pnv_npu2_init_context wasn't checking the return code from
__mmu_notifier_register. If __mmu_notifier_register failed, the
npu_context was still assigned to the mm and the caller wasn't given any
indication that things went wrong. Later on pnv_npu2_destroy_context would
be called, which in turn called mmu_notifier_unregister and dropped
mm->mm_count without having incremented it in the first place. This led to
various forms of corruption like mm use-after-free and mm double-free.
__mmu_notifier_register can fail with EINTR if a signal is pending, so
this case can be frequent.
This patch calls opal_npu_destroy_context on the failure paths, and makes
sure not to assign mm->context.npu_context until past the failure points.

Back in 2013 we added some hypercall wrappers which misspelled
"plpar" (P-series Logical PARtition) as "plapr".
Visually they're hard to distinguish and it almost doesn't matter, but
it is confusing when grepping to miss some calls because of the typo.
They've also started spreading, so before they take over let's fix
them all to be "plpar".

smp_query_cpu_stopped() and related #defines are currently in
plpar_wrappers.h. The function actually does an RTAS call, not an
hcall, and basically has nothing to do with plpar_wrappers.h
Move it into pseries.h, where it can easily be used by the only two
callers in pseries/smp.c and pseries/hotplug-cpu.c.

On MCE the current code will restart the machine with
ppc_md.restart(). This case was extremely unlikely since
prior to that a skiboot call is made and that resulted in
a checkstop for analysis.
With newer skiboots, on P9 we don't checkstop the box by
default, instead we return back to the kernel to extract
useful information at the time of the MCE. While we still
get this information, this patch converts the restart to
a panic(), so that if configured a dump can be taken and
we can track and probably debug the potential issue causing
the MCE.

P9 supports PCI tunneled operations (atomics and as_notify). This
patch adds support for tunneled operations on powernv, with a new
API, to be called by device drivers:
pnv_pci_enable_tunnel()
Enable tunnel operations, tell driver the 16-bit ASN indication
used by kernel.
pnv_pci_disable_tunnel()
Disable tunnel operations.
pnv_pci_set_tunnel_bar()
Tell kernel the Tunnel BAR Response address used by driver.
This function uses two new OPAL calls, as the PBCQ Tunnel BAR
register is configured by skiboot.
pnv_pci_get_as_notify_info()
Return the ASN info of the thread to be woken up.

When sending TLB invalidates to the NPU we need to send extra flushes due
to a hardware issue. The original implementation would lock the all the
ATSD MMIO registers sequentially before unlocking and relocking each of
them sequentially to do the extra flush.
This introduced a deadlock as it is possible for one thread to hold one
ATSD register whilst waiting for another register to be freed while the
other thread is holding that register waiting for the one in the first
thread to be freed.
For example if there are two threads and two ATSD registers:

Both threads will be stuck waiting to acquire a register resulting in an
RCU stall warning or soft lockup.
This patch solves the deadlock by refactoring the code to ensure registers
are not released between flushes and to ensure all registers are either
acquired or released together and in order.

On the 8xx, the page size is set in the PMD entry and applies to
all pages of the page table pointed by the said PMD entry.
When an app has some regular pages allocated (e.g. see below) and tries
to mmap() a huge page at a hint address covered by the same PMD entry,
the kernel accepts the hint allthough the 8xx cannot handle different
page sizes in the same PMD entry.

In order to fix this, this patch uses the address space "slices"
implemented for BOOK3S/64 and enhanced to support PPC32 by the
preceding patch.
This patch modifies the context.id on the 8xx to be in the range
[1:16] instead of [0:15] in order to identify context.id == 0 as
not initialised contexts as done on BOOK3S
This patch activates CONFIG_PPC_MM_SLICES when CONFIG_HUGETLB_PAGE is
selected for the 8xx
Alltough we could in theory have as many slices as PMD entries, the
current slices implementation limits the number of low slices to 16.
This limitation is not preventing us to fix the initial issue allthough
it is suboptimal. It will be cured in a subsequent patch.

