During debug, it was seen that the driver is issuing commands specific to
SLI3 on SLI4 devices. Although the adapter correctly rejected the command,
this should not be done.
Revise the code to stop sending these commands on a SLI4 adapter.

When unloading the driver, mailbox commands may be sent without holding a
reference on the ndlp. By the time the mailbox command completes, the ndlp
may have reduced its ref counts and been freed. The problem was reported
by KASAN.
While unregistering due to driver unload, have the completion noop'd by
setting the ndlp context NULL'd. Due to the unload, no further action was
necessary. Also, while reviewing this path, the generic nulling of the
context after handling should be slightly moved.

In summary, the new patch added taking the io_buf_list_put_lock while under
an irq-disabled hbalock. This created a lock heirarchy dependent upon irq
being disabled, and there are paths that take the io_buf_list_put_lock
without disabling irq.
Looking at the lpfc_io_free routine, which is where the new heirarchy was
introduced, there is no reason to be taking out the hbalock and raising
irq, as the functionality is replaced by the io_buf_list_xxx locks.
Resolve by removing the hbalock/irq calls in lpfc_io_free.

Driver uses "reply descriptor post queues" in round robin fashion so that
IO's are distributed to all the available reply descriptor post queues
equally. With this each reply descriptor post queue load is balanced.
This is enabled only if CPUs count to MSI-X vector count ratio is X:1
(where X > 1) This improves performance and also fixes soft lockups.

Issue Description:
We have seen cpu lock up issue from fields if system has greater (more than
96) logical cpu count. SAS3.0 controller (Invader series) supports at max
96 msix vector and SAS3.5 product (Ventura) supports at max 128 msix
vectors.
This may be a generic issue (if PCI device supports completion on multiple
reply queues). Let me explain it w.r.t to mpt3sas supported h/w just to
simplify the problem and possible changes to handle such issues. IT HBA
(mpt3sas) supports multiple reply queues in completion path. Driver creates
MSI-x vectors for controller as "min of (FW supported Reply queue, Logical
CPUs)". If submitter is not interrupted via completion on same CPU, there
is a loop in the IO path. This behavior can cause hard/soft CPU lockups, IO
timeout, system sluggish etc.
Example - one CPU (e.g. CPU A) is busy submitting the IOs and another CPU
(e.g. CPU B) is busy with processing the corresponding IO's reply
descriptors from reply descriptor queue upon receiving the interrupts from
HBA. If the CPU A is continuously pumping the IOs then always CPU B (which
is executing the ISR) will see the valid reply descriptors in the reply
descriptor queue and it will be continuously processing those reply
descriptor in a loop without quitting the ISR handler.
Mpt3sas driver will exit ISR handler if it finds unused reply descriptor in
the reply descriptor queue. Since CPU A will be continuously sending the
IOs, CPU B may always see a valid reply descriptor (posted by HBA Firmware
after processing the IO) in the reply descriptor queue. In worst case,
driver will not quit from this loop in the ISR handler. Eventually, CPU
lockup will be detected by watchdog.
Above mentioned behavior is not common if "rq_affinity" set to 2 or
affinity_hint is honored by irqbalance as "exact". If rq_affinity is set
to 2, submitter will be always interrupted via completion on same CPU. If
irqbalance is using "exact" policy, interrupt will be delivered to
submitter CPU.
If CPU counts to MSI-X vectors (reply descriptor Queues) count ratio is not
1:1, we still have exposure of issue explained above and for that we don't
have any solution.
Exposure of soft/hard lockup if CPU count is more than MSI-x supported by
device.
If CPUs count to MSI-x vectors count ratio is not 1:1, (Other way, if CPU
counts to MSI-x vector count ratio is something like X:1, where X > 1) then
'exact' irqbalance policy OR rq_affinity = 2 won't help to avoid CPU
hard/soft lockups. There won't be any one to one mapping between CPU to
MSI-x vector instead one MSI-x interrupt (or reply descriptor queue) is
shared with group/set of CPUs and there is a possibility of having a loop
in the IO path within that CPU group and may observe lockups.
For example: Consider a system having two NUMA nodes and each node having
four logical CPUs and also consider that number of MSI-x vectors enabled on
the HBA is two, then CPUs count to MSI-x vector count ratio as 4:1. e.g.
MSIx vector 0 is affinity to CPU 0, CPU 1, CPU 2 & CPU 3 of NUMA node 0 and
MSI-x vector 1 is affinity to CPU 4, CPU 5, CPU 6 & CPU 7 of NUMA node 1.

Assume that user started an application which uses all the CPUs of NUMA
node 0 for issuing the IOs. Only one CPU from affinity list (it can be any
cpu since this behavior depends upon irqbalance) CPU0 will receive the
interrupts from MSIx vector 0 for all the IOs. Eventually, CPU 0 IO
submission percentage will be decreasing and ISR processing percentage will
be increasing as it is more busy with processing the interrupts. Gradually
IO submission percentage on CPU 0 will be zero and it's ISR processing
percentage will be 100 percentage as IO loop has already formed within the
NUMA node 0, i.e. CPU 1, CPU 2 & CPU 3 will be continuously busy with
submitting the heavy IOs and only CPU 0 is busy in the ISR path as it
always find the valid reply descriptor in the reply descriptor
queue. Eventually, we will observe the hard lockup here.
Chances of occurring of hard/soft lockups are directly proportional to
value of X. If value of X is high, then chances of observing CPU lockups is
high.
Solution: Use IRQ poll interface defined in " irq_poll.c". mpt3sas driver
will execute ISR routine in Softirq context and it will always quit the
loop based on budget provided in IRQ poll interface.
In these scenarios (i.e. where CPUs count to MSI-X vectors count ratio is
X:1 (where X > 1)), IRQ poll interface will avoid CPU hard lockups due to
voluntary exit from the reply queue processing based on budget. Note -
Only one MSI-x vector is busy doing processing.

There are times when a TUR can take longer than the DEFAULT_TIMEOUT
value. The timeout code is not correct as the function exits with an
automatic as the completion variable...To be fixed later.

but we shifted some code around an now "error" is set to zero for most

If there is an error queueing an iscsi command in iscsi_queuecommand(), for
example if the transport fails to take the command in
sessuin->tt->xmit_task(), then the error path can call
iscsi_complete_task() without first aquiring the back_lock as
required. This can lead to things like ITT pool can get corrupt, resulting
in duplicate ITTs being sent out.
The solution is to hold the back_lock around iscsi_complete_task() calls,
and to add a little commenting to help others understand when back_lock
must be held.

With default value of register SERDES_CFG, the link is not stable for some
special disks when running IO. According to HW guys' suggestion, need to
make the bit10~19 value of register SERDES_CFG the max value to increase
the reliability of the HiLink.

If we exchange SAS expander from one SAS controller to other SAS controller
without powering it down, the STP target port will maintain previous
affiliation and reject all subsequent connection requests from other STP
initiator ports with OPEN_REJECT (STP RESOURCES BUSY).
To solve this issue, send HARD RESET to clear the previous affiliation of
STP target port according to SPL (chapter 6.19.4).
We (re-)introduce dev status flag to know if to sleep in NEXUS reset code
or not for remote PHYs. The idea is that if the device is being
initialised, we don't require the delay, and caller would wait for link to
be established, cf. sas_ata_hard_reset().

