Wolfram Sang 844990daa2 i2c: mxs: only flag completion when queue is completely done
The hardware generates an interrupt for every completed command in the
queue while the code assumed that it will only generate one interrupt
when the queue is empty. So, explicitly check if the queue is really
empty. This patch fixed problems which occurred due to high traffic on
the bus. While we are here, move the completion-initialization after the
parameter error checking.

Alban Browaeys aed3f09db3 drm/i915: Prevent a machine hang by checking crtc->active before loading lut
Before loading the lut (gamma), check the active state of intel_crtc,
otherwise at least on gen2 hang ensue.

This is reproducible in Xorg via:
  xset dpms force off
then
  xgamma -rgamma 2.0 # freeze.

Ohad Ben-Cohen 46451d6229 iommu/omap: fix erroneous omap-iommu-debug API calls
Adapt omap-iommu-debug to the latest omap-iommu API changes, which
were introduced by commit fabdbca "iommu/omap: eliminate the public
omap_find_iommu_device() method".

In a nutshell, iommu users are not expected to provide the omap_iommu
handle anymore - instead, iommus are attached using their user's device
handle.

omap-iommu-debug is a hybrid beast though: it invokes both public and
private omap iommu API, so fix it as necessary (otherwise a crash
is imminent).

Note: omap-iommu-debug is a bit disturbing, as it fiddles with internal
omap iommu data and requires exposing API which is otherwise not needed.
It should better be more tightly coupled with omap-iommu, to prevent
further bit rot and avoid exposing redundant API. Naturally that's out
of scope for the -rc cycle, so for now just fix the obvious.

Christian Riesch 5d69703263 davinci_emac: Do not free all rx dma descriptors during init
This patch fixes a regression that was introduced by

commit 0a5f384677
davinci_emac: Add Carrier Link OK check in Davinci RX Handler

Said commit adds a check whether the carrier link is ok. If the link is
not ok, the skb is freed and no new dma descriptor added to the rx dma
channel. This causes trouble during initialization when the carrier
status has not yet been updated. If a lot of packets are received while
netif_carrier_ok returns false, all dma descriptors are freed and the
rx dma transfer is stopped.

The bug occurs when the board is connected to a network with lots of
traffic and the ifconfig down/up is done, e.g., when reconfiguring
the interface with DHCP.

The bug can be reproduced by flood pinging the davinci board while doing
ifconfig eth0 down
ifconfig eth0 up
on the board.

After that, the rx path stops working and the overrun value reported
by ifconfig is counting up.

This patch reverts commit 0a5f384677
and instead issues warnings only if cpdma_chan_submit returns -ENOMEM.

Julia Lawall 640ba2444f drivers/video/pxa168fb.c: use devm_ functions
The various devm_ functions allocate memory that is released when a driver
detaches.  This patch uses these functions for data that is allocated in
the probe function of a platform device and is only freed in the remove
function.

By using devm_ioremap, it also removes a potential memory leak, because
there was no call to iounmap in the probe function.

Florian Tobias Schandinat 050f0e02c8 viafb: select HW scaling on VX900 for IGA2
VX900 can do hardware scaling for both IGAs in contrast to previous
hardware which could do it only for IGA2. This patch ensures that
we set the parameter for IGA2 and not for IGA1. This fixes hardware
scaling on VX900 until we have the infrastructure to support it for
both IGAs.

Giuseppe CAVALLARO e3e09f2645 phy: IC+101G and PHY_HAS_INTERRUPT flag
This patch adds the PHY_HAS_INTERRUPT flag for IC+101 device series.
Also the patch does a simple dity-up to signal that
the driver actually is for IP101A LF and IP101G devices.
In fact, these are two similar PHYs that have the same IDs
and mainly differ for the EEE capability supported in the
G series.

David McKay b8e3995af4 netdev/phy/icplus: Correct broken phy_init code
The code for ip1001_config_init() was totally broken if you were not
using RGMII. Instead of returning an error code or zero it actually
returned the value in the IP1001_SPEC_CTRL_STATUS_2 register. It was
also trying to set the IP1001_APS_ON bit , but never actually wrote
back the register.

