Current pciehp driver's intialization sequence is as follows:
(1) initialize controller data structure
(2) install interrupt handler
(3) enable software notification
(4) initialize controller specific slot data structure
(5) initialize generic slot data structure and register it to pci hotplug core
The interrupt handler of pciehp assumes that controller specific slot
data structure is already initialized. However, it is installed at (2)
before initializing controller specific slot data structure at
(4). Because of this, pciehp driver cannot handle the following cases
properly.
- If devices that shares IRQ with pciehp raise interrupts between (2) and (4).
- If hotplug events (e.g. MRL open) happen between (3) and (4).
We already have a workaround for this problem ("pciehp: fix NULL
dereference in interrupt handler: dbd79aed1a).
But we still need fundamental fix.
This patch fix the problem by changing the initilization sequence as follows:
(1) initialize controller data structure
(2) initialize controller specific slot data structure
(3) install interrupt handler
(4) enable software notification
(5) initialize generic slot data structure and register it to pci hotplug core

Currently, /sys/bus/pci/slots/ only exposes hotplug attributes when a
hotplug driver is loaded, but PCI slots have attributes such as address,
speed, width, etc. that are not related to hotplug at all.
Introduce pci_slot as the primary data structure and kobject model.
Hotplug attributes described in hotplug_slot become a secondary
structure associated with the pci_slot.
This patch only creates the infrastructure that allows the separation of
PCI slot attributes and hotplug attributes. In this patch, the PCI
hotplug core remains the only user of this infrastructure, and thus,
/sys/bus/pci/slots/ will still only become populated when a hotplug
driver is loaded.
A later patch in this series will add a second user of this new
infrastructure and demonstrate splitting the task of exposing pci_slot
attributes from hotplug_slot attributes.
- Make pci_slot the primary sysfs entity. hotplug_slot becomes a
subsidiary structure.
o pci_create_slot() creates and registers a slot with the PCI core
o pci_slot_add_hotplug() gives it hotplug capability
- Change the prototype of pci_hp_register() to take the bus and
slot number (on parent bus) as parameters.
- Remove all the ->get_address methods since this functionality is
now handled by pci_slot directly.

According to the PCI Express specification, we must wait for at least
1 second after turning power off before taking any action that relies
on power having been removed from the slot/adapter. For this, current
pciehp wait for 1 second after issuing the power off command in
hpc_power_off_slot() function. But waiting for 1 second in
hpc_power_off_slot() can make pciehp probing slow-down because pciehp
probe code calls hpc_power_off_slot() if the slot is not occupied just
in case. We don't need to wait for 1 second at the pciehp probe time
because there is no action on that empty slot. So move 1 second wait
from hpc_power_off_slot() to the caller of hpc_power_off_slot().

Fix improper long wait for command completion in pciehp probing.
As described in PCI Express specification, software notification is
not generated if the command that occurs as a result of a write to the
Slot Control register that disables software notification of command
completed events. Since pciehp driver doesn't take it into account,
such command is issued in pciehp probing, and it causes improper long
wait for command completion.

Fix the "pciehp probing slow" problem reported from Jan C. Nordholz in
http://bugzilla.kernel.org/show_bug.cgi?id=10751.
The command completed bit in Slot Status register applies only to
commands issued to control the attention indicator, power indicator,
power controller, or electromechanical interlock. However, writes to
other parts of the Slot Control register would end up writing to the
control fields. Hence, any write to Slot Control register is
considered as a command. However, if the controller doesn't support
any of attention indicator, power indicator, power controller and
electromechanical interlock, command completed bit would not set in
writing to Slot Control register. In this case, we should not wait for
command completed bit set, otherwise all commands would be considered
not completed in timeout seconds (1 sec.).
The cause of the problem is pciehp driver didn't take this situation
into account. This patch changes pciehp to take it into account. This
patch also add the check for "No Command Completed Support" bit in
Slot Capability register. If it is set, we should not wait for command
completed bit set as well.
This problem seems to be revealed by the commit
c27fb883df that fixed the bug that
pciehp did not wait for command completed properly (pciehp just
ignored the command completion event).

