Fix up trivial conflict (spelink errurs) in drivers/media/video/omap3isp/isp.c
i915 calls the panic handler function on last close to reset the modes,
however this is a really bad idea for multi-gpu machines, esp shareable
gpus machines. So add a new entry point for the driver to just restore
its own fbcon mode.
v2: move code into fb helper, fix panic code to block mode change on
powered off GPUs.

F15h CPUs may report a non-DRAM address when reporting an error address
belonging to a CC6 state save area. Add a workaround to detect this
condition and compute the actual DRAM address of the error as documented
in the Revision Guide for AMD Family 15h Models 00h-0Fh Processors.

F15h and later use a portion of DRAM as a CC6 storage area. BIOS
programs D18F1x[17C:140,7C:40] DRAM Base/Limit accordingly by
subtracting the storage area from the DRAM limit setting. However, in
order for edac to consider that part of DRAM too, we need to include it
into the per-node range.

This warning was wrongfully added for a normal condition - intlvsel
actually selects the destination node when node interleaving is enabled
and it is not a mismatch. For a detailed example, see section 2.8.10.2
"Node Interleaving" in F10h BKDG.

There is at least one BIOS with a DSDT containing a power resource
object with a _PR0 entry pointing back to that power resource. In
consequence, while registering that power resource
acpi_bus_get_power_flags() sees that it depends on itself and tries
to register it again, which leads to an infinitely deep recurrence.
This problem was introduced by commit bf325f9538
(ACPI / PM: Register power resource devices as soon as they are
needed).
To fix this problem use the observation that power resources cannot
be power manageable and prevent acpi_bus_get_power_flags() from
being called for power resource objects.

It turns out that some PCI devices are only found to be
wakeup-capable during registration, in which case, when
device_set_wakeup_capable() is called, device_is_registered() already
returns 'true' for the given device, but dpm_sysfs_add() hasn't been
called for it yet. This leads to situations in which the device's
power.can_wakeup flag is not set as requested because of failing
wakeup_sysfs_add() and its wakeup-related sysfs files are not
created, although they should be present. This is a post-2.6.38
regression introduced by commit cb8f51bdad
(PM: Do not create wakeup sysfs files for devices that cannot wake
up).
To work around this problem initialize the device's power.entry
field to an empty list head and make device_set_wakeup_capable()
check if it is still empty before attempting to add the devices
wakeup-related sysfs files with wakeup_sysfs_add(). Namely, if
power.entry is still empty at this point, device_pm_add() hasn't been
called yet for the device and its wakeup-related files will be
created later, so device_set_wakeup_capable() doesn't have to create
them.

vhci_rx/vhci_tx threads are created once but stopped each
time the vdev is shut down. On subsequent attach wake_up_process()
oopses trying to access the stopped threads.
This patch does as before the kthread conversion which is to
create the threads each time a device is attached and stop the
threads when the device is shut down.

The default maximum transmit length for NCM USB frames should be so
that a short packet happens at the end if the device supports a length
greater than the defined maximum. This is achieved by adding 4 bytes
to the maximum length so that the existing logic can fit a short
packet there.

There's a code path in pmcraid that can be reached via device ioctl that
causes all sorts of ugliness, including heap corruption or triggering
the OOM killer due to consecutive allocation of large numbers of pages.
Not especially relevant from a security perspective, since users must
have CAP_SYS_ADMIN to open the character device.
First, the user can call pmcraid_chr_ioctl() with a type
PMCRAID_PASSTHROUGH_IOCTL. A pmcraid_passthrough_ioctl_buffer
is copied in, and the request_size variable is set to
buffer->ioarcb.data_transfer_length, which is an arbitrary 32-bit signed
value provided by the user.
If a negative value is provided here, bad things can happen. For
example, pmcraid_build_passthrough_ioadls() is called with this
request_size, which immediately calls pmcraid_alloc_sglist() with a
negative size. The resulting math on allocating a scatter list can
result in an overflow in the kzalloc() call (if num_elem is 0, the
sglist will be smaller than expected), or if num_elem is unexpectedly
large the subsequent loop will call alloc_pages() repeatedly, a high
number of pages will be allocated and the OOM killer might be invoked.
Prevent this value from being negative in pmcraid_ioctl_passthrough().

