u132-hcd" module.
The "u132-hcd" module is a (cut-down OHCI) host controller
that supports a single OHCI function of the CardBus
card inserted into the U132 adapter.
The problem with the initial implementation is that when
the CardBus card inserted into the U132 adapter has multiple
functions (and a CardBus card can support up to 4 functions),
it was the first function that was arbitrarily choosen.
The first batch of 3G cards tested, like the Merlin Qualcomm
V620, have two functions each supporting a seperate USB OHCI
host controller, of which it was that first function that is
wired up to the 3G modem.
Then along comes the Vodafone Mobile Connect 3G/GPRS data card,
aka "Option GT 3G Quad" as printed on it's rear or "Option N.V.
GlobeTrotter Fusion Quad Lite" as read with "lspci -v". And it
has the meaningful functionality in the second CardBus function.
That presents a problem because it was the "ftdi-elan" module
alone that knows how to communicate to the embedded CardBus slot
and the "u132-hcd" module alone that knows how to access the
pcmcia configuration and CardBus accessible memory space. And
of course, the information about attached (internally hardwired)
devices is contained within USB configuration embedded somewhere
within the CardBus card.
If only the "u132-hcd" module probe() interface could return a
result code that propagated back to the instigating function
platform_device_register() then the "ftdi-elan" module could
try an alternative CardBus function. However in spite of
the recent changes to the drivers/base/ routines that moved
device_attach() from bus_add_device() to bus_attach_device()
both of those routines lose the "failed to attach" 0 result
code and thus the calling routine, namely device_add() is
incapable of propaging the "failed to attach" condition back
to platform_device_add() and consequently back to the caller
of platform_device_register()
Experiments show that patching bus_attach_device() to return
ENODEV fails with the kernel locking up very early during
boot. But, however, if the patch is restricted to calls from
platform_device_add() then it does seem to work.
Unfortunately, until the kernel's drivers/base is properly
modified to propagate -ENODEV back to the caller of
platform_device_register(), it is necessary to "fix" the
"ftdi-elan" module by importing knowledge from the
"u132-hcd" module. This is the reason for the duplicated
functionality introduced in this patch.

I have found a problem where the root_port_reset() goes into an infinite
loop and stalls the kernel.
This happens when a hardware fault inside the machine occurs during a small
timing window. In case of USB device connection, if a USB device responds to
hcd_submit_urb(), and later the controller fails before root_port_reset(),
root_port_reset() will loop infinitely because ohci_readl() will always
return "-1". Such a failure can include ejecting a CardBus OHCI controller.
The probability of this problem is low, but it will increase if PnP type
usage is frequent. The attached patch can solve this problem and I believe
that it is better to fix this problem.

Certain boards seem to like to issue false overcurrent notifications,
for example on ports that don't have anything connected to them. This
looks like a hardware error, at the level of noise to those ports'
overcurrent input signals (or non-debounced VBUS comparators). This
surfaces to users as truly massive amounts of syslog spam from khubd
(which is appropriate for real hardware problems, except for the
volume from multiple ports).
Using this new "ignore_oc" flag helps such systems work more sanely,
by preventing such indications from getting to khubd (and spamming
syslog). The downside is of course that true overcurrent errors will
be masked; they'll appear as spontaneous disconnects, without the
diagnostics that will let users troubleshoot issues like
short-circuited cables. In addition, controllers with no devices
attached will be forced to poll for new devices rather than relying on
interrupts, since each overcurrent event would generate a new
interrupt.
This patch (as826) is essentially a copy of David Brownell's ignore_oc
patch for ehci-hcd, ported to uhci-hcd.

When u132-hcd is built, it includes local header ohci.h, which appears
to have been intended only for use by ohci-hcd.
This throws warnings about functions which are defined and not used.
The warnings thrown are because three small functions are implemented in
the header, but not declared 'inline', a rather strange affair.
Since these functions are small, let's go ahead and define them as
'inline', just like the inline functions surrounding them. This makes
things more consistent, and kills the warnings.

Unlike UHCI, OHCI does not exert any DMA load on the system when no
devices are connected. Consequently there is no advantage to doing
an autostop other than the power savings, so we shouldn't compile the
necessary code unless CONFIG_PM is enabled.
This patch (as820) makes the root-hub suspend and resume routines
conditional on CONFIG_PM. It also prevents autostop from activating
if the device_may_wakeup flag isn't set; some people use this flag to
alert the driver about Resume-Detect bugs in the hardware.

