- Remove vport params on physical hba when npiv is disabled

FUJITA Tomonori 9cb83c7529 [SCSI] add use_sg_chaining option to scsi_host_template
This option is true if a low-level driver can support sg
chaining. This will be removed eventually when all the drivers are
converted to support sg chaining. q->max_phys_segments is set to
SCSI_MAX_SG_SEGMENTS if false.

Jesper Juhl a0a74e4505 [SCSI] lpfc: fix potential overflow of hbqs array
The Coverity checker noticed that we may overrun a statically allocated
array in drivers/scsi/lpfc/lpfc_sli.c::lpfc_sli_hbqbuf_find().

The case is this; In 'struct lpfc_hba' we have

	#define LPFC_MAX_HBQS  4
	...
	struct lpfc_hba {
		...
		struct hbq_s hbqs[LPFC_MAX_HBQS];
		...
	};

But then in lpfc_sli_hbqbuf_find() we have this code

	hbqno = tag >> 16;
	if (hbqno > LPFC_MAX_HBQS)
		return NULL;

if 'hbqno' ends up as exactely 4, then we won't return, and then this

	list_for_each_entry(d_buf, &phba->hbqs[hbqno].hbq_buffer_list, list) {

will cause an overflow of the statically allocated array at index 4,
since the valid indices are only 0-3.

I propose this patch, that simply changes the 'hbqno > LPFC_MAX_HBQS'
into 'hbqno >= LPFC_MAX_HBQS' as a possible fix.

8.2.2 lpfc patches revert a change to using SCSI command accessor
functions.

This patch, to be applied on top of the 8.2.2. patches, updates the
driver for the accessor functions.

Rework the lpfc_printf_log() macro so that logging is enabled on a
per-vport basis. Used to be on a physical-port basis, thus logging
with large numbers of vports became a mess.  Required redefinition of
the macro, and an update of every use.

FUJITA Tomonori c59fd9ebc4 [SCSI] lpfc: fix NPIV mapping problems
This patch uses dma_map_sg with phba->pcidev->dev instead of
scsi_dma_map.

scsi_dma_map doesn't work for NPIV since fc_vport->dev isn't fully
initialized. check_addr() in arch/x86_64/kernel/pci-nommu.c leads to
the crash since dev->dma_mask is NULL.

Zhang Rui 91a6902958 sysfs: add parameter "struct bin_attribute *" in .read/.write methods for sysfs binary attributes
Well, first of all, I don't want to change so many files either.

What I do:
Adding a new parameter "struct bin_attribute *" in the
.read/.write methods for the sysfs binary attributes.

In fact, only the four lines change in fs/sysfs/bin.c and
include/linux/sysfs.h do the real work.
But I have to update all the files that use binary attributes
to make them compatible with the new .read and .write methods.
I'm not sure if I missed any. :(

Why I do this:
For a sysfs attribute, we can get a pointer pointing to the
struct attribute in the .show/.store method,
while we can't do this for the binary attributes.
I don't know why this is different, but this does make it not
so handy to use the binary attributes as the regular ones.
So I think this patch is reasonable. :)

Who benefits from it:
The patch that exposes ACPI tables in sysfs
requires such an improvement.
All the table binary attributes share the same .read method.
Parameter "struct bin_attribute *" is used to get
the table signature and instance number which are used to
distinguish different ACPI table binary attributes.

Without this parameter, we need to offer different .read methods
for different ACPI table binary attributes.
This is impossible as there are various ACPI tables on different
platforms, and we don't know what they are until they are loaded.

Tejun Heo 7b595756ec sysfs: kill unnecessary attribute->owner
sysfs is now completely out of driver/module lifetime game.  After
deletion, a sysfs node doesn't access anything outside sysfs proper,
so there's no reason to hold onto the attribute owners.  Note that
often the wrong modules were accounted for as owners leading to
accessing removed modules.

This patch kills now unnecessary attribute->owner.  Note that with
this change, userland holding a sysfs node does not prevent the
backing module from being unloaded.

NPIV support is added to the driver.  It utilizes the interfaces of
the fc transport for the creation and deletion of vports. Within the
driver, a new Scsi_Host is created for each NPIV instance, and is
paired with a new instance of a FC port.  This allows N FC Port
elements to share a single Adapter.

NPIV support is only available via new adapter interface extensions,
termed SLI-3. This interface changes some of the basic behaviors such
as command and response ring element sizes and data structures, as
well as a change in buffer posting.  Note: the new firmware extensions
are found only on our mid-range and enterprise 4Gig adapters - so NPIV
support is available only on these newer adapters. The latest firmware
can be downloaded from the Emulex support page.

The driver is reorganized to separate the handling of the adapter from
the handling of the FC port. Adapter handling includes submissions of
command requests, receiving responses, and managing adapter resources.
The FC port includes the discovery engine, login handling, and the
mapping of a Scsi_Host on the "port".  Although not a large functional
change, as it touches core structures and functions, resulting in a
large text delta.