SCSI uses request_queue->queuedata == NULL as a signal that the queue
is dying. We set this state in the sdev release function. However,
this allows a small window where we release the last reference but
haven't quite got to this stage yet and so something will try to take
a reference in scsi_request_fn and oops. It's very rare, but we had a
report here, so we're pushing this as a bug fix
The actual fix is to set request_queue->queuedata to NULL in
scsi_remove_device() before we drop the reference. This causes
correct automatic rejects from scsi_request_fn as people who hold
additional references try to submit work and prevents anything from
getting a new reference to the sdev that way.

At two points in handling device ioctls via /dev/mpt2ctl, user-supplied
length values are used to copy data from userspace into heap buffers
without bounds checking, allowing controllable heap corruption and
subsequently privilege escalation.
Additionally, user-supplied values are used to determine the size of a
copy_to_user() as well as the offset into the buffer to be read, with no
bounds checking, allowing users to read arbitrary kernel memory.

The firmware is cached during the first successfull call to open() and
released once the network device is unregistered. The driver uses the
cached firmware between open() and unregister_netdev().
So far the firmware is optional : a failure to load the firmware does
not prevent open() to success. It is thus necessary to 1) unregister
all 816x / 810[23] devices and 2) force a driver probe to issue a new
firmware load.

At the end of section 10.1 of AHCI spec (rev 1.3), it states
Software shall not set PxCMD.ST to 1 until it is determined that
a functoinal device is present on the port as determined by
PxTFD.STS.BSY=0, PxTFD.STS.DRQ=0 and PxSSTS.DET=3h
Even though most AHCI host controller works without this check,
specific controller will fail under this condition.

The AT91SAM9 microcontrollers with master clock higher then 105 MHz
and PIO0, have overflow of the NCS_RD_PULSE value in the MSB. This
lead to "NCS_RD_PULSE" pulse longer then "NRD_CYCLE" pulse and driver
does not detect ATA device.

the Pioneer DVR-212D not handling SETXFER correctly. An openSUSE user
reported a similar issue with his DVR-216D that the NOSETXFER horkage
worked around for him as well.

The ahci_pmp_attach() & ahci_pmp_detach() unmask port irqs, but they
are also called during port initialization, before ahci host irq
handler is registered. On ce4100 platform, this sometimes triggers
"irq 4: nobody cared" message when loading driver.
Fixed this by not touching the register if the port is in frozen
state, and mark all uninitialized port as frozen.

A deadlock was reported to me recently that occured when netconsole was being
used in a virtual guest. If the virtio_net driver was removed while netconsole
was setup to use an interface that was driven by that driver, the guest
deadlocked. No backtrace was provided because netconsole was the only console
configured, but it became clear pretty quickly what the problem was. In
netconsole_netdev_event, if we get an unregister event, we call
__netpoll_cleanup with the target_list_lock held and irqs disabled.
__netpoll_cleanup can, if pending netpoll packets are waiting call
cancel_delayed_work_sync, which is a sleeping path. the might_sleep call in
that path gets triggered, causing a console warning to be issued. The
netconsole write handler of course tries to take the target_list_lock again,
which we already hold, causing deadlock.
The fix is pretty striaghtforward. Simply drop the target_list_lock and
re-enable irqs prior to calling __netpoll_cleanup, the re-acquire the lock, and
restart the loop. Confirmed by myself to fix the problem reported.

The mechanism used to initiate work events from the interrupt
handler has a classic read/modify/write race between the interrupt
handler that sets the condition, and the worker task that reads and
clears the condition. Close these races by using atomic
bit fields.

The commit 609ff3b ("be2net: add code to display temperature of ASIC")
adds support to display temperature of ASIC but there is missing
increment of work_counter in be_worker. Because of this 1) the
function be_cmd_get_die_temperature is called every 1 second instead
of every 32 seconds 2) be_cmd_get_die_temperature is called, although
it is not supported. This patch fixes this bug.

mac-fec.c was setting individual UDP address registers instead of multicast
group address registers when joining a multicast group.
This prevented from correctly receiving UDP multicast packets.
According to datasheet, replaced hash_table_high and hash_table_low
with grp_hash_table_high and grp_hash_table_low respectively.
Also renamed hash_table_* with grp_hash_table_* in struct fec declaration
for 8xx: these registers are used only for multicast there.
Tested on a MPC5121 based board.
Build tested also against mpc866_ads_defconfig.

check_events() implementations in both ide-gd and ide-cd are
inadequate for in-kernel event polling. Both generate media change
events continuously when certain conditions are met causing infinite
event loop between the driver and userland event handler.
As disk event now supports suppression of unlisted events, simply
de-listing DISK_EVENT_MEDIA_CHANGE from disk->events resolves the
problem. Internal handling around media revalidation will behave the
same while userland will fall back to userland event polling after
detecting the device doesn't support disk events.