When the PHY comes down, we currently do not set the negotiated linkrate:

This patch fixes the driver code to set it properly when the PHY comes
down.
If the PHY had been enabled, then set unknown; otherwise, flag as disabled.
The logical place to set the negotiated linkrate for this scenario is PHY
down routine, which is called from the PHY down ISR.
However, it is not possible to know if the PHY comes down due to PHY
disable or loss of link, as sas_phy.enabled member is not set until after
the transport disable routine is complete, which races with the PHY down
ISR.
As an imperfect solution, use sas_phy_data.enable as the flag to know if
the PHY is down due to disable. It's imperfect, as sas_phy_data is internal
to libsas.
I can't see another way without adding a new field to hisi_sas_phy and
managing it, or changing SCSI SAS transport.

The later revision of v3 hw has added an function of interrupt coalesce
according to time for PHY RX errors. We set the coalesce time to 1s. Then
we print PHY RX errors count when PHY RX errors happen, and don't need to
worry that there may be too much log prints.
Besides, we use hisi_sas_phy.lock to protect error count value. Because we
update them by calling phy_get_events_v3_hw(), which is also used by core
driver (for get PHY events function).
We relocate phy_get_events_v3_hw() to avoid a further declaration.

For internal IO and SMP IO, there is a time-out timer for them. In the
timer handler, it checks whether IO is done according to the flag
task->task_state_lock.
There is an issue which may cause system suspended: internal IO or SMP IO
is sent, but at that time because of hardware exception (such as inject
2Bit ECC error), so IO is not completed and also not timeout. But, at that
time, the SAS controller reset occurs to recover system. It will release
the resource and set the status of IO to be SAS_TASK_STATE_DONE, so when IO
timeout, it will never complete the completion of IO and wait for ever.

In "XFS over network block device" scenario XFS can create IO requests with
slab-based XFS metadata. During processing such requests tcp_sendpage() can
merge skb fragments with neighbour slab objects.
If receiving side is located on the same host tcp_recvmsg() can trigger
BUG_ON in hardening check and crash the host with following message:

from XXXXXXXX (kmalloc-512) (1024 bytes)
This patch redirect such requests from sednpage to sendmsg path. The
problem is similar to one described in recent commit 7e241f647d

There are special %pad and %pap format strings in Linux that we could use
here, but since the driver already does 64-bit arithmetic on the values,
using a plain 'u64' seems more consistent here.
Note: A possible related issue may be that the driver possibly checks the
wrong kind of overflow: when an IOMMU is in use, buffers that cross a
32-bit boundary in physical addresses would still be mapped into dma
addresses within the low 4GB space, so I suspect that we actually want to
check sg_dma_address() instead of sg_phys() here.

The outer routine lpfc_sli_issue_iocb(), which decomposes into the
SLI3 (s3) or SLI4 (s4) subroutines takes out the locks. For s3, it takes
out the hbalock. For s4, it takes out the ring_lock. The lockdep check in
the s3 and s4 subroutines both check hbalock, which is incorrect for s4.
Revise the s4 subroutine to lockdep check the ring_lock.

When trying to display tgt_counters in the debugfs, a panic can result.
There is no null check for qpair after it is assigned in the for-loop.
Unless vha->hw->queue_pair_map array is completely filled with entries, the
system will panic dereferencing a null pointer.

accelerator chip. For now it is in the drivers/misc directory but will
probably move to a new directory soon along with other drivers of this
type.
Other than that, just the usual set of individual driver updates and
fixes. There's an "odd" merge in here from the DRM tree that they asked
me to do as the MEI driver is starting to interact with the i915 driver,
and it needed some coordination. All of those patches have been
properly acked by the relevant subsystem maintainers.
All of these have been in linux-next with no reported issues, most for
quite some time.

The virtio scsi spec defines struct virtio_scsi_ctrl_tmf as a set of
device-readable records and a single device-writable response entry:

The above should be organised as two descriptor entries (or potentially
more if using VIRTIO_F_ANY_LAYOUT), but without any extra data after "le64
id" or after "u8 response".
The Linux driver doesn't respect that, with virtscsi_abort() and
virtscsi_device_reset() setting cmd->sc before calling virtscsi_tmf(). It
results in the original scsi command payload (or writable buffers) added to
the tmf.
This fixes the problem by leaving cmd->sc zeroed out, which makes
virtscsi_kick_cmd() add the tmf to the control vq without any payload.

When the HARDWARE_ERROR/0x3e/0x1 case is triggered, the logical volume
is offlined. When reading the kernel log, the reason why the device
got offlined isn't reported to the user. This situation makes it
difficult for admins to root cause.
Log a message when this condition occurs.

Nine small fixes. The resume fix is a cosmetic removal of a warning
with an incorrect condition causing it to alarm people wrongly. The
other eight patches correct a thinko in Christoph Hellwig's DMA
conversion series. Without it all these drivers end up with 32 bit
DMA masks meaning they bounce any page over 4GB before sending it to
the controller. Nowadays, even laptops mostly have memory above 4GB,
so this can lead to significant performance degradation with all the
bouncing.

T6 adapters generates DDP completion message on receiving all iSCSI pdus in

Looking at the code of scsi_alloc_sdev(), and all the calling contexts,
there seems to be no reason to use GFP_ATMOIC here. All the different
call-contexts use a mutex at some point, and nothing in between that
requires no sleeping, as far as I could see. Additionally, the code that
later allocates the block queue for the device (scsi_mq_alloc_queue())
already uses GFP_KERNEL.
There are similar allocations in two other functions:
scsi_probe_and_add_lun(), and scsi_add_lun(),; that can also be done with
GFP_KERNEL.

Negative constant left-shift is undefined behaviour in the C standard, and
as such newer versions of clang (at least) warn against it. GCC supports it
for a long time, but it would be better to remove it and rely on defined
behaviour.
My understanding is "~(-1 << N)" in 2's complement is intended to generate
a bit pattern of zeroes ending with N '1' bits. The same can be achieved by
"(1 << N) - 1" in a well-defined way, so switch to it to remove the
warning.
Tested: building a kernel with generic SCSI tape, and checking basic
operations (mt status, mt eject) on a real LTO unit. Cannot test the osst
driver.

Add this functionality, placing the descriptor being read in the actual
data buffer in the bio.
That is, for both read and write descriptors query upiu, we are using the
job's request_payload. This in turn, is mapped back in user land to the
applicable sg_io_v4 xferp: dout_xferp for write descriptor, and din_xferp
for read descriptor.

When we had a write descriptor query upiu, we appended the descriptor right
after the bsg request. This was fine as the bsg driver allows to allocate
whatever buffer we needed in its job request.
Still, the proper way to deliver payload, however small (we only write
config descriptors of 144 bytes), is by using the job request payload data
buffer.
So change this ABI now, while ufs-bsg is still new, and nobody is actually
using it.