The OPAL IMC driver's shutdown handler disables nest PMU counters by
walking nodes and taking the first CPU out of their cpumask, which is
used to index into the paca (get_hard_smp_processor_id()). This does
not always do the right thing, and in particular for CPU-less nodes it
returns NR_CPUS and that overruns the paca and dereferences random
memory.
Fix it by being more careful about checking returned CPU, and only
using online CPUs. It's not clear this shutdown code makes sense after
commit 885dcd709b ("powerpc/perf: Add nest IMC PMU support"), but this
should not make things worse
Currently the bug causes us to call OPAL with a junk CPU number. A
separate patch in development to change the way pacas are allocated
escalates this bug into a crash:

some of the preparatory changes for pkeys.
Two fixes for bugs in the enhanced IRQ soft masking for local_t, one of which
broke KVM in some circumstances.
Four fixes for Power9. The most bizarre being a bug where futexes stopped
working because a NULL pointer dereference didn't trap during early boot (it
aliased the kernel mapping). A fix for memory hotplug when using the Radix MMU,
and a fix for live migration of guests using the Radix MMU.
Two fixes for hotplug on pseries machines. One where we weren't correctly
updating NUMA info when CPUs are added and removed. And the other fixes
crashes/hangs seen when doing memory hot remove during boot, which is apparently

Commit e67e02a544 ("powerpc/pseries: Fix cpu hotplug crash with
memoryless nodes") adds an unconditional call to
find_and_online_cpu_nid(), which is only declared if CONFIG_PPC_SPLPAR
is enabled. This results in the following build error if this is not
the case.

cp_abort is only required for user windows, because kernel context
must not be preempted between a copy/paste pair.
Without this patch, the init task gets used_vas set when it runs the
nx842_powernv_init initcall, which opens windows for kernel usage.
used_vas is then never cleared anywhere, so it gets propagated into
all other tasks. It's a property of the address space, so it should
really be cleared when a new mm is created (or in dup_mmap if the
mmaps are marked as VM_DONTCOPY). For now we seem to have no such
driver, so leave that for another patch.

Currently if the kernel receives a memory hot-unplug event early
enough, it may get stuck in an infinite loop in
dissolve_free_huge_pages(). This appears as a stall just after:
pseries-hotplug-mem: Attempting to hot-remove XX LMB(s) at YYYYYYYY
It appears to be caused by "minimum_order" being uninitialized, due to
init_ras_IRQ() executing before hugetlb_init().
To correct this, extract the part of init_ras_IRQ() that enables
hotplug event processing and place it in the machine_late_initcall
phase, which is guaranteed to be after hugetlb_init() is called.

NOTE! On almost all architectures, the EPOLL* constants have the same
values as the POLL* constants do. But they keyword here is "almost".
For various bad reasons they aren't the same, and epoll() doesn't

The next patch from Al will sort out the final differences, and we
should be all done.

When DLPAR removing a CPU, the unmapping of the cpu from a node in
unmap_cpu_from_node() should also invalidate the CPUs entry in the
numa_cpu_lookup_table. There is not a guarantee that on a subsequent
DLPAR add of the CPU the associativity will be the same and thus
could be in a different node. Invalidating the entry in the
numa_cpu_lookup_table causes the associativity to be read from the
device tree at the time of the add.
The current behavior of not invalidating the CPUs entry in the
numa_cpu_lookup_table can result in scenarios where the the topology
layout of CPUs in the partition does not match the device tree
or the topology reported by the HMC.
This bug looks like it was introduced in 2004 in the commit titled
"ppc64: cpu hotplug notifier for numa", which is 6b15e4e87e32 in the
linux-fullhist tree. Hence tag it for all stable releases.

"This introduces a __bitwise type for POLL### bitmap, and propagates
the annotations through the tree. Most of that stuff is as simple as
'make ->poll() instances return __poll_t and do the same to local
variables used to hold the future return value'.
Some of the obvious brainos found in process are fixed (e.g. POLLIN
misspelled as POLL_IN). At that point the amount of sparse warnings is
low and most of them are for genuine bugs - e.g. ->poll() instance
deciding to return -EINVAL instead of a bitmap. I hadn't touched those
in this series - it's large enough as it is.
Another problem it has caught was eventpoll() ABI mess; select.c and
eventpoll.c assumed that corresponding POLL### and EPOLL### were
equal. That's true for some, but not all of them - EPOLL### are
arch-independent, but POLL### are not.
The last commit in this series separates userland POLL### values from
the (now arch-independent) kernel-side ones, converting between them
in the few places where they are copied to/from userland. AFAICS, this
is the least disruptive fix preserving poll(2) ABI and making epoll()
work on all architectures.
As it is, it's simply broken on sparc - try to give it EPOLLWRNORM and
it will trigger only on what would've triggered EPOLLWRBAND on other
architectures. EPOLLWRBAND and EPOLLRDHUP, OTOH, are never triggered
at all on sparc. With this patch they should work consistently on all
architectures"

