Bart Van Assche 4fc930896d ide: Make ide_cdrom_prep_fs() initialize the sense buffer pointer
The changes introduced through commit 82ed4db499 assume that the
sense buffer pointer in struct scsi_request is initialized for all
requests - passthrough and filesystem requests. Hence make sure
that that pointer is initialized for filesystem requests. Remove
the memset() call that clears .cmd because the scsi_req_init()
call in ide_initialize_rq() already initializes the .cmd.

Borislav Petkov fbc3edf7d7 drivers/ide-cd: Handle missing driver data during status check gracefully
The 0day bot reports the below failure which happens occasionally, with
their randconfig testing (once every ~100 boots).  The Code points at
the private pointer ->driver_data being NULL, which hints at a race of
sorts where the private driver_data descriptor has disappeared by the
time we get to run the workqueue.

So let's check that pointer before we continue with issuing the command
to the drive.

This fix is of the brown paper bag nature but considering that IDE is
long deprecated, let's do that so that random testing which happens to
enable CONFIG_IDE during randconfig builds, doesn't fail because of
this.

Besides, failing the TEST_UNIT_READY command because the drive private
data is gone is something which we could simply do anyway, to denote
that there was a problem communicating with the device.

standard prep_rq_fn builds 10 byte cmds, it missed to invoke
scsi_req_init() to initialize certain fields of a scsi_request

members of struct scsi_request).

Bart Van Assche ca18d6f769 block: Make most scsi_req_init() calls implicit
Instead of explicitly calling scsi_req_init() after blk_get_request(),
call that function from inside blk_get_request(). Add an
.initialize_rq_fn() callback function to the block drivers that need
it. Merge the IDE .init_rq_fn() function into .initialize_rq_fn()
because it is too small to keep it as a separate function. Keep the
scsi_req_init() call in ide_prep_sense() because it follows a
blk_rq_init() call.

Christoph Hellwig 2a842acab1 block: introduce new block status code type
Currently we use nornal Linux errno values in the block layer, and while
we accept any error a few have overloaded magic meanings.  This patch
instead introduces a new  blk_status_t value that holds block layer specific
status codes and explicitly explains their meaning.  Helpers to convert from
and to the previous special meanings are provided for now, but I suspect
we want to get rid of them in the long run - those drivers that have a
errno input (e.g. networking) usually get errnos that don't know about
the special block layer overloads, and similarly returning them to userspace
will usually return somethings that strictly speaking isn't correct
for file system operations, but that's left as an exercise for later.

For now the set of errors is a very limited set that closely corresponds
to the previous overloaded errno values, but there is some low hanging
fruite to improve it.

blk_status_t (ab)uses the sparse __bitwise annotations to allow for sparse
typechecking, so that we can easily catch places passing the wrong values.

Christoph Hellwig 17d5363b83 scsi: introduce a result field in struct scsi_request
This passes on the scsi_cmnd result field to users of passthrough
requests.  Currently we abuse req->errors for this purpose, but that
field will go away in its current form.

Note that the old IDE code abuses the errors field in very creative
ways and stores all kinds of different values in it.  I didn't dare
to touch this magic, so the abuses are brought forward 1:1.

Christoph Hellwig b7819b9259 block: remove the blk_execute_rq return value
The function only returns -EIO if rq->errors is non-zero, which is not
very useful and lets a large number of callers ignore the return value.

Just let the callers figure out their error themselves.

Ingo Molnar 68db0cf106 sched/headers: Prepare for new header dependencies before moving code to <linux/sched/task_stack.h>
We are going to split <linux/sched/task_stack.h> out of <linux/sched.h>, which
will have to be picked up from other headers and a couple of .c files.

Create a trivial placeholder <linux/sched/task_stack.h> file that just
maps to <linux/sched.h> to make this patch obviously correct and
bisectable.

Include the new header in the files that are going to need it.

Kees Cook 853fe1bf75 cdrom: Make device operations read-only
Since function tables are a common target for attackers, it's best to keep
them in read-only memory. As such, this makes the CDROM device ops tables
const. This drops additionally n_minors, since it isn't used meaningfully,
and sets the only user of cdrom_dummy_generic_packet explicitly so the
variables can all be const.

Inspired by similar changes in grsecurity/PaX.

Christoph Hellwig aebf526b53 block: fold cmd_type into the REQ_OP_ space
Instead of keeping two levels of indirection for requests types, fold it
all into the operations.  The little caveat here is that previously
cmd_type only applied to struct request, while the request and bio op
fields were set to plain REQ_OP_READ/WRITE even for passthrough
operations.

Instead this patch adds new REQ_OP_* for SCSI passthrough and driver
private requests, althought it has to add two for each so that we
can communicate the data in/out nature of the request.

Christoph Hellwig 2f5a8e80f7 ide: don't abuse cmd_type
Currently the legacy ide driver defines several request types of it's own,
which is in the way of removing that field entirely.