60f0187031 introduced a small power optimization: ignore the vccq load
specified in the UFSHC DT node when said host controller is connected to
specific Flash chips (currently, Samsung and Hynix).

In my opinion, the rationale for the original patch is questionable. If
neither the UFSHC, nor the Flash chip, require any load from vccq, then
that power rail should simply not be specified at all in the DT.
Working around that fact in the driver is detrimental, as evidenced by the
failure to initialize the host controller on MSM8998.

scsi_device_quiesce() and scsi_device_resume() are called during
system-wide suspend and resume. scsi_device_quiesce() only succeeds for
SCSI devices that are in one of the RUNNING, OFFLINE or TRANSPORT_OFFLINE
states (see also scsi_set_device_state()). This patch avoids that the
following warning is triggered when resuming a system for which quiescing a
SCSI device failed:

It was reported that some devices report an OPTIMAL TRANSFER LENGTH of
0xFFFF blocks. That looks bogus, especially for a device with a
4096-byte physical block size.
Ignore OPTIMAL TRANSFER LENGTH if it is not a multiple of the device's
reported physical block size.
To make the sanity checking conditionals more readable--and to
facilitate printing warnings--relocate the checking to a helper
function. No functional change aside from the printks.

commit 1917d42d14 ("fcoe: use enum for fip_mode") introduces a separate
enum for the fip_mode that shall be used during initialisation handling
until it is passed to fcoe_ctrl_link_up to set the initial fip_state. That
change was incomplete and gcc quietly converted in various places between
the fip_mode and the fip_state enum values with implicit enum conversions,
which fortunately cannot cause any issues in the actual code's execution.
clang however warns about these implicit enum conversions in the scsi
drivers. This commit consolidates the use of the two enums, guided by
clang's enum-conversion warnings.
This commit now completes the use of the fip_mode: It expects and uses
fip_mode in {bnx2fc,fcoe}_interface_create and fcoe_ctlr_init, and it calls
fcoe_ctrl_set_set() with the correct values in fcoe_ctlr_link_up(). It
also breaks the association between FIP_MODE_AUTO and FIP_ST_AUTO to
indicate these two enums are distinct.

The null check on pointer sess and the subsequent call is redundant as sess
is null on all the the paths that lead to the out_term2 label. Hence the
null check and the call can be removed. Also remove the redundant setting
of sess to NULL as this is not required now.

When sending an srb with qla2x00_start_sp, the sp can complete and be freed
by the time we log the debug message saying we sent it. This can cause a
panic if sp gets reused quickly or when running a kernel that poisons freed
memory.
This was partially fixed by (not every case was addressed):

Add HI3670 SoC UFS driver support by extending the common ufs-hisi
driver. One major difference between HI3660 ad HI3670 SoCs interms of UFS
is the PHY. HI3670 has a 10nm variant PHY and hence this parameter is used

The interrupt affinity spreading mechanism supports to spread out
affinities for one or more interrupt sets. A interrupt set contains one or
more interrupts. Each set is mapped to a specific functionality of a
device, e.g. general I/O queues and read I/O queus of multiqueue block
devices.
The number of interrupts per set is defined by the driver. It depends on
the total number of available interrupts for the device, which is
determined by the PCI capabilites and the availability of underlying CPU
resources, and the number of queues which the device provides and the
driver wants to instantiate.
The driver passes initial configuration for the interrupt allocation via a
pointer to struct irq_affinity.
Right now the allocation mechanism is complex as it requires to have a loop
in the driver to determine the maximum number of interrupts which are
provided by the PCI capabilities and the underlying CPU resources. This
loop would have to be replicated in every driver which wants to utilize
this mechanism. That's unwanted code duplication and error prone.
In order to move this into generic facilities it is required to have a
mechanism, which allows the recalculation of the interrupt sets and their
size, in the core code. As the core code does not have any knowledge about the
underlying device, a driver specific callback is required in struct
irq_affinity, which can be invoked by the core code. The callback gets the
number of available interupts as an argument, so the driver can calculate the
corresponding number and size of interrupt sets.
At the moment the struct irq_affinity pointer which is handed in from the
driver and passed through to several core functions is marked 'const', but for
the callback to be able to modify the data in the struct it's required to
remove the 'const' qualifier.
Add the optional callback to struct irq_affinity, which allows drivers to
recalculate the number and size of interrupt sets and remove the 'const'
qualifier.
For simple invocations, which do not supply a callback, a default callback
is installed, which just sets nr_sets to 1 and transfers the number of
spreadable vectors to the set_size array at index 0.
This is for now guarded by a check for nr_sets != 0 to keep the NVME driver
working until it is converted to the callback mechanism.
To make sure that the driver configuration is correct under all circumstances
the callback is invoked even when there are no interrupts for queues left,
i.e. the pre/post requirements already exhaust the numner of available
interrupts.
At the PCI layer irq_create_affinity_masks() has to be invoked even for the
case where the legacy interrupt is used. That ensures that the callback is
invoked and the device driver can adjust to that situation.

Up to 4.12, __scsi_error_from_host_byte() would reset the host byte to
DID_OK for various cases including DID_NEXUS_FAILURE. Commit
2a842acab1 ("block: introduce new block status code type") replaced this
function with scsi_result_to_blk_status() and removed the host-byte
resetting code for the DID_NEXUS_FAILURE case. As the line
set_host_byte(cmd, DID_OK) was preserved for the other cases, I suppose
this was an editing mistake.
The fact that the host byte remains set after 4.13 is causing problems with
the sg_persist tool, which now returns success rather then exit status 24
when a RESERVATION CONFLICT error is encountered.

The function sd_zbc_do_report_zones() issues a REPORT ZONES command with a
buffer size calculated based on the number of zones requested by the
caller. This value should however not exceed the capabilities of the
hardware maximum command size, that is, should not exceed the
max_hw_sectors limit of the device. This problem leads to failures of
report zones commands when re-validating disks with some SAS HBAs.
Fix this by limiting a report zone command buffer size to the minimum of
the device max_hw_sectors and calculated value based on the requested
number of zones. This does not change the semantic of the report_zones file
operation as report zones can always return less zone reports than
requested. Short reports are handled using a loop execution of the
report_zones file operation in the function blk_report_zones().

Before patch 'e76239a3748c ("block: add a report_zones method")', report
zones buffer allocation was limited to max_sectors when allocated in
blk_report_zones(). This however does not consider the actual format of the
device reply which is interface dependent. Limiting the allocation based
on the size of the expected reply format rather than the size of the array
of generic sturct blkzone passed by blk_report_zones() makes more sense.

impact thanks to an unrelated change in openssl that causes sshd to
refuse logins until it has enough entropy for the session keys, which
causes tens of minutes delay before the affected systems allow logins

Commit b2bff6ceb6 ("[SCSI] sd: Quiesce mode sense error messages")
added the macro sd_first_printk(). The macro takes "sdsk" as argument
but dereferences "sdkp". This hasn't caused any real issues since all
callers of sd_first_printk() have an sdkp. But fix the typo.

In qla2x00_async_tm_cmd, we reference off sp after it has been freed. This
caused a panic on a system running a slub debug kernel. Since fcport is
passed in anyways, just use that instead.