This call was disabled as hot-unplugging one virtconsole port led to
another virtconsole port freezing.
Upon testing it again, this now works, so enable it.
In addition, a bug was found in qemu wherein removing a port of one type
caused the guest output from another port to stop working. I doubt it
was just this bug that caused it (since disabling the hvc_remove() call
did allow other ports to continue working), but since it's all solved
now, we're fine with hot-unplugging of virtconsole ports.

In the case where a virtio-console port is in use (opened by a program)
and a virtio-console device is removed, the port is kept around but all
the virtio-related state is assumed to be gone.
When the port is finally released (close() called), we call
device_destroy() on the port's device. This results in the parent
device's structures to be freed as well. This includes the PCI regions
for the virtio-console PCI device.
Once this is done, however, virtio_pci_release_dev() kicks in, as the
last ref to the virtio device is now gone, and attempts to do
pci_iounmap(pci_dev, vp_dev->ioaddr);
pci_release_regions(pci_dev);
pci_disable_device(pci_dev);
which results in a double-free warning.
Move the code that releases regions, etc., to the virtio_pci_remove()
function, and all that's now left in release_dev is the final freeing of
the vp_dev.

When detaching a buffer from a vq, the avail.idx value should be
decremented as well.
This was noticed by hot-unplugging a virtio console port and then
plugging in a new one on the same number (re-using the vqs which were
just 'disowned'). qemu reported
'Guest moved used index from 0 to 256'
when any IO was attempted on the new port.

on each VT-d engine, after DMA remapping is enabled on the engines.
This is because the behavior of having both enabled is not deterministic
and because, if TXT has been used to launch the kernel, the PMRs may be
programmed to cover memory regions that will be used for DMA.
Under some circumstances (certain quirks detected, lack of multiple
devices, etc.), the current code does not set up DMA remapping on some
VT-d engines. In such cases it also skips disabling the PMRs. This
causes failures when the kernel is launched with TXT (most often this
occurs on the graphics engine and results in colored vertical bars on
the display).
This patch detects when the kernel has been launched with TXT and then
disables the PMRs on all VT-d engines. In some cases where the reason
that remapping is not being enabled is due to possible ACPI DMAR table
errors, the VT-d engine addresses may not be correct and thus not able
to be safely programmed even to disable PMRs. Because part of the TXT
launch process is the verification of these addresses, it will always be
safe to disable PMRs if the TXT launch has succeeded and hence only
doing this in such cases.

pg_start is copied from userspace on AGPIOC_BIND and AGPIOC_UNBIND ioctl
cmds of agp_ioctl() and passed to agpioc_bind_wrap(). As said in the
comment, (pg_start + mem->page_count) may wrap in case of AGPIOC_BIND,
and it is not checked at all in case of AGPIOC_UNBIND. As a result, user
with sufficient privileges (usually "video" group) may generate either
local DoS or privilege escalation.

page_count is copied from userspace. agp_allocate_memory() tries to
check whether this number is too big, but doesn't take into account the
wrap case. Also agp_create_user_memory() doesn't check whether
alloc_size is calculated from num_agp_pages variable without overflow.
This may lead to allocation of too small buffer with following buffer
overflow.
Another problem in agp code is not addressed in the patch - kernel memory
exhaustion (AGPIOC_RESERVE and AGPIOC_ALLOCATE ioctls). It is not checked
whether requested pid is a pid of the caller (no check in agpioc_reserve_wrap()).
Each allocation is limited to 16KB, though, there is no per-process limit.
This might lead to OOM situation, which is not even solved in case of the
caller death by OOM killer - the memory is allocated for another (faked) process.

