This checks the errors the scsi-ml determined were retryable
and returns if we should fast fail it based on the request
fail fast flags.
Without the patch, drivers like lpfc, qla2xxx and fcoe would return
DID_ERROR for what it determines is a temporary communication problem.
There is no loss of connectivity at that time and the driver thinks
that it would be fast to retry at the driver level. SCSI-ml will however
sees fast fail on the request and DID_ERROR and will fast fail the io.
This will then cause dm-multipath to fail the path and possibley switch
target controllers when we should be retrying at the scsi layer.
We also were fast failing device errors to dm multiapth when
unless the scsi_dh modules think otherwis we want to retry at
the scsi layer because multipath can only retry the IO like scsi
should have done. multipath is a little dumber though because it
does not what the error was for and assumes that it should fail
the paths.

Multipath is best at handling transport errors. If it gets a device
error then there is not much the multipath layer can do. It will just
access the same device but from a different path.
This patch breaks up failfast into device, transport and driver errors.
The multipath layers (md and dm mutlipath) only ask the lower levels to
fast fail transport errors. The user of failfast, read ahead, will ask
to fast fail on all errors.
Note that blk_noretry_request will return true if any failfast bit
is set. This allows drivers that do not support the multipath failfast
bits to continue to fail on any failfast error like before. Drivers
like scsi that are able to fail fast specific errors can check
for the specific fail fast type. In the next patch I will convert
scsi.

This has qla2xxx use the new transport error values instead of
DID_BUS_BUSY. I am not sure if all the errors
in qla_isr.c I changed are transport related. We end up blocking/deleting
the rport for all of them so it is better to use the new transport error since
the fc classs will decide when to fail the IO.
With this patch if I pull a cable then IO that had reached
the driver, will be failed with DID_TRANSPORT_DISRUPTED (not including
tape). The fc class will then fail the IO when the fast io fail tmo
has fired, and the driver will flush any other commands running.

Currently, if there is a transport problem the iscsi drivers will return
outstanding commands (commands being exeucted by the driver/fw/hw) with
DID_BUS_BUSY and block the session so no new commands can be queued.
Commands that are caught between the failure handling and blocking are
failed with DID_IMM_RETRY or one of the scsi ml queuecommand return values.
When the recovery_timeout fires, the iscsi drivers then fail IO with
DID_NO_CONNECT.
For fcp, some drivers will fail some outstanding IO (disk but possibly not
tape) with DID_BUS_BUSY or DID_ERROR or some other value that causes a retry
and hits the scsi_error.c failfast check, block the rport, and commands
caught in the race are failed with DID_IMM_RETRY. Other drivers, may
hold onto all IO and wait for the terminate_rport_io or dev_loss_tmo_callbk
to be called.
The following patches attempt to unify what upper layers will see drivers
like multipath can make a good guess. This relies on drivers being
hooked into their transport class.
This first patch just defines two new host byte errors so drivers can
return the same value for when a rport/session is blocked and for
when the fast_io_fail_tmo fires.
The idea is that if the LLD/class detects a problem and is going to block
a rport/session, then if the LLD wants or must return the command to scsi-ml,
then it can return it with DID_TRANSPORT_DISRUPTED. This will requeue
the IO into the same scsi queue it came from, until the fast io fail timer
fires and the class decides what to do.
When using multipath and the fast_io_fail_tmo fires then the class
can fail commands with DID_TRANSPORT_FAILFAST or drivers can use
DID_TRANSPORT_FAILFAST in their terminate_rport_io callbacks or
the equivlent in iscsi if we ever implement more advanced recovery methods.
A LLD, like lpfc, could continue to return DID_ERROR and then it will hit
the normal failfast path, so drivers do not have fully be ported to
work better. The point of the patches is that upper layers will
not see a failure that could be recovered from while the rport/session is
blocked until fast_io_fail_tmo/recovery_timeout fires.

When we block a rport and the driver implements the terminate
callback we will fail IO that was running quickly. However
IO that was in the scsi_device/block queue sits there until
the dev_loss_tmo fires, and this can make it look like IO is
lost because new IO will get executed but that IO stuck in
the blocked queue sits there for some time longer.
With this patch when the fast io fail tmo fires, we will
fail the blocked IO and any new IO. This patch also allows
all drivers to partially support the fast io fail tmo. If the
terminate io callback is not implemented, we will still fail blocked
IO and any new IO, so multipath can handle that.
This patch also allows the fc and iscsi classes to implement the
same behavior. The timers are just unfornately named differently.
This patch also fixes the problem where drivers were unblocking
the target in their terminate callback, which was needed for
rport removal, but for fast io fail timeout it would cause
IO to bounce arround the scsi/block layer and the LLD queuecommand.
And it for drivers that could have IO stuck but did not have
a terminate callback the unblock calls in the class will fix
them.

If the fcport is not online then we do not want to block IO to all ports on
the host. We just want to stop IO on port not online, so we should be using
the SCSI_MLQUEUE_TARGET_BUSY return value.
For the case where we race with the rport memset initialization
we do not want the queuecommand to be called again so we can just use
SCSI_MLQUEUE_TARGET_BUSY for this.

When qla4xxx begins recovery and the iscsi class is firing up to handle
it, we need to retrn SCSI_MLQUEUE_TARGET_BUSY from the driver instead
of host busy, because the session recovery only affects the one target.

SCSI-ml manages the queueing limits for the device and host, but
does not do so at the target level. However something something similar
can come in userful when a driver is transitioning a transport object to
the the blocked state, becuase at that time we do not want to queue
io and we do not want the queuecommand to be called again.
The patch adds code similar to the exisiting SCSI_ML_*BUSY handlers.
You can now return SCSI_MLQUEUE_TARGET_BUSY when we hit
a transport level queueing issue like the hw cannot allocate some
resource at the iscsi session/connection level, or the target has temporarily
closed or shrunk the queueing window, or if we are transitioning
to the blocked state.
bnx2i, when they rework their firmware according to netdev
developers requests, will also need to be able to limit queueing at this
level. bnx2i will hook into libiscsi, but will allocate a scsi host per
netdevice/hba, so unlike pure software iscsi/iser which is allocating
a host per session, it cannot set the scsi_host->can_queue and return
SCSI_MLQUEUE_HOST_BUSY to reflect queueing limits on the transport.
The iscsi class/driver can also set a scsi_target->can_queue value which
reflects the max commands the driver/class can support. For iscsi this
reflects the number of commands we can support for each session due to
session/connection hw limits, driver limits, and to also reflect the
session/targets's queueing window.

Make drive->id an unnamed union so id can be accessed either by using
'u16 *id' or 'struct hd_driveid *driveid'. Then convert all existing
drive->id users accordingly (using 'u16 *id' when possible).
This is an intermediate step to make ide 'struct hd_driveid'-free.
While at it:
- Add missing KERN_CONTs in it821x.c.

__sg_start_req() was used temporarily to call blk_get_request() during
converting sg to use the block layer.
Now sg always calls blk_get_request() so we can move blk_get_request()
to sg_start_req(). We don't need __sg_start_req anymore.

sg had lots of the own functions for the direct IO but now sg uses the
block layer functions for it. There are only five lines for the direct
IO. SG_ALLOW_DIO_CODE define was used to compile out the direct IO
code but we don't need the define. If someone wants to remove the
direct IO code, he can do easily without the define.

old sg_rq_end_io() was used to wrap sg_cmd_done during converting sg
to use the block layer (in order to cover the difference
scsi_execute_async and blk_execute_rq_nowait). Now we don't need it so
let's remove it.

Right now SCSI and others do their own command timeout handling.
Move those bits to the block layer.
Instead of having a timer per command, we try to be a bit more clever
and simply have one per-queue. This avoids the overhead of having to
tear down and setup a timer for each command, so it will result in a lot
less timer fiddling.

Modify the SCSI disk driver to call the revalidate_disk()
wrapper. This allows us to do some housekeeping such as accounting for
a disk being resized online. The wrapper will call
sd_revalidate_disk() at the appropriate time.