The problem is that the default for MQ is not to gather entropy, whereas
the default for the legacy queue was always to gather it. The original
attempt to fix entropy gathering for rotational disks under MQ added an
else branch in sd_read_block_characteristics(). Unfortunately, the entire
check isn't reached if the device has no characteristics VPD page. Since
this page was only introduced in SBC-3 and its optional anyway, most less
expensive rotational disks don't have one, meaning they all stopped
gathering entropy when we made MQ the default. In a wholly unrelated
change, openssl and openssh won't function until the random number
generator is initialised, meaning lots of people have been seeing large
delays before they could log into systems with default MQ kernels due to
this lack of entropy, because it now can take tens of minutes to initialise
the kernel random number generator.
The fix is to set the non-rotational and add-randomness flags
unconditionally early on in the disk initialization path, so they can be
reset only if the device actually reports being non-rotational via the VPD
page.

Teach scsi_debug to honor SWP in the Control Mode Page and report the
resulting WP state in the Device-Specific Parameter field.

Since commit 26e85fcd15 ("[SCSI] sd: Permit merged discard requests";
kernel v3.10) sd_done() sets the residual not only for failed special
requests but also for special requests that succeeded. Hence remove the
code from functions called by sd_init_command() that sets the residual.
This patch does not change any functionality.

For auto-control irq affinity mode, choose the dq to deliver IO according
to the current CPU.
Then it decreases the performance regression that fio and CQ interrupts are
processed on different node.
For user control irq affinity mode, keep it as before.
To realize it, also need to distinguish the usage of dq lock and sas_dev
lock.

To support queue mapped to a CPU, it needs to be ensured that issuing an
internal abort is safe, in that it is guaranteed that an internal abort is
processed for a single IO or a device after all the relevant command(s)
which it is attempting to abort have been processed by the controller.
Currently we only deliver commands for any device on a single queue to
solve this problem, as we know that commands issued on the same queue will
be processed in order, and we will not have a scenario where the internal
abort is racing against a command(s) which it is trying to abort.
To enqueue commands on queue mapped to a CPU, choosing a queue for an
command is based on the associated queue for the current CPU, so this is
not safe for internal abort since it would definitely not be guaranteed
that commands for the command devices are issued on the same queue.
To solve this issue, we take a bludgeoning approach, and issue a separate
internal abort on any queue(s) relevant to the command or device, in that
we will be guaranteed that at least one of these internal aborts will be
received last in the controller.
So, for aborting a single command, we can just force the internal abort to
be issued on the same queue as the command which we are trying to abort.
For aborting all commands associated with a device, we issue a separate
internal abort on all relevant queues. Issuing multiple internal aborts in
this fashion would have not side affect.

If sending IOs to many disks from single queue, it is possible that the
queue may be full. To avoid the situation, change queue depth from 512 to
4096 which is the max number of IOs for v3 hw.

The root cause is that the _IOC macro can generate really large numbers,
which don't fit into type 'int', which is used for the cmd parameter in
the ioctls in scsi_host_template. My research into how GCC and Clang are
handling this at a low level didn't prove fruitful. However, looking at
the rest of the kernel tree, all ioctls use an 'unsigned int' for the
cmd parameter, which will fit all of the _IOC values in the scsi/ata
subsystems.
Make that change because none of the ioctls expect a negative value for
any command, it brings the ioctls inline with the reset of the kernel,
and it removes ambiguity, which is never good when dealing with compilers.

Various null pointer dereference and general protection fault panics occur
when there is a link bounce under load. There are a large number of "error"
message 6413 indicating "bad release".
The issues resolve to list corruptions due to missing or inconsistent lock
protection. Lockups are due to nested locks in the unsolicited abort
path. The unsolicited abort path calls the wrong abort processing
routine. There was also duplicate context release while aborts were still
active in the hardware.
Removed duplicate locks and added lock protection around list item
removal. Commonized lock handling around the abort processing routines.
Prevent context release while still in ABTS list.

When the transport calls into the lpfc target to release an IO job
structure, which corresponds to an exchange, and if the driver was waiting
for an exchange in order to post a previously received command to the
transport, the driver immediately takes the IO job and reuses the context
for the prior command and calls nvmet_fc_rcv_fcp_req() to tell the
transport about a newly received command.
Problem is, the execution of the IO job release may be in the context of
the back end driver and its bio completion handlers, thus it may be in a
irq context and protection code kicks in in the bio and request layers that
are subsequently called.
Rework lpfc so that instead of immediately upcalling, queue it to a
deferred work thread and have the thread make the upcall.
Took advantage of this change to remove duplicated code with the normal
command receive path that preps the IO job and upcalls nvmet_fc. Created a
common routine both paths use.
Also corrected some errors that were found during review of the context
freeing and reuse - basically unlocked operations and a somewhat disjoint
set of calls to release associated job elements. Cleaned up this path and
added locks for coherency.

The conversion to enable SCSI and NVME fc4 support ran into an issue with
NPIV support. With NVME, NPIV is not currently supported, but with SCSI it
was. The driver reverted to its lowest setting meaning NPIV with SCSI was
not allowed.
Convert the NPIV checks and implementation so that SCSI can continue to
allow NPIV support.

A scsi host lock is taken on every io completion to check whether the abort
handler is waiting on the io completion. This is an expensive lock to take
on all completion when rarely in an abort condition.
Replace scsi host lock with command-specific lock. Synchronize completion
and abort paths by new cmd lock. Ensure all flag changing and nulling of
context pointers taken under lock. When adding lock to task management
abort, realized it was missing other synchronization locks. Added that
synchronization to match normal paths.

Current driver uses the older IRQ API for MSIX allocation
Change driver to utilize pci_alloc_irq_vectors when allocating IRQ vectors.
Make lpfc_cpu_affinity_check use pci_irq_get_affinity to determine how the
kernel mapped all the IRQs.
Remove msix_entries from SLI4 structure, replaced with pci_irq_vector()
usage.