This patch (as738b) fixes numerous problems in the controller/root-hub
suspend/resume/remote-wakeup support in ehci-hcd:
The bus_resume() routine should wake up only the ports that
were suspended by bus_suspend(). Ports that were already
suspended should remain that way.
The interrupt mask is used to detect loss of power in the
bus_resume() routine (if the mask is 0 then power was lost).
However bus_suspend() always sets the mask to 0. Instead the
mask should retain its normal value, with port-change-detect
interrupts disabled if remote wakeup is turned off.
The interrupt mask should be reset to its correct value at the
end of bus_resume() regardless of whether power was lost.
bus_resume() reinitializes the operational registers if power
was lost. However those registers are not in the aux power
well, hence they can lose their values whenever the controller
is put into D3. They should always be reinitialized.
When a port-change interrupt occurs and the root hub is
suspended, the interrupt handler should request a root-hub
resume instead of starting up the controller all by itself.
There's no need for the interrupt handler to request a
root-hub resume every time a suspended port sends a
remote-wakeup request.
The pci_resume() method doesn't need to check for connected
ports when deciding whether or not to reset the controller.
It can make that decision based on whether Vaux power was
maintained.
Even when the controller does not need to be reset,
pci_resume() must undo the effect of pci_suspend() by
re-enabling the interrupt mask.
If power was lost, pci_resume() must not call ehci_run().
At this point the root hub is still supposed to be suspended,
not running. It's enough to rewrite the command register and
set the configured_flag.

Note that the misplaced prototypes for the latter ones in
drivers/usb/host/u132-hcd.c were buggy. Depending on the calling
convention of the architecture calling one of them could have turned
your stack into garbage.

Certain boards seem to like to issue false overcurrent notifications, for
example on ports that don't have anything connected to them. This looks
like a hardware error, at the level of noise to those ports' overcurrent
input signals (or non-debounced VBUS comparators). This surfaces to users
as truly massive amounts of syslog spam from khubd (which is appropriate
for real hardware problems, except for the volume from multiple ports).
Using this new "ignore_oc" flag helps such systems work more sanely, by
preventing such indications from getting to khubd (and spam syslog). The
downside is of course that true overcurrent errors will be masked; they'll
appear as spontaneous disconnects, without the diagnostics that will let
users troubleshoot issues like short circuited cables.
Note that the bulk of these reports seem to be with VIA southbridges, but
I think some were with Intel ones.

EHCI hooks for high speed electrical tests of the root hub ports.
The expectation is that a usermode program actually triggers the test,
making the same control request it would make for an external hub.
Tests for peripheral upstream ports would issue a different request.
In all cases, the hardware needs re-initialization before it could
be used "normally" again (e.g. unplug/replug, rmmod/modprobe).

This patch (as808b) moves the Root Hub Status Change interrupt-disable
code in ohci-hcd back into the interrupt handler proper, to avoid the
chance of adverse interactions with mediocre hardware implementations.
It also deletes the root-hub status timer from within the interrupt-enable
routine. There's no need to poll for status any more once interrupts are
re-enabled.

When a suspended OHCI controller sees a port's status change, it sets
both the Root-Hub-Status-Change and the Resume-Detect bits in the
Interrupt Status register. Processing both these bits, the driver
tries to resume the root hub twice!
This patch (as807) fixes the bug by ignoring RD if RHSC is set. It
also prints a slightly more informative log message when a
remote-wakeup event occurs.

This patch (as798) adds a workaround to uhci-hcd. At least one Asus
motherboard is wired in such a way that any device attached to a
suspended UHCI controller will prevent the system from entering
suspend-to-RAM by immediately waking it up. The only way around the
problem is to turn the controller off instead of suspending it.
This fixes Bugzilla #6193.

