Improve myri10ge parity error detection and recovery:
1) Don't restore PCI config space to a rebooted NIC until AFTER the
host is quiescent.
2) Let myri10ge_close() know the NIC is dead, so it won't waste time
waiting for a dead nic to respond to MXGEFW_CMD_ETHERNET_DOWN
3) When the NIC is quiet (link down, or otherwise idle link) use
a pci config space read to detect a rebooted NIC. Otherwise
we might never notice that a NIC rebooted

This patch converts the remaining occurences of raw return values to their
symbolic counterparts in ndo_start_xmit() functions that were missed by the
previous automatic conversion.
Additionally code that assumed the symbolic value of NETDEV_TX_OK to be zero
is changed to explicitly use NETDEV_TX_OK.

This patch is the result of an automatic spatch transformation to convert
all ndo_start_xmit() return values of 0 to NETDEV_TX_OK.
Some occurences are missed by the automatic conversion, those will be
handled in a seperate patch.

Convert magic values 1 and -1 to NETDEV_TX_BUSY and NETDEV_TX_LOCKED respectively.
0 (NETDEV_TX_OK) is not changed to keep the noise down, except in very few cases
where its in direct proximity to one of the other values.

Now that core network takes care of trans_start updates, dont do it
in drivers themselves, if possible. Multi queue drivers can
avoid one cache miss (on dev->trans_start) in their start_xmit()
handler.

Force a statistics update when our ethtool gstats routine
is called. Otherwise, ethtool will continue to read stale
stats until something forces an update by reading /proc/net/dev
This requires putting a lock around the stats update to guard
against 2 threads (one via ethtool, and one via procfs)
updating the stats at once.

When the napi api was changed to separate its 1:1 binding to the net_device
struct, the netif_rx_[prep|schedule|complete] api failed to remove the now
vestigual net_device structure parameter. This patch cleans up that api by
properly removing it..

The doorbell writes may be seen out of order by the firmware if they
are in WC memory since the tx spin(un)lock does not flush WC writes.
Hence if the "stop" is written on a different CPU than the "go", it
is possible that the stop will arrive after the go unless we add an
explicit memory barrier (and mmiowb() is not enough).
It fixes transmit hangs in multi tx queue mode.

The generic packet receive code takes care of setting
netdev->last_rx when necessary, for the sake of the
bonding ARP monitor.
Drivers need not do it any more.
Some cases had to be skipped over because the drivers
were making use of the ->last_rx value themselves.

Disable NAPI if a failure occurs when setting up the interface. Leaving
it enabled could cause a BUG the next time an ifconfig up is issued.

We cannot select INTEL_IOATDMA in Kconfig as soon as MYRI10GE or IXGBE
is enabled since the former is not available on all architectures.
Just use a Kconfig bool {IXGBE,MYRI10GE}_DCA set to =y when DCA
support can actually be built.
