Bart Van Assche 7760e22350 scsi: Remove Scsi_Host.uspace_req_q
A patch I wrote myself several years ago removed SCSI target support
from the code under drivers/scsi. That patch removed the code that sets
uspace_req_q to a non-NULL value. Hence also remove the code that
depends on uspace_req_q != NULL.

Bart Van Assche 64104f7032 scsi: Call scsi_initialize_rq() for filesystem requests
If a pass-through request is submitted then blk_get_request()
initializes that request by calling scsi_initialize_rq(). Also call this
function for filesystem requests. Introduce CMD_INITIALIZED to keep
track of whether or not a request has already been initialized.

Christoph Hellwig 651a013649 scsi: scsi_transport_sas: switch to bsg-lib for SMP passthrough
Simplify the SMP passthrough code by switching it to the generic bsg-lib
helpers that abstract away the details of the request code, and gets
drivers out of seeing struct scsi_request.

For the libsas host SMP code there is a small behavior difference in
that we now always clear the residual len for successful commands,
similar to the three other SMP handler implementations.  Given that
there is no partial command handling in the host SMP handler this should
not matter in practice.

Bart Van Assche ccf1e0045e scsi: Rework handling of scsi_device.vpd_pg8[03]
Introduce struct scsi_vpd for the VPD page length, data and the RCU head
that will be used to free the VPD data. Use kfree_rcu() instead of
kfree() to free VPD data. Move the VPD buffer pointer check inside the
RCU read lock in the sysfs code. Only annotate pointers that are shared
across threads with __rcu. Use rcu_dereference() when dereferencing an
RCU pointer. This patch suppresses about twenty sparse complaints about
the vpd_pg8[03] pointers. This patch also fixes a race condition, namely
that updating of the VPD pointers and length variables in struct
scsi_device was not atomic with reference to the code reading these
variables. See also "Does the update code tolerate concurrent accesses?"
in Documentation/RCU/checklist.txt.

Bart Van Assche bed2213d01 scsi: Use blk_mq_rq_to_pdu() to convert a request to a SCSI command pointer
Since commit e9c787e65c ("scsi: allocate scsi_cmnd structures as
part of struct request") struct request and struct scsi_cmnd are
adjacent. This means that there is now an alternative to reading
req->special to convert a pointer to a prepared request into a
SCSI command pointer, namely by using blk_mq_rq_to_pdu(). Make
this change where appropriate. Although this patch does not
change any functionality, it slightly improves performance and
slightly improves readability.

Damien Le Moal 70e42fd02c scsi: sd_zbc: Write unlock zone from sd_uninit_cmnd()
Releasing a zone write lock only when the write commnand that acquired
the lock completes can cause deadlocks due to potential command
reordering if the lock owning request is requeued and not executed. This
problem exists only with the scsi-mq path as, unlike the legacy path,
requests are moved out of the dispatch queue before being prepared and
so before locking a zone for a write command.

Since sd_uninit_cmnd() is now always called when a request is requeued,
call sd_zbc_write_unlock_zone() from that function for write requests
that acquired a zone lock instead of from sd_done(). Acquisition of a zone
lock by a write command is indicated using the new command
flag SCMD_ZONE_WRITE_LOCK.

Steffen Maier 67b465250e scsi: fc: start decoupling fc_block_scsi_eh from scsi_cmnd
Scsi_cmnd is an unsuitable argument for eh_device_reset_handler(),
eh_target_reset_handler(), and eh_host_reset_handler() which do not have
the scope of one single SCSI command.  These callbacks tend to use
fc_block_scsi_eh() requiring scsi_cmnd.  In order to start decoupling
above eh callbacks from scsi_cmnd, introduce a new variant of the
function called fc_block_rport() taking an fc_rport as argument.
Refactor the old fc_block_scsi_eh() to simply delegate to
fc_block_rport().

Ewan D. Milne f9279c968c scsi: Add STARGET_CREATED_REMOVE state to scsi_target_state
The addition of the STARGET_REMOVE state had the side effect of
introducing a race condition that can cause a crash.

scsi_target_reap_ref_release() checks the starget->state to
see if it still in STARGET_CREATED, and if so, skips calling
transport_remove_device() and device_del(), because the starget->state
is only set to STARGET_RUNNING after scsi_target_add() has called
device_add() and transport_add_device().

However, if an rport loss occurs while a target is being scanned,
it can happen that scsi_remove_target() will be called while the
starget is still in the STARGET_CREATED state.  In this case, the
starget->state will be set to STARGET_REMOVE, and as a result,
scsi_target_reap_ref_release() will take the wrong path.  The end
result is a panic:

Just wire up the generic TCG OPAL infrastructure to the SCSI disk driver
and the Security In/Out commands.

Note that I don't know of any actual SCSI disks that do support TCG OPAL,

Johannes Thumshirn 6d311fa7d2 scsi: sas: scsi_queue_work can fail, so make callers aware
libsas uses scsi_queue_work() to queue its internal event notifications.
scsi_queue_work() can return -EINVAL if the work queue doesn't exist and
it does call queue_work() which can return false if the work is already
queued.

Make the SAS event code capable of returning errors up to the caller,
which is handy when changing to dynamically allocated work in libsas
as well, as discussed here: https://lkml.org/lkml/2017/6/14/121.

Bart Van Assche 6e9017feaa scsi: Remove the definition of VLC_SA_RECEIVE_CREDENTIAL
The symbolic name VLC_SA_RECEIVE_CREDENTIAL is not used anywhere in the
kernel. Additionally, since SPC 5 the RECEIVE CREDENTIAL command is
obsolete. The VLC_SA_RECEIVE_CREDENTIAL definition is misleading since
it occurs outside the list of other variable length CDB service action
codes (READ_32, WRITE_32, ...). Hence remove this definition.

