We had a multi-partition SD-Card with two ext2 file systems. The partition
table was getting overwritten by a race between the card removal and
the unmount of the 2nd ext2 partition.
What was observed:
1. Suspend/resume would call to remove the device. The clearing
of the device information is done asynchronously.
2. A request is made to unmount the file system (this is called
after the removal has started).
3. The remapping table was cleared by the asynchronous part of
the device removal.
4. A write request to the super block (block 0 of the partition)
was sent down and instead of being remapped to the partition
offset, it was remapped to block 0 of the device which is where
the partition table is located.
5. Write was queued and written resulting in the overwriting
of the partition table with the ext2 super block.
6. The mmc_queue is cleaned up.
The mmc card device driver used to access SD cards, was calling del_gendisk
before calling mmc_cleanup-queue. The comment in the mmc_blk_remove_req
code indicated that it expected del_gendisk to block all further requests
from being queued but it doesn't. The mmc driver uses the presences of the
mmc_queue to determine if the request should be queued.
The fix was to clean up the mmc_queue before the rest of the
the delete partition code is called.
This prevents the overwriting of the partition table.
However, the umount gets an error trying to write the super block.
The umount should be issued before the device is removed but that
is not always possible. The umount is still needed to cleanup other
data structures.

Once the mmc blkdevice is being probed, runtime pm will be enabled.
By using runtime autosuspend, the power save operations can be done
when request inactivity occurs for a certain time. Right now the
selected timeout value is set to 3 s. Obviously this value will likely
need to be configurable somehow since it needs to be trimmed depending
on the power save algorithm.
For SD-combo cards, we are still leaving the enablement of runtime PM
to the SDIO init sequence since it depends on the capabilities of the
SDIO func driver.
Moreover, when the blk device is being suspended, we make sure the device
will be runtime resumed. The reason for doing this is that we want the
host suspend sequence to be unaware of any runtime power save operations
done for the card in this phase. Thus it can just handle the suspend as
the card is fully powered from a runtime perspective.
Finally, this patch prepares to make it possible to move BKOPS handling
into the runtime callbacks for the mmc bus_ops. Thus IDLE BKOPS can be
accomplished.

The sanitize support is added as a user-app ioctl call, and
was removed from the block-device request, since its purpose is
to be invoked not via File-System but by a user.
This feature deletes the unmap memory region of the eMMC card,
by writing to a specific register in the EXT_CSD.
unmap region is the memory region that was previously deleted
(by erase, trim or discard operation).
In order to avoid timeout when sanitizing large-scale cards,
the timeout for sanitize operation is 240 seconds.

For normal request mmc_blk_issue_rq is called twice with asynchronous
transfer(cur and prev). Host's claim and release can be done in each
mmc_blk_issue_rq. However, Special request is currently excluded in
asynchronous transfer. After special request is finished, if there is
no new request, mmc_release_host won't be called in mmc_blk_issue_rq.
The problem is founded during mmc_suspend.

It allows for cleaning up on a considerable amount of places. They did
port_get, hangup, kref_put. Now the only thing needed is to call
tty_port_tty_hangup which does exactly that. And they can also decide
whether to consider CLOCAL or completely ignore that.

This patch supports packed write command of eMMC4.5 devices. Several
writes can be grouped in packed command and all data of the individual
commands can be sent in a single transfer on the bus. Large amounts of
data in one transfer rather than several data of small size are
effective for eMMC write internally. As a result, packed command help
write throughput be improved. The following tables show the results
of packed write.

When current request is running on the bus and if next request fetched
by mmcqd is NULL, mmc context (mmcqd thread) gets blocked until the
current request completes. This means that if new request comes in while
the mmcqd thread is blocked, this new request can not be prepared in
parallel to current ongoing request. This may result in delaying the new
request execution and increase it's latency.
This change allows to wake up the MMC thread on new request arrival.
Now once the MMC thread is woken up, a new request can be fetched and
prepared in parallel to the current running request which means this new
request can be started immediately after the current running request
completes.
With this change read throughput is improved by 16%.

The option allows you to remove TTY and compile without errors. This
saves space on systems that won't support TTY interfaces anyway.