With the older SG interface, we don't know a user-space address to
trasfer data when executing a SCSI command. So we can't pass a
user-space address to blk_rq_map_user.
This patch fixes sg to pass a NULL user-space address to
blk_rq_map_user so that it just sets up a request and bios with page
frames propely without data transfer.

This patch introduces struct rq_map_data to enable bio_copy_use_iov()
use reserved pages.
Currently, bio_copy_user_iov allocates bounce pages but
drivers/scsi/sg.c wants to allocate pages by itself and use
them. struct rq_map_data can be used to pass allocated pages to
bio_copy_user_iov.
The current users of bio_copy_user_iov simply passes NULL (they don't
want to use pre-allocated pages).

Now that disk and partition handlings are mostly unified, it's easy to
allow disk to have extended device number. This patch makes
add_disk() use extended device number if disk->minors is zero. Both
sd and ide-disk are updated to use this.
* sd_format_disk_name() is implemented which can generically determine
the drive name. This removes disk number restriction stemming from
limited device names.
* If sd index goes over SD_MAX_DISKS (which can be increased now BTW),
sd simply doesn't initialize minors letting block layer choose
extended device number.
* If CONFIG_DEBUG_EXT_DEVT is set, both sd and ide-disk always set
minors to 0 and use extended device numbers.

With previous changes, it's meaningless to limit the number of
partitions. Replace @ext_minors with GENHD_FL_EXT_DEVT such that
setting the flag allows the disk to have maximum number of allowed
partitions (only limited by the number of entries in parsed_partitions
as determined by MAX_PART constant).
This kills not-too-pretty alloc_disk_ext[_node]() functions and makes
@minors parameter to alloc_disk[_node]() unnecessary. The parameter
is left alone to avoid disturbing the users.

Extended devt introduces non-contiguos device numbers. This patch
implements a debug option which forces most devt allocations to be
from the extended area and spreads them out. This is enabled by
default if DEBUG_KERNEL is set and achieves...
1. Detects code paths in kernel or userland which expect predetermined
consecutive device numbers.
2. When something goes wrong, avoid corruption as adding to the minor
of earlier partition won't lead to the wrong but valid device.

Update sd and ide-disk such that they can take advantage of extended
minors.
ide-disk already has 64 minors per device and currently doesn't use
extended minors although after this patch it can be turned on by
simply tweaking constants.
sd only had 16 minors per device causing problems on certain peculiar
configurations. This patch lifts the restriction and enables it to
use upto 64 minors.

* Implement disk_devt() and part_devt() and use them to directly
access devt instead of computing it from ->major and ->first_minor.
Note that all references to ->major and ->first_minor outside of
block layer is used to determine devt of the disk (the part0) and as
->major and ->first_minor will continue to represent devt for the
disk, converting these users aren't strictly necessary. However,
convert them for consistency.
* Implement disk_max_parts() to avoid directly deferencing
genhd->minors.
* Update bdget_disk() such that it doesn't assume consecutive minor
space.
* Move devt computation from register_disk() to add_disk() and make it
the only one (all other usages use the initially determined value).
These changes clean up the code and will help disk->part dereference
fix and extended block device numbers.

The Flash Layout Table (FLT) present on many recent HBAs encodes
flash usage information, organizes data stored into separate
regions and presents the information uniformly to the driver.
Use this information rather than using specific hard-coded values
based on ISP type.

Currently Firmware does not have counters for input megabytes and
output megabytes, therefore driver counts these values depending
on the status of the scsi command and direction of the command.
The values are exported in the FC_HOST path.

Due to patch building error on my side, we are still passing DID_BUS_BUSY
for commands that are running, when we want to return whatever the caller
of fail_all_commands wanted. This replaces the hardcoded error code with
the value that is passed in.

There's already a fc_vport_termintate() call exported by
the transport. This patch adds a symmetric call to the API to allow
an NPIV-capable LLD to instantiate vports sans user intervention.

Brian King <brking@linux.vnet.ibm.com> reported that fibre channel
devices can oops during scanning if their ports block (because the
device goes from CREATED -> BLOCK -> RUNNING rather than CREATED ->
BLOCK -> CREATED).
Fix this by adding a new state: CREATED_BLOCK which can only transition
back to CREATED and disallow the CREATED -> BLOCK transition. Now both
the created and blocked states that the mid-layer recognises can include
CREATED_BLOCK.

This patch adds scsi netlink recieve and event support for transport
and scsi LLDD's. It is a reimplementation of the patch posted last
week by David Somayajulu.

There are a few things done differently:
- Transport support is included
- Event delivery is included
- The vendor message is now its own unique message type, considered
part of the generic "SCSI Transport".
- LLDD entry points are now registered rather than included in the
scsi_host_template.
Background: When I started to implement the event handler via template,
I had to either: muck up scsi_add_host and scsi_remove_host; or have
the event handler search all possible shosts. Neither was acceptable.
Moving to a registration solves this, and also limits the scope of
the changes to something that could be backported to a distro without
breaking an already-released-distro kabi. However, I admit it isn't
as elegant, as the passing of the LLDD host template in the
registration and the complexity around dynamic add/remove shows.
- The receive path was augmented to require a unique identifier for
the LLDD before the message was allowed to be handed off to the
driver. Given how quickly very fatal errors occur if there's msg
mismatches (which I saw in testing my own tools :), I believe this
to be a very good thing. The id plays off the vendor id scheme already
introduced for the vendor unique event messages used by FC.
Additionally, the id use as the basis of the registration/deregistration.
- Send assist functions, for both the transport and LLDDs are included.

This patch cleans up the behavior of scsi_host_lookup().
The original implementation attempted to use the dual role of
either returning a pointer value, or a negative error code.
User's needed to use IS_ERR() to check the result. Additionally,
the IS_ERR() macro never checks for when a NULL pointer was
returned, so a NULL pointer actually passes with a success case.
Note: scsi_host_get(), used by scsi_host_lookup(), can return
a NULL pointer.
Talk about a mudhole for the unitiated to step into....
This patch converts scsi_host_lookup() to return either NULL
or a valid pointer. The consumers were updated for the change.

Logically, SCR access ops should take @link; however, there was no
compelling reason to convert all SCR access ops when adding @link
abstraction as there's one-to-one mapping between a port and a non-PMP
link. However, that assumption won't hold anymore with the scheduled
addition of slave link.
Make SCR access ops per-link.

The current sg list traversal logic for the continuation entries
doesn't advance the list pointer once all seven slots are used, so the
next continuation entry (if there is one) wrongly begins again at the
start of the sg list.
Fix by advancing the sg pointer after the for_each_sg().

Sometimes, particularly for USB devices with the last sector bug,
requests get completed in chunks. There's a bug in this in that if
one of the chunks gets an error, we complete that chunk with an error
but never move on to the remaining ones, leading to the request
hanging (because it's not fully completed).
Fix this by completing all remaining chunks if an error is encountered.

For IBM z series certain LUNs can no longer be accessed.
This is because kernel version 2.6.19 a check was introduced not to
create a generic SCSI device for devices that return PQ=1 and
PDT=0x1f. For WLUNs (see SAM-3, p. 41ff) generic SCSI devices should
be created unconditionally without looking at the PQ bit, so add a
check for WLUNs in with this test.

This thing was completely pointless.
Just find the OF device in the parent of drivers that want to program
this device, and map the DMA regs inside such drivers too.
This also moves the dummy claim_dma_lock() and release_dma_lock()
implementation to floppy_32.h, which makes it handle this issue
just like floppy_64.h does.

cmd_filter works only for the block layer SG_IO with SCSI block
devices. It breaks scsi/sg.c, bsg, and the block layer SG_IO with SCSI
character devices (such as st). We hit a kernel crash with them.
The problem is that cmd_filter code accesses to gendisk (having struct
blk_scsi_cmd_filter) via inode->i_bdev->bd_disk. It works for only
SCSI block device files. With character device files, inode->i_bdev
leads you to struct cdev. inode->i_bdev->bd_disk->blk_scsi_cmd_filter
isn't safe.
SCSI ULDs don't expose gendisk; they keep it private. bsg needs to be
independent on any protocols. We shouldn't change ULDs to expose their
gendisk.
This patch moves struct blk_scsi_cmd_filter from gendisk to
request_queue, a common object, which eveyone can access to.
The user interface doesn't change; users can change the filters via
/sys/block/. gendisk has a pointer to request_queue so the cmd_filter
code accesses to struct blk_scsi_cmd_filter.

