The check_intr_schedule() routine in ehci-hcd looks at the wrong
microframes when checking to see if a full-speed or low-speed
interrupt endpoint will fit in the periodic schedule. If the
Start-Split transaction is scheduled for microframe N then the
Complete-Split transactions get scheduled for microframes N+2, N+3, and
N+4. However the code considers N+1, N+2, and N+3 instead.
This patch fixes the limits on the "for" loop and also improves the
use of whitespace.

usb_wait_anchor_empty_timeout() should wait till the completion handler
has run. Both the zd1211rw driver and the uas driver (in its task mgmt) depend
on the completion handler having completed when usb_wait_anchor_empty_timeout()
returns, as they read state set by the completion handler after an
usb_wait_anchor_empty_timeout() call.
But __usb_hcd_giveback_urb() calls usb_unanchor_urb before calling the
completion handler. This is necessary as the completion handler may
re-submit and re-anchor the urb. But this introduces a race where the state
these drivers want to read has not been set yet by the completion handler
(this race is easily triggered with the uas task mgmt code).
I've considered adding an anchor_count to struct urb, which would be
incremented on anchor and decremented on unanchor, and then only actually
do the anchor / unanchor on 0 -> 1 and 1 -> 0 transtions, combined with
moving the unanchor call in hcd_giveback_urb to after calling the completion
handler. But this will only work if urb's are only re-anchored to the same
anchor as they were anchored to before the completion handler ran.
And at least one driver re-anchors to another anchor from the completion
handler (rtlwifi).
So I have come up with this patch instead, which adds the ability to
suspend wakeups of usb_wait_anchor_empty_timeout() waiters to the usb_anchor
functionality, and uses this in __usb_hcd_giveback_urb() to delay wake-ups
until the completion handler has run.

Hi Greg,
Here's four patches for 3.12.
The first patch is a bug fix for the USB 2.0 Link PM registers that I sent
out to the list a long time ago (August), but forgot to queue up. The
second and fourth patches are quirks for xHCI hosts. These patches are
marked for stable. The third patch fixes a bug uncovered with sparse.

I have am335x-evm with one port running in OTG mode. Since commit
fe4cb09 ("usb: musb: gadget: remove hcd initialization") the loaded
gadget does non pop up on the host. All I see is
|usb 4-5: new high-speed USB device number 52 using ehci-pci
|usb 4-5: device descriptor read/64, error -110
Since a later commit 2cc65fe ("usb: musb: add musb_host_setup() and
musb_host_cleanup()) the gadget shows up on the host again but only
in OTG mode (because we have the host init code running). It does not
work in device only mode.
If running in OTG mode and the gadget is removed and added back (rmmod
followed by modprobe of a gadget) then the same error is pops up on the
host side.
This patch ensures that the gadget side also executes musb_start() which
puts the chip in "connect accept" mode. With this change the device
works in OTG & device mode and the gadget can be added & removed
multiple times.
A device (if musb is in OTG mode acting as a host) is only recognized if
it is attached during module load (musb_hdrc module). After the device
unplugged and plugged again the host does not recognize it. We get a
buch of errors if musb running in OTG mode, attached to a host and no
gadget is loaded. Bah.
This is one step forward. Host & device only mode should work. I will
look at OTG later. I looked at this before commit fe4cb09 and OTG wasn't
working there perfectly so I am not sure that it is a regression :)

When configfs is in place, the luns will not be represented in sysfs,
so there will be no struct device associated with a lun.
In order to maintain compatibility and allow configfs adoption
sysfs is made optional in this patch.
As a consequence some debug macros need to be adjusted. Two new
fields are added to struct fsg_lun: name and name_pfx.
The "name" is for storing a string which is presented to the user
instead of the dev_name. The "name_pfx", if non-NULL, is prepended
to the "name" at printing time.
The name_pfx is for a future lun.0, which will be a default group in
mass_storage.<name>. By design at USB function configfs group's creation
time its name is not known (but instead set a bit later in
drivers/usb/gadget/configfs.c:function_make) and it is this name that
serves the purpose of the said name prefix. So instead of copying
a yet-unknown string a pointer to it is stored in struct fsg_lun.