Anton Blanchard 07da60c1f4 [SCSI] lpfc: fix oops when parsing dodgy VPD
We have seen two cases where VPD on an emulex card has been incorrect
and we end up walking off the end of memory. It looks like someone made
an update (increased the length of a string) without increasing the
Length field. Then we do:

	Length -= (3+i);

And since Length is unsigned it becomes very large and we loop forever
in the encapsulating:

	while (Length > 0) {

If we make Length signed then we fall out of the loop and proceed on.

Its important to note we have only seen this in the lab and it may be
the only two cases of this in existence, but since the rest of the code
has been written to be resilient against bad VPD we may as well fix this
too.

Linas Vepstas 5daa49ef05 [SCSI] lpfc: avoid double-free during PCI error failure
If a PCI error is detected that cannot be recovered from, there
will be a double call of lpfc_pci_remove_one(), with the second call
resulting in a null-pointer dereference. The first call occurs in
lpfc_io_error_detected(), and the second call during pci device
remove. This patch eliminates the first call; its un-needed.

David Howells 7d12e780e0 IRQ: Maintain regs pointer globally rather than passing to IRQ handlers
Maintain a per-CPU global "struct pt_regs *" variable which can be used instead
of passing regs around manually through all ~1800 interrupt handlers in the
Linux kernel.

The regs pointer is used in few places, but it potentially costs both stack
space and code to pass it around.  On the FRV arch, removing the regs parameter
from all the genirq function results in a 20% speed up of the IRQ exit path
(ie: from leaving timer_interrupt() to leaving do_IRQ()).

Where appropriate, an arch may override the generic storage facility and do
something different with the variable.  On FRV, for instance, the address is
maintained in GR28 at all times inside the kernel as part of general exception
handling.

Having looked over the code, it appears that the parameter may be handed down
through up to twenty or so layers of functions.  Consider a USB character
device attached to a USB hub, attached to a USB controller that posts its
interrupts through a cascaded auxiliary interrupt controller.  A character
device driver may want to pass regs to the sysrq handler through the input
layer which adds another few layers of parameter passing.

I've build this code with allyesconfig for x86_64 and i386.  I've runtested the
main part of the code on FRV and i386, though I can't test most of the drivers.
I've also done partial conversion for powerpc and MIPS - these at least compile
with minimal configurations.

This will affect all archs.  Mostly the changes should be relatively easy.
Take do_IRQ(), store the regs pointer at the beginning, saving the old one:

	struct pt_regs *old_regs = set_irq_regs(regs);

And put the old one back at the end:

	set_irq_regs(old_regs);

Don't pass regs through to generic_handle_irq() or __do_IRQ().

In timer_interrupt(), this sort of change will be necessary:

	-	update_process_times(user_mode(regs));
	-	profile_tick(CPU_PROFILING, regs);
	+	update_process_times(user_mode(get_irq_regs()));
	+	profile_tick(CPU_PROFILING);

I'd like to move update_process_times()'s use of get_irq_regs() into itself,
except that i386, alone of the archs, uses something other than user_mode().

Some notes on the interrupt handling in the drivers:

 (*) input_dev() is now gone entirely.  The regs pointer is no longer stored in
     the input_dev struct.

 (*) finish_unlinks() in drivers/usb/host/ohci-q.c needs checking.  It does
     something different depending on whether it's been supplied with a regs
     pointer or not.

 (*) Various IRQ handler function pointers have been moved to type
     irq_handler_t.

Serge E. Hallyn 96b644bdec [PATCH] namespaces: utsname: use init_utsname when appropriate

Stall error handler if attempting resets/aborts while an rport is blocked.
This avoids device offline scenarios due to errors in the error handler.

Background:
  Although the transport is using the scsi_timed_out functionality to
  restart the timeout if the rport is blocked, if the timeout has already
  fired before the block occurs, the eh handler still runs and can take
  the device offline. Ultimately, this window cannot be resolved without
  significant work in the error handler thread. Christoph noted the first
  level of these issues when he noted the poor error response handling
  by the error thread.

  We found, under heavy load and error testing, that time window from when
  the scsi_times_out() adds the io to the queue to when the scsi_error_handler
  gets around to servicing it, can be in the several seconds range. In most
  cases, these test conditions are highly unusual, but possible.
  As a result, we're stalling the error handler in this race window so that
  we can avoid the device_offline transitions.

Introduce lpfc_reset_barrier() function for resets on dual channel adapters

Workaround for a hardware errata on dual channel asics. There is a
potential for the chip to lock up on a reset if a shared dma engine is in
use. The (ugly) work around requires a reset process which uses a mailbox
command to synchronize the independent channels prior to the reset to
avoid the issue. Unfortunately, the timing windows required to ensure this
workaround succeeds are very specific, meaning we can't release the cpu
during the barrier.

Fix deadlock in lpfc_fdmi_tmo_handler

lpfc_fdmi_tmo_handler was calling lpfc_fdmi_cmd with the host_lock
held.  lpfc_fdmi_cmd assumes the host_lock is released as it calls functions
that acquire the host_lock.  lpfc_fdmi_tmo_handler acquired the host_lock to
protect access to work_hba_events.  This was already checked in the worker
thread so we can remove that code completely and remove access to the
host_lock.