If the client virtual fibre channel adapter is already logged into the
server and does an NPIV Login again, the async queue, which is used for
reporting Link Up/Link Down type of events, does not get reset on the
server side. Fix up the client driver so that we also do not reset it.
This fixes a problem of lost async events following relogins.

If an ELS is received while the virtual fibre channel adapter is going
through its discovery, a flag is set which causes discovery to get
re-driven. However, the hosts's state does not get set back to
IBMVFC_INITIALIZING and scsi_block_requests does not get called again,
which can result in queuecommand ops getting sent during
discovery. This should not occur and may cause problems. One example
is that we may no longer be logged into the target we send the command
to, resulting in a failure which should not have occurred.

This fixes a hang on module removal. The module removal code was setting
the hosts's state to IBMVFC_HOST_OFFLINE before tearing down the kernel
thread, but, due to a bug in ibmvfc_wait_while_resetting, was not waiting
for the kernel thread's offlining work to be done prior to destroying
the kernel thread, which left the scsi host in a blocked state which we
never got out of.

When running ibmvscsi in a shared memory partition, it must provide
a default value for the amount of DMA resources it will need in order to
perform reasonably well. This was being calculated in sectors rather than
bytes, as it should. This patch fixes this.

/sys/bus/pci/drivers/megaraid_sas/dbg_lvl defaults to being
world-writable, which seems bad (letting any user affect kernel driver
behavior and logging level).
This turns off group and user write permissions, so that on typical
production systems only root can write to it.

During internal testing, we've seen issues (hangs) with the
'deferred' vport tear-down-processing typically accompanied with
the fc_remove_host() call. This is due to the current
implementation's back-end vport handling being performed by the
physical-HA's DPC thread where premature shutdown could lead to
latent vport requests without a processor.
This should also address a problem reported by Gal Rosen
(http://marc.info/?l=linux-scsi&m=121731664417358&w=2) where the
driver would attempt to awaken a previously torn-down DPC thread
from interrupt context by implicitly calling wake_up_process()
rather than the driver's qla2xxx_wake_dpc() helper. Rather, than
reshuffle the remove_one() device-removal code, during unload,
depend on the driver's timer to wake-up the DPC process, by
limiting wake-ups based on an 'unloading' flag.

* Use correct 'ha' to mark a device lost from ISR.
I/Os will always be returned on the physical-HA.
qla2x00_mark_device_lost() should be called with the HA bound
to the fcport.
* Mark *all* devices lost during ISP-ABORT (bighammer).
These fixes correct issues discovered locally where during
link-perturbation and heavy vport-I/O fcport/rport states would
stray and an rport's scsi-target lost (timed-out).

on a reboot of a previously recognized SCST target, the initiator
driver would be unable to re-recognize the device as a target.
It turns out that prior to the SCST software reloading and
returning it's "target-capable" abilities in the PRLI payload,
the HBA would be re-initialized as an initiator-only type port.
Since initiators typically classify themselves as an FCP-2
capable device, both software and firmware do not perform an
explicit logout during port-loss. Unfortunately, as can be seen
by the failure case, when the port (now target-capable) returns,
firmware performs an ADISC without a follow-on PRLI, leaving
stale 'initiator-only' data in the firmware's port database.
Correct the discrepancy by performing the explicit logout during
the transport's request to terminate-rport-io, thus synchronizing
port states and ensuring a follow-on PRLI is performed.

which was reverted due to the regression it caused by commit
fca082c9f1.
That regression was not root-caused by the original commit, it was just
uncovered by it, and the real fix was done by Alan Stern in commit
580da34847 ("Fix USB storage hang on
command abort").
We can thus re-introduce the change that was confirmed by Alan Jenkins
to be still required by his odd card reader.

The guilty commit turned out to be 08da591e14
("ide: add ide_device_{get,put}() helpers"). ide_device_put() is called
before kref_put() in ide_cd_put() so IDE device is already gone by the time
ide_cd_release() is reached.
Fix it by calling ide_device_get() before kref_get() and ide_device_put()
after kref_put() in all affected device drivers.

Brown paper bag time. In v1 cd->drive was referenced after dropping last
reference on cd object (which could result in OOPS in ide_device_put() as
reported/debugged by Mariusz Kozlowski). Fix it by caching cd->drive in
the local variable (fix other device drivers too).

Converting page lock to new locking bitops requires a change of page flag
operation naming, so we might as well convert it to something nicer
(!TestSetPageLocked_Lock => trylock_page, SetPageLocked => set_page_locked).
This also facilitates lockdeping of page lock.

This reverts commit 2b14290078, since it
seems to break some other USB storage devices (at least a JMicron USB to
ATA bridge). As such, while it apparently fixes some cardreaders, it
would need to be made conditional on the exact reader it fixes in order
to avoid causing regressions.

The class_device->device conversion is causing an oops in revalidate
because it's assuming that the device_for_each_child iterator will only
return struct scsi_device children. The conversion made all former
class_devices children of the device as well, so this assumption is
broken. Fix it.

The last_sector_bug flag was added to work around a bug in certain usb
cardreaders, where they would crash if a multiple sector read included the
last sector. The original implementation avoids this by e.g. splitting an 8
sector read which includes the last sector into a 7 sector read, and a single
sector read for the last sector. The flag is enabled for all USB devices.
This revealed a second bug in other usb cardreaders, which crash when they
get a multiple sector read which stops 1 sector short of the last sector.
Affected hardware includes the Kingston "MobileLite" external USB cardreader
and the internal USB cardreader on the Asus EeePC.
Extend the last_sector_bug workaround to ensure that any access which touches
the last 8 hardware sectors of the device is a single sector long. Requests
are shrunk as necessary to meet this constraint.
This gives us a safety margin against potential unknown or future bugs
affecting multi-sector access to the end of the device. The two known bugs
only affect the last 2 sectors. However, they suggest that these devices
are prone to fencepost errors and that multi-sector access to the end of the
device is not well tested. Popular OS's use multi-sector accesses, but they
rarely read the last few sectors. Linux (with udev & vol_id) automatically
reads sectors from the end of the device on insertion. It is assumed that
single sector accesses are more thoroughly tested during development.

As the original code would incorrectly call the non-ISP24xx/25xx
callbacks during recovery, a stop-firmware failure could result
in improper bit-banging of the RISC and in some cases manifest in
a NMI-watchdog trigger due to the RISC not coming out of its
reset state.

SCSI: fix race in device_create
There is a race from when a device is created with device_create() and
then the drvdata is set with a call to dev_set_drvdata() in which a
sysfs file could be open, yet the drvdata will be NULL, causing all
sorts of bad things to happen.
This patch fixes the problem by using the new function,
device_create_drvdata(). It fixes the problem in all of the scsi
drivers that need it.

The problem is ch_probe stores ch's private data at a wrong place.
We need to store it at scsi_device->sdev_gendev but the above patch
stores it at device struct that device_create_drvdata returns. So we
hit an oops when ch_remove accesses
scsi_device->sdev_gendev->driver_data, which is NULL.
Actually, there wasn't a race because ch doesn't create sysfs files
with device struct that device_create returns. This patch puts back
dev_set_drvdata() to set ch's private data properly.

Properly setup the size of the async event queue. This fixes a bug where async events

If certain ELS events are received during module removal, after the kthread
is stopped, the rmmod can hang. This fixes the ibmvfc driver so that ELS
events during rmmod are ignored by stopping all device activity prior to
killing the kthread and also changes reinitialization to not attempt a reinit
if the adapter has been taken offline.

