Michael Holzheu d57af9b214 taskstats: use real microsecond granularity for CPU times
The taskstats interface uses microsecond granularity for the user and
system time values.  The conversion from cputime to the taskstats values
uses the cputime_to_msecs primitive which effectively limits the
granularity to milliseconds.  Add the cputime_to_usecs primitive for
architectures that have better, more precise CPU time values.  Remove
cputime_to_msecs primitive because there are no more users left.

Takuya Yoshikawa 55438cc751 KVM: ia64: define kvm_lapic_enabled() to fix a compile error
The following patch

  commit 57ce1659316f4ca298919649f9b1b55862ac3826
  KVM: x86: In DM_LOWEST, only deliver interrupts to vcpus with enabled LAPIC's

ignored the fact that kvm_irq_delivery_to_apic() was also used by ia64.

We define kvm_lapic_enabled() to fix a compile error caused by this.
This will have the same effect as reverting the problematic patch for ia64.

Jeff Mahoney e207386865 ioctl: Use asm-generic/ioctls.h on ia64 (enables termiox)
This patch converts ia64 to use asm-generic/ioctls.h instead of its
own version.

The differences between the arch-specific version and the generic
version are as follows:

- The generic version adds TIOCSRS485 and TIOCGRS485, which are unused
  by any driver available on this architecture.
- The generic version adds support for termiox

Arnd Bergmann 6038f373a3 llseek: automatically add .llseek fop
All file_operations should get a .llseek operation so we can make
nonseekable_open the default for future file operations without a
.llseek pointer.

The three cases that we can automatically detect are no_llseek, seq_lseek
and default_llseek. For cases where we can we can automatically prove that
the file offset is always ignored, we use noop_llseek, which maintains
the current behavior of not returning an error from a seek.

New drivers should normally not use noop_llseek but instead use no_llseek
and call nonseekable_open at open time.  Existing drivers can be converted
to do the same when the maintainer knows for certain that no user code
relies on calling seek on the device file.

The generated code is often incorrectly indented and right now contains
comments that clarify for each added line why a specific variant was
chosen. In the version that gets submitted upstream, the comments will
be gone and I will manually fix the indentation, because there does not
seem to be a way to do that using coccinelle.

Some amount of new code is currently sitting in linux-next that should get
the same modifications, which I will do at the end of the merge window.

David Howells df9ee29270 Fix IRQ flag handling naming
Fix the IRQ flag handling naming.  In linux/irqflags.h under one configuration,
it maps:

	local_irq_enable() -> raw_local_irq_enable()
	local_irq_disable() -> raw_local_irq_disable()
	local_irq_save() -> raw_local_irq_save()
	...

and under the other configuration, it maps:

	raw_local_irq_enable() -> local_irq_enable()
	raw_local_irq_disable() -> local_irq_disable()
	raw_local_irq_save() -> local_irq_save()
	...

This is quite confusing.  There should be one set of names expected of the
arch, and this should be wrapped to give another set of names that are expected
by users of this facility.

Change this to have the arch provide:

	flags = arch_local_save_flags()
	flags = arch_local_irq_save()
	arch_local_irq_restore(flags)
	arch_local_irq_disable()
	arch_local_irq_enable()
	arch_irqs_disabled_flags(flags)
	arch_irqs_disabled()
	arch_safe_halt()

Then linux/irqflags.h wraps these to provide:

	raw_local_save_flags(flags)
	raw_local_irq_save(flags)
	raw_local_irq_restore(flags)
	raw_local_irq_disable()
	raw_local_irq_enable()
	raw_irqs_disabled_flags(flags)
	raw_irqs_disabled()
	raw_safe_halt()

with type checking on the flags 'arguments', and then wraps those to provide:

	local_save_flags(flags)
	local_irq_save(flags)
	local_irq_restore(flags)
	local_irq_disable()
	local_irq_enable()
	irqs_disabled_flags(flags)
	irqs_disabled()
	safe_halt()

with tracing included if enabled.

The arch functions can now all be inline functions rather than some of them
having to be macros.

Tony Luck 4de0a75948 [IA64] Initialize interrupts later (from init_IRQ())
Thomas Gleixner is cleaning up the generic irq code, and ia64 ran
into problems because it calls register_intr() before early_irq_init()
is called.  Move the call to acpi_boot_init() from setup_arch() to
init_IRQ().

As a bonus - moving the call later means we no longer need the
hacks in iosapic.c to switch between the bootmem and regular
allocator - we can just used kzalloc() for allocation.