The error checking was also incorrect in both this function and the
reset function, so this patch fixes that up in a consistent fashion.

Nikolaus Schulz c1c1a3d012 hwmon: (f75375s) Fix register write order when setting fans to full speed
By hwmon sysfs interface convention, setting pwm_enable to zero sets a fan
to full speed.  In the f75375s driver, this need be done by enabling
manual fan control, plus duty mode for the F875387 chip, and then setting
the maximum duty cycle.  Fix a bug where the two necessary register writes
were swapped, effectively discarding the setting to full-speed.

Rob Clark ca888a7958 OMAPDSS: HDMI: hot plug detect fix
The "OMAPDSS: HDMI: PHY burnout fix" commit switched the HDMI driver
over to using a GPIO for plug detect.  Unfortunately the ->detect()
method was not also updated, causing HDMI to no longer work for the
omapdrm driver (because it would actually check if a connection was
detected before attempting to enable display).

Archit Taneja a247ce78ca OMAPDSS: HACK: Ensure DSS clock domain gets out of idle when HDMI is enabled
For DSS clock domain to transition from idle to active state. It's necessary
to enable the optional clock DSS_FCLK before we enable the module using the
MODULEMODE bits in the clock domain's CM_DSS_DSS_CLKCTRL register.

This sequence was not followed correctly for the 'dss_hdmi' hwmod and it led
to DSS clock domain not getting out of idle when pm_runtime_get_sync() was
called for hdmi's platform device.

Since the clock domain failed to change it's state to active, the hwmod code
disables any clocks it had enabled before for this hwmod. This led to the clock
'dss_48mhz_clk' gettind disabled.

When hdmi's runtime_resume() op is called, the call to dss_runtime_get()
correctly enables the DSS clock domain this time. However, the clock
'dss_48mhz_clk' is needed for HDMI's PHY to function correctly. Since it was
disabled previously, the driver fails when it tries to enable HDMI's PHY.

Fix this for now by ensuring that dss_runtime_get() is called before we call
pm_runtime_get_sync() for hdmi's platform device. A correct fix for later would
be to modify the DSS related hwmod's mainclks, and also some changes in how
opt clocks are handled in the DSS driver.

This fixes the issue of HDMI not working when it's the default display. The
issue is not seen if any other display is already enabled as the first display
would have correctly enabled the DSS clockdomain.

Scott Talbert ee932bf9ac Move Logitech Harmony 900 from cdc_ether to zaurus
In the current kernel implementation, the Logitech Harmony 900 remote
control is matched to the cdc_ether driver through the generic
USB_CDC_SUBCLASS_MDLM entry.  However, this device appears to be of the
pseudo-MDLM (Belcarra) type, rather than the standard one.  This patch
blacklists the Harmony 900 from the cdc_ether driver and whitelists it for
the pseudo-MDLM driver in zaurus.

Elevating the lock for both kref_put and also for controller node deletion.
Previously, controller deletion was protected but the not the kref_put. This

Jerome Glisse 6b7746e876 drm/radeon/kms: properly set accel working flag and bailout when false
If accel is not working many subsystem such as the ib pool might not be
initialized properly that can lead to segfault inside kernel when cs
ioctl is call with non working acceleration. To avoid this make sure
the accel working flag is false when an error in GPU startup happen and
return EBUSY from cs ioctl if accel is not working.

Hitoshi Mitake 797a796a13 asm-generic: architecture independent readq/writeq for 32bit environment
This provides unified readq()/writeq() helper functions for 32-bit
drivers.

For some cases, readq/writeq without atomicity is harmful, and order of
io access has to be specified explicitly.  So in this patch, new two
header files which contain non-atomic readq/writeq are added.