The rtc_device_register() call has changed semantics so that it
will immediately call out to rtc_read_alarm() and since the
callbacks require the drvdata to be set, we need to set it before

The slave member of struct aggregator does not necessarily point
to a slave which is part of the aggregator. It points to the
slave structure containing the aggregator structure, while
completely different slaves (or no slaves at all) may be part of
the aggregator.
The agg_device_up() function wrongly uses agg->slave to find the state
of the aggregator. Use agg->lag_ports->slave instead. The bug has
been introduced by commit 4cd6fe1c64
("bonding: fix link down handling in 802.3ad mode").

The qdio hardware may surpress further interrupts as long as a SBAL is in
the error state. That can lead to unnotified data in the SBALs following
the error state. To prevent this behaviour change the SBAL[s] in error
state immediately to another program owned state so interrupts are again
received for further traffic on the device.

The dasd_open function uses the private_data pointer of the gendisk to
find the dasd_block structure that matches the gendisk. When a DASD
device is set offline, we set the private_data pointer of the gendisk
to NULL and later remove the dasd_block structure, but there is still
a small race window, in which dasd_open could first read a pointer
from the private_data field and then try to use it, after the structure
has already been freed.
To close this race window, we will store a pointer to the dasd_devmap
structure of the base device in the private_data field. The devmap
entries are not deleted, and we already have proper locking and
reference counting in place, so that we can safely get from a devmap
pointer to the dasd_device and dasd_block structures of the device.

After raid4->raid0 takeover operation, another takeover operation
(e.g raid0->raid10) results "kernel oops".
Root cause:
Variables 'degraded' in mddev structure is not cleared

A raid0 array doesn't set 'dev_sectors' as each device might
contribute a different number of sectors.
So when converting to a RAID4 or RAID5 we need to set dev_sectors
as they need the number.
We have already verified that in fact all devices do contribute
the same number of sectors, so use that number.

We previously needed to set ->queue_lock to match the raid5
device_lock so we could safely use queue_flag_* operations (e.g. for
plugging). which test the ->queue_lock is in fact locked.
However that need has completely gone away and is unlikely to come
back to remove this now-pointless setting.

Problem description:
gsm_queue() calculate a CRC for arrived frames. As a last step of
CRC calculation it call
gsm->fcs = gsm_fcs_add(gsm->fcs, gsm->received_fcs);
This work perfectly for the case of GSM0 mode as gsm->received_fcs
contain the last piece of data required to generate final CRC.
gsm->received_fcs is not used for GSM1 mode. Thus we put an
additional byte to CRC calculation. As result we get a wrong CRC
and reject incoming frame.

If cts changes between reading the level at the cts input (USR1_RTSS)
and acking the irq (USR1_RTSD) the last edge doesn't generate an irq and
uart_handle_cts_change is called with a outdated value for cts.

nouveau_bo_wr32 expects offset to be in words, but we pass value in bytes,
so after commit 73412c3854 ("drm/nouveau: allocate
kernel's notifier object at end of block") we started to overwrite some memory
after notifier buffer object (previously m2mf_ntfy was always 0, so it didn't
matter it was a value in bytes).

Device suspend/resume infrastructure is used not only by the suspend
and hibernate code in kernel/power, but also by APM, Xen and the
kexec jump feature. However, commit 40dc166cb5
(PM / Core: Introduce struct syscore_ops for core subsystems PM)
failed to add syscore_suspend() and syscore_resume() calls to that
code, which generally leads to breakage when the features in question
are used.
To fix this problem, add the missing syscore_suspend() and
syscore_resume() calls to arch/x86/kernel/apm_32.c, kernel/kexec.c
and drivers/xen/manage.c.

Videobuf2 core assumes that driver doesn't set any buffer flags.
This is correct for buffer state flags that videobuf2 manages,
but the other flags like V4L2_BUF_FLAG_{KEY,P,B}FRAME,
V4L2_BUF_FLAG_TIMECODE and V4L2_BUF_FLAG_INPUT should be passed from or to
the driver.

Protecting the color format with vb2_is_streaming() is not sufficient
as this prevents changing the format only after VIDIOC_STREAMON.
To prevent the color format reconfiguration as soon as buffers
are allocated use vb2_is_busy() instead.

When open a device node, all entities part of the same pipeline are
powered on. If one of the entities fails to be powered on, the open
operations fails. In that case the device node entity use count must not
be incremented.

