The asynchronous aborts are generally fatal for the kernel but they can
be masked via the pstate A bit. If a system error happens while in
kernel mode, it won't be visible until returning to user space. This
patch enables this kind of abort early to help identifying the cause.

There is a rather horrid merge conflict in tools/perf - I was never
notified of the conflict because it originally occurred between Will's

mail.
The other notable thing is I'm expecting some build breakage in the
crypto stuff on ARM only with Ard's AES patches. These were merged
into a stable git branch which others had already pulled, so there's
little I can do about this. The problem is caused because these
patches have a dependency on some code in the crypto git tree - I
tried requesting a branch I can pull to resolve these, and all I got
each time from the crypto people was "we'll revert our patches then"
which would only make things worse since I still don't have the
dependent patches. I've no idea what's going on there or how to
resolve that, and since I can't split these patches from the rest of
this pull request, I'm rather stuck with pushing this as-is or
reverting Ard's patches.
Since it should "come out in the wash" I've left them in - the only
build problems they seem to cause at the moment are with randconfigs,
and since it's a new feature anyway. However, if by -rc1 the
dependencies aren't in, I think it'd be best to revert Ard's patches"
I resolved the perf conflict roughly as per the patch sent by Russell,
but there may be some differences. Any errors are likely mine. Let's
see how the crypto issues work out..

When booting a vcpu using PSCI, make sure we start it with the
endianness of the caller. Otherwise, secondaries can be pretty
unhappy to execute a BE kernel in LE mode...
This conforms to PSCI spec Rev B, 5.13.3.

Do the necessary byteswap when host and guest have different
views of the universe. Actually, the only case we need to take
care of is when the guest is BE. All the other cases are naturally
handled.
Also be careful about endianness when the data is being memcopy-ed
from/to the run buffer.

This patch expands the VA_BITS to 42 when the 64K page configuration is
enabled allowing 2TB kernel linear mapping. Linux still uses 2 levels of
page tables in this configuration with pgd now being a full page.

Some drivers (ACPI notably) use ioremap_cache() to map an area which could
either be outside of kernel RAM or in an already mapped reserved area of
RAM. To avoid aliases with different caching attributes, ioremap() does
not allow RAM to be remapped. But for ioremap_cache(), the existing kernel
mapping may be used.

On an (even slightly) oversubscribed system, spinlocks are quickly
becoming a bottleneck, as some vcpus are spinning, waiting for a
lock to be released, while the vcpu holding the lock may not be
running at all.
The solution is to trap blocking WFEs and tell KVM that we're
now spinning. This ensures that other vpus will get a scheduling
boost, allowing the lock to be released more quickly. Also, using
CONFIG_HAVE_KVM_CPU_RELAX_INTERCEPT slightly improves the performance
when the VM is severely overcommited.

Currently, the code for setting the __cpu_boot_mode flag is munged in
with el2_setup. This makes things difficult on a BE bringup as a
memory access has to have occurred before el2_setup which is the place
that we'd like to set the endianess on the current EL.
Create a new function for setting __cpu_boot_mode and have el2_setup
return the mode the CPU. Also define a new constant in virt.h,
BOOT_CPU_MODE_EL1, for readability.

The arm64 port contains wrappers for arm32 syscalls that pass 64-bit
values. These wrappers concatenate the two registers to hold a 64-bit
value in a single X register. On BE, however, the lower and higher
words are swapped.
Create a new assembler macro, regs_to_64, that when on BE systems
swaps the registers in the orr instruction.

With the advent of CPU_HOTPLUG, the enable-method property for CPU0 may
tells us something useful (i.e. how to hotplug it back on), so we must
read it along with all the enable-method for all the other CPUs. Even
on UP the enable-method may tell us useful information (e.g. if a core
has some mechanism that might be usable for cpuidle), so we should
always read it.
This patch factors out the reading of the enable method, and ensures
that CPU0's enable method is read regardless of whether the kernel is
built with SMP support.