 - <asm-generic/io-64-nonatomic-lo-hi.h> provides non-atomic readq/
   writeq with the order of lower address -> higher address

 - <asm-generic/io-64-nonatomic-hi-lo.h> provides non-atomic readq/
   writeq with reversed order

This allows us to remove some readq()s that were added drivers when the
default non-atomic ones were removed in commit dbee8a0aff ("x86:
remove 32-bit versions of readq()/writeq()")

The drivers which need readq/writeq but can do with the non-atomic ones
must add the line:

  #include <asm-generic/io-64-nonatomic-lo-hi.h> /* or hi-lo.h */

But this will be nop in 64-bit environments, and no other #ifdefs are
required.  So I believe that this patch can solve the problem of
 1. driver-specific readq/writeq
 2. atomicity and order of io access

This patch is tested with building allyesconfig and allmodconfig as
ARCH=x86 and ARCH=i386 on top of tip/master.

Alan Stern bb94a40668 usb-storage: fix freezing of the scanning thread
This patch (as1521b) fixes the interaction between usb-storage's
scanning thread and the freezer.  The current implementation has a
race: If the device is unplugged shortly after being plugged in and
just as a system sleep begins, the scanning thread may get frozen
before the khubd task.  Khubd won't be able to freeze until the
disconnect processing is complete, and the disconnect processing can't
proceed until the scanning thread finishes, so the sleep transition
will fail.

The implementation in the 3.2 kernel suffers from an additional
problem.  There the scanning thread calls set_freezable_with_signal(),
and the signals sent by the freezer will mess up the thread's I/O
delays, which are all interruptible.

The solution to both problems is the same: Replace the kernel thread
used for scanning with a delayed-work routine on the system freezable
work queue.  Freezable work queues have the nice property that you can
cancel a work item even while the work queue is frozen, and no signals
are needed.

The 3.2 version of this patch solves the problem in Bugzilla #42730.

Sarah Sharp 340a3504fd xhci: Fix encoding for HS bulk/control NAK rate.
The xHCI 0.96 spec says that HS bulk and control endpoint NAK rate must
be encoded as an exponent of two number of microframes.  The endpoint
descriptor has the NAK rate encoded in number of microframes.  We were
just copying the value from the endpoint descriptor into the endpoint
context interval field, which was not correct.  This lead to the VIA
host rejecting the add of a bulk OUT endpoint from any USB 2.0 mass
storage device.

The fix is to use the correct encoding.  Refactor the code to convert
number of frames to an exponential number of microframes, and make sure
we convert the number of microframes in HS bulk and control endpoints to
an exponent.

This should be back ported to kernels as old as 2.6.31, that contain the
commit dfa49c4ad1 "USB: xhci - fix math
in xhci_get_endpoint_interval"

Elric Fu a45aa3b305 USB: Set hub depth after USB3 hub reset
The superspeed device attached to a USB 3.0 hub(such as VIA's)
doesn't respond the address device command after resume. The
root cause is the superspeed hub will miss the Hub Depth value
that is used as an offset into the route string to locate the
bits it uses to determine the downstream port number after
reset, and all packets can't be routed to the device attached
to the superspeed hub.

Hub driver sends a Set Hub Depth request to the superspeed hub
except for USB 3.0 root hub when the hub is initialized and
doesn't send the request again after reset due to the resume
process. So moving the code that sends the Set Hub Depth request
to the superspeed hub from hub_configure() to hub_activate()
is to cover those situations include initialization and reset.

The patch should be backported to kernels as old as 2.6.39.

Sarah Sharp cab928ee1f USB: Fix handoff when BIOS disables host PCI device.
On some systems with an Intel Panther Point xHCI host controller, the
BIOS disables the xHCI PCI device during boot, and switches the xHCI
ports over to EHCI.  This allows the BIOS to access USB devices without
having xHCI support.

The downside is that the xHCI BIOS handoff mechanism will fail because
memory mapped I/O is not enabled for the disabled PCI device.
Jesse Barnes says this is expected behavior.  The PCI core will enable
BARs before quirks run, but it will leave it in an undefined state, and
it may not have memory mapped I/O enabled.

Make the generic USB quirk handler call pci_enable_device() to re-enable
MMIO, and call pci_disable_device() once the host-specific BIOS handoff
is finished.  This will balance the ref counts in the PCI core.  When
the PCI probe function is called, usb_hcd_pci_probe() will call
pci_enable_device() again.