Mostly trivial stuff for this. The only tricky part was realizing
that the new handler_*_irq() paths do not use desc->chip->end(irq).
Not a problem for the edge case as the ia64 iosapic routine for
that was nop(). But the "level" case handled interrupt migration
there.  Just use a slightly modified version of the "end" routine
as "unmask" for the level triggered case.

H. Peter Anvin c41d68a513 compat: Make compat_alloc_user_space() incorporate the access_ok()
compat_alloc_user_space() expects the caller to independently call
access_ok() to verify the returned area.  A missing call could
introduce problems on some architectures.

This patch incorporates the access_ok() check into
compat_alloc_user_space() and also adds a sanity check on the length.
The existing compat_alloc_user_space() implementations are renamed
arch_compat_alloc_user_space() and are used as part of the
implementation of the new global function.

This patch assumes NULL will cause __get_user()/__put_user() to either
fail or access userspace on all architectures.  This should be
followed by checking the return value of compat_access_user_space()
for NULL in the callers, at which time the access_ok() in the callers
can also be removed.

Tony Luck f574c84319 [IA64] fix siglock
When ia64 converted to using ticket locks, an inline implementation
of trylock/unlock in fsys.S was missed.  This was not noticed because
in most circumstances it simply resulted in using the slow path because
the siglock was apparently not available (under old spinlock rules).

Problems occur when the ticket spinlock has value 0x0 (when first
initialised, or when it wraps around). At this point the fsys.S
code acquires the lock (changing the 0x0 to 0x1. If another process
attempts to get the lock at this point, it will change the value from
0x1 to 0x2 (using new ticket lock rules). Then the fsys.S code will
free the lock using old spinlock rules by writing 0x0 to it. From
here a variety of bad things can happen.

David Howells d7627467b7 Make do_execve() take a const filename pointer
Make do_execve() take a const filename pointer so that kernel_execve() compiles
correctly on ARM:

arch/arm/kernel/sys_arm.c:88: warning: passing argument 1 of 'do_execve' discards qualifiers from pointer target type

This also requires the argv and envp arguments to be consted twice, once for
the pointer array and once for the strings the array points to.  This is
because do_execve() passes a pointer to the filename (now const) to
copy_strings_kernel().  A simpler alternative would be to cast the filename
pointer in do_execve() when it's passed to copy_strings_kernel().

do_execve() may not change any of the strings it is passed as part of the argv
or envp lists as they are some of them in .rodata, so marking these strings as
const should be fine.

Further kernel_execve() and sys_execve() need to be changed to match.

This has been test built on x86_64, frv, arm and mips.

FUJITA Tomonori 3b9c6c11f5 dma-mapping: remove dma_is_consistent API
Architectures implement dma_is_consistent() in different ways (some
misinterpret the definition of API in DMA-API.txt).  So it hasn't been so
useful for drivers.  We have only one user of the API in tree.  Unlikely
out-of-tree drivers use the API.

Even if we fix dma_is_consistent() in some architectures, it doesn't look
useful at all.  It was invented long ago for some old systems that can't
allocate coherent memory at all.  It's better to export only APIs that are
definitely necessary for drivers.

Let's remove this API.

FUJITA Tomonori 4565f0170d dma-mapping: unify dma_get_cache_alignment implementations
dma_get_cache_alignment returns the minimum DMA alignment.  Architectures
defines it as ARCH_DMA_MINALIGN (formally ARCH_KMALLOC_MINALIGN).  So we
can unify dma_get_cache_alignment implementations.

Note that some architectures implement dma_get_cache_alignment wrongly.
dma_get_cache_alignment() should return the minimum DMA alignment.  So
fully-coherent architectures should return 1.  This patch also fixes this
issue.

This patch is against the 2.6.34 source.

Paraphrased from the 1989 BSD patch by David Borman @ cray.com:

     These are the changes needed for the kernel to support
     LINEMODE in the server.

     There is a new bit in the termios local flag word, EXTPROC.
     When this bit is set, several aspects of the terminal driver
     are disabled.  Input line editing, character echo, and mapping
     of signals are all disabled.  This allows the telnetd to turn
     off these functions when in linemode, but still keep track of
     what state the user wants the terminal to be in.

     New ioctl:
         TIOCSIG         Generate a signal to processes in the
                         current process group of the pty.

     There is a new mode for packet driver, the TIOCPKT_IOCTL bit.
     When packet mode is turned on in the pty, and the EXTPROC bit
     is set, then whenever the state of the pty is changed, the
     next read on the master side of the pty will have the TIOCPKT_IOCTL
     bit set.  This allows the process on the server side of the pty
     to know when the state of the terminal has changed; it can then
     issue the appropriate ioctl to retrieve the new state.