When driving high iop counts, auto_imax coalescing kicks in and drives the
performance to extremely small iops levels.
There are two issues:
1) auto_imax is enabled by default. The auto algorithm, when iops gets
high, divides the iops by the hdwq count and uses that value to
calculate EQ_Delay. The EQ_Delay is set uniformly on all EQs whether
they have load or not. The EQ_delay is only manipulated every 5s (a
long time). Thus there were large 5s swings of no interrupt delay
followed by large/maximum delay, before repeating.
2) When processing a CQ, the driver got mixed up on the rate of when
to ring the doorbell to keep the chip appraised of the eqe or cqe
consumption as well as how how long to sit in the thread and
process queue entries. Currently, the driver capped its work at
64 entries (very small) and exited/rearmed the CQ. Thus, on heavy
loads, additional overheads were taken to exit and re-enter the
interrupt handler. Worse, if in the large/maximum coalescing
windows,k it could be a while before getting back to servicing.
The issues are corrected by the following:
- A change in defaults. Auto_imax is turned OFF and fcp_imax is set
to 0. Thus all interrupts are immediate.
- Cleanup of field names and their meanings. Existing names were
non-intuitive or used for duplicate things.
- Added max_proc_limit field, to control the length of time the
handlers would service completions.
- Reworked EQ handling:
Added common routine that walks eq, applying notify interval and max
processing limits. Use queue_claimed to claim ownership of the queue
while processing. Always rearm the queue whenever the common routine
is called.
Rework queue element processing, namely to eliminate hba_index vs
host_index. Only one index is necessary. The queue entry can be
marked invalid and the host_index updated immediately after eqe
processing.
After rework, xx_release routines are now DB write functions. Renamed
the routines as such.
Moved lpfc_sli4_eq_flush(), which does similar action, to same area.
Replaced the 2 individual loops that walk an eq with a call to the
common routine.
Slightly revised lpfc_sli4_hba_handle_eqe() calling syntax.
Added per-cpu counters to detect interrupt rates and scale
interrupt coalescing values.
- Reworked CQ handling:
Added common routine that walks cq, applying notify interval and max
processing limits. Use queue_claimed to claim ownership of the queue
while processing. Always rearm the queue whenever the common routine
is called.
Rework queue element processing, namely to eliminate hba_index vs
host_index. Only one index is necessary. The queue entry can be
marked invalid and the host_index updated immediately after cqe
processing.
After rework, xx_release routines are now DB write functions. Renamed
the routines as such.
Replaced the 3 individual loops that walk a cq with a call to the
common routine.
Redefined lpfc_sli4_sp_handle_mcqe() to commong handler definition with
queue reference. Add increment for mbox completion to handler.
- Added a new module/sysfs attribute: lpfc_cq_max_proc_limit To allow
dynamic changing of the CQ max_proc_limit value being used.
Although this leaves an EQ as an immediate interrupt, that interrupt will
only occur if a CQ bound to it is in an armed state and has cqe's to
process. By staying in the cq processing routine longer, high loads will
avoid generating more interrupts as they will only rearm as the processing
thread exits. The immediately interrupt is also beneficial to idle or
lower-processing CQ's as they get serviced immediately without being
penalized by sharing an EQ with a more loaded CQ.

Review of the eq coalescing logic showed the code was a bit fragmented.
Sometimes it would save/set via an interrupt max value, while in others it
would do so via a usdelay. There were also two places changing eq delay,
one place that issued mailbox commands, and another that changed via
register writes if supported.
Clean this up by:
- Standardizing the operation of lpfc_modify_hba_eq_delay() routine so
that it is always told of a us delay to impose. The routine then chooses
the best way to set that - via register or via mbx.
- Rather than two value types stored in eq->q_mode (usdelay if change via
register, imax if change via mbox) - q_mode always contains usdelay.
Before any value change, old vs new value is compared and only if
different is a change done.
- Revised the dmult calculation. dmult is not set based on overall imax
divided by hardware queues - instead imax applies to a single cpu and
the value will be replicated to all cpus.

So far MSIX vector allocation assumed it would be 1:1 with hardware
queues. However, there are several reasons why fewer MSIX vectors may be
allocated than hardware queues such as the platform being out of vectors or
adapter limits being less than cpu count.
This patch reworks the MSIX/EQ relationships with the per-cpu hardware
queues so they can function independently. MSIX vectors will be equitably
split been cpu sockets/cores and then the per-cpu hardware queues will be
mapped to the vectors most efficient for them.

The desired affinity for the hardware queue behavior is for hdwq 0 to be
affinitized with cpu 0, hdwq 1 to cpu 1, and so on. The implementation so
far does not do this if the number of cpus is greater than the number of
hardware queues (e.g. hardware queue allocation was administratively
reduced or hardware queue resources could not scale to the cpu count).
Correct the queue affinitization logic when queue count is less than
cpu count.

Default behavior is to use the information from the upper IO stacks to
select the hardware queue to use for IO submission. Which typically has
good cpu affinity.
However, the driver, when used on some variants of the upstream kernel, has
found queuing information to be suboptimal for FCP or IO completion locked
on particular cpus.
For command submission situations, the lpfc_fcp_io_sched module parameter
can be set to specify a hardware queue selection policy that overrides the
os stack information.
For IO completion situations, rather than queing cq processing based on the
cpu servicing the interrupting event, schedule the cq processing on the cpu
associated with the hardware queue's cq.

The XRI get/put lists were partitioned per hardware queue. However, the
adapter rarely had sufficient resources to give a large number of resources
per queue. As such, it became common for a cpu to encounter a lack of XRI
resource and request the upper io stack to retry after returning a BUSY
condition. This occurred even though other cpus were idle and not using
their resources.
Create as efficient a scheme as possible to move resources to the cpus that
need them. Each cpu maintains a small private pool which it allocates from
for io. There is a watermark that the cpu attempts to keep in the private
pool. The private pool, when empty, pulls from a global pool from the
cpu. When the cpu's global pool is empty it will pull from other cpu's
global pool. As there many cpu global pools (1 per cpu or hardware queue
count) and as each cpu selects what cpu to pull from at different rates and
at different times, it creates a radomizing effect that minimizes the
number of cpu's that will contend with each other when the steal XRI's from
another cpu's global pool.
On io completion, a cpu will push the XRI back on to its private pool. A
watermark level is maintained for the private pool such that when it is
exceeded it will move XRI's to the CPU global pool so that other cpu's may
allocate them.
On NVME, as heartbeat commands are critical to get placed on the wire, a
single expedite pool is maintained. When a heartbeat is to be sent, it will
allocate an XRI from the expedite pool rather than the normal cpu
private/global pools. On any io completion, if a reduction in the expedite
pools is seen, it will be replenished before the XRI is placed on the cpu
private pool.
Statistics are added to aid understanding the XRI levels on each cpu and
their behaviors.

Now that the lower half has much better per-cpu parallelization using the
hardware queues, the SCSI MQ support needs to be tied into it.
The involves the following mods:
- Use the hardware queue info from the midlayer to help select the
hardware queue to utilize. This required change to the get_scsi-buf_xxx
routines.
- Remove lpfc_sli4_scmd_to_wqidx_distr() routine. No longer needed.
- Includes fix for SLI-3 that does not have multi queue parallelization.

SLI4 nvme functions are passing the SLI3 ring number when posting wqe to
hardware. This should be indicating the hardware queue to use, not the ring
number.
Replace ring number with the hardware queue that should be used.
Note: SCSI avoided this issue as it utilized an older lfpc_issue_iocb
routine that properly adapts.

Many io statistics were being sampled and saved using adapter-based data
structures. This was creating a lot of contention and cache thrashing in
the I/O path.
Move the statistics to the hardware queue data structures. Given the
per-queue data structures, use of atomic types is lessened.
Add new sysfs and debugfs stat routines to collate the per hardware queue
values and report at an adapter level.