This should be back ported to kernels as old as 2.6.31.  That was the
first kernel with xHCI support, and no one has complained about BIOS
handoffs failing due to memory mapped I/O being disabled on other hosts
(EHCI, UHCI, or OHCI).

BF can be disabled in some cases, the capability field, bf_reg_size is set

Neil Horman b203262de6 vmxnet3: cap copy length at size of skb to prevent dropped frames on tx
I was recently shown that vmxnet3 devices on transmit, will drop very small udp
frames consistently.  This is due to a regression introduced by commit
39d4a96fd7.  This commit attempts to introduce an
optimization to the tx path, indicating that the underlying hardware behaves
optimally when at least 54 bytes of header data are available for direct access.
This causes problems however, if the entire frame is less than 54 bytes long.
The subsequent pskb_may_pull in vmxnet3_parse_and_copy_hdr fails, causing an
error return code, which leads to vmxnet3_tq_xmit dropping the frame.

Fix it by placing a cap on the copy length.  For frames longer than 54 bytes, we
do the pull as we normally would.  If the frame is shorter than that, copy the
whole frame, but no more.  This ensures that we still get the optimization for
qualifying frames, but don't do any damange for frames that are too short.

Also, since I'm unable to do this, it wuold be great if vmware could follow up
this patch with some additional code commentary as to why 54 bytes is an optimal
pull length for a virtual NIC driver.  The comment that introduced this was
vague on that.  Thanks!

Eric Dumazet 11aad99af6 atl1c: dont use highprio tx queue
This driver attempts to use two TX rings but lacks proper support :

1) IRQ handler only takes care of TX completion on first TX ring
2) the stop/start logic uses the legacy functions (for non multiqueue
drivers)

This means all packets witk skb mark set to 1 are sent through high
queue but are never cleaned and queue eventualy fills and block the
device, triggering the infamous "NETDEV WATCHDOG" message.

Lets use a single TX ring to fix the problem, this driver is not a real
multiqueue one yet.

Minimal fix for stable kernels.

Alan Stern fea6d607e1 [SCSI] scsi_pm: Fix bug in the SCSI power management handler
This patch (as1520) fixes a bug in the SCSI layer's power management
implementation.

LUN scanning can be carried out asynchronously in do_scan_async(), and
sd uses an asynchronous thread for the time-consuming parts of disk
probing in sd_probe_async().  Currently nothing coordinates these
async threads with system sleep transitions; they can and do attempt
to continue scanning/probing SCSI devices even after the host adapter
has been suspended.  As one might expect, the outcome is not ideal.

This is what the "prepare" stage of system suspend was created for.
After the prepare callback has been called for a host, target, or
device, drivers are not allowed to register any children underneath
them.  Currently the SCSI prepare callback is not implemented; this
patch rectifies that omission.

For SCSI hosts, the prepare routine calls scsi_complete_async_scans()
to wait until async scanning is finished.  It might be slightly more
efficient to wait only until the host in question has been scanned,
but there's currently no way to do that.  Besides, during a sleep
transition we will ultimately have to wait until all the host scanning
has finished anyway.

For SCSI devices, the prepare routine calls async_synchronize_full()
to wait until sd probing is finished.  The routine does nothing for
SCSI targets, because asynchronous target scanning is done only as
part of host scanning.

Kleber Sacilotto de Souza a92fa25c63 [SCSI] ipr: fix eeh recovery for 64-bit adapters
In some scenarios, an EEH error can take a long time to be detected, since the
driver issues an MMIO read only after a device reset command times out and we
try to reset the adapter. This patch adds some code in ipr_cancel_op() to read
a hardware register so we detect the error earlier in case the op is being
aborted because of a timeout caused by a frozen adapter slot.

Another problem in such scenarios is that in __ipr_eh_host_reset() we change the
dump state flag from WAIT_FOR_DUMP to GET_DUMP, and the flag is later changed
from GET_DUMP to READ_DUMP in ipr_reset_restore_cfg_space(). However, if when
__ipr_eh_host_reset() is called by the SCSI error handling the function
ipr_reset_restore_cfg_space() has already been called by the PCI EEH code, we
end up with the flag in an inconsistent state. This patch also prevents this
problem.