Since the original BSD patches accompanied the source code for telnet
I've left that reference here, but obviously the feature is useful for
any remote terminal protocol, including ssh.

The corresponding feature has existed in the BSD tty driver since 1989.

from this.

Takuya Yoshikawa 979586e0b5 KVM: ia64: cleanup kvm_ia64_sync_dirty_log()
kvm_ia64_sync_dirty_log() is a helper function for kvm_vm_ioctl_get_dirty_log()
which copies ia64's arch specific dirty bitmap to general one in memslot.
So doing sanity checks in this function is unnatural. We move these checks
outside of this and change the prototype appropriately.

Ben Hutchings 30da552428 PCI: MSI: Restore read_msi_msg_desc(); add get_cached_msi_msg_desc()
commit 2ca1af9aa3285c6a5f103ed31ad09f7399fc65d7 "PCI: MSI: Remove
unsafe and unnecessary hardware access" changed read_msi_msg_desc() to
return the last MSI message written instead of reading it from the
device, since it may be called while the device is in a reduced
power state.

However, the pSeries platform code really does need to read messages
from the device, since they are initially written by firmware.
Therefore:
- Restore the previous behaviour of read_msi_msg_desc()
- Add new functions get_cached_msi_msg{,_desc}() which return the
  last MSI message written
- Use the new functions where appropriate

Tony Luck a95f9ac271 [IA64] increase ia64 static per cpu area
I've been trying to avoid this for a long time ... but per-cpu space
has slowly been growing.  Tejun has some patches in linux-next that
pre-reserve some space (PERCPU_DYNAMIC_EARLY_SIZE) for use before
slab comes online ... and this pushes ia64 above the 64K current
limit on static percpu space.

I could probably squeeze it back under (we are only over by 512 bytes).
But I don't think that I'll be able to squeeze it down enough to build
a comfortable breathing space - and I don't want to keep nibbling off
a dozen bytes here and there every time some generic code bumps us
back over the limit.

Next available supported page size is 256K ... so we have to quadruple
the available space - a bigger jump than I'd like. But perhaps it will
be enough to last a few more years before it needs to be increased again.

John Stultz 7615856ebf timkeeping: Fix update_vsyscall to provide wall_to_monotonic offset
update_vsyscall() did not provide the wall_to_monotoinc offset,
so arch specific implementations tend to reference wall_to_monotonic
directly. This limits future cleanups in the timekeeping core, so
this patch fixes the update_vsyscall interface to provide
wall_to_monotonic, allowing wall_to_monotonic to be made static
as planned in Documentation/feature-removal-schedule.txt

Tony Luck b70f4e85bf [IA64] Fix spinaphore down_spin()
Typo in down_spin() meant it only read the low 32 bits of the
"serve" value, instead of the full 64 bits. This results in the
system hanging when the values in ticket/serve get larger than
32-bits. A big enough system running the right test can hit this
in a just a few hours.

Tejun Heo b71ab8c202 workqueue: increase max_active of keventd and kill current_is_keventd()
Define WQ_MAX_ACTIVE and create keventd with max_active set to half of
it which means that keventd now can process upto WQ_MAX_ACTIVE / 2 - 1
works concurrently.  Unless some combination can result in dependency
loop longer than max_active, deadlock won't happen and thus it's
unnecessary to check whether current_is_keventd() before trying to
schedule a work.  Kill current_is_keventd().

(Lockdep annotations are broken.  We need lock_map_acquire_read_norecurse())

Peter Zijlstra 1996bda2a4 arch: Implement local64_t
On 64bit, local_t is of size long, and thus we make local64_t an alias.
On 32bit, we fall back to atomic64_t. (architecture can provide optimized
32-bit version)

(This new facility is to be used by perf events optimizations.)

This partially reverts commit 4ec37de89d
("[IA64] Fix build breakage"), since the commit that made it necessary
got reverted earlier (see commit 35926ff5fb, 'Revert "cpusets:
randomize node rotor used in cpuset_mem_spread_node()"')

Even if we ever re-introduce this, there is no reason to make
__node_random be some architecture-specific function.

cpusets: randomize node rotor used in cpuset_mem_spread_node()

Jack Steiner fixed a problem with too many small tasks being
assigned to node 0. Copy his code to ia64 to avoid build error.

    arch/ia64/kernel/smpboot.c:641: error: ‘cpu_to_node_map’ undeclared (first use in this function)

In commit 3bccd99627
numa: ia64: use generic percpu var numa_node_id() implementation

Lee Schermerhorn added some set_numa_node() calls - but these
only work on CONFIG_NUMA=y configurations. Surround the calls
with #ifdef CONFIG_NUMA

Lee Schermerhorn fd1197f113 numa: ia64: support numa_mem_id() for memoryless nodes
Enable 'HAVE_MEMORYLESS_NODES' by default when NUMA configured on ia64.
Initialize percpu 'numa_mem' variable when starting secondary cpus.
Generic initialization will handle the boot cpu.