The bulk of this patch consists of Kconfig changes adding "depends on
TTY" to various serial devices and similar drivers that require the TTY
layer. Ideally, these dependencies would occur on a common intermediate
symbol such as SERIO, but most drivers "select SERIO" rather than
"depends on SERIO", and "select" does not respect dependencies.
bloat-o-meter output comparing our previous minimal to new minimal by
removing TTY. The list is filtered to not show removed entries with awk

Now, we start converting tty buffer functions to actually use
tty_port. This will allow us to get rid of the need of tty in many
call sites. Only tty_port will needed and hence no more
tty_port_tty_get in those paths.
Now, the one where most of tty_port_tty_get gets removed:
tty_flip_buffer_push.
IOW we also closed all the races in drivers not using tty_port_tty_get
at all yet.
Also we move tty_flip_buffer_push declaration from include/linux/tty.h
to include/linux/tty_flip.h to all others while we are changing it
anyway.

There are infinite loops in the mmc code that can be caused by bad
hardware. The code will loop forever if the device never comes back
from program mode, R1_STATE_PRG, and it is not ready for data,
R1_READY_FOR_DATA.
A long timeout is added to prevent the code from looping forever.
The timeout will occur if the device never comes back from program
state or the device never becomes ready for data.
It's not clear whether the timeout will do more than log a pr_err()
and then start a fresh hang all over again. We may need to extend
this patch later to perform some kind of reset of the device (is
that possible?) or rejection of new I/O to the device.

In case both 'req' and 'mq->mqrq_prev->req' are null, there is no request
to be processed. That means there is no need to switch buffer.
Switching buffer is required only after finishing 'issue_fn'.

After commit "TTY: move tty buffers to tty_port", the tty buffers are
not freed in some drivers. This is because tty_port_destructor is not
called whenever a tty_port is freed. This was an assumption I counted
with but was unfortunately untrue. So fix the drivers to fulfil this
assumption.
Here it is enough to switch to refcounting in tty_port.

tty from struct sdio_uart_port is unused. Proper refcounted tty in
tty_port->tty is used instead. So remove the member from that
structure.

For several MoviNAND eMMC parts, there are known issues with secure
erase and secure trim. For these specific MoviNAND devices, we skip
these operations.
Specifically, there is a bug in the eMMC firmware that causes
unrecoverable corruption when the MMC is erased with MMC_CAP_ERASE
enabled.

Currently we have no way to assign tty->port while performing tty
installation. There are two ways to provide the link tty_struct =>
tty_port. Either by calling tty_port_install from tty->ops->install or
tty_port_register_device called instead of tty_register_device when
the device is being set up after connected.
In this patch we modify most of the drivers to do the latter. When the
drivers use tty_register_device and we have tty_port already, we
switch to tty_port_register_device. So we have the tty_struct =>
tty_port link for free for those.

alternative would be to keep points and refcount the termios objects.

For completing any block request, MMC block driver is calling:
spin_lock_irq(queue)
__blk_end_request()
spin_unlock_irq(queue)
But if we analyze the sources of latency in kernel using ftrace,
__blk_end_request() function at times may take up to 6.5ms with
spinlock held and irq disabled.
__blk_end_request() calls couple of functions and ftrace output
shows that blk_update_bidi_request() function is almost taking 6ms.
There are 2 function to end the current request: ___blk_end_request()
and blk_end_request(). Both these functions do same thing except
that blk_end_request() function doesn't take up the spinlock
while calling the blk_update_bidi_request().
This patch replaces all __blk_end_request() calls with
blk_end_request() and __blk_end_request_all() calls with
blk_end_request_all().
Testing done: 20 process concurrent read/write on sd card
and eMMC. Ran this test for almost a day on multicore system
and no errors observed.
This change is not meant for improving MMC throughput; it's basically
about becoming fair to other threads/interrupts in the system. By
holding spin lock and interrupts disabled for longer duration, we
won't allow other threads/interrupts to run at all. Actually slight
performance degradation at file system level can be expected as we
are not holding the spin lock during blk_update_bidi_request() which
means our mmcqd thread may get preempted for other high priority
thread or any interrupt in the system.
These are performance numbers (100MB file write) with eMMC running
in DDR mode:

If multi block write operation fails for SD card, during
error handling we send the SD_APP_SEND_NUM_WR_BLKS (ACMD22)
to know how many blocks were already programmed by card.
But mmc_sd_num_wr_blocks() function which sends the ACMD22
calculates the data timeout value from csd.tacc_ns and
csd.tacc_clks parameters which will be 0 for block addressed
(>2GB cards) SD card. This would result in timeout_ns and
timeout_clks being 0 in the mmc_request passed to host driver.
This means host controller would program its data timeout timer
value with 0 which could result in DATA TIMEOUT errors from
controller.
To fix this issue, mmc_sd_num_wr_blocks() should instead
just call the mmc_set_data_timeout() to calculate the
data timeout value. mmc_set_data_timeout() function
ensures that non zero timeout value is set even for
block addressed SD cards.

This patch adds support for large sector size of 4KB by disabling
emulation. This patch passes eMMC DATA_SECTOR_SIZE as the logical
block size during mmc_blk_alloc_req.
In order to use this patch for 4KB sector size, ensure that
USE_NATIVE_SECTOR is enabled, partition table is 4KB sector size
aligned and file system block and sector size are 4KB multiples.

MMC bus is using legacy suspend/resume method, which is not compatible if
runtime pm callbacks are used. In this scenario, MMC bus suspend/resume
callbacks cannot be called when system entering S3. So change to use the
new defined dev_pm_ops for system sleeping mode.
Tested on AM335x Platform. Solves major issue/crash reported at

eMMC v4.5 sanitize operation erases all copies of unmapped
data. However trim or erase operations must be used first
to unmap the required sectors. That was not being done.

eMMC v4.5 discard operation is significantly different from the
existing trim operation because it is not guaranteed to work with
the new sanitize operation. Consequently mmc_can_trim() is
separated from mmc_can_discard().
Also the new discard operation does not result in the sectors being
set to all-zeros, so discard_zeroes_data must not be set.
In addition, the new discard has the same timeout as trim, but from
v4.5 trim is defined to use the hc timeout. The timeout calculation
is adjusted accordingly.

According to the specifications for SD and (e)MMC default
blocksize (named BLOCKLEN in Spec.) must always be 512
bytes. Since we hardcoded to always use 512 bytes, we do
not explicitly have to set it. Future improvements should
potentially make it possible to use a greater blocksize
than 512 bytes, but until then let's skip this.

All num, magic and owner are set by alloc_tty_driver. No need to
re-set them on each allocation site.
pti driver sets something different to what it passes to
alloc_tty_driver. It is not a bug, since we don't use the lines
parameter in any way. Anyway this is fixed, and now we do the right

Kill block requests when the host realizes that the card is
removed from the slot and is sure that subsequent requests
are bound to fail. Do this silently so that the block
layer doesn't output unnecessary error messages.

Enable boot partitions to be read-only locked until next power on via
a sysfs entry. There will be one sysfs entry for each boot partition:
/sys/block/mmcblkXbootY/ro_lock_until_next_power_on
Each boot partition is locked by writing 1 to its file.

If the read or write buffer size associated with the command sent
through the mmc_blk_ioctl is zero, do not prepare data buffer.
This enables a ioctl(2) call to for instance send a MMC_SWITCH to set
a byte in the ext_csd.

Adds a quirk that sets the data read timeout to a fixed value instead
of relying on the information in the CSD. The timeout value chosen
is 300ms since that has proven enough for the problematic cards found,
but could be increased if other cards require this.
This patch also enables this quirk for certain Micron cards known to
have this problem.

Due to hardware bugs, some MMC host controllers don't support
multiple-block reads[1]. To resolve, add a new MMC capability flag,
MMC_CAP2_NO_MULTI_READ, which can be set by affected host controller
drivers. When this capability is set, all reads will be issued one
sector at a time.

It allows gerneral purpose partitions in MMC Device. And I try to simply
make mmc_blk_alloc_parts using mmc_part structure suggested by Andrei
Warkentin. After patching, we see general purpose partitions like this:

results in switch errors. Since there is no access, mmcboot0/1

For cards that support hardware reset (just eMMC), try a reset and
retry before returning an I/O error. However this is not done for
ECC errors and is never done twice for the same operation type
(READ, WRITE, DISCARD, SECURE DISCARD) until that type of operation
again succeeds.