Instead add a type field to struct ide_request and use that to distinguish
the different types of IDE-internal requests.

It's a bit of a mess, but so is the surrounding code..

as the first thing of their private data.  To support this the legacy
IDE and BSG code is switched to set cmd_size on their queues to let
the block layer allocate the additional space.

Christoph Hellwig e806402130 block: split out request-only flags into a new namespace
A lot of the REQ_* flags are only used on struct requests, and only of
use to the block layer and a few drivers that dig into struct request
internals.

This patch adds a new req_flags_t rq_flags field to struct request for
them, and thus dramatically shrinks the number of common requests.  It
also removes the unfortunate situation where we have to fit the fields
from the same enum into 32 bits for struct bio and 64 bits for
struct request.

Mel Gorman 71baba4b92 mm, page_alloc: rename __GFP_WAIT to __GFP_RECLAIM
__GFP_WAIT was used to signal that the caller was in atomic context and
could not sleep.  Now it is possible to distinguish between true atomic
context and callers that are not willing to sleep.  The latter should
clear __GFP_DIRECT_RECLAIM so kswapd will still wake.  As clearing
__GFP_WAIT behaves differently, there is a risk that people will clear the
wrong flags.  This patch renames __GFP_WAIT to __GFP_RECLAIM to clearly
indicate what it does -- setting it allows all reclaim activity, clearing
them prevents it.

Kees Cook 02aa2a3763 drivers: avoid format string in dev_set_name
Calling dev_set_name with a single paramter causes it to be handled as a
format string.  Many callers are passing potentially dynamic string
content, so use "%s" in those cases to avoid any potential accidents,
including wrappers like device_create*() and bdi_register().

Al Viro db2a144bed block_device_operations->release() should return void
The value passed is 0 in all but "it can never happen" cases (and those
only in a couple of drivers) *and* it would've been lost on the way
out anyway, even if something tried to pass something meaningful.

Al Viro d9dda78bad procfs: new helper - PDE_DATA(inode)
The only part of proc_dir_entry the code outside of fs/proc
really cares about is PDE(inode)->data.  Provide a helper
for that; static inline for now, eventually will be moved
to fs/proc, along with the knowledge of struct proc_dir_entry
layout.

Jens' back-merge commit 698567f3fa ("Merge commit 'v2.6.39' into
for-2.6.40/core") was incorrectly done, and re-introduced the
DISK_EVENT_MEDIA_CHANGE lines that had been removed earlier in commits

 - 9fd097b149 ("block: unexport DISK_EVENT_MEDIA_CHANGE for
   legacy/fringe drivers")

 - 7eec77a181 ("ide: unexport DISK_EVENT_MEDIA_CHANGE for ide-gd
   and ide-cd")

because of conflicts with the "g->flags" updates near-by by commit
d4dc210f69 ("block: don't block events on excl write for non-optical
devices")

As a result, we re-introduced the hanging behavior due to infinite disk
media change reports.

Tssk, tssk, people! Don't do back-merges at all, and *definitely* don't
do them to hide merge conflicts from me - especially as I'm likely
better at merging them than you are, since I do so many merges.

Tejun Heo d4dc210f69 block: don't block events on excl write for non-optical devices
Disk event code automatically blocks events on excl write.  This is
primarily to avoid issuing polling commands while burning is in
progress.  This behavior doesn't fit other types of devices with
removeable media where polling commands don't have adverse side
effects and door locking usually doesn't exist.

This patch introduces new genhd flag which controls the auto-blocking
behavior and uses it to enable auto-blocking only on optical devices.

Note for stable: 2.6.38 and later only

Tejun Heo 5b03a1b140 ide: Convert to bdops->check_events()
Convert ->media_changed() to the new ->check_events() method.  The
conversion is mostly mechanical.  The only notable change is that
cdrom now doesn't generate any event if @slot_nr isn't CDSL_CURRENT.
It used to return -EINVAL which would be treated as media changed.  As
media changer isn't supported anyway, this doesn't make any
difference.

This makes ide emit the standard disk events and allows kernel event
polling.  Currently, only MEDIA_CHANGE event is implemented.  Adding
support for EJECT_REQUEST shouldn't be difficult; however, given that
ide driver is already deprecated, it probably is best to leave it
alone.

Borislav Petkov 1107128283 ide-cd: Do not access completed requests in the irq handler
ide_cd_error_cmd() can complete an erroneous request with leftover
buffers. Signal this with its return value so that the request is not
accessed after its completion in the irq handler and we oops.

Arnd Bergmann 6e9624b8ca block: push down BKL into .open and .release
The open and release block_device_operations are currently
called with the BKL held. In order to change that, we must
first make sure that all drivers that currently rely
on this have no regressions.

This blindly pushes the BKL into all .open and .release
operations for all block drivers to prepare for the
next step. The drivers can subsequently replace the BKL
with their own locks or remove it completely when it can
be shown that it is not needed.