This patch frees the luntbl dma area in sym_hcb_free if allocated.
Since the luntbl is part of a larger dma coherent area not freeing the
luntbl kept a 64k dma coherent area previous allocated through
dma_alloc_coherent allocated. This prevented a DLPAR remove IO
operation from completing successfully.

This patch (as1116) fixes a bug in scsi_eh_prep_cmnd() and
scsi_eh_restore_cmnd(). These routines are supposed to save any
values they change and restore them later, but someone forgot to
save & restore scmd->underflow.
This fixes part of the problem reported in Bugzilla #9638.

Support for controllers and disks that implement DIF protection
information:
- During command preparation the RDPROTECT/WRPROTECT must be set
correctly if the target has DIF enabled.
- READ(6) and WRITE(6) are not supported when DIF is on.
- The controller must be told how to handle the I/O via the
protection operation field in scsi_cmnd.
- Refactor the I/O completion code that extracts failed LBA from the
returned sense data and handle DIF failures correctly.
- sd_dif.c implements the functions required to prepare and complete
requests with protection information attached.

If a disk is formatted with protection information (Inquiry bit
PROTECT=1) it is required to support Read Capacity(16). Force use of
the 16-bit command in this case and extract the P_TYPE field which
indicates whether the disk is formatted using DIF Type 1, 2 or 3.
The ATO (App Tag Own) bit in the Control Mode Page indicates whether
the storage device or the initiator own the contents of the
DIF application tag.

are data integrity capable.
- Add support for mapping an extra scatter-gather list containing
the protection information.
- Allocate protection scsi_data_buffer if host is DIX (integrity DMA)
capable.
- Accessor function for checking whether a device has protection
enabled.

Controllers that support DMA of protection information must be told
explicitly how to handle the I/O. The controller has no knowledge of
the protection capabilities of the target device so this information
must be passed in the scsi_cmnd.
- The protection operation tells the HBA whether to generate, strip or
verify protection information.
- The protection type tells the HBA which layout the target is
formatted with. This is necessary because the controller must be
able to correctly interpret the included protection information in
order to verify it.
- When a scsi_cmnd is reused for error handling the protection
operation must be cleared and saved while error handling is in
progress.
- prot_op and prot_type are placed in an existing hole in scsi_cmnd
and don't cause the structure to grow.

Controllers that support protection information must indicate this to
the SCSI midlayer so that the ULD can prepare scsi_cmnds accordingly.
This patch implements a host mask and various types of protection:
- DIF Type 1-3 (between HBA and disk)
- DIX Type 0-3 (between OS and HBA)
The patch also allows the HBA to set the guard type to something
different than the T10-mandated CRC.

multipath keeps a separate device table which may be
more current than the built-in one.
So we should make sure to always call ->attach whenever
a multipath map with hardware handler is instantiated.
And we should call ->detach on removal, too.

This patch converts the EMC device handler to use a proper
state machine. We now also parse the extended INQUIRY
information to determine if long trespass commands are
supported. And we're now using the long trespass command
correctly. And finally there's now an check at init time
to refuse to attach to devices not supporting EMC-specific
VPD pages.

Implement a 'dh_state' sdev attribute for dynamic device handler
manipulation. A read on the attribute will return the name of
the currently attached device handler or 'detached' if no handler
is attached.
The attribute allows the following strings to be written:
- The name of the device handler to be attached if the state is
'detached'.
- 'activate' to trigger path activation if a device handler
is attached.
- 'detach' to detach the currently attached device handler.

Instead of having each and every driver implement its own
device table scanning code we should rather implement a common
routine and scan the device tables there.
This allows us also to implement a general notifier chain
callback for all device handler instead for one per handler.

Update index allocation as follows.
* sd_index_idr is used only for ID allocation and mapping
functionality is not used. Use more memory efficient ida instead.
* idr and ida have their own locks inside them and don't need them for
operation. Drop it.
* index wasn't freed if probing failed after index allocation. fix
it.
* ida allocation should be repeated if it fails with -EAGAIN.

When drivers use a shared tag map we can end up with more requests
than tags, because the tag map is shost->can_queue tags and there
can be sdevs * sdev->queue_depth requests. In scsi_request_fn
if tag allocation fails we just drop down to just dequeueing the
tag without a tag. The problem is that drivers using the shared tag
map rely on a valid tag always being set, because it will use the
tag number to lookup commands later.
This patch has us check if we got a valid tag when the host lock
is held right before we check if the host queue is ready. We do the
check here because to allocate the tag we need the q lock, but
if the tag is bad we want to add the device/q onto the starved list
which requires the host lock.

Currently qla4xxx and stex pass in their can_queue values into
scsi_activate_tcq because they wanted the tag map that large.
The problem with this is that it ends up also setting the queue
depth to that large value. All we want to do this in this case
is set the device queue depth and the other device settings.
We do not need to touch the tag map sizing because the drivers
had setup that map according to their can_queue limits when the
shared map was created.
The scsi mid layer in request_fn will then handle the case where we
have more requests than available tags when it checks the host
queue ready function.

Mike Christie noticed a bogus memset. It can be removed as dead code
since the number of bytes in the driver buffer in fixed block mode is
always a multiple of the tape block size.

There were several issues here, one, during RSCN handling if a
follow-on RSCN occurred (within interrupt context) the DPC thread
could inadvertantly leave the fcport in a stale lost state.
Secondly, scheduled rport removal is handled exclusively by the
'parent' DPC thread, so wake up the proper thread. Finally,
process vport loop-resync's only when the vport has in an
"active" state (ID acquired).

The firmware group has suggested that FCE (Fibre Channel Event)
tracing be enabled prior to EFT (Extended Firmware Tracing) to
maximize the capturing of data on the wire. This change has no
real semantic effect on driver operation, as it's mostly a
shuffling of code.

Recent ISPs have this information written at manufacturing time,
so use the information. This also reduces future churn of the
qla_devtbl.h file contents, as the driver can now depend on the
information to be present in VPD.

iIDMA support requires the driver issue several additional
fabric-managegment (FM) commands per port discovered during SNS
scanning -- GFPN (Get Fabric Port Name) and GPSC (Get Port Speed
Capabilities). It has been found during testing that some
switches do not respond as *well* as expected to these commands
(silence -- no ACC nor BS_RJT). So, to handle such conditions,
allow the user the ability to indirectly disable the FM commands
by disabling iIDMA with the ql2xiidmaenable module-parameter.

Add per-device dma_mapping_ops support for CONFIG_X86_64 as POWER
architecture does:
This enables us to cleanly fix the Calgary IOMMU issue that some devices
are not behind the IOMMU (http://lkml.org/lkml/2008/5/8/423).
I think that per-device dma_mapping_ops support would be also helpful for
KVM people to support PCI passthrough but Andi thinks that this makes it
difficult to support the PCI passthrough (see the above thread). So I
CC'ed this to KVM camp. Comments are appreciated.
A pointer to dma_mapping_ops to struct dev_archdata is added. If the
pointer is non NULL, DMA operations in asm/dma-mapping.h use it. If it's
NULL, the system-wide dma_ops pointer is used as before.
If it's useful for KVM people, I plan to implement a mechanism to register
a hook called when a new pci (or dma capable) device is created (it works
with hot plugging). It enables IOMMUs to set up an appropriate
dma_mapping_ops per device.
The major obstacle is that dma_mapping_error doesn't take a pointer to the
device unlike other DMA operations. So x86 can't have dma_mapping_ops per
device. Note all the POWER IOMMUs use the same dma_mapping_error function
so this is not a problem for POWER but x86 IOMMUs use different
dma_mapping_error functions.
The first patch adds the device argument to dma_mapping_error. The patch
is trivial but large since it touches lots of drivers and dma-mapping.h in
all the architecture.
This patch:
dma_mapping_error() doesn't take a pointer to the device unlike other DMA
operations. So we can't have dma_mapping_ops per device.
Note that POWER already has dma_mapping_ops per device but all the POWER
IOMMUs use the same dma_mapping_error function. x86 IOMMUs use device
argument.