Add a new REQ_PRIO to let requests preempt others in the cfq I/O schedule,
and lave REQ_META purely for marking requests as metadata in blktrace.
All existing callers of REQ_META except for XFS are updated to also
set REQ_PRIO for now.

During card removal and inserting cycle the test file in the debugfs could be
stalled until the host driver removes it. Let's keep the file in the linked
list and destroy it when card is removed.

requests, but not in mmc_blk_issue_flush(). This patch fixes that

Change mmc_blk_issue_rw_rq() to become asynchronous.
The execution flow looks like this:
* The mmc-queue calls issue_rw_rq(), which sends the request
to the host and returns back to the mmc-queue.
* The mmc-queue calls issue_rw_rq() again with a new request.
* This new request is prepared in issue_rw_rq(), then it waits for
the active request to complete before pushing it to the host.
* When the mmc-queue is empty it will call issue_rw_rq() with a NULL
req to finish off the active request without starting a new request.

The way the request data is organized in the mmc queue struct, it only
allows processing of one request at a time. This patch adds a new struct
to hold mmc queue request data such as sg list, request, blk request and
bounce buffers, and updates any functions depending on the mmc queue
struct. This prepares for using multiple active requests in one mmc queue.

Add a test that measures how the mmc bandwidth depends on the numbers of
sg elements in the sg list. The transfer size if fixed and sg length goes
from a few up to 512. The purpose is to measure overhead caused by
multiple sg elements.

Some host controllers will not operate without a hardware
timeout that is limited in value. However large discards
require large timeouts, so there needs to be a way to
specify the maximum discard size.
A host controller driver may now specify the maximum discard
timeout possible so that max_discard_sectors can be calculated.
However, for eMMC when the High Capacity Erase Group Size
is not in use, the timeout calculation depends on clock
rate which may change. For that case Preferred Erase Size
is used instead.

Command channel errors fall into four classes:
1. The command was issued with the card in the wrong state
2. The command failed to be received by the card correctly
3. The cards response failed to be received by the host (CRC error)
4. The card failed to respond to the card
For (1), in theory we should know that the card is in the correct state.
However, a failed stop command (or other failure) may result in the card
remaining in a data transfer state from the previous command. If we
detect this condition, we try to recover by sending a stop command.
For the initial commands (set block count and the read/write command)
no data will have been transferred. All that we need deal with is
retrying at this point. A failed stop command can be remedied as
above.
If we are unable to recover the card (eg, the card ignores our requests
for status, or we don't recognise the error code) then we immediately
fail the request.

If the MMC_SEND_STATUS command is not successful, we should not return
a zero status word, but instead allow the caller to know positively
that an error occurred.
Convert the open-coded get_card_status() to use the helper function,
and provide definitions for the card state field.

SCSI defines discard alignment as the offset to the first
optimal discard. In the case of SD/MMC, that is always zero
which is the default.
SCSI defines discard granularity as a hint of a optimal
discard size. That is much better expressed by the MMC
"preferred erase size" (pref_erase) field.

The MMC block driver and other drivers (e.g. mmc-test) will expect
the card to be switched to the User Data Area eMMC partition when
they start. Hence the MMC block driver should ensure it is that
way when it is removed.

Allows appropriately-privileged applications to send CMD (normal) and ACMD
(application-specific; preceded with CMD55) commands to cards/devices on
the mmc bus. This is primarily useful for enabling the security
functionality built in to every SD card.
It can also be used as a generic passthrough (e.g. to enable virtual
machines to control mmc bus devices directly). However, this use case has
not been tested rigorously. Generic passthrough testing was only conducted
for a few non-security opcodes to prove the feasibility of the passthrough.
Since any opcode can be sent using this passthrough, it is very possible to
render the card/device unusable. Applications that use this ioctl must
have CAP_SYS_RAWIO.

TI OMAP3621/OMAP3630 SoC, Samsung S5PC110 SoC, Qualcomm MSM7200A SoC.