We are currently using this flag to check whether it's safe
to call into ->request_fn(). If it is set, we punt to kblockd.
But we get a lot of false positives and excessive punts to
kblockd, which hurts performance.
The only real abuser of this infrastructure is SCSI. So export
the async queue run and convert SCSI over to use that. There's
room for improvement in that SCSI need not always use the async
call, but this fixes our performance issue and they can fix that
up in due time.

Mouse gets "stuck" after restore of PV guest but buttons are in working
condition.
If driver has been configured for ABS coordinates at start it will get
XENKBD_TYPE_POS events and then suddenly after restore it'll start getting
XENKBD_TYPE_MOTION events, that will be dropped later and they won't get
into user-space.
Regression was introduced by hunk 5 and 6 of
5ea5254aa0ad269cfbd2875c973ef25ab5b5e9db
("Input: xen-kbdfront - advertise either absolute or relative
coordinates").
Driver on restore should ask xen for request-abs-pointer again if it is
available. So restore parts that did it before 5ea5254.

For some reason, sending QoS configuration causes transmission to stop
after a single frame on HT channels when not associated. Removing the
extra QoS configuration has no effect on station mode, and fixes
injection mode.

add a new blk_run_queue_async helper to do it explicitly. I've kept
the blk_queue_stopped check for now, but I suspect it's not needed
as the check we do when the workqueue items runs should be enough.

In raid5 plugging is used for 2 things:
1/ collecting writes that require a bitmap update
2/ collecting writes in the hope that we can create full
stripes - or at least more-full.
We now release these different sets of stripes when plug_cnt
is zero.
Also in make_request, we call mddev_check_plug to hopefully increase
plug_cnt, and wake up the thread at the end if plugging wasn't
achieved for some reason.

When an md device adds a request to a queue, it can call
mddev_check_plugged.
If this succeeds then we know that the md thread will be woken up
shortly, and ->plug_cnt will be non-zero until then, so some
processing can be delayed.
If it fails, then no unplug callback is expected and the make_request
function needs to do whatever is required to make the request happen.

md has some plugging infrastructure for RAID5 to use because the
normal plugging infrastructure required a 'request_queue', and when
called from dm, RAID5 doesn't have one of those available.
This relied on the ->unplug_fn callback which doesn't exist any more.
So remove all of that code, both in md and raid5. Subsequent patches
with restore the plugging functionality.

Several tests in the ipv6 routing code check IFF_LOOPBACK, and
allowing stacking such as VLAN'ing on top of loopback results in a
netdevice which reports IFF_LOOPBACK but really isn't the loopback
device.
Instead of spamming the ipv6 routing code with even more special tests,
simply disallow VLAN over loopback.
The result of this patch is:

When a failure to stop a module in the pipeline is detected, the only
way to recover is to reset the ISP. However, as other users can be using
a different pipeline with other modules, the ISP can't be reset
synchronously with the error detection.
Mark the ISP as needing a reset when a failure to stop a pipeline is
detected, and reset the ISP when the last user releases the last
reference to the ISP.
Modify the omap3isp_pipeline_set_stream() function to record the new ISP
pipeline state only when no error occurs, except when stopping the
pipeline in which case the pipeline is still marked as stopped.

As the number of phases/taps, used to select the correct equations to
compute the ratio, depends on the ratio, start with the 7-tap mode
equations to compute an approximation of the ratio, and switch to the
4-tap mode equations if the approximation is lower than or equal to 512.

Apparently some distros set i2c-algo-bit.bit_test to 1 by
default. In some cases this causes i2c_bit_add_bus
to fail and prevents the i2c bus from being added. In the
radeon case, we fail to add the ddc i2c buses which prevents
the driver from being able to detect attached monitors.
The i2c bus works fine even if bit_test fails. This is likely
due to gpio switching that is required and handled in the
pre/post_xfer hooks, so call the pre/post_xfer hooks in the
bit test as well.

This patch fixes two issues:
- A disabled crtc does not use any lb, so return 0 for
lb size. This makes the display priority calculation
more exact.
- Only use 1/2 and whole lb partitions. Using smaller
partitions can cause underflow to one of the displays
if you have multiple large displays on the same lb.

Fix a possible problem with mport registration left non-cleared after
fsl_rio_setup() exits on link error. Abort mport initialization if
registration failed.
This patch is applicable to 2.6.39-rc1 only. The problem does not exist
for earlier versions.