The arm64 kernel has an internal holding pen, which is necessary for
some systems where we can't bring CPUs online individually and must hold
multiple CPUs in a safe area until the kernel is able to handle them.
The current SMP infrastructure for arm64 is closely coupled to this
holding pen, and alternative boot methods must launch CPUs into the pen,
where they sit before they are launched into the kernel proper.
With PSCI (and possibly other future boot methods), we can bring CPUs
online individually, and need not perform the secondary_holding_pen
dance. Instead, this patch factors the holding pen management code out
to the spin-table boot method code, as it is the only boot method
requiring the pen.
A new entry point for secondaries, secondary_entry is added for other
boot methods to use, which bypasses the holding pen and its associated
overhead when bringing CPUs online. The smp.pen.text section is also
removed, as the pen can live in head.text without problem.
The cpu_operations structure is extended with two new functions,
cpu_boot and cpu_postboot, for bringing a cpu into the kernel and
performing any post-boot cleanup required by a bootmethod (e.g.
resetting the secondary_holding_pen_release to INVALID_HWID).
Documentation is added for cpu_operations.

For hotplug support, we're going to want a place to store operations
that do more than bring CPUs online, and it makes sense to group these
with our current smp_enable_ops. For cpuidle support, we'll want to
group additional functions, and we may want them even for UP kernels.
This patch renames smp_enable_ops to the more general cpu_operations,
and pulls the definitions out of smp code such that they can be used in
UP kernels. While we're at it, fix up instances of the cpu parameter to
be an unsigned int, drop the init markings and rename the *_cpu
functions to cpu_* to reduce future churn when cpu_operations is
extended.

The functions in psci.c are only used from smp_psci.c, and smp_psci
cannot function without psci.c. Additionally psci.c is built when !SMP,
where it's expected that cpu_suspend may be useful.
This patch unifies the two files, removing pointless duplication and
paving the way for PSCI support in UP systems.

memcpy(..., n * sizeof(args[0]));
If 'number of arguments(n)' is zero and 'argument index(i)' is also zero in
syscall_get_arguments(), none of arguments should be copied by memcpy().
Otherwise 'n--' can be a big positive number and unexpected amount of data
will be copied. Tracing system calls which take no argument, say sync(void),
may hit this case and eventually make the system corrupted.
This patch fixes the issue both in syscall_get_arguments() and
syscall_set_arguments().

The KVM PSCI code blindly assumes that vcpu_id and MPIDR are
the same thing. This is true when vcpus are organized as a flat
topology, but is wrong when trying to emulate any other topology
(such as A15 clusters).
Change the KVM PSCI CPU_ON code to look at the MPIDR instead
of the vcpu_id to pick a target CPU.

Support huge pages in KVM/ARM and KVM/ARM64. The pud_huge checking on
the unmap path may feel a bit silly as the pud_huge check is always
defined to false, but the compiler should be smart about this.
Note: This deals only with VMAs marked as huge which are allocated by
users through hugetlbfs only. Transparent huge pages can only be
detected by looking at the underlying pages (or the page tables
themselves) and this patch so far simply maps these on a page-by-page
level in the Stage-2 page tables.

xen_swiotlb_alloc_coherent needs to allocate a coherent buffer for cpu
and devices. On native x86 is sufficient to call __get_free_pages in
order to get a coherent buffer, while on ARM (and potentially ARM64) we
need to call the native dma_ops->alloc implementation.
Introduce xen_alloc_coherent_pages to abstract the arch specific buffer
allocation.
Similarly introduce xen_free_coherent_pages to free a coherent buffer:
on x86 is simply a call to free_pages while on ARM and ARM64 is
arm_dma_ops.free.

get_user() is defined as a function macro in arm64, and trace_get_user()
calls it as followed:
get_user(ch, ptr++);
Since the second parameter occurs twice in the definition, 'ptr++' is
unexpectedly evaluated twice and trace_get_user() will generate a bogus
string from user-provided one. As a result, some ftrace sysfs operations,
like "echo FUNCNAME > set_ftrace_filter," hit this case and eventually fail.
This patch fixes the issue both in get_user() and put_user().

TCR.TBI0 can be used to cause hardware address translation to ignore the
top byte of userspace virtual addresses. Whilst not especially useful in
standard C programs, this can be used by JITs to `tag' pointers with
various pieces of metadata.