Enable the driver to function in a Cooperative Memory Overcommitment (CMO)
environment.
The following changes are made to enable the driver for CMO:
* DMA mapping errors will not result in error messages if entitlement has
been exceeded and resources were not available.
* The driver has a get_desired_dma function defined to function
in a CMO environment. It will indicate how much IO memory it would like
to function.

On 32-bit architectures PAGE_ALIGN() truncates 64-bit values to the 32-bit
boundary. For example:
u64 val = PAGE_ALIGN(size);
always returns a value < 4GB even if size is greater than 4GB.
The problem resides in PAGE_MASK definition (from include/asm-x86/page.h for
example):

The "~" is performed on a 32-bit value, so everything in "and" with
PAGE_MASK greater than 4GB will be truncated to the 32-bit boundary.
Using the ALIGN() macro seems to be the right way, because it uses
typeof(addr) for the mask.
Also move the PAGE_ALIGN() definitions out of include/asm-*/page.h in
include/linux/mm.h.

This mirrors the functionality that driver_find_device has as well.
We add a start variable, and all callers of the function are fixed up at

With broken Sparc64 IOMMU accounting, the kernel submits larger requests

ide_do_drive_cmd is called only with ide_preempt action argument. So
we can remove the action argument in ide_do_drive_cmd and ide_action_t
typedef.
This patch also includes two minor cleanups: 1) ide_do_drive_cmd
always succeeds so we don't need the return value; 2) the callers use
blk_rq_init before ide_do_drive_cmd so there is no need to initialize
rq->errors.

All the callers of ide_do_drive_cmd() except for ide-scsi use
ide_preempt action argument. This converts ide-scsi to use
blk_execute_rq_nowait instead of ide_do_drive_cmd so that we can
remove the action argument in ide_do_drive_cmd and ide_action_t
typedef.

Always call SELECT_MASK(..., 0) in ide_tf_load() (needs to be done
to match ide_set_irq(..., 1)) and then remove IDE_TFLAG_NO_SELECT_MASK
taskfile flag.
This change should only affect hpt366 and icside host drivers since
->maskproc(..., 0) for sgiioc4 is equivalent to ide_set_irq(..., 1).

Adds a new scsi_device flag, start_stop_pwr_cond: If enabled, the sd
driver will not send plain START STOP UNIT commands but ones with the
power condition field set to 3 (standby) or 1 (active) respectively.
Some FireWire disk firmwares do not stop the motor if power condition is
zero. Or worse, they become unresponsive after a START STOP UNIT with
power condition = 0 and start = 0.

This patch only adds the necessary code to sd_mod but doesn't activate
it. Follow-up patches to the FireWire drivers will add detection of
affected devices and enable the code for them.
I did not add power condition values to scsi_error.c::scsi_eh_try_stu()
for now. The three firmwares which suffer from above mentioned problems
do not need START STOP UNIT in the error handler, and they are not
adversely affected by START STOP UNIT with power condition = 0 and start
= 1 (like scsi_eh_try_stu() sends it if scsi_device.allow_restart is
enabled).

The sym53c8xx driver, for some reason, seems to mostly re-implement
linux/list.h with added bogosity. The main one is it's implementation
of sym_que_entry which spits warnings with recent gcc's on some
64 bits architectures.

scsi_lib.c:scsi_host_queue_ready() plugs the device with incorrect
locking. It should actually have the queue lock held, but it's
holding the host lock. Fix this by eliminating the call. The host
ready has no need to plug the queue because if it returns 0 in
scsi_request_function control transfers to not_ready which acquires
the queue lock and plugs the device if its at zero depth.

This patch adds a new device driver to support the Virtual Fibre Channel
interface on IBM Power based servers. The Virtual I/O Server on IBM Power
servers utilizes N-Port ID Virtualization to export a Virtual Fibre Channel
adapter to the client. This driver is the client device driver.

The features enable bit has to be set in the config2 register
before we can be absolutely sure we will probe a correct
part unique ID and family code from the transfer-count-high
register.
Also, reload the CFACT, STP, SOFF, and TIMEO near the end of
esp_reset_esp().

class_find_device gets a ref to the device so we must release it.
The class will serialize access to the ep so we do not have to worry
about a remove racing with the callers access, so we can simplify the
use and drop the ref right away.

This patch fixes two bugs that are related.
1. Old tools did not set can_queue/cmds_max. This patch modifies
libiscsi so that when we add the host we catch this and set it
to the default.
2. iscsi_tcp thought that the scsi command that was passed to
the eh functions needed a iscsi_cmd_task allocated for it. It
only needed a mgmt task, and now it does not matter since it
all comes from the same pool and libiscsi handles this for the
drivers. ib_iser had copied iscsi_tcp's code and set can_queue
to its max - 1 to handle this. So this patch removes the max -1,
and just sets it to the max.

Prior handler was only waiting for I/O on one lun to finish before
returning completion. Now, wait for all LUNs on the target.

under a 2.6.18-8.el5 kernel. Traced it to a garbage-in/garbage-out
ioctl condition in the aacraid driver.
Adaptec's special ioctl scb passthrough needs to check the validity of
the individual scatter gather count fields to the maximum the adapter
supports. Doing so will have the side effect of preventing
copy_from_user() from bugging out while populating the dma buffers.
This is a hardening effort, issue was triggered by an errant version
of the management tools and thus the BUG should not be seen in the
field.

The recv lock was defined so the iscsi layer could block
the recv path from processing IO during recovery. It
turns out iser just set a lock to that pointer which was pointless.
We now disconnect the transport connection before doing recovery
so we do not need the recv lock. For iscsi_tcp we still stop
the recv path incase older tools are being used.
This patch also has iscsi_itt_to_ctask user grab the session lock
and has the caller access the task with the lock or get a ref
to it in case the target is broken and sends a tmf success response
then sends data or a response for the command that was supposed to
be affected bty the tmf.

This adds two new attrs used for creating initiator ports and
binding sessions to hardware.
The session level initiatorname:
Since bnx2i does a scsi_host per host device, we need to add the
iface initiator port settings on the session, so we can create
multiple initiator ports (each with different inames) per device/scsi_host.
The current iname reflects that qla4xxx can have one iname per hba, and we are
allocating a host per session for software. The iname on the host will
remain so we can export and set the hba level qla4xxx setting.
The ifacename attr:
To bind a session to a some peice of hardware in userspace we maintain
some mappings, but during boot or iscsid restart (iscsid contains the user
space part of the driver) we need to be able to figure out which of those
host mappings abstractions maps to certain sessions. This patch adds
a ifacename attr, which userspace can set to id the host side of the
endpoint across pivot_roots and iscsid restarts.

it is virtual, so we just check if we are sent a ep and fail
if we are.

Add sysfs representation for the endpoint, so userspace can match the
host and session to the endpoint. This will allow us to set the host's
parent correctly at host creation time.
The next patches will convert tcp and iser, and fix iser's dma_mask
bug.

Currently we duplicate the list of sessions, because we were using the
test for if a session was on the host list to indicate if the session
was bound or unbound. We can instead use the target_id and fix up
the class so that drivers like bnx2i do not have to manage the target id
space.

There is no need to have the mgmt and cmd tasks separate
structs. It used to save a lot of memory when we overprealocated
memory for tasks, but the next patches will set up the
driver so in the future they can use a mempool or some other
common scsi command allocator and common tagging.

Currently to get a ctask from the session cmd array, you have to
know to use the itt modifier. To make this easier on LLDs and
so in the future we can easilly kill the session array and use
the host shared map instead, this patch adds a nice wrapper
to strip the itt into a session->cmds index and return a ctask.

After the stop_conn callback has returned the LLD should not
touch the scsi cmds. iscsi_tcp and libiscsi use the
conn->recv_lock and suspend_rx field to halt recv path
processing, but iser does not have any protection.
This patch modifies iser so that userspace can just
call the ep_disconnect callback, which will halt
all recv IO, before calling the stop_conn callback so
we do not have to worry about the conn->recv_lock and
suspend rx field. iser just needs to stop the send side
from accessing the ib conn.
Fixup to handle when the ep poll fails and ep disconnect
is called from Erez.