This patch fixes max packet size check in s3c_hsotg_set_ep_maxpacket()

check if maxpacket isn't grater than 1024 which is maximum size od single USB

Haswell LynxPoint and LynxPoint-LP with the recent Intel BIOS show
mysterious wakeups after shutdown occasionally. After discussing with
BIOS engineers, they explained that the new BIOS expects that the
wakeup sources are cleared and set to D3 for all wakeup devices when
the system is going to sleep or power off, but the current xhci driver
doesn't do this properly (partly intentionally).
This patch introduces a new quirk, XHCI_SPURIOUS_WAKEUP, for
fixing the spurious wakeups at S5 by calling xhci_reset() in the xhci
shutdown ops as done in xhci_stop(), and setting the device to PCI D3
at shutdown and remove ops.
The PCI D3 call is based on the initial fix patch by Oliver Neukum.
[Note: Sarah changed the quirk name from XHCI_HSW_SPURIOUS_WAKEUP to
XHCI_SPURIOUS_WAKEUP, since none of the other quirks have system names
in them. Sarah also fixed a collision with a quirk submitted around the
same time, by changing the xhci->quirks bit from 17 to 18.]

The function pci_write_config_dword() sets the appropriate byteordering
internally so the value argument should not be converted to little-endian.
This bug was found by sparse.
This patch is not suitable for stable. Since cpu_to_lei32 is a no-op on
little endian systems, this bug would only affect big endian Intel
systems with the EHCI to xHCI port switchover, which are non-existent,

The RWE bit of the USB 2.0 PORTPMSC register is supposed to enable
remote wakeup for devices in the lower power link state L1. It has
nothing to do with the device suspend remote wakeup from L2. The RWE
bit is designed to be set once (when USB 2.0 LPM is enabled for the
port) and cleared only when USB 2.0 LPM is disabled for the port.
The xHCI bus suspend method was setting the RWE bit erroneously, and the
bus resume method was clearing it. The xHCI 1.0 specification with
errata up to Aug 12, 2012 says in section 4.23.5.1.1.1 "Hardware
Controlled LPM":
"While Hardware USB2 LPM is enabled, software shall not modify the
HIRDBESL or RWE fields of the USB2 PORTPMSC register..."
If we have previously enabled USB 2.0 LPM for a device, that means when
the USB 2.0 bus is resumed, we violate the xHCI specification by
clearing RWE. It also means that after a bus resume, the host would
think remote wakeup is disabled from L1 for ports with USB 2.0 Link PM
enabled, which is not what we want.
This patch should be backported to kernels as old as 3.2, that
contain the commit 65580b4321 "xHCI: set
USB2 hardware LPM". That was the first kernel that supported USB 2.0
Link PM.

This patch adds support for isochronous out transfers to the HWA. The
primary changes are:
1. Add a isoc_pack_desc_urb field to struct wa_seg. This urb is used
to send the isochronous packet info message to the HWA which describes
the isoc data segment(s) that will be sent as the payload of the
transfer request.
2. Use the URB iso_frame_desc field to populate the isochronous packet
info message and data segments sent to the HWA.
3. After the data is sent and transfer result is returned from the
HWA, read the isoc packet status message from the HWA. The contents of
the isoc packet status message are used to set the iso_frame_desc
status and actual_length fields in the original isoc URB. This feature
required the addition of a some state tracking variables in struct wahc
so the dti_urb knows what type of packet it expects to receive next.

Commit "usb: pci-quirks: refactor AMD quirk to abstract AMD chipset types"
introduced a new AMD chipset type to filter AMD platforms with different
chipsets.
According to a recent thread [1], this patch updates USB subsystem hang
symptom quirk which is observed on AMD all SB600 and SB700 revision
0x3a/0x3b. And make it use the new chipset type to represent.