Pull siginfo cleanups from Eric Biederman:
"Long ago when 2.4 was just a testing release copy_siginfo_to_user was
made to copy individual fields to userspace, possibly for efficiency
and to ensure initialized values were not copied to userspace.
Unfortunately the design was complex, it's assumptions unstated, and
humans are fallible and so while it worked much of the time that
design failed to ensure unitialized memory is not copied to userspace.
This set of changes is part of a new design to clean up siginfo and
simplify things, and hopefully make the siginfo handling robust enough
that a simple inspection of the code can be made to ensure we don't
copy any unitializied fields to userspace.
The design is to unify struct siginfo and struct compat_siginfo into a
single definition that is shared between all architectures so that
anyone adding to the set of information shared with struct siginfo can
see the whole picture. Hopefully ensuring all future si_code
assignments are arch independent.
The design is to unify copy_siginfo_to_user32 and
copy_siginfo_from_user32 so that those function are complete and cope
with all of the different cases documented in signinfo_layout. I don't
think there was a single implementation of either of those functions
that was complete and correct before my changes unified them.
The design is to introduce a series of helpers including
force_siginfo_fault that take the values that are needed in struct
siginfo and build the siginfo structure for their callers. Ensuring
struct siginfo is built correctly.
The remaining work for 4.17 (unless someone thinks it is post -rc1
material) is to push usage of those helpers down into the
architectures so that architecture specific code will not need to deal
with the fiddly work of intializing struct siginfo, and then when
struct siginfo is guaranteed to be fully initialized change copy
siginfo_to_user into a simple wrapper around copy_to_user.
Further there is work in progress on the issues that have been
documented requires arch specific knowledge to sort out.
The changes below fix or at least document all of the issues that have
been found with siginfo generation. Then proceed to unify struct
siginfo the 32 bit helpers that copy siginfo to and from userspace,
and generally clean up anything that is not arch specific with regards
to siginfo generation.

The QS21/22 IBM Cell blades had a southbridge chip called Axon. This
could have DDR DIMMs attached to it, though they were not directly
usable as RAM, instead they could be used as some sort of buffer, if
applications were written specifically to use the block device
provided by the driver.
Although the driver supposedly had direct access support, it was
apparently never tested (see commit 91117a2024 ("axonram: Fix bug in
direct_access")).
These machines have not been available for over 5 years, and were
never widely in use. It seems highly unlikely anyone is using this
driver.
In general we're happy to leave old drivers in the tree, but because
DAX is involved this driver is caught up in the ongoing work in that
area, but none of the DAX folks are able to test it.
So remove the driver, if any one *is* using it, we'll be happy to put
it back.

The mpc52xx_gpt code currently implements an irq_chip for handling
interrupts; due to how irq_chip handling is done, it's necessary for
the irq_chip methods to be invoked from hardirq context, even on a a
real-time kernel. Because the spinlock_t type becomes a "sleeping"
spinlock w/ RT kernels, it is not suitable to be used with irq_chips.
A quick audit of the operations under the lock reveal that they do
only minimal, bounded work, and are therefore safe to do under a raw
spinlock.

On powerpc systems with shared configurations of CPUs and memory and
memoryless nodes at boot, an event ordering problem was observed on a
SLES12 build platforms with the hot-add of CPUs to the memoryless
nodes.
* The most common error occurred when the memory SLAB driver attempted
to reference the memoryless node to which a CPU was being added
before the kernel had finished initializing all of the data
structures for the CPU and exited 'device_online' under
DLPAR/hot-add.
Normally the memoryless node would be initialized through the call
path device_online ... arch_update_cpu_topology ... find_cpu_nid ...
try_online_node. This patch ensures that the powerpc node will be
initialized as early as possible, even if it was memoryless and
CPU-less at the point when we are trying to hot-add a new CPU to it.

The pcidev value stored in pci_dn is only used for NPU/NPU2
initialization. We can easily drop the cached pointer and
use an ancient helper - pci_get_domain_bus_and_slot() instead in order
to reduce complexity.

When enabling SR-IOV in pseries platform, the VF bar properties for a
PF are reported on the device node in the device tree.
This patch adds the IOV Bar resources to Linux structures from the
device tree for later use when configuring SR-IOV by PF driver.

After initial validation of SR-IOV resources, firmware will associate
PEs to the dynamic VFs created within this call. This patch adds the
association of PEs to the PF array of PE numbers indexed by VF.

When pseries SR-IOV is enabled and after a PF driver has resumed from
EEH, platform has to be notified of the event so the child VFs can be
allowed to resume their normal recovery path.
This patch makes the EEH operation allow unfreeze platform dependent
code and adds the call to pseries EEH code.

To correctly use EEH code one has to make sure that the EEH_PE_VF is
set for dynamic created VFs. Therefore this patch allocates an eeh_pe
of eeh type EEH_PE_VF and associates PE with parent.

Add user APIs through ioctl to allocate, free, and be notified of an
AFU interrupt.
For opencapi, an AFU can trigger an interrupt on the host by sending a
specific command targeting a 64-bit object handle. On POWER9, this is
implemented by mapping a special page in the address space of a
process and a write to that page will trigger an interrupt.