Make the driver aware of the initial status of the regulator.
The leds-regulator driver was ignoring the initial status of the
regulator; this resulted in rdev->use_count being incremented to 2 after
calling regulator_led_set_value() in the .probe method when a regulator
was already enabled at insmod time, which made it impossible to ever
disable the regulator.

Sometimes the i2c test in i2c_bit_add_bus fails
if this happens we fail to register the i2c adapter and
eventually fail to add the connector. If i2c fails,
add the connector to the user can at least force
it on.
Note that some distros set i2c-algo-bit.bit_test to 1 by
default which sometimes fails preventing the ddc i2c adapter
from being added. The i2c adapter works even if the bit test
fails, probably due to pre/post_xfer not getting called in
the test_bit function. I have another patch to follow
up on that.

During a kernel crash, bna control path state machine and firmware do not
get a notification and hence are not cleanly shutdown. The registers
holding driver/IOC state information are not reset back to valid
disabled/parking values. This causes subsequent driver initialization
to hang during kdump kernel boot. This patch, during the initialization
of first PCI function, resets corresponding register when unclean shutown
is detect by reading chip registers. This will make sure that ioc/fw
gets clean re-initialization.

This patch disable the optional PM feature inside the Hudson3 platform under
the following conditions:
1. If an isochronous device is connected to xHCI port and is active;
2. Optional PM feature that powers down the internal Bus PLL when the link is
in low power state is enabled.
The PM feature needs to be disabled to eliminate PLL startup delays when the
link comes out of low power state. The performance of DMA data transfer could
be impacted if system delay were encountered and in addition to the PLL start
up delays. Disabling the PM would leave room for unpredictable system delays
in order to guarantee uninterrupted data transfer to isochronous audio or
video stream devices that require time sensitive information. If data in an
audio/video stream was interrupted then erratic audio or video performance
may be encountered.
AMD PLL quirk is already implemented in OHCI/EHCI driver. After moving the
quirk code to pci-quirks.c and export them, xHCI driver can call it directly
without having the quirk implementation in itself.

On a resume, when the power is lost during hibernate, the USB core will
call hub_reset_resume for the xHCI USB 2.0 roothub, but not for the USB
3.0 roothub:

This patch clear PORT_POWER when suspend a USB3.0 device behind a USB3.0
external hub, so the system can suspend and resume.
Note USB3.0 device may not work after system resume and this is a temporary
workaround. The correct fix will be in future patches.

If I unplug a device while the UAS driver is loaded, I get an oops
in usb_free_streams(). This is because usb_unbind_interface() calls
usb_disable_interface() which calls usb_disable_endpoint() which sets
ep_out and ep_in to NULL. Then the UAS driver calls usb_pipe_endpoint()
which returns a NULL pointer and passes an array of NULL pointers to
usb_free_streams().
I think the correct fix for this is to check for the NULL pointer
in usb_free_streams() rather than making the driver check for this
situation. My original patch for this checked for dev->state ==
USB_STATE_NOTATTACHED, but the call to usb_disable_interface() is
conditional, so not all drivers would want this check.
Note from Sarah Sharp: This patch does avoid a potential dereference,
but the real fix (which will be implemented later) is to set the
.soft_unbind flag in the usb_driver structure for the UAS driver, and
all drivers that allocate streams. The driver should free any streams
when it is unbound from the interface. This avoids leaking stream rings
in the xHCI driver when usb_disable_interface() is called.
This should be queued for stable trees back to 2.6.35.

Currently, when resetting a device, xHCI driver disables all but one
endpoints and frees their rings, but leaves alone any streams that
might have been allocated. Later, when users try to free allocated
streams, we oops in xhci_setup_no_streams_ep_input_ctx() because
ep->ring is NULL.
Let's free not only rings but also stream data as well, so that
calling free_streams() on a device that was reset will be safe.
This should be queued for stable trees back to 2.6.35.

When we get a port status change event, we need to figure out what type of
port it came from: a USB 3.0 port, or a USB 2.0/1.1 port. We can't know
which usb_hcd to use until that point, so hcd will be NULL for part of the
function. Unfortunately, if any of the sanity checks fail, we'll jump to
the cleanup label before hcd is set to a valid pointer, and then we'll
attempt to tell the USB core to kick the hcd, which is NULL.
Skip kicking the roothub if the sanity checks fail.