Nothing uses 'numa_mem_id()' yet.  Subsequent patch with modify slab to
use this.

FUJITA Tomonori 1ef04370d8 asm-generic: remove ARCH_HAS_SG_CHAIN in scatterlist.h
There are more architectures that don't support ARCH_HAS_SG_CHAIN than
those that support it.  This removes removes ARCH_HAS_SG_CHAIN in
asm-generic/scatterlist.h and lets arhictectures to define it.

It's clearer than defining ARCH_HAS_SG_CHAIN asm-generic/scatterlist.h and
undefing it in arhictectures that don't support it.

Oleg Nesterov ffdf91856c ia64: ptrace_attach_sync_user_rbs: avoid "task->signal != NULL" checks
Preparation to make task->signal immutable, no functional changes.

It doesn't matter which pointer we check under tasklist to ensure the task
was not released, ->signal or ->sighand.  But we are going to make
->signal refcountable, change the code to use ->sighand.

Note: this code doesn't need this check and tasklist_lock at all, it
should be converted to use lock_task_sighand().  And, the code under
SIGNAL_STOP_STOPPED check looks wrong.

Eric W. Biederman 2c2df8418a x86, acpi/irq: Introduce apci_isa_irq_to_gsi
There are a number of cases where the current code makes the assumption
that isa irqs identity map to the first 16 acpi global system intereupts.
In most instances that assumption is correct as that is the required
behaviour in dual i8259 mode and the default behavior in ioapic mode.

However there are some systems out there that take advantage of acpis
interrupt remapping  for the isa irqs to have a completely different
mapping of isa_irq to gsi.

Introduce acpi_isa_irq_to_gsi to perform this mapping explicitly in the
code that needs it.  Initially this will be just the current assumed
identity mapping to ensure it's introduction does not cause regressions.

Takuya Yoshikawa 87bf6e7de1 KVM: fix the handling of dirty bitmaps to avoid overflows
Int is not long enough to store the size of a dirty bitmap.

This patch fixes this problem with the introduction of a wrapper
function to calculate the sizes of dirty bitmaps.

Note: in mark_page_dirty(), we have to consider the fact that
  __set_bit() takes the offset as int, not long.

Bjorn Helgaas 57283776b2 ACPI: pci_root: pass acpi_pci_root to arch-specific scan
The acpi_pci_root structure contains all the individual items (acpi_device,
domain, bus number) we pass to pci_acpi_scan_root(), so just pass the
single acpi_pci_root pointer directly.

This will make it easier to add _CBA support later.  For _CBA, we need the
entire downstream bus range, not just the base bus number.  We have that in
the acpi_pci_root structure, so passing the pointer makes it available to
the arch-specific code.

Tejun Heo 5a0e3ad6af include cleanup: Update gfp.h and slab.h includes to prepare for breaking implicit slab.h inclusion from percpu.h
percpu.h is included by sched.h and module.h and thus ends up being
included when building most .c files.  percpu.h includes slab.h which
in turn includes gfp.h making everything defined by the two files
universally available and complicating inclusion dependencies.

percpu.h -> slab.h dependency is about to be removed.  Prepare for
this change by updating users of gfp and slab facilities include those
headers directly instead of assuming availability.  As this conversion
needs to touch large number of source files, the following script is
used as the basis of conversion.

The script does the followings.

* Scan files for gfp and slab usages and update includes such that
  only the necessary includes are there.  ie. if only gfp is used,
  gfp.h, if slab is used, slab.h.

* When the script inserts a new include, it looks at the include
  blocks and try to put the new include such that its order conforms
  to its surrounding.  It's put in the include block which contains
  core kernel includes, in the same order that the rest are ordered -
  alphabetical, Christmas tree, rev-Xmas-tree or at the end if there
  doesn't seem to be any matching order.

* If the script can't find a place to put a new include (mostly
  because the file doesn't have fitting include block), it prints out
  an error message indicating which .h file needs to be added to the
  file.

The conversion was done in the following steps.