Current pciehp implementation disables and re-enables hotplug interrupts
in its interrupt handler. This operation might be intend to guarantee
that interrupts for the events newly occured during previous events are
being handled will be successfully generated. But current implementaion
has the following prolems.
- Current interrupt service routin clears status changes without
waiting command completion. Because of this, events might not be
cleared properly.
- Current interrupt service routine clears status changes caused by
disabling or enabling hotplug interrupts itself. This will lose new
events that occurs during previous interrupts are being handled.
- Current implementation doesn't have any serialization mechanism
between the code to wait for command completion and the interrupt
handler that clears the command completion events caused by itself.
There is clearly race conditions between them, and it may cause
the problem that waiting for command completion doesn't work for
example.
To fix those problems, this patch stops disabling/re-enabling hotplug
interrupts in interrupt service routine. Instead of this, this patch
re-inspects Slot Status register after clearing what is presumed to
be the last bending interrupt in order to guarantee that all interrupt
events are serviced.

The 'power' attribute of the fakephp driver originally only let one turn a
slot off. If one tried to turn a slot on (echo 1 > .../power), it would
return ENODEV, as fakephp did not support this function.
An old (pre-git) patch changed this:
2004/11/11 16:33:31-08:00 jdittmer
[PATCH] fakephp: add pci bus rescan ability
http://article.gmane.org/gmane.linux.kernel/251183
Now writing "1" to the power attribute has the effect of triggering a bus
rescan, but it still returns ENODEV, probably an oversight in the above
patch.
Using the BusyBox echo will not produce an error message, but will
trigger *two* bus rescans (and return an exit code of 1):
~ # strace echo -n 1 > /sys/bus/pci/slots/0000:00:00.0/power
...
write(1, "1", 1) = -1 ENODEV (No such device)
write(1, "1", 1) = -1 ENODEV (No such device)
exit(1) = ?
Using cp gives a write error, even though the write did happen and a rescan
was triggered:
~ # echo -n 1 > tmp ; cp tmp /sys/bus/pci/slots/0000:00:00.0/power
cp: Write Error: No such device
It seems much better to return success instead of failure. The actual
status of the bus rescan is hard to return. It happens asynchronously in a
work thread, so the sysfs store functions returns before any status is
ready (the whole point of the work queue). And even if it didn't do this,
the rescan doesn't have any clear status to return.

In drivers/pci/hotplug/ibmphp_ebda.c::ebda_rsrc_controller(), storage is
allocated with kzalloc() and assigned to 'tmp_slot'. Then lots of
stuff, like ->flag, ->supported_speed etc is set in tmp_slot. A bit
further down there's then this test :
if (!bus_info_ptr1) {
rc = -ENODEV;
goto error;
}
At this point, tmp_slot has not been assigned to anything, so when
erroring-out we want to free it, but nothing at the 'error:' label
free's 'tmp_slot' - and we can't really free 'tmp_slot' at 'error:'
since we may jump to that label later when 'tmp_slot' *has* been used
and we do not want it freed. So, the only sane option left seems to be
to kfree(tmp_slot) just before jumping to the 'error:' label in the one
place where this is what actually makes sense. The following patch does
just that and thus kills off a tiny potential memory leak.

they were not so. So annotate these function and fix the references

drivers/pci/hotplug/acpiphp_ibm.c:ibm_find_acpi_device() is not

If the fakephp driver is used to emulate removal of a PCI device by
writing text string "0" to the "power" sysfs attribute file, this causes
its parent directory and its contents (including the "power" file) to be
deleted before the write operation returns. Unfortunately, it ends up
in a deadlock waiting for itself to complete.
The deadlock is as follows: sysfs_write_file calls flush_write_buffer
which calls sysfs_get_active_two before calling power_write_file in
pci_hotplug_core.c via the sysfs store operation. The power_write_file
function calls disable_slot in fakephp.c via the slot operation. The
disable_slot function calls remove_slot which calls pci_hp_deregister
(back in pci_hotplug_core.c) which calls fs_remove_slot which calls
sysfs_remove_file to remove the "power" file. The sysfs_remove_file
function calls sysfs_hash_and_remove which calls sysfs_addrm_finish
which calls sysfs_deactivate. The sysfs_deactivate function sees that
something has an active reference on the sysfs_dirent (from the
previous call to sysfs_get_active_two back up the call stack somewhere)
so waits for the active reference to go away, which is of course
impossible.
The problem has been present since 2.6.21.
This patch breaks the deadlock by queuing work queue items on a single-
threaded work queue to remove a slot from sysfs, and to rescan the PCI
buses. There is also some protection against disabling a slot that is
already being removed.