The test area size was set to the preferred erase size but for comparison
purposes it is better if it is the same size for different devices. Make
it a multiple of preferred erase size that is greater than or equal to 4MiB.

If the MMC host controller does not support waiting for card signaling
busy state (MMC_CAP_WAIT_WHILE_BUSY cap), there is no point in prining
the relevant warning message.

While the MMC handled the card's read only flag correctly on open,
it did not setup the flag in the allocated disk structure. The
consequence being that probing the /sys/class/block/mmcblkX/ro
attribute always reported 0.

We don't actually need this and it causes problems for internal use of
this functionality. Currently there is a single use of the FILE * pointer.
That is the serial core which uses it to check tty_hung_up_p. However if
that is true then IO_ERROR is also already set so the check may be removed.

The old limit of number of minor numbers per mmcblk device was hardcoded
at 8. This isn't enough for some of the more elaborate partitioning
schemes, for example those used by Chrome OS.
Since there might be a bunch of systems out there with static /dev
contents that relies on the old numbering scheme, let's make it a
build-time option with the default set to the previous 8.
Also provide a boot/modprobe-time parameter to override the config
default: mmcblk.perdev_minors.

The DDR support patch needs the following fixes:
- The block driver does not need to know about DDR, any more
than it needs to know about bus width.
- Not only the card must be switched to DDR mode. The host
controller must also be configured, which is done through
the 'set_ios()' function.
- Do not set the DDR mode state until after the switch command
is successful.
- Setting block length is not supported in DDR mode. Make that
a core function and change the other place it is used (mmc_test)
also.

The EXTRA_CFLAGS assignment in mmc/Makefile was not accomplishing
anything because this flag only has effect on sources at the same level
as the makefile (i.e., per directory). Since card/, core/, and host/
rely on MMC_DEBUG, the subdir-ccflags-y variant seems to be the
appropriate choice.

As proposed by Greg K-H it is more logical to keep files for the mmc_test
driver under debugfs.
Additionally this patch brings seq_file API for show() method. It allows
to write unlimited data to the file.

Make it possible to get test results via sysfs. It helps to do tests
non-interactively. We have the file created under sysfs already and
can use it to show test results.
Prior to this patch, the "test" file under each card's sysfs node was
write-only, and results were obtained by looking at dmesg. This patch
improves programmatic access to the test results, making them available by
reading back from the same "test" file:

It's better to use strict_strtol() to convert user's input and strictly
check it. At least it forbids to interpret wrong input as a 0 and
prevents to run all tests.

The host controller driver limits I/O transfers to maximum
transfer size, maximum block count, maximum segment size
and maximum segment count. The performance tests were
not obeying these limits which meant they would not work
with some drivers. This patch fixes that.

calls from a recent pushdown, and some of the drivers
were already using the BKL before.

Barrier is deemed too heavy and will soon be replaced by FLUSH/FUA
requests. Deprecate barrier. All REQ_HARDBARRIERs are failed with
-EOPNOTSUPP and blk_queue_ordered() is replaced with simpler
blk_queue_flush().
blk_queue_flush() takes combinations of REQ_FLUSH and FUA. If a
device has write cache and can flush it, it should set REQ_FLUSH. If
the device can handle FUA writes, it should also set REQ_FUA.
All blk_queue_ordered() users are converted.
* ORDERED_DRAIN is mapped to 0 which is the default value.
* ORDERED_DRAIN_FLUSH is mapped to REQ_FLUSH.
* ORDERED_DRAIN_FLUSH_FUA is mapped to REQ_FLUSH | REQ_FUA.

- Use nr_free_buffer_pages() instead of sysinfo.totalram to determine
total lowmem pages.
- Change variables containing memory sizes to unsigned long.
- Limit maximum test area size to 128MiB because that is the maximum MMC
high capacity erase size (the maxmium SD allocation unit size is just
4MiB)

mmc_test provides tests aimed at testing SD/MMC hosts. This patch adds
performance tests.
It is advantageous to have performance tests in a kernel
module like mmc_test for the following reasons:
- transfer times can be measured very accurately
- arbitrarily large transfers are possible
- the effect of contiguous vs scattered pages
can be determined
The new tests are:

Enable MMC to service discard requests. In the case of SD and MMC cards
that do not support trim, discards become erases. In the case of cards
(MMC) that only allow erases in multiples of erase group size, round to
the nearest completely discarded erase group.