This is needed to prepare for configfs integration.
So far the luns have been allocated during gadget's initialization, based
on the nluns module parameter's value; the exact number is known when the
gadget is initialized and that number of luns is allocated in one go; they
all will be used.
When configfs is in place, the luns will be created one-by-one by the user.
Once the user is satisfied with the number of luns, they activate the
gadget. The number of luns must be <= FSG_MAX_LUN (currently 8), but other
than that it is not known up front and the user need not use contiguous
numbering (apart from the default lun #0). On the other hand, the function
code uses lun numbers to identify them and the number needs to be used
as an index into an array.
Given the above, an array needs to be allocated, but it might happen that
7 out of its 8 elements will not be used. On my machine
sizeof(struct fsg_lun) == 462, so > 3k of memory is allocated but not used
in the worst case.
By adding another level of indirection (allocating an array of pointers
to struct fsg_lun and then allocating individual luns instead of an array
of struct fsg_luns) at most 7 pointers are wasted, which is much less.
This patch also changes some for/while loops to cope with the fact
that in the luns array some entries are potentially empty.

Converting to configfs requires making the f_mass_storage.c a module.
But first we need to get rid of "#include "storage_common.c".
This patch makes storage_common.c a separately compiled file, which is
built as a utility module named u_ms.ko. After all mass storage users are
converted to the new function interface this module can be eliminated
by merging it with the mass storage function's module.
USB descriptors are exported so that they can be accessed from
f_mass_storage.
FSG_VENDOR_ID and FSG_PRODUCT_ID are moved to their only user.
Handling of CONFIG_USB_GADGET_DEBUG_FILES is moved to f_mass_storage.c.
The fsg_num_buffers static is moved to FSG_MODULE_PARAMETER users, so
instead of using a global variable the f_mass_storage introduces
fsg_num_buffers member in fsg_common (and fsg_config).
fsg_strings and fsg_stringtab are moved to f_mass_storage.c.

After commit 09fc7d22b0 (usb: musb: fix incorrect
usage of resource pointer), CPPI DMA driver on DaVinci DM6467 can't detect its
dedicated IRQ and so the MUSB IRQ is erroneously used instead. This is because
only 2 resources are passed to the MUSB driver from the DaVinci glue layer, so
fix this by always copying 3 resources (it's safe since a placeholder for the
3rd resource is always there) and passing 'pdev->num_resources' instead of the
size of musb_resources[] to platform_device_add_resources().

This went unnoticed in durin the merge window:
The dsps driver creates a child device for the musb core driver _and_
attaches the of_node to it so devm_usb_get_phy_by_phandle() grabs the
correct phy and attaches the devm resources to the proper device. We
could also use the parent device but then devm would attach the
resource to the wrong device and it would be destroyed once the parent
device is gone - not the device that is used by the musb core driver.
If the phy is now not available then dsps_musb_init() /
devm_usb_get_phy_by_phandle() returns with EPROBE_DEFER. Since the
of_node is attached it tries OF drivers as well and matches the driver
against DSPS. That one creates a new child device for the musb core
driver which gets probed immediately.
The whole thing repeats itself until the stack overflows.
I belive the same problem exists in ux500 glue code (since 313bdb11
("usb: musb: ux500: add device tree probing support") but the drivers are
now probed in the right order so they don't see it.
The problem is that the dsps driver gets bound to the musb-child device
due to the same of_node / matching binding. I don't really agree with
having yet another child node in DT to fix this. Ideally we would have
musb core driver with DT bindings and according to the binding we would
select the few extra hacks / gleue layer.
Therefore I suggest the driver to reject the musb-core device.

This patch updates URB dequeue handling in wusbcore to make it more
reliable when a URB has been broken up into multiple WUSB transfer
request segments.
In wa_urb_dequeue, don't mark segments in the WA_SEG_SUBMITTED,
WA_SEG_PENDING or WA_SEG_DTI_PENDING states as completed if an ABORT
TRANSFER request was sent to the HWA to clean them up. Wait for the
HWA to return a transfer result indicating that it has aborted the
request before cleaning it up. This prevents the DTI state machine

musb->xceiv can't be removed as of now because musb core uses xceiv.state and

moved out of xceiv and then we can start using the generic PHY framework.