This is the workaround against Bad DLLP error that sometimes happen

The PCI code in 32 and 64 bits fixes up resources differently.
32 bits uses a header quirk plus handles bridges in pcibios_fixup_bus()
while 64 bits does things in various places depending on whether you
are using OF probing, using PCI hotplug, etc...
This merges those by basically using the 32 bits approach for both,
with various tweaks to make 64 bits work with the new approach.

Well, first of all, I don't want to change so many files either.
What I do:
Adding a new parameter "struct bin_attribute *" in the
.read/.write methods for the sysfs binary attributes.
In fact, only the four lines change in fs/sysfs/bin.c and
include/linux/sysfs.h do the real work.
But I have to update all the files that use binary attributes
to make them compatible with the new .read and .write methods.
I'm not sure if I missed any. :(
Why I do this:
For a sysfs attribute, we can get a pointer pointing to the
struct attribute in the .show/.store method,
while we can't do this for the binary attributes.
I don't know why this is different, but this does make it not
so handy to use the binary attributes as the regular ones.
So I think this patch is reasonable. :)
Who benefits from it:
The patch that exposes ACPI tables in sysfs
requires such an improvement.
All the table binary attributes share the same .read method.
Parameter "struct bin_attribute *" is used to get
the table signature and instance number which are used to
distinguish different ACPI table binary attributes.
Without this parameter, we need to offer different .read methods
for different ACPI table binary attributes.
This is impossible as there are various ACPI tables on different
platforms, and we don't know what they are until they are loaded.

sysfs is now completely out of driver/module lifetime game. After
deletion, a sysfs node doesn't access anything outside sysfs proper,
so there's no reason to hold onto the attribute owners. Note that
often the wrong modules were accounted for as owners leading to
accessing removed modules.
This patch kills now unnecessary attribute->owner. Note that with
this change, userland holding a sysfs node does not prevent the
backing module from being unloaded.

On some systems, the ACPI bus check event can reference a bridge that is
higher in the ACPI hierarchy than the bridge immediately above the
hotplug PCI slot into which an adapter was just inserted. The current
'acpiphp' code expects the bus check event to reference the bridge
immediately above the slot that received the adapter so the hotplug
operation can fail on these systems with the message "acpiphp_glue:
cannot get bridge info". This change fixes the problem by
re-enumerating all slots that lie below the bridge referenced by the bus
check event, including those slots that may be located under lower level
PCI-to-PCI bridge(s).

Instead of all drivers reading pci config space to get the revision
ID, they can now use the pci_device->revision member.
This exposes some issues where drivers where reading a word or a dword
for the revision number, and adding useless error-handling around the
read. Some drivers even just read it for no purpose of all.
In devices where the revision ID is being copied over and used in what
appears to be the equivalent of hotpath, I have left the copy code
and the cached copy as not to influence the driver's performance.

This rewrites pretty much from scratch the handling of MMIO and PIO
space allocations on powerpc64. The main goals are:
- Get rid of imalloc and use more common code where possible
- Simplify the current mess so that PIO space is allocated and
mapped in a single place for PCI bridges
- Handle allocation constraints of PIO for all bridges including
hot plugged ones within the 2GB space reserved for IO ports,
so that devices on hotplugged busses will now work with drivers
that assume IO ports fit in an int.
- Cleanup and separate tracking of the ISA space in the reserved
low 64K of IO space. No ISA -> Nothing mapped there.
I booted a cell blade with IDE on PIO and MMIO and a dual G5 so
far, that's it :-)
With this patch, all allocations are done using the code in
mm/vmalloc.c, though we use the low level __get_vm_area with
explicit start/stop constraints in order to manage separate
areas for vmalloc/vmap, ioremap, and PCI IOs.
This greatly simplifies a lot of things, as you can see in the
diffstat of that patch :-)
A new pair of functions pcibios_map/unmap_io_space() now replace
all of the previous code that used to manipulate PCI IOs space.
The allocation is done at mapping time, which is now called from
scan_phb's, just before the devices are probed (instead of after,
which is by itself a bug fix). The only other caller is the PCI
hotplug code for hot adding PCI-PCI bridges (slots).
imalloc is gone, as is the "sub-allocation" thing, but I do beleive
that hotplug should still work in the sense that the space allocation
is always done by the PHB, but if you unmap a child bus of this PHB
(which seems to be possible), then the code should properly tear
down all the HPTE mappings for that area of the PHB allocated IO space.
I now always reserve the first 64K of IO space for the bridge with
the ISA bus on it. I have moved the code for tracking ISA in a separate
file which should also make it smarter if we ever are capable of
hot unplugging or re-plugging an ISA bridge.
This should have a side effect on platforms like powermac where VGA IOs
will no longer work. This is done on purpose though as they would have
worked semi-randomly before. The idea at this point is to isolate drivers
that might need to access those and fix them by providing a proper
function to obtain an offset to the legacy IOs of a given bus.