This removes the session and conn data_size fields from the iscsi_transport.
Just pass in the value like with host allocation. This patch also makes
it so the LLD iscsi_conn data is allocated with the iscsi_cls_conn.

max_cmd_len and max_conn are not really used. max_cmd_len is
always 16 and can be set by the LLD. max_conn is always one
since we do not support MCS.

iscsi offload (bnx2i and qla4xx) allocate a scsi host per hba,
so the session creation path needs a shost/host_no argument.
Software iscsi/iser will follow the same behabior as before
where it allcoates a host per session, but in the future iser
will probably look more like bnx2i where the host's parent is
the hardware (rnic for iser and for bnx2i it is the nic), because
it does not use a socket layer like how iscsi_tcp does.

Currently, ipr does not support HDIO_GET_IDENTITY to SATA devices.
An oops occurs if userspace attempts to send the command. Since hald
issues the command, ensure we fail the ioctl in ipr. This is a
temporary solution to the oops. Once the ipr libata EH conversion
is upstream, ipr will fully support HDIO_GET_IDENTITY.

This patch (as1108) fixes a problem that can occur with certain USB
mass-storage devices: They return invalid data together with a residue
indicating that the data should be ignored. Rather than leave the
invalid data in a transfer buffer, where it can get misinterpreted,
the patch clears the invalid portion of the buffer.
This solves a problem (wrong write-protect setting detected) reported
by Maciej Rutecki and Peter Teoh.

blk_rq_map_kern can handle the stack buffers correctly (avoid DMA
from/to the stack buffers by using the bounce buffer) so we don't need
to complicate the code by allocating just 8 bytes.

Update acornscsi as per all the other ecard drivers to use MMIO
accessors rather than the obsolete 'pc io' style inb/outb accessors.
Use ecard_request_resources()/ecard_release_resources() for easier

The esp driver currently does hand rolled reference counting of its
target. It's much easier to do what it needs to do if it's plugged into
the mid-layer callbacks (target_alloc and target_destroy) which were
designed for this case, so do it this way and get rid of the internal
target reference count.

The problem here is that tp->starget is set every time a lun
is allocated for a particular target so we can catch the
sdev_target parent value.
The reset handler uses the NULL'ness of this value to determine
which targets are active.
But esp_slave_destroy() does not NULL out this value when appropriate.
So for every target that doesn't respond, the SCSI bus scan causes
a stale pointer to be left here, with ensuing crashes like you're
seeing.

All of the open() functions which don't need the BKL on their face may
still depend on its acquisition to serialize opens against driver
initialization. So make those functions acquire then release the BKL to be
on the safe side.

scsi: use class iteration api
Isn't a correct replacement for the original hand rolled host
lookup. The problem is that class_find_child would get a reference to
the host's class device which is never released. Since the host class
device holds a reference to the host gendev, the host can never be
freed.
In 2.6.26 we started using class_find_device, and this function also
gets a reference to the device, so we end up with an extra ref
and the host will not get released.
This patch adds a put_device to balance the class_find_device() get. I
kept the scsi_host_get in scsi_host_lookup, because the target layer
is using scsi_host_lookup and it looks like it needs the SHOST_DEL
check.

If you delay 30s or more before mounting a CD after inserting it then
the kernel has the wrong value for the CD size.

The problem is in sr_test_unit_ready(): the function eats unit
attentions without adjusting the sdev->changed status. This means
that when the CD signals changed media via unit attention, we can
ignore it. Fix by making sr_test_unit_ready() adjust the changed
status.

USB sometimes doesn't return an error but instead returns a residue
value indicating part (or all) of the command wasn't completed. So if
the driver _done() error processing indicates the command was fully
processed, subtract off the residue so that this USB error gets
propagated.

Some of the storage devices (that can be accessed through multiple paths),
do need some special handling for
1. Activating the passive path of the storage access.
2. Decode and handle the special sense codes returned by the devices.
3. Handle the I/Os being sent to the passive path, especially
during the device probe time.
when accessed through multiple paths.
As of today this special device handling is done at the dm-multipath
layer using dm-handlers. That works well for (1); for (2) to be handled
at dm layer, scsi sense information need to be exported from SCSI to dm-layer,
which is not very attractive; (3) cannot be done at all at the dm layer.
Device handler has been moved to SCSI mainly to handle (2) and (3) properly.

[SCSI] add scsi_host and scsi_target to scsi_bus
which added scsi_bus_type to the struct scsi_target device. This
causes both the scsi_device and scsi_target to fire scsi_bus_uevents.
However, the actualy scsi_bus_uevent() call assumes blindly that it's
a struct scsi_device. Check for this and return immediately if it
isn't.

There is a race from when a device is created with device_create() and
then the drvdata is set with a call to dev_set_drvdata() in which a
sysfs file could be open, yet the drvdata will be NULL, causing all
sorts of bad things to happen.
This patch fixes the problem by using the new function,
device_create_drvdata(). It fixes the problem in all of the scsi
drivers that need it.

Which shows that lockdep is detecting the driver's
interrupt-handler is run in both process and interrupt context
with irqs-enabled in the former case.
During init-time and error-recovery (after a RISC reset), the
driver disables interrupts and 'polls' for completions by calling
qla2x00_poll():

Recent firmwares no longer return the 'number of buffers' in
mailbox6. The original code may result in a potential panic
during a FW-dump process due to the driver misinterpreting the
size of the allocated buffer.

The qla1280 driver was ANDing the output value of mailbox register
0 with (1 << target-number) to determine whether to enable queueing
on the target in question.
But mailbox register 0 has the status code for the mailbox command
(in this case, Set Target Parameters). Potential values are:

So clearly that is in error. I can't think what the author of that
line was looking for in a mailbox register, so I just eliminated the
AND. flag is used later in the function, and I think that the later
usage was also wrong, though it was used to set values that aren't
used. Oh well, an overhaul of this driver is not what I want to do
now -- just a bugfix.
After the fix, I found that my disks were getting a queue depth of
255, which is far too many. Most SCSI disks are limited to 32 or
64. In any case, there's no point, queueing up a bunch of commands
to the adapter that will just result in queue full or starve other
targets from being issued commands due to running out of internal
memory. So I dropped default queue depth to 32 (from which 1 is
subtracted elsewhere, giving net of 31).
I tested with a Seagate ST336753LC, and results look good, so
I'm satisfied with this patch.

If the ping tmo is longer than the recv tmo then we could miss a window
where we were supposed to check the recv tmo. This happens because
the ping code will set the next timeout for the ping timeout, and if the
ping executes quickly there will be a long chunk of time before the
timer wakes up again.
This patch has the ping processing code kick off a recv
tmo check when getting a nop in response to our ping.

The following patch fixes a bug in the iscsi nop processing.
The target sends iscsi nops to ping the initiator and the
initiator has to send nops to reply and can send nops to
ping the target.
In 2.6.25 we moved the nop processing to the kernel to handle
problems when the userspace daemon is not up, but the target
is pinging us, and to handle when scsi commands timeout, but
the transport may be the cause (we can send a nop to check
the transport). When we added this code we added a bug where
if the transport timer wakes at the exact same time we are supposed to check
for a nop timeout we drop the session instead of checking the transport.
This patch checks if a iscsi ping is outstanding and if the ping has
timed out, to determine if we need to signal a connection problem.

This message appears on modprobe/rmmod/modprobe of the driver. It's
caused because if the driver has no instances, it returns an error
from gdth_init, which causes the module to fail to load.
Unfortunately, the module's pci driver is still registered at this
point.
Fix this by making gdth behave like a modern driver and insert even if
it doesn't find any instances (in case of hot plug or software driven
binding).

The global timer handling is problematic in that if someone unbinds a
PCI gdth instance, the BUG_ON() in the timer will cause a panic.
Fix this by making the timer start and stop depending on whether there
are instances present. This should also permit binding and unbinding
to work.