Maintain a per-CPU global "struct pt_regs *" variable which can be used instead
of passing regs around manually through all ~1800 interrupt handlers in the
Linux kernel.
The regs pointer is used in few places, but it potentially costs both stack
space and code to pass it around. On the FRV arch, removing the regs parameter
from all the genirq function results in a 20% speed up of the IRQ exit path
(ie: from leaving timer_interrupt() to leaving do_IRQ()).
Where appropriate, an arch may override the generic storage facility and do
something different with the variable. On FRV, for instance, the address is
maintained in GR28 at all times inside the kernel as part of general exception
handling.
Having looked over the code, it appears that the parameter may be handed down
through up to twenty or so layers of functions. Consider a USB character
device attached to a USB hub, attached to a USB controller that posts its
interrupts through a cascaded auxiliary interrupt controller. A character
device driver may want to pass regs to the sysrq handler through the input
layer which adds another few layers of parameter passing.
I've build this code with allyesconfig for x86_64 and i386. I've runtested the
main part of the code on FRV and i386, though I can't test most of the drivers.
I've also done partial conversion for powerpc and MIPS - these at least compile
with minimal configurations.
This will affect all archs. Mostly the changes should be relatively easy.
Take do_IRQ(), store the regs pointer at the beginning, saving the old one:
struct pt_regs *old_regs = set_irq_regs(regs);
And put the old one back at the end:
set_irq_regs(old_regs);
Don't pass regs through to generic_handle_irq() or __do_IRQ().
In timer_interrupt(), this sort of change will be necessary:
- update_process_times(user_mode(regs));
- profile_tick(CPU_PROFILING, regs);
+ update_process_times(user_mode(get_irq_regs()));
+ profile_tick(CPU_PROFILING);
I'd like to move update_process_times()'s use of get_irq_regs() into itself,
except that i386, alone of the archs, uses something other than user_mode().
Some notes on the interrupt handling in the drivers:
(*) input_dev() is now gone entirely. The regs pointer is no longer stored in
the input_dev struct.
(*) finish_unlinks() in drivers/usb/host/ohci-q.c needs checking. It does
something different depending on whether it's been supplied with a regs
pointer or not.
(*) Various IRQ handler function pointers have been moved to type
irq_handler_t.

This patch (as790b) adds "autostop" support to ohci-hcd: the driver
will automatically stop the host controller when no devices have been
connected for at least one second. This feature is useful when the
USB autosuspend facility isn't available, such as when
CONFIG_USB_SUSPEND hasn't been set.

The autosuspend technique used by ohci-hcd doesn't mesh well with the
newer USB core autosuspend code. This patch (as789) removes ohci-hcd's
autosuspend support. Now the driver will be usable, but it won't
automatically go into a low-power state when no devices are connected.
That's for a later patch.

This revamps handling of the hardware "async advance" IRQ, and its watchdog
timer. Basically it dis-entangles that important timeout from the others,
simplifying the associated state and code to make it more robust.
This reportedly improves behavior of EHCI on some systems with VIA chips,
and AFAIK won't affect non-VIA hardware. VIA systems need this code to
recover from silcon bugs whereby the "async advance" IRQ isn't issued.

When ohci-hcd is shutting down (for rmmod or PC-card removal), there is
a window when the device is shut down, HC communication area (->hcca)
is freed, but the core has not called "free_irq" yet. If another device
triggers a shared interrupt in this window, we oops when trying to
access the freed ->hcca.
This patch removes the window by calling free_irq before ->hcca is freed.
The patch is tested at the PC hotplug test rig at Stratus, and with
rmmod by Rafael Wysocki.

The purpose of this patch is to split off the case when a device does
not reply on the lower level (which is reported by HC hardware), and
a case when the device accepted the request, but does not reply at
upper level. This redefinition allows to diagnose issues easier,
without asking the user if the -110 happened "immediately".
The usbmon splits such cases already thanks to its timestamp, but
it's not always available.
I adjusted all drivers which I found affected (by searching for "urb").
Out of tree drivers may suffer a little bit, but I do not expect much
breakage. At worst they may print a few messages.

This "u132-hcd" module is one half of the "driver" for
ELAN's U132 which is a USB to CardBus OHCI controller
adapter. This module needs the "ftdi-elan" module in
order to communicate to CardBus OHCI controller inserted
into the U132 adapter.
When the "ftdi-elan" module detects a supported CardBus
OHCI controller in the U132 adapter it loads this "u132-hcd"
module.
Upon a successful device probe() the single workqueue
is started up which does all the processing of commands
from the USB core that implement the host controller.
The workqueue maintains the urb queues and issues commands
via the functions exported by the "ftdi-elan" module. Each
such command will result in a callback.
Note that the "ftdi-elan" module is a USB client driver.
Note that this "u132-hcd" module is a (cut-down OHCI)
host controller.
Thus we have a topology with the parent of a host controller
being a USB client! This really stresses the USB subsystem
semaphore/mutex handling in the module removal.

root hubs. That support fit in rather awkwardly with the rest of
usbcore and it was used only by ohci-hcd. It won't be needed any more

As part of the ongoing program to flatten out the HCD bus-glue layer,
this patch (as771b) eliminates the hcpriv, release, and kref fields
from struct usb_bus. hcpriv and release were not being used for
anything worthwhile, and kref has been moved into the enclosing
usb_hcd structure.
Along with those changes, the patch gets rid of usb_bus_get and
usb_bus_put, replacing them with usb_get_hcd and usb_put_hcd.
The one interesting aspect is that the dev_set_drvdata call was
removed from usb_put_hcd, where it clearly doesn't belong. This means
the driver private data won't get reset to NULL. It shouldn't cause
any problems, since the private data is undefined when no driver is
bound.