The open and release block_device_operations are currently
called with the BKL held. In order to change that, we must
first make sure that all drivers that currently rely
on this have no regressions.
This blindly pushes the BKL into all .open and .release
operations for all block drivers to prepare for the
next step. The drivers can subsequently replace the BKL
with their own locks or remove it completely when it can
be shown that it is not needed.
The functions blkdev_get and blkdev_put are the only
remaining users of the big kernel lock in the block
layer, besides a few uses in the ioctl code, none
of which need to serialize with blkdev_{get,put}.
Most of these two functions is also under the protection
of bdev->bd_mutex, including the actual calls to
->open and ->release, and the common code does not
access any global data structures that need the BKL.

Remove all the trivial wrappers for the cmd_type and cmd_flags fields in
struct requests. This allows much easier grepping for different request
types instead of unwinding through macros.

percpu.h is included by sched.h and module.h and thus ends up being
included when building most .c files. percpu.h includes slab.h which
in turn includes gfp.h making everything defined by the two files
universally available and complicating inclusion dependencies.
percpu.h -> slab.h dependency is about to be removed. Prepare for
this change by updating users of gfp and slab facilities include those
headers directly instead of assuming availability. As this conversion
needs to touch large number of source files, the following script is
used as the basis of conversion.

The script does the followings.
* Scan files for gfp and slab usages and update includes such that
only the necessary includes are there. ie. if only gfp is used,
gfp.h, if slab is used, slab.h.
* When the script inserts a new include, it looks at the include
blocks and try to put the new include such that its order conforms
to its surrounding. It's put in the include block which contains
core kernel includes, in the same order that the rest are ordered -
alphabetical, Christmas tree, rev-Xmas-tree or at the end if there
doesn't seem to be any matching order.
* If the script can't find a place to put a new include (mostly
because the file doesn't have fitting include block), it prints out
an error message indicating which .h file needs to be added to the
file.
The conversion was done in the following steps.
1. The initial automatic conversion of all .c files updated slightly
over 4000 files, deleting around 700 includes and adding ~480 gfp.h
and ~3000 slab.h inclusions. The script emitted errors for ~400
files.
2. Each error was manually checked. Some didn't need the inclusion,
some needed manual addition while adding it to implementation .h or
embedding .c file was more appropriate for others. This step added
inclusions to around 150 files.
3. The script was run again and the output was compared to the edits
from #2 to make sure no file was left behind.
4. Several build tests were done and a couple of problems were fixed.
e.g. lib/decompress_*.c used malloc/free() wrappers around slab
APIs requiring slab.h to be added manually.
5. The script was run on all .h files but without automatically
editing them as sprinkling gfp.h and slab.h inclusions around .h
files could easily lead to inclusion dependency hell. Most gfp.h
inclusion directives were ignored as stuff from gfp.h was usually
wildly available and often used in preprocessor macros. Each
slab.h inclusion directive was examined and added manually as
necessary.
6. percpu.h was updated not to include slab.h.
7. Build test were done on the following configurations and failures
were fixed. CONFIG_GCOV_KERNEL was turned off for all tests (as my
distributed build env didn't work with gcov compiles) and a few
more options had to be turned off depending on archs to make things
build (like ipr on powerpc/64 which failed due to missing writeq).
* x86 and x86_64 UP and SMP allmodconfig and a custom test config.
* powerpc and powerpc64 SMP allmodconfig
* sparc and sparc64 SMP allmodconfig
* ia64 SMP allmodconfig
* s390 SMP allmodconfig
* alpha SMP allmodconfig
* um on x86_64 SMP allmodconfig
8. percpu.h modifications were reverted so that it could be applied as
a separate patch and serve as bisection point.
Given the fact that I had only a couple of failures from tests on step
6, I'm fairly confident about the coverage of this conversion patch.
If there is a breakage, it's likely to be something in one of the arch
headers which should be easily discoverable easily on most builds of
the specific arch.

Except for SCSI no device drivers distinguish between physical and
hardware segment limits. Consolidate the two into a single segment
limit.