Used the generic PHY framework API to create the PHY. Now the power off and
power on are done in omap_usb_power_off and omap_usb_power_on respectively.
The omap-usb2 driver is also moved to driver/phy.
However using the old USB PHY library cannot be completely removed
because OTG is intertwined with PHY and moving to the new framework
will break OTG. Once we have a separate OTG state machine, we
can get rid of the USB PHY library.

This patch updates __wa_xfer_setup_segs error path to only clean up the
xfer->seg entry that it failed to create and then set that entry to
NULL. wa_xfer_destroy will clean up the remaining xfer->segs that were
fully created. It also moves the code to create the dto sg list to an
out of line function to make __wa_xfer_setup_segs easier to read. Prior
to this change, __wa_xfer_setup_segs would clean up all entries in the
xfer->seg array in case of an error but it did not set them to NULL.
This resulted in a double free when wa_xfer_destroy was eventually
called by the higher level error handler.

If __wa_xfer_setup fails, it can leave a partially constructed wa_xfer
object. The error handling code eventually calls wa_xfer_destroy which
does not check for NULL before dereferencing xfer->seg which could cause
a kernel panic. This change also makes sure to free xfer->seg which was
being leaked for all transfers before this change.

There's a bunch of failure exits in ffs_fs_mount() with
seriously broken recovery logics. Most of that appears to stem
from misunderstanding of the ->kill_sb() semantics; unlike
->put_super() it is called for *all* superblocks of given type,
no matter how (in)complete the setup had been. ->put_super()
is called only if ->s_root is not NULL; any failure prior to
setting ->s_root will have the call of ->put_super() skipped.
->kill_sb(), OTOH, awaits every superblock that has come from
sget().
Current behaviour of ffs_fs_mount():
We have struct ffs_sb_fill_data data on stack there. We do
ffs_dev = functionfs_acquire_dev_callback(dev_name);
and store that in data.private_data. Then we call mount_nodev(),
passing it ffs_sb_fill() as a callback. That will either fail
outright, or manage to call ffs_sb_fill(). There we allocate an
instance of struct ffs_data, slap the value of ffs_dev (picked
from data.private_data) into ffs->private_data and overwrite
data.private_data by storing ffs into an overlapping member
(data.ffs_data). Then we store ffs into sb->s_fs_info and attempt
to set the rest of the things up (root inode, root dentry, then
create /ep0 there). Any of those might fail. Should that
happen, we get ffs_fs_kill_sb() called before mount_nodev()
returns. If mount_nodev() fails for any reason whatsoever,
we proceed to
functionfs_release_dev_callback(data.ffs_data);
That's broken in a lot of ways. Suppose the thing has failed in
allocation of e.g. root inode or dentry. We have
functionfs_release_dev_callback(ffs);
ffs_data_put(ffs);
done by ffs_fs_kill_sb() (ffs accessed via sb->s_fs_info), followed by
functionfs_release_dev_callback(ffs);
from ffs_fs_mount() (via data.ffs_data). Note that the second
functionfs_release_dev_callback() has every chance to be done to freed memory.
Suppose we fail *before* root inode allocation. What happens then?
ffs_fs_kill_sb() doesn't do anything to ffs (it's either not called at all,
or it doesn't have a pointer to ffs stored in sb->s_fs_info). And
functionfs_release_dev_callback(data.ffs_data);
is called by ffs_fs_mount(), but here we are in nasal daemon country - we
are reading from a member of union we'd never stored into. In practice,
we'll get what we used to store into the overlapping field, i.e. ffs_dev.
And then we get screwed, since we treat it (struct gfs_ffs_obj * in
disguise, returned by functionfs_acquire_dev_callback()) as struct
ffs_data *, pick what would've been ffs_data ->private_data from it
(*well* past the actual end of the struct gfs_ffs_obj - struct ffs_data
is much bigger) and poke in whatever it points to.
FWIW, there's a minor leak on top of all that in case if ffs_sb_fill()
fails on kstrdup() - ffs is obviously forgotten.
The thing is, there is no point in playing all those games with union.
Just allocate and initialize ffs_data *before* calling mount_nodev() and
pass a pointer to it via data.ffs_data. And once it's stored in
sb->s_fs_info, clear data.ffs_data, so that ffs_fs_mount() knows that
it doesn't need to kill the sucker manually - from that point on
we'll have it done by ->kill_sb().