The functions blkdev_get and blkdev_put are the only
remaining users of the big kernel lock in the block
layer, besides a few uses in the ioctl code, none
of which need to serialize with blkdev_{get,put}.

Most of these two functions is also under the protection
of bdev->bd_mutex, including the actual calls to
->open and ->release, and the common code does not
access any global data structures that need the BKL.

Alexey Dobriyan 6d703a81ad ide: convert to ->proc_fops
->read_proc, ->write_proc are going away, ->proc_fops should be used instead.

The only tricky place is IDENTIFY handling: if for some reason
taskfile_lib_get_identify() fails, buffer _is_ changed and at least
first byte is overwritten. Emulate old behaviour with returning
that first byte to userspace and reporting length=1 despite overall -E.

Borislav Petkov 9c72ebef5a ide-cd: handle fragmented packet commands gracefully
There are some devices in the wild that clear the DRQ bit during the
last word of a packet command and therefore could use a "second chance"
for that last word of data to be xferred instead of simply failing the
request. Do that by attempting to suck in those last bytes in PIO mode.

In addition, the ATA_ERR bit has to be cleared for we cannot be sure the
data is valid otherwise.

Currently the error gets repeated too frequently, for example each
time HAL polls the device when a disc is present. Avoid that by using
printk_once instead of printk.

Frans Pop reported that his CDROM drive reports a blocksize of 2352,
and this causes new warnings due to commit
e8e7b9eb11 ("ide-cd: fix oops when using
growisofs").

What we're trying to do is make sure that "blocklen >> SECTOR_BITS"
is something the block layer won't choke on.

And for Frans' case "2352 >> SECTOR_BITS" is equal to
"2048 >> SECTOR_BITS", and thats "4".

So warning in this case gives no real benefit.

Rainer Weikusat 39c58f37a1 ide-cd: prevent null pointer deref via cdrom_newpc_intr
With 2.6.30, the error handling code in cdrom_newpc_intr was changed
to deal with partial request failures by normally completing the 'good'
parts of a request and only 'error' the last (and presumably,
incompletely transferred) bio associated with a particular
request. In order to do this, ide_complete_rq is called over
ide_cd_error_cmd() to partially complete the rq. The block layer
does partial completion only for requests with bio's and if the
rq doesn't have one (eg 'GPCMD_READ_DISC_INFO') the request is
completed as a whole and the drive->hwif->rq pointer set to NULL
afterwards. When calling ide_complete_rq again to report
the error, this null pointer is derefenced, resulting in a kernel
crash.

This fixes http://bugzilla.kernel.org/show_bug.cgi?id=13399.

Martin K. Petersen e1defc4ff0 block: Do away with the notion of hardsect_size
Until now we have had a 1:1 mapping between storage device physical
block size and the logical block sized used when addressing the device.
With SATA 4KB drives coming out that will no longer be the case.  The
sector size will be 4KB but the logical block size will remain
512-bytes.  Hence we need to distinguish between the physical block size
and the logical ditto.

This patch renames hardsect_size to logical_block_size.

Tejun Heo 5f49f63178 block: set rq->resid_len to blk_rq_bytes() on issue
In commit c3a4d78c58, while introducing
rq->resid_len, the default value of residue count was changed from
full count to zero.  The conversion was done under the assumption that
when a request fails residue count wasn't defined.  However, Boaz and
James pointed out that this wasn't true and the residue count should
be preserved for failed requests too.

This patchset restores the original behavior by setting rq->resid_len
to blk_rq_bytes(rq) on request start and restoring explicit clearing
in affected drivers.  While at it, take advantage of the fact that
rq->resid_len is set to full count where applicable.

* ide-cd: rq->resid_len cleared on pc success

* mptsas: req->resid_len cleared on success

* sas_expander: rsp/req->resid_len cleared on success

* mpt2sas_transport: req->resid_len cleared on success

* ide-cd, ide-tape, mptsas, sas_host_smp, mpt2sas_transport, ub: take
  advantage of initial full count to simplify code

Boaz Harrosh spotted bug in resid_len initialization.  Fixed as
suggested.

Tejun Heo 8f6205cd57 ide: dequeue in-flight request
ide generally has single request in flight and tracks it using
hwif->rq and all state handlers follow the following convention.

* ide_started is returned if the request is in flight.

* ide_stopped is returned if the queue needs to be restarted.  The
  request might or might not have been processed fully or partially.

* hwif->rq is set to NULL, when an issued request completes.

So, dequeueing model can be implemented by dequeueing after fetch,
requeueing if hwif->rq isn't NULL on ide_stopped return and doing
about the same thing on completion / port unlock paths.  These changes
can be made in ide-io proper.

In addition to the above main changes, the following updates are
necessary.

* ide-cd shouldn't dequeue a request when issuing REQUEST SENSE for it
  as the request is already dequeued.

* ide-atapi uses request queue as stack when issuing REQUEST SENSE to
  put the REQUEST SENSE in front of the failed request.  This now
  needs to be done using requeueing.