Remove virt_to_bus/bus_to_virt code from dpt_i2o, and use
dma_alloc_coherent() / dma_free_coherent().
This is in preparation of 64-bit support, dma_alloc_coherent()
can allocate memory in the lower 32 bits of physical memory
which is needed because the HBA only supports message blocks under 4GB
This code is based in part on the unofficial adaptec 64-bit
dpt_i2o driver update that I got from Mark Salyzyn at Adaptec.

Update dpt_i2o.c to use the standard __init / __exit
code instead of the legacy '#include "scsi_module.c"' code.
This is needed in preparation of 64-bit support. scsi_module.c
calls scsi_add_host() with the device pointer set to NULL, and that
crashes code like arch/x64/kernel/pci-gart_64.c::need_iommu().
The reboot_notifier code is deleted because it wasn't compiled
in ever anyway, and it would be useless to duplicate it in
the new code.

For firmware that supports the feature(s), add the ability to start or
stop an array using the associated SCSI commands, to automatically
manage the spin-up of an array on new I/O reporting back the
appropriate check conditions and actions in cooperation with the
normal timeout mechanisms and enable the blackout period management in
the Firmware associated with the background spin-down of the arrays
when the Firmware times out and deems the arrays as idle.

As JBOD devices (really just Simple Single Drive Volumes exported to
the SCSI channel) are managed, they fail to update correctly when the
driver triggers a SCSI scan. In addition, the ability to change
multiple arrays or JBODs at the same time was resulting in dropped
scans, set up a mechanism to issue a list of single target scans on a
single configuration change notification from the Firmware.
Performed some additional sundry cosmetic code style cleanups.

Add support for variable-length, extended, and vendor specific
CDBs to scsi-ml. It is now possible for initiators and ULD's
to issue these types of commands. LLDs need not change much.
All they need is to raise the .max_cmd_len to the longest command
they support (see iscsi patch).
- clean-up some code paths that did not expect commands to be
larger than 16, and change cmd_len members' type to short as
char is not enough.

- struct scsi_cmnd had a 16 bytes command buffer of its own.
This is an unnecessary duplication and copy of request's
cmd. It is probably left overs from the time that scsi_cmnd
could function without a request attached. So clean that up.
- Once above is done, few places, apart from scsi-ml, needed
adjustments due to changing the data type of scsi_cmnd->cmnd.
- Lots of drivers still use MAX_COMMAND_SIZE. So I have left
that #define but equate it to BLK_MAX_CDB. The way I see it
and is reflected in the patch below is.
MAX_COMMAND_SIZE - means: The longest fixed-length (*) SCSI CDB
as per the SCSI standard and is not related
to the implementation.
BLK_MAX_CDB. - The allocated space at the request level
- I have audit all ISA drivers and made sure none use ->cmnd in a DMA
Operation. Same audit was done by Andi Kleen.
(*)fixed-length here means commands that their size can be determined
by their opcode and the CDB does not carry a length specifier, (unlike
the VARIABLE_LENGTH_CMD(0x7f) command). This is actually not exactly
true and the SCSI standard also defines extended commands and
vendor specific commands that can be bigger than 16 bytes. The kernel
will support these using the same infrastructure used for VARLEN CDB's.
So in effect MAX_COMMAND_SIZE means the maximum size command
scsi-ml supports without specifying a cmd_len by ULD's

[SCSI] fix SLUB WARN_ON
Fixed another problem in free list handling by moving list allocation
from scsi_host_alloc() to scsi_add_host(). Unfortunately it
introduced a new failure mode in that hosts can pass straight from
alloc to put without going through add, leaving the free list
uninitialised.
Fix by checking shost->cmd_pool on the release path to see if it got
initialised.

collisions. This causes a problem for ipr whose "state" attribute
collides with an identically named one in the SCSI mid-layer. Rename

Newer Dell CERC firmware (>= 6.62) implement a random deletion handling
compatible with the legacy megaraid driver. The legacy handling shifted
the target ID by 0x80 only for I/O commands (READ/WRITE/etc), whereas
megaraid_mbox shifts the target ID always if random deletion is supported.
The resulted in megaraid_mbox sending an INQUIRY to the wrong channel, and
not finding any devices, obviously.

Remove proc_root export. Creation and removal works well if parent PDE is
supplied as NULL -- it worked always that way.
So, one useless export removed and consistency added, some drivers created
PDEs with &proc_root as parent but removed them as NULL and so on.

The block layer initializes rq->cmd (queue_flush calls rq_init) so
prepare_flush_fn hooks don't need to do that.
The purpose of this patch is to remove sizeof(rq->cmd), as a
preparation for large command support, which changes rq->cmd from the
static array to a pointer. sizeof(rq->cmd) will not make sense.

transfer as an argument and always do padding.
While at it:

There should be no functional changes caused by this patch (all users

Add a 'count' variable to each symbol which gets increased every time
the symbol is referenced. And then modify the register definition to
include counts for symbols which are referenced from the source code
only and not from the sequencer code.
This will give us an automatic usage count for the symbols with only
minimal hand-crafting.

Commit 63a8651f25 ([SCSI] qla2xxx:
Correct infinite-login-retry issue.) introduced a small
regression where a successful relogin would result in an fcport's
loop_id to be incorrectly reset to FC_NO_LOOP_ID. Only clear-out
loopid, if retries have been 'truly' exhausted.

The driver is incorrectly assuming that the 'sp' reference held
in qla2[x00|4xx]_abort_command() is valid after the mailbox
command is issued to abort the exchange. It is *not*, as the
command may be completed during interrupt context before control
is returned to the mailbox caller.

Use the more efficient read-DMA'ble-buffer mailbox commands
rather than reading a single word/dword at a time. We also
remove a bulk of the duplicate mailbox command-handling codes in
favor of more generic read-memory() routines (qla2xxx_dump_ram()
and qla24xx_dump_ram()).

Replace the mac_esp driver with a new one based on the esp_scsi core.
For esp_scsi: add support for sync transfers for the PIO mode, add a new
esp_driver_ops method to get the maximum dma transfer size (like the old
NCR53C9x driver), and some cleanups.

Way back when, when the fc_user_scan routine was created, it kept some
of its original logic that walked the rport list and kicked off a scan.
Unfortunately, it didn't keep any of the locking around the rport list,
nor did it consider the synchronous nature of the scan invoked. The result,
there are some scan requests where the rport list changes, thus a subsequent
scan is called on a bogus rport structure and the system NMI's.

The function type_check() in aicasm grammar code was
never used properly due to a bug.
This patch fixes it up and ensures it's only called if appropriate.
In addition the unused 16bit instruction are disabled, but left in
the code for reference.

We're getting a WARN_ON from SLUB indicating that we're trying to free
caches with in-use objects. The root cause is a new dependency in the
command/sense free on unchecked_isa_dma. The WARN_ON is caused by
drivers which change this in their setup after the command/sense cache
is allocated.
The fix is to move the allocation of this cache into scsi_add_host()
so things like gdth have an opportunity to modify it between alloc and
add (but *not* after).
The true fix would be to move unchecked_isa_dma into the template and
out of the host, so it because a truly read only variable.

calling ioremap_nocache() on it. This warrants kernel oops when the registers
are accessed on PPC 44x platforms which (being 32-bit) have PCI memory space
mapped beyond 4 GB.
The arch/ppc/ kernel has a fixup in ioremap() that helps create an illusion
that the PCI memory resources are mapped below 4 GB, but arch/powerpc/ code
got rid of this trick, having instead CONFIG_RESOURCES_64BIT enabled.

scsi_transport_sas calls blk_cleanup_queue too early for bsg
queues. If a user holds a sas_host, end_device, or expander device
open, remove the device, then send a request to it, we get a kernel
crash. We need to call blk_cleanup_queue in the release callback as we
do with scsi devices.
This patch moves blk_cleanup_queue to sas_expander_release and
sas_end_device_release from sas_bsg_remove. sas_host can't use the
release callback in struct device so use bsg's release callback.