These are needed to wrap uses of NEON in kernel mode. The names are

"Since v3.7.2 on two independent machines a very specific Git
repository fails in 9/10 cases on git-fsck due to an SHA1/memory
failures. This only occurs on a very specific repository and can be
reproduced stably on two independent laptops. Git mailing list ran
out of ideas and for me this looks like some very exotic kernel issue"
and bisected the failure to the backport of commit 53a59fc67f ("mm:
limit mmu_gather batching to fix soft lockups on !CONFIG_PREEMPT").
That commit itself is not actually buggy, but what it does is to make it
much more likely to hit the partial TLB invalidation case, since it
introduces a new case in tlb_next_batch() that previously only ever
happened when running out of memory.
The real bug is that the TLB gather virtual memory range setup is subtly
buggered. It was introduced in commit 597e1c3580 ("mm/mmu_gather:
enable tlb flush range in generic mmu_gather"), and the range handling
was already fixed at least once in commit e6c495a96c ("mm: fix the TLB
range flushed when __tlb_remove_page() runs out of slots"), but that fix
was not complete.
The problem with the TLB gather virtual address range is that it isn't
set up by the initial tlb_gather_mmu() initialization (which didn't get
the TLB range information), but it is set up ad-hoc later by the
functions that actually flush the TLB. And so any such case that forgot
to update the TLB range entries would potentially miss TLB invalidates.
Rather than try to figure out exactly which particular ad-hoc range
setup was missing (I personally suspect it's the hugetlb case in
zap_huge_pmd(), which didn't have the same logic as zap_pte_range()
did), this patch just gets rid of the problem at the source: make the
TLB range information available to tlb_gather_mmu(), and initialize it
when initializing all the other tlb gather fields.
This makes the patch larger, but conceptually much simpler. And the end
result is much more understandable; even if you want to play games with
partial ranges when invalidating the TLB contents in chunks, now the
range information is always there, and anybody who doesn't want to
bother with it won't introduce subtle bugs.
Ben verified that this fixes his problem.

Not saving PAR_EL1 is an unfortunate oversight. If the guest
performs an AT* operation and gets scheduled out before reading
the result of the translation from PAREL1, it could become
corrupted by another guest or the host.
Saving this register is made slightly more complicated as KVM also
uses it on the permission fault handling path, leading to an ugly
"stash and restore" sequence. Fortunately, this is already a slow
path so we don't really care. Also, Linux doesn't do any AT*
operation, so Linux guests are not impacted by this bug.

Using an enum for the register we wish to access allows newer
compilers to determine if we've forgotten a case in our switch
statement. This allows us to remove the BUILD_BUG() instances in
the arm64 port, avoiding problems where optimizations may not
happen.
To try and force better code generation we're currently marking
the accessor functions as inline, but newer compilers can ignore
the inline keyword unless it's marked __always_inline. Luckily on
arm and arm64 inline is __always_inline, but let's make
everything __always_inline to be explicit.

Secondary CPUs write to __boot_cpu_mode with caches disabled, and thus a
cached value of __boot_cpu_mode may be incoherent with that in memory.
This could lead to a failure to detect mismatched boot modes.
This patch adds flushing to ensure that writes by secondaries to
__boot_cpu_mode are made visible before we test against it.

The __cpuinit type of throwaway sections might have made sense
some time ago when RAM was more constrained, but now the savings
do not offset the cost and complications. For example, the fix in
commit 5e427ec2d0 ("x86: Fix bit corruption at CPU resume time")
is a good example of the nasty type of bugs that can be created
with improper use of the various __init prefixes.
After a discussion on LKML[1] it was decided that cpuinit should go
the way of devinit and be phased out. Once all the users are gone,
we can then finally remove the macros themselves from linux/init.h.
Note that some harmless section mismatch warnings may result, since
notify_cpu_starting() and cpu_up() are arch independent (kernel/cpu.c)
are flagged as __cpuinit -- so if we remove the __cpuinit from
arch specific callers, we will also get section mismatch warnings.
As an intermediate step, we intend to turn the linux/init.h cpuinit
content into no-ops as early as possible, since that will get rid
of these warnings. In any case, they are temporary and harmless.
This removes all the arch/arm64 uses of the __cpuinit macros from
all C files. Currently arm64 does not have any __CPUINIT used in
assembly files.