The UHCI controller in my laptop takes longer to turn off the
Resume-Detect bit than the 4 us allowed by uhci-hcd. Presumably other
computers will have the same problem.
This patch (as752) increases the maximum delay to 10 us, which should be
plenty, and uses polling to avoid penalizing systems which can turn the
bit off more quickly.

If some problem occurs during ehci startup, for instance, request_irq fails,
echi hcd driver tries it best to cleanup, but fails to unregister reboot
notifier, which in turn leads to crash on reboot/poweroff.
The following patch resolves this problem by not using reboot notifiers
anymore, but instead making ehci/ohci driver get its own shutdown method. For
PCI, it is done through pci glue, for everything else through platform driver
glue.
One downside: sa1111 does not use platform driver stuff, and does not have its
own shutdown hook, so no 'shutdown' is called for it now. I'm not sure if it
is really necessary on that platform, though.

With the newer Samsung S3C2412 and S3C2413 SoC devices,
the 48MHz USB clock has been given an individual gate
into the USB OHCI and gadget blocks.
This clock is called usb-bus-clock, and we need to
replace the old use of the USB PLL (upll) directly
with the new usb-bus-host.
The S3C2410 clock driver has been updated already to
provide a virtual clock which is a child of the UPLL
to maintain compatibility. The S3C2412 clock driver
correctly enables the PLL when either usb-bus clock
is active.

This teaches OHCI to use the root hub status change (RHSC) IRQ, bypassing
root hub timers most of the time and switching over to the "new" root hub
polling scheme. It's complicated by the fact that implementations of OHCI
trigger and ack that IRQ differently (the spec is vague there).
Avoiding root hub timers helps mechanisms like "dynamic tick" leave the
CPU in lowpower modes for longer intervals.

The following patches reduce the size of the VFS inode structure by 28 bytes
on a UP x86. (It would be more on an x86_64 system). This is a 10% reduction
in the inode size on a UP kernel that is configured in a production mode
(i.e., with no spinlock or other debugging functions enabled; if you want to
save memory taken up by in-core inodes, the first thing you should do is
disable the debugging options; they are responsible for a huge amount of bloat
in the VFS inode structure).
This patch:
The filesystem or device-specific pointer in the inode is inside a union,
which is pretty pointless given that all 30+ users of this field have been
using the void pointer. Get rid of the union and rename it to i_private, with
a comment to explain who is allowed to use the void pointer. This is just a
cleanup, but it allows us to reuse the union 'u' for something something where
the union will actually be used.

Apparently some UHCI controllers change the value of the Short Packet
Detect (SPD) bit in the TD status word -- presumably when they receive a
short packet. This patch (as759) changes uhci-hcd to avoid assuming
that the bit is unchanged; in fact, the driver no longer looks at SPD at
all.
This fixes the second problem reported in Bugzilla #6752.

OHCI updates for AT91 series processors:
- Get ready for at91sam926x processors (ARMv5tej not ARMv4t)
- Suspend/resume support now behaves properly
- In "standby" mode, OHCI can be a source of system wakeup events
(remote wakeup, device connect/disconnect, etc)
And minor cleanups.

In a rare and all-but-unused path, the EHCI driver could reuse a variable
in a way that'd make trouble. Specifically, if the first root hub port
gets an overcurrent event (rare) during a remote wakeup scenario (all but
unused in today's Linux, except for folk working with suspend-to-RAM and
similar sleep states), that would look like a fatal error which would shut
down the controller. Fix by not reusing that variable.

I received an DBAU1200 eval kit from AMD a few days ago and tried to
enable the USB2 port, but the current linux-2.6 GIT did not even
compile with CONFIG_SOC_1200, CONFIG_SOC_AU1X00, CONFIG_USB_EHCI and
CONFIG_USB_OHCI set.
Furthermore, in ehci-hcd.c, platform_driver_register() was called with
an improper argument of type 'struct device_driver *' which of course
ended up in a kernel oops. How could that ever have worked on your
machines?
Anyway, here's a trivial patch that makes the USB subsystem working
on my board for both OHCI and EHCI.
It also removes the /* FIXME use "struct platform_driver" */.