First thing mm.h does is including sched.h solely for can_do_mlock() inline
function which has "current" dereference inside. By dealing with can_do_mlock()
mm.h can be detached from sched.h which is good. See below, why.
This patch
a) removes unconditional inclusion of sched.h from mm.h
b) makes can_do_mlock() normal function in mm/mlock.c
c) exports can_do_mlock() to not break compilation
d) adds sched.h inclusions back to files that were getting it indirectly.
e) adds less bloated headers to some files (asm/signal.h, jiffies.h) that were
getting them indirectly
Net result is:
a) mm.h users would get less code to open, read, preprocess, parse, ... if
they don't need sched.h
b) sched.h stops being dependency for significant number of files:
on x86_64 allmodconfig touching sched.h results in recompile of 4083 files,
after patch it's only 3744 (-8.3%).

After Al Viro (finally) succeeded in removing the sched.h #include in module.h
recently, it makes sense again to remove other superfluous sched.h includes.
There are quite a lot of files which include it but don't actually need
anything defined in there. Presumably these includes were once needed for
macros that used to live in sched.h, but moved to other header files in the
course of cleaning it up.
To ease the pain, this time I did not fiddle with any header files and only
removed #includes from .c-files, which tend to cause less trouble.
Compile tested against 2.6.20-rc2 and 2.6.20-rc2-mm2 (with offsets) on alpha,

Support for dynamic loading and unloading of ACPI SSDT tables upon slot
hotplugs and unplugs.
On SN platforms, we now represent every populated root bus slot with a single
ACPI SSDT table containing info for every device and PPB attached to the slot.
These SSDTs are generated by the prom at initial boot and hotplug time. The
info in these SSDT tables is used by the SN kernel IO "fixup" code (which is
called at boot and hotplug time).
On hotplugs (i.e. enable_slot()), if running with an ACPI capable prom,
attempt to obtain a new ACPI SSDT table for the slot being hotplugged. If
successful, add the table to the ACPI namespace (acpi_load_table()) and then
walk the new devices and add them to the ACPI infrastructure (acpi_bus_add()).
On hot unplugs (i.e. disable_slot()), if running with an ACPI capable prom,
attempt to remove the SSDT table associated with the slot from the ACPI
namespace (acpi_unload_table_id()) and infastructure (acpi_bus_trim()).
From: John Keller <jpk@sgi.com>

the slot's SSDT table from the ACPI namespace a bit too early in

Change memory allocation for acpiphp slots
Change the "struct slot" that acpiphp uses for managing it's slots to
directly contain the memory for the needed struct hotplug_slot_info and
the slot's name. This way we need only two memory allocations per slot
instead of four.
While we are at it: make_slot_name() is just a wrapper around snprintf()
knowing the right arguments to call it. Since the function makes just one
function call and is only called from one place I inlined it by hand.
Finally this fixes a possible bug waiting for someone to hit it. There were
two unused local variables in acpiphp_register_hotplug_slot(). gcc did not
find them because they were used in memory allocations with sizeof(*var).
They had the same types as the target of the allocation, but nevertheless
this was just weird.

