We weren't properly NULL terminating protocol error strings for our debug
printk resulting in garbage being included in the output when debug was
enabled.

Update license boilerplate to specify GPLv2 and remove the (at your option
clause). This change was agreed to by all the copyright holders (approvals

Fix more include file problems that surfaced since I submitted the previous
fix-missing-includes.patch. This should now allow not to include sched.h
from module.h, which is done by a followup patch.