This patch (as707) improves the FSBR operation in uhci-hcd by turning it
off more quickly when it isn't needed. FSBR puts a noticeable load on a
computer's PCI bus, so it should be disabled as soon as possible when it
isn't in use. The patch leaves it running for only 10 ms after the last
URB stops using it, on the theory that this should be long enough for a
driver to submit another URB if it wants keep FSBR going.

This patch (as706) removes the private hc_inaccessible flag from
uhci-hcd. It's not needed because it conveys exactly the same
information as the generic HCD_FLAG_HW_ACCESSIBLE bit.
In its place goes a new flag recording whether the controller is dead.
The new code allows a complete device reset to resurrect a dead
controller (although usbcore doesn't yet implement such a facility).

This patch (as705) contains a small set of updates for uhci-hcd written
mostly by Dave Brownell:
* Root hub suspend messages come out labeled as root hub messages;
PCI messages should only come out when the pci device suspends.
* Rename the reset() method to better match its init() role
* Behave more like the other HCDs by returning -ESHUTDOWN for root-hub
suspend/resume errors.
* When an URB fails, associate the message with the usb device not
the host controller (it still hides endpoint and direction)

This fixes coverity Bug #390.
With the following code
ret = ep->branch = balance(isp116x, ep->period, ep->load);
if (ret < 0)
goto fail;
the problem is that ret and balance are of the type int, and ep->branch is u16.
so the int balance() returns gets reduced to u16 and then converted to an int again,
which removes the sign. Maybe the following little c program can explain it better:
This updates the EHCI driver by adding an improved scheduler for the
transaction translators, found in USB 2.0 hubs and used for low and
full speed devices.
- adds periodic_tt_usecs() and some helper functions, which does
the same thing that "periodic_usecs" does, except on the other
side of the TT, i.e. it calculates the low/fullspeed bandwidth
usage instead of highspeed.
- adds a tt_available() function which is the new implementation
of what tt_no_collision() does ... while tt_no_collision() ensures
that each TT handles only 1 periodic transfer at a time (a very
pessimistic approach) this version instead tracks bandwidth and
allows each TT to handle as many transfers as will fit on each TT's
downstream bus (closer to best-case).
The new scheduler is selected by a config option, marked as EXPERIMENTAL
so it can be tested (and more broadly reviewed) for a while until it
seems safe to remove the original scheduler.

This patch (as690) does the same thing for ISO TDs as as680 did for
non-ISO TDs: free them as they are used rather than all at once when an
URB is complete. At the same time it fixes a minor buglet (I'm not
aware of it ever affecting anyone): An ISO TD should be retired when its
frame is over, regardless of whether or not the hardware has marked it
inactive.

This patch (as689) stores the period for periodic transfers (interrupt
and ISO) in the queue header. This is necessary for proper bandwidth
tracking (not yet implemented). It also makes the scheduling of ISO
transfers a bit more rigorous, with checks for out-of-bounds frame
numbers.

This patch (as688) fixes a small race in uhci-hcd. Because ISO queues
aren't controlled by queue headers, they can't be unlinked. Only
individual URBs can. So whenever multiple ISO URBs are dequeued, it's
necessary to make sure the hardware is done with each one. We can't
assume that dequeuing the first URB will suffice to unlink the entire
queue.

This patch (as687) changes uhci-hcd to keep track of frame numbers as
full-sized integers rather than 11-bit values. This makes them a lot
easier to handle and makes it possible to schedule beyond a 2-second
window, should anyone ever want to do so.

Some old Intel UHCI controllers have a bug that has shown up in a few
systems (the PIIX3 "Neptune" chip set). Until now there has not been
any simple way to work around the bug, but the lastest changes in
uhci-hcd have made it easy. This patch (as684) adds the work-around.

This patch (as683) re-implements Full-Speed Bandwidth Reclamation (FSBR)
properly. It keeps track of which endpoint queues have advanced, and
when none have advanced for a sufficiently long time, FSBR is turned
off. The next TD on each of the non-moving queues is modified to
generate an interrupt on completion, so that FSBR can be re-enabled as
soon as the hardware starts to make some progress.

This patch (as682) gets rid of the TD-removal list in uhci-hcd. It is
no longer needed because now TDs are not freed until we know the
hardware isn't using them. It also simplifies the code for adding and
removing TDs to/from URBs.