In the opencapi protocol, host memory contexts are referenced by a
'actag'. During setup, a driver must tell the device how many actags
it can used, and what values are acceptable.
On POWER9, the NPU can handle 64 actags per link, so they must be
shared between all the PCI functions of the link. To get a global
picture of how many actags are used by each AFU of every function, we
capture some data at the end of PCI enumeration, so that actags can be
shared fairly if needed.
This is not powernv specific per say, but rather a consequence of the
opencapi configuration specification being quite general. The number
of available actags on POWER9 makes it more likely to be hit. This is
somewhat mitigated by the fact that existing AFUs are coded by
requesting a reasonable count of actags and existing devices carry
only one AFU.

Implement a few platform-specific calls which can be used by drivers:
- provide the Transaction Layer capabilities of the host, so that the
driver can find some common ground and configure the device and host
appropriately.
- provide the hw interrupt to be used for translation faults raised by
the NPU
- map/unmap some NPU mmio registers to get the fault context when the
NPU raises an address translation fault
The rest are wrappers around the previously-introduced opal calls.

Add opal calls to interact with the NPU:
OPAL_NPU_SPA_SETUP: set the Shared Process Area (SPA)
The SPA is a table containing one entry (Process Element) per memory
context which can be accessed by the opencapi device.
OPAL_NPU_SPA_CLEAR_CACHE: clear the context cache
The NPU keeps a cache of recently accessed memory contexts. When a
Process Element is removed from the SPA, the cache for the link must
be cleared.
OPAL_NPU_TL_SET: configure the Transaction Layer
The Transaction Layer specification defines several templates for
messages to be exchanged on the link. During link setup, the host and
device must negotiate what templates are supported on both sides and
at what rates those messages can be sent.

The configuration space for opencapi devices doesn't have a PCI
Express capability, therefore confusing linux in thinking it's of an
old PCI type with a 256-byte configuration space size, instead of the
desired 4k. So add a PCI fixup to declare the correct size.

The NPU was already abstracted by opal as a virtual PHB for nvlink,
but it helps to be able to differentiate between a nvlink or opencapi
PHB, as it's not completely transparent to linux. In particular, PE
assignment differs and we'll also need the information in later
patches.
So rename existing PNV_PHB_NPU type to PNV_PHB_NPU_NVLINK and add a
new type PNV_PHB_NPU_OCAPI.

Today 4 architectures set ARCH_SUPPORTS_MEMORY_FAILURE (arm64, parisc,
powerpc, and x86), while 4 other architectures set __ARCH_SI_TRAPNO
(alpha, metag, sparc, and tile). These two sets of architectures do
not interesect so remove the trapno paramater to remove confusion.

Platforms with a panic handler that halts the system can have problems
getting kernel messages out, because the panic notifiers are called
before kernel/panic.c does its flushing of printk buffers an console
etc.
This was attempted to be solved with commit a3b2cb30f2 ("powerpc: Do
not call ppc_md.panic in fadump panic notifier"), but that wasn't the
right approach and caused other problems, and was reverted by commit
ab9dbf771f.
Instead, the powernv shutdown paths have already had a similar
problem, fixed by taking the message flushing sequence from
kernel/panic.c. That's a little bit ugly, but while we have the code
duplicated, it will work for this case as well. So have ppc panic
handlers do the same flushing before they terminate.
Without this patch, a qemu pseries_le_defconfig guest stops silently
when issued the nmi command when xmon is off and no crash dumpers
enabled. Afterwards, an oops is printed by each CPU as expected.

During a kdump kernel boot in PowerPC, we request a reset of the PHBs
to the FW. It makes sense, since if we are booting a kdump kernel it
means we had some trouble before and we cannot rely in the adapters'
health; they could be in a bad state, hence the reset is needed.
But this reset is useful not only in kdump - there are situations,
specially when debugging drivers, that we could break an adapter in
a way it requires such reset. One can tell to just go ahead and
reboot the machine, but happens that many times doing kexec is much
faster, and so preferable than a full power cycle.
This patch adds the ppc_pci_reset_phbs parameter to perform such reset.

Symbolic macros are unintuitive and hard to read, whereas octal constants
are much easier to interpret. Replace macros for the basic permission
flags (user/group/other read/write/execute) with numeric constants
instead, across the whole powerpc tree.
Introducing a significant number of changes across the tree for no runtime
benefit isn't exactly desirable, but so long as these macros are still
used in the tree people will keep sending patches that add them. Not only
are they hard to parse at a glance, there are multiple ways of coming to
the same value (as you can see with 0444 and 0644 in this patch) which
hurts readability.