might_sleep()/might_fault() preemption point annotations for uaccess
functions, by Michael S Tsirkin:
1. The only reason uaccess routines might sleep is if they fault.
Make this explicit for all architectures.
2. A voluntary preemption point in uaccess functions means compiler
can't inline them efficiently, this breaks assumptions that they
are very fast and small that e.g. net code seems to make. Remove
this preemption point so behaviour matches with what callers
assume.
3. Accesses (e.g through socket ops) to kernel memory with KERNEL_DS
like net/sunrpc does will never sleep. Remove an unconditinal
might_sleep() in the might_fault() inline in kernel.h (used when
PROVE_LOCKING is not set).
4. Accesses with pagefault_disable() return EFAULT but won't cause
caller to sleep. Check for that and thus avoid might_sleep() when
PROVE_LOCKING is set.
These changes offer a nice speedup for CONFIG_PREEMPT_VOLUNTARY=y
kernels, here's a network bandwidth measurement between a virtual
machine and the host:

Bring Transparent HugePage support to ARM. The size of a
transparent huge page depends on the normal page size. A
transparent huge page is always represented as a pmd.
If PAGE_SIZE is 4KB, THPs are 2MB.
If PAGE_SIZE is 64KB, THPs are 512MB.

Add huge page support to ARM64, different huge page sizes are
supported depending on the size of normal pages:
PAGE_SIZE is 4KB:
2MB - (pmds) these can be allocated at any time.
1024MB - (puds) usually allocated on bootup with the command line
with something like: hugepagesz=1G hugepages=6
PAGE_SIZE is 64KB:
512MB - (pmds) usually allocated on bootup via command line.

Under ARM64, PTEs can be broadly categorised as follows:
- Present and valid: Bit #0 is set. The PTE is valid and memory
access to the region may fault.
- Present and invalid: Bit #0 is clear and bit #1 is set.
Represents present memory with PROT_NONE protection. The PTE
is an invalid entry, and the user fault handler will raise a
SIGSEGV.
- Not present (file or swap): Bits #0 and #1 are clear.
Memory represented has been paged out. The PTE is an invalid
entry, and the fault handler will try and re-populate the
memory where necessary.
Huge PTEs are block descriptors that have bit #1 clear. If we wish
to represent PROT_NONE huge PTEs we then run into a problem as
there is no way to distinguish between regular and huge PTEs if we
set bit #1.
To resolve this ambiguity this patch moves PTE_PROT_NONE from
bit #1 to bit #2 and moves PTE_FILE from bit #2 to bit #3. The
number of swap/file bits is reduced by 1 as a consequence, leaving
60 bits for file and swap entries.

If we consider the following code sequence:
my_pte = pte_modify(entry, myprot);
x = pte_write(my_pte);
y = pte_exec(my_pte);
If myprot comes from a PROT_NONE page, then x and y will both be
true which is undesireable behaviour.
This patch sets the no-execute and read-only bits for PAGE_NONE
such that the code above will return false for both x and y.

Wire the init of a 32bit vcpu by allowing 32bit modes in pstate,
and providing sensible defaults out of reset state.
This feature is of course conditioned by the presence of 32bit
capability on the physical CPU, and is checked by the KVM_CAP_ARM_EL1_32BIT
capability.

Define the 32bit specific registers (SPSRs, cp15...).
Most CPU registers are directly mapped to a 64bit register
(r0->x0...). Only the SPSRs have separate registers.
cp15 registers are also mapped into their 64bit counterpart in most
cases.

The software breakpoint handlers are hooked in directly from ptrace,
which makes it difficult to add additional handlers for things like
kprobes and kgdb.
This patch moves the handling code into debug-monitors.c, where we can

When using an IOMMU for device mappings, it is necessary to keep a
pointer between the device and the IOMMU to which it is attached in
order to obtain the correct IOMMU when attaching the device to a domain.
This patch adds an iommu pointer to the dev_archdata structure, in a
similar manner to other architectures (ARM, PowerPC, x86, ...).

Under arm64, we will calibrate the delay loop statically using a known
timer frequency, so delete read_current_timer(), or it will cause
compiling issue with allmodconfig.

An exclusive store instruction may fail for reasons other than lock
contention (e.g. a cache eviction during the critical section) so, in
line with other architectures using similar exclusive instructions
(alpha, mips, powerpc), retry the trylock operation if the lock appears
to be free but the strex reported failure.