1. The initial automatic conversion of all .c files updated slightly
   over 4000 files, deleting around 700 includes and adding ~480 gfp.h
   and ~3000 slab.h inclusions.  The script emitted errors for ~400
   files.

2. Each error was manually checked.  Some didn't need the inclusion,
   some needed manual addition while adding it to implementation .h or
   embedding .c file was more appropriate for others.  This step added
   inclusions to around 150 files.

3. The script was run again and the output was compared to the edits
   from #2 to make sure no file was left behind.

4. Several build tests were done and a couple of problems were fixed.
   e.g. lib/decompress_*.c used malloc/free() wrappers around slab
   APIs requiring slab.h to be added manually.

5. The script was run on all .h files but without automatically
   editing them as sprinkling gfp.h and slab.h inclusions around .h
   files could easily lead to inclusion dependency hell.  Most gfp.h
   inclusion directives were ignored as stuff from gfp.h was usually
   wildly available and often used in preprocessor macros.  Each
   slab.h inclusion directive was examined and added manually as
   necessary.

6. percpu.h was updated not to include slab.h.

7. Build test were done on the following configurations and failures
   were fixed.  CONFIG_GCOV_KERNEL was turned off for all tests (as my
   distributed build env didn't work with gcov compiles) and a few
   more options had to be turned off depending on archs to make things
   build (like ipr on powerpc/64 which failed due to missing writeq).

   * x86 and x86_64 UP and SMP allmodconfig and a custom test config.
   * powerpc and powerpc64 SMP allmodconfig
   * sparc and sparc64 SMP allmodconfig
   * ia64 SMP allmodconfig
   * s390 SMP allmodconfig
   * alpha SMP allmodconfig
   * um on x86_64 SMP allmodconfig

8. percpu.h modifications were reverted so that it could be applied as
   a separate patch and serve as bisection point.

Given the fact that I had only a couple of failures from tests on step
6, I'm fairly confident about the coverage of this conversion patch.
If there is a breakage, it's likely to be something in one of the arch
headers which should be easily discoverable easily on most builds of
the specific arch.

Len Brown 68ca406930 ACPI: delete the "acpi=ht" boot option
acpi=ht was important in 2003 -- before ACPI was
universally deployed and enabled by default in
the major Linux distributions.

At that time, there were a fair number of people who
or chose to, or needed to, run with acpi=off,
yet also wanted access to Hyper-threading.

Today we find that many invocations of "acpi=ht"
are accidental, and thus is it possible that it
is doing more harm than good.

In 2.6.34, we warn on invocation of acpi=ht.
In 2.6.35, we delete the boot option.

Christoph Hellwig dacbe41f77 ptrace: move user_enable_single_step & co prototypes to linux/ptrace.h
While in theory user_enable_single_step/user_disable_single_step/
user_enable_blockstep could also be provided as an inline or macro there's
no good reason to do so, and having the prototype in one places keeps code
size and confusion down.

Roland said:

  The original thought there was that user_enable_single_step() et al
  might well be only an instruction or three on a sane machine (as if we
  have any of those!), and since there is only one call site inlining
  would be beneficial.  But I agree that there is no strong reason to care
  about inlining it.

  As to the arch changes, there is only one thought I'd add to the
  record.  It was always my thinking that for an arch where
  PTRACE_SINGLESTEP does text-modifying breakpoint insertion,
  user_enable_single_step() should not be provided.  That is,
  arch_has_single_step()=>true means that there is an arch facility with
  "pure" semantics that does not have any unexpected side effects.
  Inserting a breakpoint might do very unexpected strange things in
  multi-threaded situations.  Aside from that, it is a peculiar side
  effect that user_{enable,disable}_single_step() should cause COW
  de-sharing of text pages and so forth.  For PTRACE_SINGLESTEP, all these
  peculiarities are the status quo ante for that arch, so having
  arch_ptrace() itself do those is one thing.  But for building other
  things in the future, it is nicer to have a uniform "pure" semantics
  that arch-independent code can expect.

  OTOH, all such arch issues are really up to the arch maintainer.  As
  of today, there is nothing but ptrace using user_enable_single_step() et
  al so it's a distinction without a practical difference.  If/when there
  are other facilities that use user_enable_single_step() and might care,
  the affected arch's can revisit the question when someone cares about
  the quality of the arch support for said new facility.

Christoph Hellwig e28cbf2293 improve sys_newuname() for compat architectures
On an architecture that supports 32-bit compat we need to override the
reported machine in uname with the 32-bit value.  Instead of doing this
separately in every architecture introduce a COMPAT_UTS_MACHINE define in
<asm/compat.h> and apply it directly in sys_newuname().
