Define name tables for these enumerations in a similar way as for
loopback. Move the loopback name table together with them.

All files that include ethtool.h, rx.h or tx.h are also including
efx.h, and there is no good reason to separate out the few
declarations they contain. Therefore fold them into efx.h.

Now that core network takes care of trans_start updates, dont do it
in drivers themselves, if possible. Multi queue drivers can
avoid one cache miss (on dev->trans_start) in their start_xmit()
handler.

Make use of the newly-added generic MDIO clause 45 support and remove
redundant definitions.
Add an 'efx_' prefix to the remaining driver-specific MDIO functions
and remove arguments which are redundant with efx->mdio.prtad.

MAC, PHY and board events may be separately enabled and signalled.
Our current arrangement of chaining the polling functions can result
in events being missed. Change them to be more independent.

The SFC4000 has a separate MAC for use at sub-10G speeds. Introduce
an efx_mac_operations structure with implementations for the two MACs.
Switch between the MACs as necessary.
PHY settings are independent of the MAC, so add get_settings() and
set_settings() to efx_phy_operations. Also add macs field to indicate
which MACs the PHY is connected to.

Add efx_poll_loopback() function to test for successful completion of test.
Change efx_test_loopback() to end the test after 1 ms if
efx_poll_loopback() indicates success, and otherwise to wait for 100 ms
as before.
While we're here, rename efx_{rx,tx}_loopback() to
efx_{begin,end}_loopback() which more accurately reflect their
purpose.

efx_test_loopback() used "|" to combine the results of the RX and TX
phases. If both phases fail with different error codes, this results
in a bogus error code.

Checksum generation is an attribute of our hardware TX queues, not TX
descriptors. We previously used a single queue and turned checksum
generation on or off as requested through ethtool. However, this can
result in regenerating checksums in raw packets that should not be
modified. We now create 2 hardware TX queues with checksum generation
on or off. They are presented to the net core as one queue since it
does not know how to select between them.
The self-test verifies that a bad checksum is unaltered on the queue
with checksum generation off.