Switching between reading the virtual or physical counters is
problematic, as some core code wants a view of time before we're fully
set up. Using a function pointer and switching the source after the
first read can make time appear to go backwards, and having a check in
the read function is an unfortunate block on what we want to be a fast
path.
Instead, this patch makes us always use the virtual counters. If we're a
guest, or don't have hyp mode, we'll use the virtual timers, and as such
don't care about CNTVOFF as long as it doesn't change in such a way as
to make time appear to travel backwards. As the guest will use the
virtual timers, a (potential) KVM host must use the physical timers
(which can wake up the host even if they fire while a guest is
executing), and hence a host must have CNTVOFF set to zero so as to have
a consistent view of time between the physical timers and virtual
counters.

When we take an exception at EL1, we only want to enable debug
exceptions if we're not currently stepping, otherwise we can easily get
stuck in a loop stepping into interrupt handlers.
Unfortunately, the current code tests the wrong bit in the mdscr, so fix
that.

For compiling with allmodconfig, need vga.h file, so generate it which
just only include the asm-generic one.
It is firstly used by drivers/gpu/drm/drm_irq.c.

Drivers use cmpxchg64, cmpxchg64_local to perform 64-bit operation, so
they can cross 32-bit and 64-bit platforms (it is a standard way).

This has a nasty set of conflicts with the exynos MCT code, which was
moved in a separate branch, and then fixed up when merged in, but still
conflicts a bit here. It should have been sorted out by this merge though.
The ESR_EL1 decoding process is a bit cryptic, and KVM has also
a need for the same constants.
Add a new esr.h file containing the appropriate exception classes
constants, and change entry.S to use it. Fix a small bug in the
EL1 breakpoint check while we're at it.

This converts arm and arm64 to use CLKSRC_OF DT based initialization for
the arch timer. A new function arch_timer_arch_init is added to allow for
arch specific setup.
This has a side effect of enabling sched_clock on omap5 and exynos5. There
should not be any reason not to use the arch timers for sched_clock.

The reg property of the cpu nodes in the DT now contains all the
affinity levels in (MPIDR[39:32] and MPIDR[23:0]) and that's what
boot_secondary() writes in the pen, so increase the mask in
secondary_holding_pen accordingly.

When booting the kernel, the cpu logical id map must be initialised
using device tree data passed by FW or through an embedded blob.
This patch parses the reg property in device tree "cpu" nodes,
retrieves the corresponding CPUs hardware identifiers (MPIDR) and
initialises the cpu logical map accordingly.
The device tree HW identifiers are considered valid if all CPU nodes
contain a "reg" property, there are no duplicate "reg" entries and the
DT defines a CPU node whose "reg" property defines affinity levels
that matches those of the boot CPU.
The primary CPU is assigned cpu logical number 0 to keep the current
convention valid.

The expression to compute the padding needed to fill the uc_sigmask field
to 1024 bits actually computes the padding needed for 1080 bits.
Fortunately, due to the 16-byte alignment of the following field
(uc_mcontext) the definition in glibc contains enough bytes of padding
after uc_sigmask so that the overall offsets and size match in both
definitions.

register to contain PID of current process") from ARM that introduces a
new Kconfig option which, when enabled, causes the kernel to write the
PID of the current task into the CONTEXTIDR register on context switch.
This is useful when analysing hardware trace, since writes to this
register can be configured to emit an event into the trace stream.

Our uses of inline asm constraints for atomic operations are fairly
wild and varied. We basically need to guarantee the following:
1. Any instructions with barrier implications
(load-acquire/store-release) have a "memory" clobber
2. When performing exclusive accesses, the addresing mode is generated
using the "Q" constraint
3. Atomic blocks which use the condition flags, have a "cc" clobber
This patch addresses these concerns which, as well as fixing the
semantics of the code, stops GCC complaining about impossible asm
constraints.

The arch_timer driver supports a superset of the functionality of the
arm_generic driver, and is not tied to a particular arch.
This patch moves arm64 to use the arch_timer driver, gaining additional
functionality in doing so, and removes the (now unused) arm_generic
driver. Timer-related hooks specific to arm64 are moved into
arch/arm64/kernel/time.c.

Currently arch_counter_get_cnt{p,v}ct can be speculated, allowing for
stale time values to be read. This could be problematic for the delay
loop and other sensitive functions, as the time delta could jump around
unexpectedly.
This patch adds isbs to arch_counter_get_cnt{p,v}ct, preventing this
possibility.