Martin Schwidefsky 656d912537 [S390] 3215 deadlock with tty_wakeup
The 3215 driver calls tty_wakeup from irq context while holding the
device spinlock. If printk is called by any function on the callchain
starting from tty_wakeup the system deadlocks on the device spinlock.
Using a tasklet to call tty_wakup solves the problem.

Eugeni Dodonov 1c8ecf80fd drm/i915: do not enable RC6p on Sandy Bridge
With base on latest findings, RC6p seems to be respondible for RC6-related
issues on Sandy Bridge platform. To work-around those issues, the previous
solution was to completely disable RC6 on Sandy Bridge for the past few
releases, even if plain RC6 was not giving any issues.

What this patch does is preventing RC6p from being enabled on Sandy Bridge
even if users enable RC6 via a kernel parameter. So it won't change the
defaults in any way, but will ensure that if users do enable RC6 manually
it won't break their machines by enabling this extra state.

Proper fix for this (enabling specific RC6 states according to the GPU
generation) were proposed for the -next kernel, but we are too late in the
release process now to pick such changes.

Giuseppe CAVALLARO cf3f047b9a stmmac: move hw init in the probe (v2)
This patch moves the MAC HW initialization and
the HW feature verification from the open to the probe
function as D. Miller suggested.
So the patch actually reorganizes and tidies-up some parts of
the driver and indeed fixes some problem when tune its HW features.
These can be overwritten by looking at the HW cap register at
run-time and that generated problems.

Pavel Roskin 2504a6423b ath9k: stop on rates with idx -1 in ath9k rate control's .tx_status
Rate control algorithms are supposed to stop processing when they
encounter a rate with the index -1.  Checking for rate->count not being
zero is not enough.

Allowing a rate with negative index leads to memory corruption in
ath_debug_stat_rc().

Amitkumar Karwar 6670f15b1f mwifiex: clear previous security setting during association
Driver maintains different flags for WEP, WPA, WPA2 security modes.
Appropriate flag is set using security information provided in
connect request. mwifiex_is_network_compatible() routine uses them
to check if driver's setting is compatible with AP. Association is
aborted if the routine fails.

For some corner cases, it is observed that association is failed
even for valid security information based on association history.
This patch fixes the problem by clearing previous security setting
during each association.

We should set WEP key provided in connect request as default tx key.
This missing change is also added here.

Lars-Peter Clausen 61cddc57dc regmap: Fix cache defaults initialization from raw cache defaults
Currently registers with a value of 0 are ignored when initializing the register
defaults from raw defaults. This worked in the past, because registers without a
explicit default were assumed to have a default value of 0. This was changed in
commit b03622a8 ("regmap: Ensure rbtree syncs registers set to zero properly").
As a result registers, which have a raw default value of 0 are now assumed to
have no default. This again can result in unnecessary writes when syncing the
cache. It will also result in unnecessary reads for e.g. the first update
operation. In the case where readback is not possible this will even let the
update operation fail, if the register has not been written to before.

So this patch removes the check. Instead it adds a check to ignore raw defaults
for registers which are volatile, since those registers are not cached.

Taylor Ralph fda27874de [media] hdpvr: update picture controls to support firmware versions > 0.15
Correctly sets the max/min/default values for the hdpvr picture
controls. The reason the current values didn't cause a problem until now
is because any firmware <= 0.15 didn't support them. The latest firmware
releases properly support picture controls and the values in the patch
are derived from the windows driver using SniffUSB2.0.

Ondrej Zary 5350c65f4f Resurrect Intel740 driver: i740fb
This is a resurrection of an old (like 2.4.19) out-of-tree driver for
Intel740 graphics cards and adaptation for recent kernels. The old driver by
Andrey Ulanov is located at: http://sourceforge.net/projects/i740fbdev/files/

This is a new driver based on skeletonfb, using most of the low level HW code
from the old driver. The DDC code is completely new.

The driver was tested on two 8MB cards: Protac AG240D and Diamond Stealth II
G460.

Eric Dumazet 3013dc0cce 3c59x: shorten timer period for slave devices
Jean Delvare reported bonding on top of 3c59x adapters was not detecting
network cable removal fast enough.