This patch adds release callback support, which is called when a bsg
device goes away. bsg_register_queue() takes a pointer to a callback
function. This feature is useful for stuff like sas_host that can't
use the release callback in struct device.
If a caller doesn't need bsg's release callback, it can call
bsg_register_queue() with NULL pointer (e.g. scsi devices can use
release callback in struct device so they don't need bsg's callback).
With this patch, bsg uses kref for refcounts on bsg devices instead of
get/put_device in fops->open/release. bsg calls put_device and the
caller's release callback (if it was registered) in kref_put's
release.

The current target allocation code registeres each possible target
with sysfs; it will be deleted again if no useable LUN on this target
was found. This results in a string of 'target add/target remove' uevents.
Based on a patch by Hannes Reinecke <hare@suse.de> this patch reworks
the target allocation code so that only uevents for existing targets
are sent. The sysfs registration is split off from the existing
scsi_target_alloc() into a in a new scsi_add_target() function, which
should be called whenever an existing target is found. Only then a
uevent is sent, so we'll be generating events for existing targets
only.

Fixed trivial conflict in include/linux/memory.h due to semaphore header
file change (made irrelevant by the change to mutex).
blk_rq_map_user adjusts bi_size of the last bio. It breaks the rule
that req->data_len (the true data length) is equal to sum(bio). It
broke the scsi command completion code.
commit e97a294ef6 was introduced to fix
the above issue. However, the partial completion code doesn't work
with it. The commit is also a layer violation (scsi mid-layer should
not know about the block layer's padding).
This patch moves the padding adjustment to blk_rq_map_sg (suggested by
James). The padding works like the drain buffer. This patch breaks the
rule that req->data_len is equal to sum(sg), however, the drain buffer
already broke it. So this patch just restores the rule that
req->data_len is equal to sub(bio) without breaking anything new.
Now when a low level driver needs padding, blk_rq_map_user and
blk_rq_map_user_iov guarantee there's enough room for padding.
blk_rq_map_sg can safely extend the last entry of a scatter list.
blk_rq_map_sg must extend the last entry of a scatter list only for a
request that got through bio_copy_user_iov. This patches introduces
new REQ_COPY_USER flag.

None of these files use any of the functionality promised by
asm/semaphore.h. It's possible that they rely on it dragging in some
unrelated header file, but I can't build all these files, so we'll have
fix any build failures as they come up.

Now that all SFF stuff is separated out of core layer, core layer
doesn't call ops->tf_read directly. It gets called only via
ops->qc_fill_rtf() for non-SFF drivers. This patch directly
implements private ops->qc_fill_rtf() for non-SFF controllers and kill
ops->tf_read().
This is much cleaner for non-SFF controllers as some of them have to
cache SFF register values in private data structure and report the
cached values via ops->tf_read(). Also, ops->tf_read() gets nasty for
controllers which don't have clear notion of TF registers when
operation is not in progress.
As this change makes default ops->qc_fill_rtf unnecessary, move
ata_sff_qc_fill_rtf() form ata_base_port_ops to ata_sff_port_ops where
it belongs.

On command completion, ata_qc_complete() directly called ops->tf_read
to fill qc->result_tf. This patch adds ops->qc_fill_rtf to replace
hardcoded ops->tf_read usage.
ata_sff_qc_fill_rtf() which uses ops->tf_read to fill result_tf is
implemented and set in ata_base_port_ops and other ops tables which
don't inherit from ata_base_port_ops, so this patch doesn't introduce
any behavior change.
ops->qc_fill_rtf() is similar to ops->sff_tf_read() but can only be
called when a command finishes. As some non-SFF controllers don't
have TF registers defined unless they're associated with in-flight
commands, this limited operation makes life easier for those drivers
and help lifting SFF assumptions from libata core layer.

Currently reset methods are not specified directly in the
ata_port_operations table. If a LLD wants to use custom reset
methods, it should construct and use a error_handler which uses those
reset methods. It's done this way for two reasons.
First, the ops table already contained too many methods and adding
four more of them would noticeably increase the amount of necessary
boilerplate code all over low level drivers.
Second, as ->error_handler uses those reset methods, it can get
confusing. ie. By overriding ->error_handler, those reset ops can be
made useless making layering a bit hazy.
Now that ops table uses inheritance, the first problem doesn't exist
anymore. The second isn't completely solved but is relieved by
providing default values - most drivers can just override what it has
implemented and don't have to concern itself about higher level
callbacks. In fact, there currently is no driver which actually
modifies error handling behavior. Drivers which override
->error_handler just wraps the standard error handler only to prepare
the controller for EH. I don't think making ops layering strict has
any noticeable benefit.
This patch makes ->prereset, ->softreset, ->hardreset, ->postreset and
their PMP counterparts propoer ops. Default ops are provided in the
base ops tables and drivers are converted to override individual reset
methods instead of creating custom error_handler.
* ata_std_error_handler() doesn't use sata_std_hardreset() if SCRs
aren't accessible. sata_promise doesn't need to use separate
error_handlers for PATA and SATA anymore.
* softreset is broken for sata_inic162x and sata_sx4. As libata now
always prefers hardreset, this doesn't really matter but the ops are
forced to NULL using ATA_OP_NULL for documentation purpose.
* pata_hpt374 needs to use different prereset for the first and second
PCI functions. This used to be done by branching from
hpt374_error_handler(). The proper way to do this is to use
separate ops and port_info tables for each function. Converted.

Most of the cpu_to_le32() usage was wrong in one way or another.
Compiler warning on BE builds was just the tip of the iceberg.

Upon having configuration changes on vports only, the driver
handles SCR regardless physical port state and, in turn, it
results mailbox error as below:

Now that infrastructure is present within the midlayer and there
is a clear distinction between what is expected from a device and
target reset, convert the current device-reset codes to a
target-reset, and add codes to perform a proper device-reset (LUN
reset).
In the process of adding reset support, collapse and consolidate
large sections of mailbox-command (TMF issuance) codes,
generalize the two 'wait-for-commands-to-complete' functions, and
add a generic-reset routine for use by midlayer reset functions.

The Flash Descriptor Table (FDT) present on many recent HBAs
encodes flash accessing characteristics of the flash-part used on
the HBA. Use this information during flash manipulation (writes)
rather than using specific hard-coded values based on queried
manufacturer and device IDs.

Supported events include LIP, LIP reset, RSCN, link up, and link
down.
To support AEN (and additional forthcoming features), we also
introduce a simple deferred-work construct to manage events which
require a non-atomic sleeping-capable context. This work-list is
processed as part of the driver's standard DPC routine.

There's no need to use the heavier (albiet safer)
*_irq[save|restore]() locking primitives within the driver's
interrupt handlers, interrupts are guaranteed to be
non-reentrant. Use lightweight spin_lock() and spin_unlock()
primitives while acquiring the hardware_lock.

Every current transport class calls transport_container_release but
ignores the return value. This is catastrophic if it returns an error
because the containers are part of a global list and the next action of
almost every transport class is to free the memory used by the
container.
Fix this by making transport_container_release a void, but making it BUG
if attribute_container_release returns an error ... this catches the
root cause of a system panic much earlier. If we don't do this, we get
an eventual BUG when the attribute container list notices the corruption
caused by the freed memory it's still referencing.
Also made attribute_container_release __must_check as a reminder.

Currently, if the barrier command fails, the error return isn't seen
by the block layer and it proceeds on regardless. The problem is that
SCSI always returns no error for REQ_TYPE_BLOCK_PC ... it expects the
submitter to pick the errors out of req->errors, which the block
barrier functions don't do.
Since it appears that the way SG_IO and scsi_execute_request() work
they discard the block error return and always use req->errors, the
best fix for this is to have the SCSI layer return an error to block
if one actually occurred (this also allows us to filter out spurious
errors, like deferred sense).
This patch is a bug fix that will need backporting to stable, but it's
also quite a big change and in need of testing, so we'll incubate in
the main kernel tree and backport at the -rc2 or so stage if no
problems turn up.

header.context is a 64 bit field, but it's deliberately split into
context and context_hi32. Thus cpu_to_le64 assignments are wrong on
this. Replace them with the correct settings of both the low and high
words.

The three drivers: a2091, gvp11 and mvme147 have erroneous references

variable.