Once the IO buff allocations were made shared, there was a single XRI
buffer list shared by all hardware queues. A single list isn't great for
performance when shared across the per-cpu hardware queues.
Create a separate XRI IO buffer get/put list for each Hardware Queue. As
SGLs and associated IO buffers get allocated/posted to the firmware; round
robin their assignment across all available hardware Queues so that there
is an equitable assignment.
Modify SCSI and NVME IO submit code paths to use the Hardware Queue logic
for XRI allocation.
Add a debugfs interface to display hardware queue statistics
Added new empty_io_bufs counter to track if a cpu runs out of XRIs.
Replace common_ variables/names with io_ to make meanings clearer.

Currently, both nvme and fcp each have their own concept of an io_channel,
which is a combination wq/cq and associated msix. Different cpus would
share an io_channel.
The driver is now moving to per-cpu wq/cq pairs and msix vectors. The
driver will still use separate wq/cq pairs per protocol on each cpu, but
the protocols will share the msix vector.
Given the elimination of the nvme and fcp io channels, the module
parameters will be removed. A new parameter, lpfc_hdw_queue is added which
allows the wq/cq pair allocation per cpu to be overridden and allocated to
lesser value. If lpfc_hdw_queue is zero, the number of pairs allocated will
be based on the number of cpus. If non-zero, the parameter specifies the
number of queues to allocate. At this time, the maximum non-zero value is
64.
To manage this new paradigm, a new hardware queue structure is created to
track queue activity and relationships.
As MSIX vector allocation must be known before setting up the
relationships, msix allocation now occurs before queue datastructures are
allocated. If the number of vectors allocated is less than the desired
hardware queues, the hardware queue counts will be reduced to the number of
vectors

There is a extra queue and msix vector for expresslane. Now that the driver
will be doing queues per cpu, this oddball queue is no longer needed.
Expresslane will utilize the normal per-cpu queues.

Currently, both NVME and SCSI get their IO buffers from separate
pools. XRI's are associated 1:1 with IO buffers, so XRI's are also split
between protocols.
Eliminate the independent pools and use a single pool. Each buffer
structure now has a common section and a protocol section. Per protocol
routines for SGL initialization are removed and replaced by common
routines. Initialization of the buffers is only done on the common area.
All other fields, which are protocol specific, are initialized when the
buffer is allocated for use in the per-protocol allocation routine.
In the past, the SCSI side allocated IO buffers as part of slave_alloc
calls until the maximum XRIs for SCSI was reached. As all XRIs are now
common and may be used for either protocol, allocation for everything is
done as part of adapter initialization and the scsi side has no action in
slave alloc.
As XRI's are no longer split, the lpfc_xri_split module parameter is
removed.
Adapters based on SLI3 will continue to use the older scsi_buf_list_get/put
routines. All SLI4 adapters utilize the new IO buffer scheme

An hba-wide lock is taken in the nvme io completion routine. The lock
covers null'ing of the nrport pointer in the cmd structure.
The nrport member isn't necessary. After extracting the pointer from the
command, the pointer was dereferenced to get the fc discovery node
pointer. But the fc discovery node pointer is alrady in the command
structure so the dereferrence was unnecessary.
Eliminated the nrport structure member and its use, which also eliminates
the port-wide lock.

When loop was made down explicitly due to cable pull, then for N2N toplogy,
if FAWWPN BIT is enabled by user, then it would restore some default
(garbage) value for Physical port WWPN, so this show garbage WWPN for the
port. Fix is, to restore physical port WWPN, if it is fabric
configuration. When loop is explicitly made down, and FAWWPN feature is
enabled, then driver need to restore original flashed WWPN.

On Transmit respond in target mode, if the chip is already reset or the
session is already deleted, then advance the command to the free step.
There is no need to abort the command, because the chip has already flushed

On session cleanup, either an implicit LOGO or an implicit PRLO is used to
flush IOs. If the flush command hit Queue Full condition, then it is
dropped. This patch adds retry code to prevent command drop.

This patch provide call back functions to stop the chip and resume the chip
if the PCI lower level driver wants to perform function level reset/FLR.
Before the FLR, the chip will be stopped to turn off all DMA activity.
After the FLR, the chip is reset to resume previous operation state.

This patch adds support for the new DEVICE_LIST DCMD.
Driver currently sends two separate DCMDs for getting the list of PDs and
LDs that are exposed to host. The new DCMD provides a single interface to
get a list of both PDs and LDs that are exposed to the host. Based on the
list of target IDs that are returned by this DCMD, driver will add the
devices (PD/LD) to SML. Driver will check for FW support for this new DCMD
and based on the support will either send the new DCMD or will fall back to
the earlier method of sending two separate DCMDs for PD and LD list.

The deadlock occurs as cxlflash_remove() is called from cxlflash_probe()
without setting 'cxlflash_cfg->state' to STATE_PROBED and the probe thread
starts to wait on 'cxlflash_cfg->reset_waitq'. Since the device was never
successfully probed the 'cxlflash_cfg->state' never changes from
STATE_PROBING hence the deadlock occurs.
We fix this deadlock by setting the variable 'cxlflash_cfg->state' to
STATE_PROBED in case an error occurs during cxlflash_probe() and just
before calling cxlflash_remove().

It added a warning whose intent was to check whether the rport was still
linked into the peer list. It doesn't work as intended and gives false
positive warnings for two reasons:
1) If the rport is never linked into the peer list it will not be
considered empty since the list_head is never initialized.
2) If the rport is deleted from the peer list using list_del_rcu(), then
the list_head is in an undefined state and it is not considered empty.

Commit bf50545696 ("block: Introduce blk_revalidate_disk_zones()")
inadvertently broke the message output of sd_zbc_print_zones() because the
zone information initialization of the scsi disk structure was moved to the
second scan run while sd_zbc_print_zones() is called on the first
scan. This leads to the following incorrect message to be printed for any
ZBC or ZAC zoned disks.

Fix this by initializing sdkp zone size and number of zones early on the
first scan. This does not impact the execution of
blk_revalidate_zones(). This functions is still called only once the block
device capacity is set on the second revalidate run on boot, or if the disk
zone configuration changed (i.e. the disk changed).

There is no need to call twice string_get_size() when the capacity messages

The aic94xx driver is currently failing to load with errors like
sysfs: cannot create duplicate filename '/devices/pci0000:00/0000:00:03.0/0000:02:00.3/0000:07:02.0/revision'
Because the PCI code had recently added a file named 'revision' to every
PCI device. Fix this by renaming the aic94xx revision file to
aic_revision. This is safe to do for us because as far as I can tell,
there's nothing in userspace relying on the current aic94xx revision file
so it can be renamed without breaking anything.

Hot-plugging SAS wire of direct hard disk backplane may cause disk lost. We
have done this test with several types of SATA disk from different venders,
and only two models from Seagate has this problem, ST4000NM0035-1V4107 and
ST3000VM002-1ET166.
The root cause is that the disk doesn't send D2H frame after OOB finished.
SAS controller will issue phyup interrupt only when D2H frame is received,
otherwise, will be waiting there all the time.
When this issue happen, we can find the disk again with link reset. To fix
this issue, we setup an timer after OOB finished. If the PHY is not up in
20s, do link reset. Notes: the 20s is an experience value.

The SAS controller cannot support a programmed minimum linkrate of > 1.5G
(it will always negotiate to 1.5G at least), so just reject it.
This solves a strange situation where the PHY negotiated linkrate may be
less than the programmed minimum linkrate.