Bart Van Assche ca18d6f769 block: Make most scsi_req_init() calls implicit
Instead of explicitly calling scsi_req_init() after blk_get_request(),
call that function from inside blk_get_request(). Add an
.initialize_rq_fn() callback function to the block drivers that need
it. Merge the IDE .init_rq_fn() function into .initialize_rq_fn()
because it is too small to keep it as a separate function. Keep the
scsi_req_init() call in ide_prep_sense() because it follows a
blk_rq_init() call.

Bart Van Assche 0db6ca8a5e scsi: Protect SCSI device state changes with a mutex
Serializing SCSI device state changes avoids that two state changes can
occur concurrently, e.g. the state changes in scsi_target_block() and
__scsi_remove_device(). This serialization is essential to make patch
"Make __scsi_remove_device go straight from BLOCKED to DEL" work
reliably.

Enable this mechanism for all scsi_target_*block() callers but not for
the scsi_internal_device_unblock() calls from the mpt3sas driver because
that driver can call scsi_internal_device_unblock() from atomic context.

Bart Van Assche 8e6882545d scsi: Avoid that scsi_exit_rq() triggers a use-after-free
Dereferencing shost from scsi_exit_rq() is not safe because the SCSI
host may already have been freed when scsi_exit_rq() is called.
Increasing the shost reference count in scsi_init_rq() and dropping that
reference in scsi_exit_rq() is nontrivial since scsi_host_dev_release()
may sleep and since scsi_exit_rq() may be called from interrupt
context. Since scsi_exit_rq() only needs a single bit from shost, copy
that bit into struct scsi_cmnd.

Christoph Hellwig 2a842acab1 block: introduce new block status code type
Currently we use nornal Linux errno values in the block layer, and while
we accept any error a few have overloaded magic meanings.  This patch
instead introduces a new  blk_status_t value that holds block layer specific
status codes and explicitly explains their meaning.  Helpers to convert from
and to the previous special meanings are provided for now, but I suspect
we want to get rid of them in the long run - those drivers that have a
errno input (e.g. networking) usually get errnos that don't know about
the special block layer overloads, and similarly returning them to userspace
will usually return somethings that strictly speaking isn't correct
for file system operations, but that's left as an exercise for later.

For now the set of errors is a very limited set that closely corresponds
to the previous overloaded errno values, but there is some low hanging
fruite to improve it.

blk_status_t (ab)uses the sparse __bitwise annotations to allow for sparse
typechecking, so that we can easily catch places passing the wrong values.

Christoph Hellwig 17d5363b83 scsi: introduce a result field in struct scsi_request
This passes on the scsi_cmnd result field to users of passthrough
requests.  Currently we abuse req->errors for this purpose, but that
field will go away in its current form.

Note that the old IDE code abuses the errors field in very creative
ways and stores all kinds of different values in it.  I didn't dare
to touch this magic, so the abuses are brought forward 1:1.

This patch allows scsi drivers that expose virturalized fibre channel
devices but that do not expose rports to successfully rescan the scsi
bus via echo "- - -" > /sys/class/scsi_host/hostX/scan.  Drivers can
create a pseudo rport and indicate FC_PORT_ROLE_FCP_DUMMY_INITIATOR as
the rport's role in fc_rport_identifiers. This insures that a valid
scsi_target_id is assigned to the newly created rport and it can meet
the requirements of fc_user_scan_tgt calling scsi_scan_target.

Hannes Reinecke 8e8c9d01c5 scsi: make eh_eflags persistent
If a failed command is retried and fails again we need
to enter SCSI EH, otherwise we will never be able to
recover the command.
To detect this situation we must not clear scmd->eh_eflags
when EH finishes but rather make it persistent throughout
the lifetime of the command.

Hannes Reinecke 7a38dc0bfb scsi: scsi_error: count medium access timeout only once per EH run
The current medium access timeout counter will be increased for
each command, so if there are enough failed commands we'll hit
the medium access timeout for even a single device failure and
the following kernel message is displayed:

sd H:C:T:L: [sdXY] Medium access timeout failure. Offlining disk!

Fix this by making the timeout per EH run, ie the counter will
only be increased once per device and EH run.

Bart Van Assche 8893cf6cb1 scsi: mpt3sas: Avoid sleeping in interrupt context
Commit 669f044170 ("scsi: srp_transport: Move queuecommand() wait code
to SCSI core") can make scsi_internal_device_block() sleep.  However,
the mpt3sas driver can call this function from an interrupt
handler. Hence add a second argument to scsi_internal_device_block()
that restores the old behavior of this function for the mpt3sas handler.

The call chain that triggered an "IRQ handler enabled interrupts"
complaint is as follows:

_base_interrupt()
-> _base_async_event()
   -> mpt3sas_scsih_event_callback()
      -> _scsih_check_topo_delete_events()
         -> _scsih_block_io_to_children_attached_directly()
            -> _scsih_block_io_device()

Chris Leech 6f8830f5bb scsi: libiscsi: add lock around task lists to fix list corruption regression
There's a rather long standing regression from the commit "libiscsi:
Reduce locking contention in fast path"

Depending on iSCSI target behavior, it's possible to hit the case in
iscsi_complete_task where the task is still on a pending list
(!list_empty(&task->running)).  When that happens the task is removed
from the list while holding the session back_lock, but other task list
modification occur under the frwd_lock.  That leads to linked list
corruption and eventually a panicked system.

Rather than back out the session lock split entirely, in order to try
and keep some of the performance gains this patch adds another lock to
maintain the task lists integrity.

Major enterprise supported kernels have been backing out the lock split
for while now, thanks to the efforts at IBM where a lab setup has the
most reliable reproducer I've seen on this issue.  This patch has been
tested there successfully.