When parsing exponent-expressed intervals we subtract 1 from the
value and then expect it to match with original + 1, which is
highly unlikely, and we end with frequent spew:
usb 3-4: ep 0x83 - rounding interval to 512 microframes
Also, parsing interval for fullspeed isochronous endpoints was
incorrect - according to USB spec they use exponent-based
intervals (but xHCI spec claims frame-based intervals). I trust
USB spec more, especially since USB core agrees with it.
This should be queued for stable kernels back to 2.6.31.

The logic of the handling Missed Service Error Events was pretty
confusing as we were checking the same condition several times.
In addition, it caused compiler warning since the compiler could
not figure out that event_trb is actually unused in case we are
skipping current TD.
Fix that by rearranging "skip" condition checks, and factor out
skip_isoc_td() so that it is called explicitly.

Isochronous and interrupt SuperSpeed endpoints use the same mechanisms
for decoding bInterval values as HighSpeed ones so adjust the code
accordingly.
Also bandwidth reservation for SuperSpeed matches highspeed, not
low/full speed.

This patch moves the HcInterrupt register write to clear the
pending interrupt to after the isr work is done, doing this removes
glitches in the irq line.

The driver did not take the zero flag in the USB request. If the
request length is the same as the endpoint's maxpacket, an additional
ZLP with no data has to be transmitted.
The method used here is inspired to what is done in fsl_udc_core.c
(and pxa27x_udc.c and at91_udc.c) where this is supported.
There already was a discussion about this topic with people from
Keymile, and I propose here a better implementation:

Also I did a small cleanup by changing ep->dev->lock for just dev->lock.
They're the same lock, but dev->lock is shorter and that's how it is
used for the spin_unlock_irqrestore() call.

It turns out that this function calls pci_get_device under a spin_lock
with irqs disabled, but the pci_get_device function is only allowed in
preemptible context.
This patch fixes the warning by making all data-structure
modifications on temporal storage and commiting this back
into the visible structure at the end. While at it, this
patch also moves the pci_dev_put calls out of the spinlocks
because this function might sleep too.

This patch (as1458) fixes a problem affecting ultra-reliable systems:
When hardware failover of an EHCI controller occurs, the data
structures do not get released correctly. This is because the routine
responsible for removing unused QHs from the async schedule assumes
the controller is running properly (the frame counter is used in
determining how long the QH has been idle) -- but when a failover
causes the controller to be electronically disconnected from the PCI
bus, obviously it stops running.
The solution is simple: Allow scan_async() to remove a QH from the
async schedule if it has been idle for long enough _or_ if the
controller is stopped.

During processing of bunch of eem frames if "echo" command is found
skb is cloned and the cloned version should be used to send reply.
Unfortunately, the data of the original skb were actually used and
the cloned skb is never freed.
Using the cloned skb and freeing the skb in the completion callback
for usb request.

ep_read() acquires data->lock mutex in get_ready_ep() and releases it on
all paths except for one: when usb_endpoint_xfer_isoc() failed. The
patch adds mutex_unlock(&data->lock) at that path.

priv->tx_power_next is not initialized to max supported power,
but instead default value is used, what cause errors like

Machines are getting deadlock in four node cluster environment.
All nodes are accessing (find /gfs2 -depth -print|cpio -ocv > /dev/null)
200 GB storage on a GFS2 filesystem.
This result in memory fragmentation and driver receives 18 frags for
1448 byte packets.
For non tso packet, fw drops the tx request, if it has >14 frags.
Fixing it by pulling extra frags.

We were using uninitialised watermarks values for disabled pipes which
were combined into a single WM register and so corrupting the values for
the enabled pipe and upsetting the display hardware.

When the crop rectangle needs to be modified to match hardware
requirements, center the resulting rectangle on the requested rectangle

If the camera host driver (sh_mobile_ceu_camera.c) is unloaded and then
reloaded, probe will fail, because camera client .set_bus_param() and
.query_bus_param() methods have been set to NULL. Fix this by caching
the original pointers and restoring them on driver-unbind.

With the introduction of CSI2 support on sh-mobile, the host driver
switched to using v4l2_device_call_until_err() with grp_id == 0 to
call subdev operations on the sensor and the CSI2 subdev. However,
this has broken multi-client set ups like the one on migor, because
that way all operations get called on both clients. To fix this add
a grp_id and set it to the client private context.