When issing a hardreset to a SATA device when running IO, it is possible
that abnormal CQs of the device are returned. Then enter error handler, it
doesn't enter function hisi_sas_abort_task() as there is no timeout IO, and
it doesn't set device as HISI_SAS_DEV_EH. So when hardreset by libata
later, it actually doesn't issue hardreset as there is a check to judge
whether device is in error.
For this situation, actually need to hardreset the device to recover.
So remove the check of sas_dev status in hisi_sas_I_T_nexus_reset().
Before we add the check to avoid the endless loop of reset for
directly-attached SATA device at probe time, actually we flutter it for
it, so it is not necessary to add the check now.

When injecting 2 bit ECC error, it will cause fatal AXI interrupts. Before
the recovery of SAS controller reset, the internal of SAS controller is in
error. If CQ interrupts return at the time, actually it is exception CQ
interrupt, and it may cause resource release in disorder.
To avoid the exception situation, shutdown AXI bus after fatal AXI
interrupt. In SAS controller reset, it will restart AXI bus. For later
version of v3 hw, hardware will shutdown AXI bus for this situation, so
just fix current ver of v3 hw.

The WRITE SAME(10) and (16) implementations didn't take account of the
buffer wrap required when the virtual_gb parameter is greater than 0.
Fix that and rename the fake_store() function to lba2fake_store() to lessen
confusion with the global fake_storep pointer. Bump version date.

Currently, the Kbuild core manipulates header search paths in a crazy way
[1].
To fix this mess, I want all Makefiles to add explicit $(srctree)/ to the
search paths in the srctree. Some Makefiles are already written in that
way, but not all. The goal of this work is to make the notation consistent,
and finally get rid of the gross hacks.
Having whitespaces after -I does not matter since commit 48f6e3cf5b
("kbuild: do not drop -I without parameter").

I was able to build without these extra header search paths.
Especially, the header search path -I. in kernel Makefiles is always
suspicious; it allows the compiler to search for headers in the top of
$(srctree), where obviously no header file exists.

The issue to be fixed in this commit is when libfc found it received a
invalid FLOGI response from FC switch, it would return without freeing the
fc frame, which is just the skb data. This would cause memory leak if FC
switch keeps sending invalid FLOGI responses.
This fix is just to make it execute `fc_frame_free(fp)` before returning
from function `fc_lport_flogi_resp`.

Rework sd_setup_read_write_cmnd() so it becomes more readable. Put all the
sanity checking at the head of the function and sanitize the logged error
messages. Move the legacy SCSI logging calls to the end of the functions
and reduce conditional nesting.

We have had several bugs due mixing sector and logical block size
terminology. In the block layer, a sector is a 512-byte unit regardless of
the logical block size of the underlying device. But the term "sector" is
still widely used in sd.c when referring to logical block sized units.
We previously introduced helper functions such as sectors_to_logical() and
logical_to_sectors() to make the distinction clear. Use these to make the
code in sd.c consistent wrt. logical blocks and block layer sectors.
Use "lba" to describe a logical block address and "nr_blocks" when counting
logical blocks. SBC uses "TRANSFER LENGTH" to describe the latter but this
term was avoided to prevent confusion with the very similar DMA transfer
size (->transfersize) which is counted in bytes.

We cannot wait on a completion object in the lpfc_nvme_targetport structure
in the _destroy_targetport() code path because the NVMe/fc transport will
free that structure immediately after the .targetport_delete() callback.
This results in a use-after-free, and a hang if slub_debug=FZPU is enabled.
Fix this by putting the completion on the stack.

When a host driver sets a maximum segment size we should not only propagate
that setting to the block layer, which can merge segments, but also to the
DMA mapping layer which can merge segments as well.

Replace nvram_* functions with static functions in nvram.h. These will
become wrappers for struct nvram_ops method calls.
This patch effectively disables existing NVRAM functionality so as to
allow the rest of the series to be bisected without build failures.
That functionality is gradually re-implemented in subsequent patches.
Replace the sole validate-checksum-and-read-byte sequence with a call to
nvram_read() which will gain the same semantics in subsequent patches.
Remove unused exports.

On powerpc, setting CONFIG_NVRAM=n builds a kernel with no NVRAM support.
Setting CONFIG_NVRAM=m enables the /dev/nvram misc device module without
enabling NVRAM support in drivers. Setting CONFIG_NVRAM=y enables the
misc device (built-in) and also enables NVRAM support in drivers.
m68k shares the valkyriefb driver with powerpc, and since that driver uses
NVRAM, it is affected by CONFIG_ATARI_SCSI, because of the use of
"select NVRAM". We can avoid the "select" here, but drivers still have
to interpret the CONFIG_NVRAM symbol consistently regardless of platform.
In this patch and the subsequent fbdev driver patch, the convention is
adopted across all relevant platforms whereby NVRAM functionality gets
enabled in a given device driver when the nvram misc device is built-in
or when both drivers are modules.

CONFIG_SCSI_UFS_QCOM selects CONFIG_PHY_QCOM_UFS, assuming that this was
the only possible PHY driver Qualcomm's UFS controller would use. But in
SDM845, the UFS driver is bundled into phy-qcom-qmp, and phy-qcom-ufs is
unused.
Remove the select, since for SDM845 it adds useless drivers to the build.

After Commit 54aed4dd35 ("MIPS: IP27: use dma_direct_ops") qla1280 driver
failed on SGI IP27 machines with
qla1280: QLA1040 found on PCI bus 0, dev 0
qla1280 0000:00:00.0: enabling device (0006 -> 0007)
qla1280: Failed to get request memory
qla1280: probe of 0000:00:00.0 failed with error -12
Reason is that SGI IP27 always generates 64bit DMA addresses and has no
fallback mode for 32bit DMA addresses implemented. QLA1280 supports 64bit
addressing for all DMA accesses so setting coherent mask to 64bit fixes the
issue.

Albeit we no longer rely on those hard-coded descriptor sizes, we still use
them as our defaults, so better get it right. While adding its sysfs
entries, we forgot to update the geometry descriptor size. It is 0x48
according to UFS2.1, and wasn't changed in UFS3.0.

When the driver finds invalid destination MAC for the first un-reachable
target, and before completes the PATH_REQ operation, set new ep_state to
OFFLDCONN_NONE so that as part of driver ep_poll mechanism, the upper
open-iscsi layer is notified to complete the login process on the first
un-reachable target and thus proceed login to other reachable targets.

hba->is_sys_suspended is set after successful system suspend but
not clear after successful system resume.
According to current behavior, hba->is_sys_suspended will not be set if
host is runtime-suspended but not system-suspended. Thus we shall aligh the
same policy: clear this flag even if host remains runtime-suspended after
ufshcd_system_resume is successfully returned.
Simply fix this flag to correct host status logs.