3c59x indeed uses a 60 seconds timer to check link status if carrier is
on, and 5 seconds if carrier is off.

This patch reduces timer period to 5 seconds if device is a bonding
slave.

Sarah Sharp 68d07f64b8 USB: Don't fail USB3 probe on missing legacy PCI IRQ.
Intel has a PCI USB xhci host controller on a new platform. It doesn't
have a line IRQ definition in BIOS.  The Linux driver refuses to
initialize this controller, but Windows works well because it only depends
on MSI.

Actually, Linux also can work for MSI.  This patch avoids the line IRQ
checking for USB3 HCDs in usb core PCI probe.  It allows the xHCI driver
to try to enable MSI or MSI-X first.  It will fail the probe if MSI
enabling failed and there's no legacy PCI IRQ.

This patch should be backported to kernels as old as 2.6.32.

Sometimes a software reset is needed. Then some registers are saved and
restored but the interrupt mask register is missing. It causes issues

Guennadi Liakhovetski 5ba85d95ca mmc: sh_mmcif: fix late delayed work initialisation
If the driver is loaded with a card in the slot, mmc_add_host() will
schedule an immediate card-detection work, which will start IO and wait
for command completion. Usually the kernel first returns to the sh_mmcif
probe function, lets it finish and only then schedules the rescan work.
But sometimes, expecially under heavy system load, the work will be
scheduled immediately before returning to the probe method. In this case
it is important for the driver to be fully prepared for IO. For sh_mmcif
this means, that also the timeout work has to be initialised before
calling mmc_add_host(). It is also better to prepare interrupts
beforehand. Besides, since mmc_add_host() does card-detection itself,
there is no need to do it again immediately afterwards.

Girish K S 3e73c36b4d mmc: core: Fix PowerOff Notify suspend/resume
Modified the mmc_poweroff to resume before sending the poweroff
notification command. In sleep mode only AWAKE and RESET commands are
allowed, so before sending the poweroff notification command resume from
sleep mode and then send the notification command.

PowerOff Notify is tested on a Synopsis Designware Host Controller
(eMMC 4.5). The suspend to RAM and resume works fine.

Jaehoon Chung 6e8201f57c mmc: core: add the capability for broken voltage
There is an understood mismatch between the voltage the host controller is
set to and the voltage supplied to the card by a fixed voltage regulator.
Teaching the driver to accept the mismatch is overly complicated.  Instead
just accept the regulator's voltage.

This patch adds MMC_CAP2_BROKEN_VOLTAGE.

If the voltage didn't satisfy between min_uV and max_uV, try to change
the voltage in core.c.  When changing the voltage, maybe use
regulator_set_voltage().

In regulator_set_voltage(), check the below condition.

Then, eMMC didn't initialize always.

So if use a fixed-regulator, we need to add the MMC_CAP2_BROKEN_VOLTAGE.

Jerry Huang 147c3b338d mmc: esdhc: set the timeout to the max value
When accessing the card on some FSL platform boards (e.g p2020, p1010,
mpc8536), the following error is reported with the timeout value calculated:

mmc0: Got data interrupt 0x00000020 even though no data operation was
in progress.
mmc0: Got data interrupt 0x00000020 even though no data operation was
in progress.

So we skip the calculation of timeout and use the max value to fix it.

Jerry Huang 81e499224a mmc: esdhc: add PIO mode support
For some FSL ESDHC controllers (e.g. P2020E, Rev1.0), the SDHC can not
work on DMA mode because of the hardware bug, so we set a broken dma flag
and use PIO mode.

Ensure clocks are always enabled before any interaction with the
host controller driver. This makes sure that there is no race
between host execution and the core layer turning off clocks

Philip Rakity 2d0d68f583 mmc: core: UHS sdio card that fails should not exceed 50MHz
A UHS sdio card that fails initialization at 1.8v signaling is not in
UHS mode.  We cannot use the speed in the the cis to reflect the bus
speed as this is the maxiumum value and will not reflect the fact
that the host is operating at a lower (non uhs) bus speed.