Maintain a per-CPU global "struct pt_regs *" variable which can be used instead
of passing regs around manually through all ~1800 interrupt handlers in the
Linux kernel.
The regs pointer is used in few places, but it potentially costs both stack
space and code to pass it around. On the FRV arch, removing the regs parameter
from all the genirq function results in a 20% speed up of the IRQ exit path
(ie: from leaving timer_interrupt() to leaving do_IRQ()).
Where appropriate, an arch may override the generic storage facility and do
something different with the variable. On FRV, for instance, the address is
maintained in GR28 at all times inside the kernel as part of general exception
handling.
Having looked over the code, it appears that the parameter may be handed down
through up to twenty or so layers of functions. Consider a USB character
device attached to a USB hub, attached to a USB controller that posts its
interrupts through a cascaded auxiliary interrupt controller. A character
device driver may want to pass regs to the sysrq handler through the input
layer which adds another few layers of parameter passing.
I've build this code with allyesconfig for x86_64 and i386. I've runtested the
main part of the code on FRV and i386, though I can't test most of the drivers.
I've also done partial conversion for powerpc and MIPS - these at least compile
with minimal configurations.
This will affect all archs. Mostly the changes should be relatively easy.
Take do_IRQ(), store the regs pointer at the beginning, saving the old one:
struct pt_regs *old_regs = set_irq_regs(regs);
And put the old one back at the end:
set_irq_regs(old_regs);
Don't pass regs through to generic_handle_irq() or __do_IRQ().
In timer_interrupt(), this sort of change will be necessary:
- update_process_times(user_mode(regs));
- profile_tick(CPU_PROFILING, regs);
+ update_process_times(user_mode(get_irq_regs()));
+ profile_tick(CPU_PROFILING);
I'd like to move update_process_times()'s use of get_irq_regs() into itself,
except that i386, alone of the archs, uses something other than user_mode().
Some notes on the interrupt handling in the drivers:
(*) input_dev() is now gone entirely. The regs pointer is no longer stored in
the input_dev struct.
(*) finish_unlinks() in drivers/usb/host/ohci-q.c needs checking. It does
something different depending on whether it's been supplied with a regs
pointer or not.
(*) Various IRQ handler function pointers have been moved to type
irq_handler_t.

The following patches reduce the size of the VFS inode structure by 28 bytes
on a UP x86. (It would be more on an x86_64 system). This is a 10% reduction
in the inode size on a UP kernel that is configured in a production mode
(i.e., with no spinlock or other debugging functions enabled; if you want to
save memory taken up by in-core inodes, the first thing you should do is
disable the debugging options; they are responsible for a huge amount of bloat
in the VFS inode structure).
This patch:
The filesystem or device-specific pointer in the inode is inside a union,
which is pretty pointless given that all 30+ users of this field have been
using the void pointer. Get rid of the union and rename it to i_private, with
a comment to explain who is allowed to use the void pointer. This is just a
cleanup, but it allows us to reuse the union 'u' for something something where
the union will actually be used.

o hotplug slots add
When the hot-added PCI device is p2p bridge, acpiphp calls
find_p2p_bridge() to add hotplug slots.
o hotplug slots remove
When the hot-removing PCI device is p2p bridge, acpiphp
calls cleanup_p2p_bridge() to remove hotplug slots.
o notify handler exchange
When the p2p bridge is added, acpiphp changes the notify
hanlder.
If no bridge device is inserted into the hotpluggable PCI
slot, acpiphp installs the notify handler for function.
After the p2p bridge hot-add, acpiphp has to install the
notify handler for bridge. Because, the role of the
handlers are not same. The hot-remove case is ditto.

The init function for the RPA PCI Hotplug driver returns -ENODEV in the
case that no hotplug-capable slots are detected in the system. This is
bad, since hot-capable slots can be added after boot to a purely virtual
POWER partition. This is also bad because DLPAR I/O operations depend
on the rpaphp module.
Change the rpaphp init module to return success for the case of
partitions that own no hotplug-capable slots at boot. Such slots can be
dynamically added after boot.

These patches add generic dock event handling to acpiphp. If there are
pci devices that need to be inserted/removed after the dock event, the
event notification will be handed down to the normal pci hotplug event
handler in acpiphp so that new bridges/devices can be enumerated.
Because some dock stations do not have pci bridges or pci devices that
need to be inserted after a dock, acpiphp will remain loaded to handle
dock events even if no hotpluggable pci slots are discovered.
You probably need to have the pci=assign-busses kernel parameter enabled
to use these patches, and you may not allow ibm_acpi to handle docking
notifications and use this patch.