According to Serial Attached SCSI - 1.1 (SAS-1.1):
If an expander PHY attached to a SATA PHY is using a physical link rate
greater than the maximum connection rate supported by the pathway from an
STP initiator port, a management application client should use the SMP PHY
CONTROL function (see 10.4.3.10) to set the PROGRAMMED MAXIMUM PHYSICAL
LINK RATE field of the expander PHY to the maximum connection rate
supported by the pathway from that STP initiator port.
Currently libsas does not support checking if this condition occurs, nor
rectifying when it does.
Such a condition is not at all common, however it has been seen on some
pre-silicon environments where the initiator PHY only supports a 1.5 Gbit
maximum linkrate, mated with 12G expander PHYs and 3/6G SATA phy.
This patch adds support for checking and rectifying this condition during
initial device discovery only.
We do support checking min pathway connection rate during revalidation phase,
when new devices can be detected in the topology. However we do not
support in the case of the the user reprogramming PHY linkrates, such that
min pathway condition is not met/maintained.
A note on root port PHY rates:
The libsas root port PHY rates calculation is broken. Libsas sets the
rates (min, max, and current linkrate) of a root port to the same linkrate
of the first PHY member of that same port. In doing so, it assumes that
all other PHYs which subsequently join the port to have the same
negotiated linkrate, when they could actually be different.
In practice this doesn't happen, as initiator and expander PHYs are
normally initialised with consistent min/max linkrates.
This has not caused an issue so far, so leave alone for now.

Currently the SMP PHY control execution result is checked, however the
function result for the command is not.
As such, we may be missing all potential errors, like SMP FUNCTION FAILED,
INVALID REQUEST FRAME LENGTH, etc., meaning the PHY control request has
failed.
In some scenarios we need to ensure the function result is accepted, so add
a check for this.

When SGE buffer containing DIF information crosses 4G boundary, it results
in DMA error. This patch fixes this issue by calculating SGE buffer size
and if it crosses 4G boundary, driver will split it into multiple SGE
buffers to avoid DMA error.

When SCSI-MQ is enabled, in some case system would present
nr_possible_cpus() which is greater than requested vectors by the
driver. This results into driver being able to get larger number of MSI-X
vectors than actual online CPUs. Driver then uses
pci_alloc_irq_vectors_affinity() to assign 1:1 mapping and affinity for
each MSI-x vector to CPUs. When the command is submitted using MSI-x
vector, assigned to offline CPU, it results in an ABTS and system
hang. This hang is result of a driver not being able to process interrupt
on a vector assigned to an Off-line CPUs
This patch fixes this issue by setting irq_offset value for the
blk_mq_pci_map_queues() to use only those CPUs which has CPU mask affinity
assigned and are online. By using the irq_offset value, driver will allow
online cpumask to decide which vectors are used in blk_mq_pci_map_queues().

Currently we set the protection parameters after calling scsi_add_host()
for v3 hw.
They should be set beforehand, so make this change.
Appearantly this fixes our DIX issue (not mainline yet) also, but more
testing required.

Currently the protection lookup tables in sd_prot_flag_mask() and
sd_prot_op() are declared as non-static. As such, they will be rebuilt for
each respective function call.
Optimise by making them static.
This saves ~100B object code for sd.c:

scsi_mq_setup_tags(), which is called by scsi_add_host(), calculates the
command size to allocate based on the prot_capabilities. In the isci
driver, scsi_host_set_prot() is called after scsi_add_host() so the command
size gets calculated to be smaller than it needs to be. Eventually,
scsi_mq_init_request() locates the 'prot_sdb' after the command assuming it
was sized correctly and a buffer overrun may occur.
However, seeing blk_mq_alloc_rqs() rounds up to the nearest cache line
size, the mistake can go unnoticed.
The bug was noticed after the struct request size was reduced by commit
9d037ad707 ("block: remove req->timeout_list")
Which likely reduced the allocated space for the request by an entire cache
line, enough that the overflow could be hit and it caused a panic, on boot,

sd_done() would call scsi_prot_sg_count() which reads the number of
entities in 'prot_sdb', but seeing 'prot_sdb' is located after the end of
the allocated space it reads a garbage number and erroneously calls
t10_pi_complete().
To prevent this, the calls to scsi_host_set_prot() are moved into
isci_host_alloc() before the call to scsi_add_host(). Out of caution, also
move the similar call to scsi_host_set_guard().

This patch allocates snapshot memory for global reg, port regs, CQ, DQ,
IOST, ITCT.
When we fail to allocate memory for some registers, we free the memory
and set hisi_sas_debugfs_enable as 0 to stop loading debugfs from running.

The non-PCI code has bitrotted for quite a while and will just oops
on load because it passes a NULL pointer to the PCI DMA routines.
Lets kill it for good - if someone really wants to use one of these
cards I'll help mentoring them to write a proper driver glue.

Out of the three callers once insists on the scratch buffer, and the
others are fine with a new allocation. Switch those two to just use
pci_alloc_consistent directly, and open code the scratch buffer
allocation in the remaining one. This avoids a case where we might
be doing a memory allocation under a spinlock with irqs disabled.

The commit 356fd2663c ("scsi: Set request queue runtime PM status back to
active on resume") fixed up the inconsistent RPM status between request
queue and device. However changing request queue RPM status shall be done
only on successful resume, otherwise status may be still inconsistent as
below,
Request queue: RPM_ACTIVE
Device: RPM_SUSPENDED
This ends up soft lockup because requests can be submitted to underlying
devices but those devices and their required resource are not resumed.
For example,

device driver but required resource (like clock) is not resumed yet thus

Changing of caching mode via /sys/devices/.../scsi_disk/.../cache_type may
fail if device responds to MODE SENSE command with DPOFUA flag set, and
then checks this flag to be not set on MODE SELECT command.
In this scenario, when trying to change cache_type, write always fails:

Nobody has actually used the type (VERIFY_READ vs VERIFY_WRITE) argument
of the user address range verification function since we got rid of the
old racy i386-only code to walk page tables by hand.
It existed because the original 80386 would not honor the write protect
bit when in kernel mode, so you had to do COW by hand before doing any
user access. But we haven't supported that in a long time, and these
days the 'type' argument is a purely historical artifact.
A discussion about extending 'user_access_begin()' to do the range
checking resulted this patch, because there is no way we're going to
move the old VERIFY_xyz interface to that model. And it's best done at
the end of the merge window when I've done most of my merges, so let's
just get this done once and for all.
This patch was mostly done with a sed-script, with manual fix-ups for
the cases that weren't of the trivial 'access_ok(VERIFY_xyz' form.
There were a couple of notable cases:
- csky still had the old "verify_area()" name as an alias.
- the iter_iov code had magical hardcoded knowledge of the actual
values of VERIFY_{READ,WRITE} (not that they mattered, since nothing
really used it)
- microblaze used the type argument for a debug printout
but other than those oddities this should be a total no-op patch.
I tried to fix up all architectures, did fairly extensive grepping for
access_ok() uses, and the changes are trivial, but I may have missed
something. Any missed conversion should be trivially fixable, though.

This is two simple target fixes and one discard related I/O starvation
problem in sd. The discard problem occurs because the discard page
doesn't have a mempool backing so if the allocation fails due to
memory pressure, we then lose the forward progress we require if the
writeout is on the same device. The fix is to back it with a mempool.