in MUSB's Device Tree node for the 'clock-names' property. If it's absent
the clock request will fail. However, if we don't supply the con_id then
clk_get() will call into clk_sys() which will use the device name to
search for the appropriate clock, which is much more natural than forcing
'usb'.

Trying to read data from the Pegasus Technologies NoteTaker (0e20:0101)
[1] with the Windows App (EasyNote) works natively but fails when
Windows is running under KVM (and the USB device handed to KVM).
The reason is a USB control message
usb 4-2.2: control urb: bRequestType=22 bRequest=09 wValue=0200 wIndex=0001 wLength=0008
This goes to endpoint address 0x01 (wIndex); however, endpoint address
0x01 does not exist. There is an endpoint 0x81 though (same number,
but other direction); the app may have meant that endpoint instead.
The kernel thus rejects the IO and thus we see the failure.
Apparently, Linux is more strict here than Windows ... we can't change
the Win app easily, so that's a problem.
It seems that the Win app/driver is buggy here and the driver does not
behave fully according to the USB HID class spec that it claims to
belong to. The device seems to happily deal with that though (and
seems to not really care about this value much).
So the question is whether the Linux kernel should filter here.
Rejecting has the risk that somewhat non-compliant userspace apps/
drivers (most likely in a virtual machine) are prevented from working.
Not rejecting has the risk of confusing an overly sensitive device with
such a transfer. Given the fact that Windows does not filter it makes
this risk rather small though.
The patch makes the kernel more tolerant: If the endpoint address in
wIndex does not exist, but an endpoint with toggled direction bit does,
it will let the transfer through. (It does NOT change the message.)
With attached patch, the app in Windows in KVM works.
usb 4-2.2: check_ctrlrecip: process 13073 (qemu-kvm) requesting ep 01 but needs 81
I suspect this will mostly affect apps in virtual environments; as on
Linux the apps would have been adapted to the stricter handling of the
kernel. I have done that for mine[2].

This patch straightens out some locking issues in the USB sysfs
interface:
Deauthorization will destroy existing configurations.
Attributes that read from udev->actconfig need to lock the
device to prevent races. Likewise for the rawdescriptor
values.
Attributes that access an interface's current alternate
setting should use ACCESS_ONCE() to obtain the cur_altsetting
pointer, to protect against concurrent altsetting changes.
The supports_autosuspend() attribute routine accesses values
from an interface's driver, so it should lock the interface
(rather than the usb_device) to protect against concurrent
unbinds. Once this is done, the routine can be simplified
considerably.
Scalar values that are stored directly in the usb_device structure are
always available. They do not require any locking. The same is true
of the cached interface string descriptor, because it is not
deallocated until the usb_host_interface structure is destroyed.

bmAttributes in endpoint descriptor might not be set zero. So it's better
to use usb_endpoint_type routine to mask bmAttributes with
USB_ENDPOINT_XFERTYPE_MASK to judge the transfter type later.