Current SHPCHP driver doesn't care about the confliction between
hotplug operation via sysfs and hotplug operation via attention
button. So if those ware conflicted, slot could be an unexpected
state.
This patch changes SHPCHP driver to handle slot state properly. With
this patch, slot events are handled according to the current slot
state as shown at the Table below.
Table. Slot States and Event Handling
=========================================================================
Slot State Event and Action
=========================================================================
STATIC - Go to POWERON state if user initiates
(Slot enabled, insertion request via sysfs
Slot disabled) - Go to POWEROFF state if user initiates removal
request via sysfs
- Go to BLINKINGON state if user presses
attention button when the slot is disabled
- Go to BLINKINGOFF state if user presses
attention button when the slot is enabled
The event handler of SHPCHP driver is unnecessarily very complex. In
addition, current event handler can only a fixed number of events at
the same time, and some of events would be lost if several number of
events happened at the same time.
This patch simplify the event handler by using 'work queue', and it
also fix the above-mentioned issue.

Removing and then adding a PCI slot to a running partition results in
a kernel panic. The current code attempts to add iospace for an entire
root bus, which is inappropriate, and silently fails. When a pci device
tries to use the iospace, a page fault is taken, as the iospace had not
been mapped, and of course the page fault cannot be resolved.
This only occurs for PCI adapters using pio, which may be why it hadn't
been seen earlier (this seems to have been broken for a while).

Current SHPCHP driver writes a '0' to the Command Completion Detect
bit to clear the Command Complete Interrupt Pending. But according to
the SHPC spec (See 4.7.3.1 System Interrupts), SHPCHP driver must
write '1'. This patch fixes this bug.

The hpc_get_mode1_ECC_cap() function of SHPCHP driver seems to refer
the wrong bit for refering the "Mode 1 ECC Capability" bit. This bug
seems not to cause any problem so far. But I think this should be
fixed. This patch fixes this bug.

refer wrong bits in the "Slot Avail Register I" and "Slot Avail
Register II". This patch fixes this bug. And this also cleanup the
code.

This patch replaces pci_find_slot() with pci_get_slot() in the SHPCHP
driver. This enables SHPCHP driver to work on multiple PCI segment
systems.

Include fixes for 2.6.14-git11. Should allow to remove sched.h from
module.h on i386, x86_64, arm, ia64, ppc, ppc64, and s390. Probably more
to come since I haven't yet checked the other archs.

Per the PCI Express spec, the power-fault-detected bit in the
slot status register can be set anytime hardware detects a power
fault, regardless of whether the slot has a device populated in
it or not. This bit is sticky and must be explicitly cleared.
This patch is needed to allow hot-add after such a power fault
has been detected.

This patch contains the following cleanups:
- access.c should #include "pci.h" for getting the prototypes of it's
global functions
- hotplug/shpchp_pci.c: make the needlessly global function
program_fw_provided_values() static

even if the condition has cleared by the time the corresponding
interrupt handling code gets a chance to run. This patch

This patch further tweaks how we request control of hotplug
controller hardware from BIOS. We first search the ACPI namespace
corresponding to a specific hotplug controller looking for an
_OSC or OSHP method. On failure, we successively move to the
ACPI parent object, till we hit the highest level host bridge
in the hierarchy. This allows for different types of BIOS's
which place the _OSC/OSHP methods at various places in the acpi
namespace, while still not encroaching on the namespace of
some other root level host bridge.
This patch also introduces a new load time option (pciehp_force)
that allows us to bypass all _OSC/OSHP checking. Not supporting
these methods seems to be be the most common ACPI firmware problem
we've run into. This will still _not_ allow the pciehp driver to
work correctly if the BIOS really doesn't support pciehp (i.e. if
it doesn't generate a hotplug interrupt). Use this option with
caution. Some BIOS's may deliberately not build any _OSC/OSHP
methods to make sure it retains control the hotplug hardware.
Using the pciehp_force parameter for such systems can lead to
two separate entities trying to control the same hardware.

This patch tweaks the way pciehp requests control of the hotplug
hardware from BIOS. It now tries to invoke the ACPI _OSC method
for a specific hotplug controller only, rather than walking the
entire acpi namespace invoking all possible _OSC methods under
all host bridges. This allows us to gain control of each hotplug
controller individually, even if BIOS fails to give us control of
some other hotplug controller in the system.

Reduce the number of debug messages generated if pciehp debug is

are either early-driver-debug type messages, or print information
that can be inferred through other debug prints.

State information is currently stored in per-slot as well as
per-pci-function data structures in pciehp. There's a lot of
overlap in the information kept, and some of it is never used.
This patch consolidates the state information to per-slot and
eliminates unused data structures. The biggest change is to
eliminate the pci_func structure and the code around managing
its lists.
