Don't do the num_tx_queues based masking on calculating tx queue
index.
1) num_tx_queues is not always power-of-2, because it also depends on
the online cpu numbers. So the masking could be a performance bug
on a 6 cpu system.
2) queue_mapping will be limited by real_num_tx_queues=num_tx_queues
in the generic netdev function set_cur_queue_map(). So the bound
limiting here is not necessary.

This patch does two things:
1) On 82599, the PHY is emedded in the MAC. On 82598, the SFP+ NIC has an external PHY. The reset in the SFP+ setup patch for 82598 is unnecessary on 82599, and adds extra dead time to device initialization. This removes that PHY reset for 82599 only.
2) On 82599, the SFP+ modules are multispeed fiber modules (10G/1G). We need to make sure to identify them properly for the remaining init sections to properly set them up.

since with a 64kB PAGE_SIZE MAX_SKB_FRAGS = 3, the way the driver was

The failure happens when an interrupt occurs and the driver is reading
EICR. This read will cause a clear-by-read which leads to two TLP
being inserted in the PCIe retry buffer leading to an overflow of the
buffer and corruption of TLPs.
The solution is different depending where the reading of EICR takes place.
For ixgbe_msix_lsc() since we are in MSIX mode and know OCD is enabled a
clear-by-write is done instead of the normal clear-by-read.
For ixgbe_intr() 0xffffffff is written to EIMC before the read, masking the
interrupts.

We have some reasons to kill netdev->priv:
1. netdev->priv is equal to netdev_priv().
2. netdev_priv() wraps the calculation of netdev->priv's offset, obviously
netdev_priv() is more flexible than netdev->priv.
But we cann't kill netdev->priv, because so many drivers reference to it
directly.
This patch is a safe convert for netdev->priv to netdev_priv(netdev).
Since all of the netdev->priv is only for read.
But it is too big to be sent in one mail.

queues and the driver has been refreshed to better handle a dynamic
interrupt mode as well as multiple queues.

1) reading some of the registers in our hardware causes them to clear,
so don't read ICR in the ethtool register dump function.
2) several register iterators were not iterating

This change makes it so only one srrctl register is programmed per VMDq id, and if VMDq is not enabled it is one register per RSS queue. Currently this function is working correctly for the multiqueue RSS and single queue cases, but if any advances features such as VMDq or DCB would have been enabled this function would have caused issues as it was not correct.

DCA related fixes
=================
- ixgbe was not compiling and using DCA correctly if dca was a module
- DCA interface changed with new kernel
- ixgbe was not correctly configured to indicate DCA hints to the
correct CPU.

as mentioned by Herbert, our hardware supports IP offloads, not full
checksum offloads for any protocol in existence (even though the
hardware just provides generic csum support over any range of bytes)

This locks the seed down so loading/unloading the driver will present
predictable hashing from RSS. Also move the rx_buf_len out of the adapter
struct, and into the Rx ring struct.

the driver was misinterpreting rx_csum return value in the descriptor
so occassionally we would indicate an rx_csum error in our stats
when there was none. This would have no effect on traffic because
we would just hand the packet to the stack anyway without the offload
flag set, but would increase CPU for those packets that needed a recompute.

ixgbe: Introduce MSI-X queue vector code
The following patch adds it back. Without this the default value of 0
causes the performance of this card to be awful. Restoring these to the
default values yields much better performance.
This regression has been around since 2.6.25.

The ixgbe driver was untested with device ID 8086:10c8 but still advertises
support. Currently if this device is present in the system when the driver
is loaded, the system will panic.

hardware. This patch is necessary for 2.6.24, 2.6.25 and 2.6.26

With the recent changes to tx mutiqueue, igb/ixgbe/e1000e was not calling
netif_tx_start_all_queues() before calling netif_tx_wake_all_queues().
This causes an issue during loading of the driver.
In addition, updated e1000e to use the updated tx mutliqueue api.