The following patch is required to resolve remote wake issues with
certain devices.
Issue description:
If the remote wake is issued from the device in a specific timing
condition while the system is entering sleep state then it may cause
system to auto wake on subsequent sleep cycle.
Root cause:
Host controller rebroadcasts the Resume signal > 100 µseconds after
receiving the original resume event from the device. For proper
function, some devices may require the rebroadcast of resume event
within the USB spec of 100µS.
Workaroud:
1. Filter the AMD platforms with Yangtze chipset, then judge of all the usb
devices are mouse or not. And get out the port id which attached a mouse
with Pixart controller.
2. Then reset the port which attached issue device during system resume
from S3.
[Q] Why the special devices are only mice? Would high speed devices
such as 3G modem or USB Bluetooth adapter trigger this issue?
- Current this sensitivity is only confined to devices that use Pixart
controllers. This controller is designed for use with LS mouse
devices only. We have not observed any other devices failing. There
may be a small risk for other devices also but this patch (reset
device in resume phase) will cover the cases if required.
[Q] Shouldn’t the resume signal be sent within 100 us for every
device?
- The Host controller may not send the resume signal within 100us,
this our host controller specification change. This is why we
require the patch to prevent side effects on certain known devices.
[Q] Why would clicking mouse INTENSELY to wake the system up trigger
this issue?
- This behavior is specific to the devices that use Pixart controller.
It is timing dependent on when the resume event is triggered during
the sleep state.
[Q] Is it a host controller issue or mouse?
- It is the host controller behavior during resume that triggers the
device incorrect behavior on the next resume.
This patch sets USB_QUIRK_RESET_RESUME flag for these Pixart-based mice
when they attached to platforms with AMD Yangtze chipset.

This patch abstracts out a AMD chipset type which includes southbridge
generation and its revision. When os excutes usb_amd_find_chipset_info
routine to initialize AMD chipset type, driver will know which kind of
chipset is used.
This update has below benifits:
- Driver is able to confirm which southbridge generations and their
revision are used, with chipset detection once.
- To describe chipset generations with enumeration types brings better
readability.
- It's flexible to filter AMD platforms to implement new quirks in future.

There is a pending TD which is not freed after request finishes,
we do this due to a controller bug. This TD needs to be freed when
the driver is removed. It prints below error message when unload
chipidea driver at current code:
"ci_hdrc ci_hdrc.0: dma_pool_destroy ci_hw_td, b0001000 busy"
It indicates the buffer at dma pool are still in use.
This commit will free the pending TD at driver's removal procedure,
it can fix the problem described above.

Support for specifying soft dependencies in the modules themselves was
introduced in commit 7cb14ba.
In Arch we have always been shipping a module.d(5) fragment ordering ohci/uhci
after ehci. If this ordering is really necessary, it would be great to move it
to the kernel and getting the correct fragment generated by depmod.

Since uhci-hcd, ehci-hcd, and xhci-hcd support runtime PM, the .pm
field in their pci_driver structures should be protected by CONFIG_PM
rather than CONFIG_PM_SLEEP. The corresponding change has already
been made for ohci-hcd.
Without this change, controllers won't do runtime suspend if system
suspend or hibernation isn't enabled.

Commit 24f531371d (USB: EHCI: accept very late isochronous URBs)
changed the isochronous API provided by ehci-hcd. URBs submitted too
late, so that the time slots for all their packets have already
expired, are no longer rejected outright. Instead the submission is
accepted, and the URB completes normally with a -EXDEV error for each
packet. This is what client drivers expect.
This patch implements the same policy in ohci-hcd. The change is more
complicated than it was in ehci-hcd, because ohci-hcd doesn't scan for
isochronous completions in the same way as ehci-hcd does. Rather, it
depends on the hardware adding completed TDs to a "done queue". Some
OHCI controller don't handle this properly when a TD's time slot has
already expired, so we have to avoid adding such TDs to the schedule
in the first place. As a result, if the URB was submitted too late
then none of its TDs will get put on the schedule, so none of them
will end up on the done queue, so the driver will never realize that
the URB should be completed.
To solve this problem, the patch adds one to urb_priv->td_cnt for such
URBs, making it larger than urb_priv->length (td_cnt already gets set
to the number of TD's that had to be skipped because their slots have
expired). Each time an URB is given back, the finish_urb() routine
looks to see if urb_priv->td_cnt for the next URB on the same endpoint
is marked in this way. If so, it gives back the next URB right away.
This should be applied to all kernels containing commit 815fa7b917
(USB: OHCI: fix logic for scheduling isochronous URBs).
